1.The clinical significance of ANCA detction in patients with systemic lupus erythematosus

Objective To study the diagnostic value of anti-neutrophil cytoplasmic antibody (ANCA) detction in patients with systemic lupus erythematosus (SLE). Methods Indirect immunofluorescence (IIF) and Western blotting were applied to detecting the expression of sera ANCA, antinuclear antibody (ANA), anti-double-stranded DNA antibody, anti-SmithD1 antibody in 25 samples of control group and 106 samples of patients with SLE. Results ALL samples of control group showed negative; 47.2% (50/106) sera showed ANCA positive in patients with SLE,and all of them were peripheral nuclear pattern (pANCA). In ANCA positive group, the positive rate of ANA, anti-SmithD1 antibody and anti-ds-DNA antibody was respectively 100% (50/50), 82% (41/50) and 94% (47/50); in ANCA negative group, the positive rate was respectively 91.1% (51/56), 67.9% (38/56)and 50% (28/56). There were no statistical differences in positive rates of ANA and anti-SmithD1 antibody between ANCA positive and negative groups (both P>0. 05), but there was in positive rates of anti-ds-DNA antibody (P<0.01). ANCA positive rate was respectively 84.6% (44/52) and 11.1%(6/54) in active SLE group (n=52) and inactive SLE group (n=54) (P<0.01). Conclusion The determined ANCA in patients with SLE is not correlative with ANA and anti-SmithD1 antibody, but positively relates to anti-double-stranded DNA antibody. The positive rate of ANCA in patients with active lupus is significantly higher than that in non-active lupus.

The results indicate that the ion beam bioengineering of China get a fast development under the support of the nation and the local government and college. The development in the field of microbe is the fastest. in 21st century, the local government and colleges gradually enlarge the support of the ion beam bioengineering, some articles are under the support of enterprise researcher and fund. The contents of these articles mainly about application study. After analysing the data, the future of ion beam bioengineering of China was forecasted.
3.Identification of the genus and species of the dacryocystitis-causing bacteria by 16S rRNA gene

Background Dacryocystitis is one of the most common infectious eye diseases.The gold standard for the identification of bacteria causing dacryocystitis is bacterial culture.The combination of regular culture method with molecular biology techeniques will generate more reliable results.However,very few research data are available in ophthalmological studies in this area.Objective This study was to identify the genera and species of the dacryocystitis-causing bacteria by PCR amplification of the 16S rRNA sequences.Methods Ten cases of qualified standardized bacteria samples were taken,and the nucleic acids were released in the heating process of the PCR procedure.The 16S rRNA genes were amplified and sequenced,and the genera and species were identified using BLAST from GenBank,and the results were used to compare with the results from biochemical identification to test the reliability of this method.The cultured bacterial species from the lacrimal sac secretions from 30 cases of dacryocystitis patients were identified with the above method.Results The outcome of the PCR identification for the 10 cases of quality control standard bacterial specimens was consistent with the results from the biochemical identification.The identification of the 30 cases of dacryocystitis through sequencing the 16S rRNA revealed there were 13 cases of Staphylococcus epidermidis infection,2 cases of Staphylococcus warneri infection,1 case of Staphylococcus hominis infection,5 cases of Corynebacterium macginleyi infection,3 cases of Streptococcus pneumonia infection,2 cases of Bacillus cereus infection,1 case of Micrococcus luteus infection,1 case of Moraxella catarrhalis infection,1 case of Moraxella osloensis infection and 1 case of Pseudomonas aeruginosa infection.Conclusions Sequencing the 16S rRNA is an accurate and specific way for the identification of the genera and species of bacteria that cause dacryocystitis in patients.This sequencing method is feasible in monitoring a variety of dacryocystitis-causing pathogens.More information and epidemiological statistics about dacryocystitis can be obtained from 16S rRNA sequencing.
4.Further enhancing patient satisfaction after laser corneal refractive surgery

Corneal refractive surgery has been the major solution for the correction of ametropia.With the standardization of preoperative examination,intraoperative procedure and perioperative drugs,corneal refractive surgery has become much safer.Meanwhile,patients can get not only good visual acuity,but also favorable visual quality.However,the patient satisfaction has not been improved dramatically with the enhance of effectiveness and safety after surgery.Increase of the satisfaction degree to corneal refractive surgery is related to multiple factors such as experienced and highly skilled operation,operative safety,lessening operative complication,good postoperative visual and life quality of patients.Comprehensively analyzing existing problems in corneal refractive surgery in China and further obtain satisfaction of patient will be of an important significance for the healthy development of laser corneal refractive surgery.
6.Clinical study on mild cognitive impairment converting to dementia

Objective To study the natural history of mild cognitive impairment (MCI) progress to dementia and to evaluate the efficacy of acetylcholinesterase inhibitor (AChEI) donepezil on MCI.Methods Ninty-eight patients with MCI including amnestic MCI and non-amnestic MCI were retrospectively analyzed. The patients with donepezil or not were separately divided to two groups while they were matched by sex, age,degree of MCI and possession of the ApoE?4 allele.The rate of conversion from MCI to dementia, measured with the Mini-Mental State Examination (MMSE),Alzheimer's Disease Assessment Scale- Cognitive section(ADAS-Cog) and Wechsler Memory Scale-Revised (WMS-R),was compared between the two groups.Results In aMCI group,the rate of conversion dropped by 15.1% and 8.3% (P0.05).Conclusion Donepezil could postpone the conversion of MCI to dementia.
8.Analysis of the failed internal fixations for thoracolumbar fractures

Objective To discuss the common causes for postoperative complications after treatment of spinal fractures and their preventive measures.Methods To analyze the 32 failed eases who received internal fixation for their thoracolumbar fractures between July 1998 and April 2005 in our department.Their preoperative and postoperative results of X-ray,CT and MIR examinations were reviewed to find out the causes for failure.Three of them were treated through anterior approach and 29 through posterior approach.Eighteen screws broke in eight eases, the rods got loosened in eight cases and broken in four cases,15 pedicte screws were mis-located in eight cases and the implants got loosened in three cases.Results Thirty patients had to get a reoperation.Two were cured through non-surgical treatment.The causes for failure were found to be as follows:1,incorrect operation approach;2,in- stability of the front-middle colunm;3,unskillful operation;4,ignorance or the lesion to the adjacent vertebral discs; 5,flaws in pedicle screw design.Conclusion To prevent postoperative complications after internal fixation for spinal fractures,surgeons should strictly stick to the indications before operation,choose a proper approach and method for internal fixation,try to stabilize the front-middle column by the first intention,and have a good command of anatomy and operation skills.
