1.Progress in treatment of liver cirrhosis with ascites

Ascites formation represents a hallmark of decompensated liver cirrhosis.It predicts a poor outcome.Patients with ascites are at high risk of developing complications such as SBP (spontaneous bacterial peritonitis),hyponatremia and HRS (hepatorenal syndrome).Adequate treatment of cirrhotic ascites increases survival and betters quality of life.Despite improved current medical treatment of ascites,liver transplantation remains the ultimate treatment of refractory ascites.This paper summarizes the treatment of liver cirrhosis with ascites.
3.Mediating effect of social support on relationship between childhood abuses and non-suicidal self-injuries in adolescents

Objective:To investigate the relationship among childhood abuses,social support and non-suicidal self-injury (NSSI) in adolescents,and to examine the mediating effect of social support on the relationship between childhood abuses and non-suicidal self-injury.Methods:A total of 9704 middle school students were selected from junior and senior middle schools in Zhengzhou,Guiyang city.The Childhood Trauma Questionnaire-28 item Short Form (CTQ-SF),Non-Suicidal Self-Injury Questionnaire,Social Support Scale for University Students and Multidimensional Sub-health Questionnaire of Adolescents (MSQA) were used to measure the different types of adverse childhood experiences,non-suicidal self-injury,social support and psychopathological status.Results:The CTQ-SF-scores were positively correlated with non-suicidal self-injury scores (r =0.20,P < 0.01) and negatively correlated with social support scores (r =-0.22,P < 0.01).The total scores and factor scores of social supportscale were negatively correlated with non-suicidal self-injury scores in adolescents (P < 0.01).When control of the area,grade,self-reported learning burden,self-reported grades and psychological pathology symptoms,social support in childhood abuse and the relation between the suicide self-injury the mediation effect by the single factor of 17.04% to 12.57%,the mediation effect of three subcomponents accounted for 7.12% ~ 13.85%.Conclusion:It suggests that social support may play a mediating effect on the relationship between childhood abuses and non-suicidal selfinjury.

7.Effect of nuclear factor-kappa B decoy oligodeoxynncleotides on respiratory function and cytokine expression after severe lung contusion in rabbits

Objective To explore the effect of nuclear factor-kappa B (NF-κB) decoy oligode-oxynucleotides (ODN) on respiratory function and expressions of IL-1β and IL-13 in serum following se-vere lung contusion in rabbits. Methods A total of 40 New-Zealand rabbits were randomly divided into four groups, ie, severe lung contusion group (Group A, n=12), severe lung contusion with NF-κB scrambled decoy ODN intervention group (Group B, n=12), severe lung contusion with sense NF- B de-coy ODN intervention group (Group C, n=12) and normal control group (Group D, n =4). After the contusion model was set up, the sense and scrambled NF-κB decoy ODN were infused into the rabbits via the jugular veins in different groups, with 20 g per experimental rabbit. After contusion, respiratory fre-quency, tidal volume, airway pressure, respiration flow rate curve and end expiration nitric oxide concen-tration were detected at 1, 2, 3 and 4 hours. The expressions of IL-1β and IL-13 in serum were observed by means of ELISA. Results After sense NF-κB decoy ODN intervention, alveolar ventilation, arteri-al PO_2 and pulmonary compliance were improved, compared with Group A and Group B, with statistical difference (P<0.01). The expression of IL-1β was decreased and that of IL-13 increased after sense NF-κB decoy ODN intervention to the severe lung contusion, compared with Groups A and B, with statis-tical difference (P <0.01). The expression of IL-1β was increased to peak level at 1 hour after contu-sion, which continued to the end of the experiment. While expression of IL-13 was decreased at 1 hour af-ter contusion and reached the minimum level at 4 hours. With intervention with sense decoy ODN, the in-creased expression of IL-1β was down-regulated, but expression of IL-13 remained at high level, with sta-tistical difference compared with Group A and Group B (P < 0.01). Conclusions Intervention with sense NF-κB decoy ODN can significantly protect the respiratory function, reduce the expression of IL-1β and increase expression of IL-13 after severe lung contusion.
8.Preparation and characterization of hydroxyapatite/collagen/poly-L-lactic acid three-dimensional porous reservoir type drug carrier for controlled release

BACKGROUND: In the clinical therapy for orthopedic diseases, the traditional administration easily induces the great fluctuation of drug concentration and side effect, and implanted drug carrier material hydroxyapatite (Hap) cannot be used in human's load bearing parts due to its poor mechanical performances, and poly-L-lactic acid (PLLA) is effortlessly degraded to acid byproducts. Drug carriers made from Hap/PLLA are supposed to not only persist releasing drug onto the lesion and reduce the side effect,but also obtain strength enhancement and eliminate tissue inflammatory reaction in favor of bone regeneration.OBJECTIVE: To observe the preparation of absorbable target control-release drug carrier and simulate the application of drugs.DESIGN: Observation trial.SETTING: Chongqing Institute of Technology and Wuhan University of Technology.MATERIALS: Tails of albino rats and PLLA (offered by Biological Center of Wuhan University of Technology), pepsin (1:10 000, Sigma company), Ca(OH)2, strong phosphoric, NaOH, glacial acetic acid, phosphate buffer (pH=7.4), 1,4-dioxane, absolute alcohol (Analytical reagent available on market).METHODS: The experiment was carried out in Chongqing Institute of Technology and Wuhan University of Technology. ①Type Ⅰ collagen (Col)was prepared by acid dissolution and alkali purification methods. Hap/Col bone-like biomimetic composite was synthesized through self-assembly mechanism of materials simulating in vitro biomineralization process of nature bone. X ray diffraction analysis and transmission electron microscope were applied to observe the characterization of composite's structure and morphology. ②The resultant product of HAP/Col composite was further synthesized with PLLA to prepare the three-dimensional porous reservoir type carrier for controlled release of drug employing thermal-induced phase separation technique. Scanning electron microscope, material test machine and specific gravity test method were used to investigate the pore structure and mechanical property of carrier material. ③The prepared Hap/Col/PLLA drug carrier was loaded with bromothymol blue to investigate in vitro control-release characteristic in simulated body fluid.MAIN OUTCOME MEASURES: ①Characterization of structure and morphology of Hap/Col bone-like biomimetic composite; ②Evaluation on the property and preparation technique of Hap/Col/PLLA drug carrier; ③Assessment on results of in vitro control-release trail of model composite.RESULTS: ①Hap nanocrystals formed as slender needles aligning with its crystalline c-axis preferentially oriented.②Hap/Col/PLLA drug carrier possessed apt pore configuration and physical properties for drug controlrelease.③In vitro release test by model compound revealed an approximate zero-orderslow release prior to its 80% release percentage.CONCLUSION: Hap/Col composite material is similar with natural bone, and Hap/Col/PLLA reservoir carrier can control drug release.
9.A clinical study on dystonia manifested as parkinsonism( report of 1 next-generation sequencing attached case)

Objective To discuss the clinical features of dystonia manifested as Parkinsonism ( PKS) . Methods Clinical materials of a patient with dystonia manifested as PKS were analyzed retrospectively. Results The onset age of the young women was 31 years old, who was started asymmetrically with symptoms of claudication and tremor of the right foot. Levodopa had a short-term effect. The results of dopamine transporter ( DAT) PET showed that DAT in retrolentiform part were decreased significantly. Atypical Parkinson's disease was considered and she was treated as PKS long-termly. Subsequently, heterozygous mutation of c. 268-4T>A (NM_018105) in DYT6 gene was found through the next-generation sequencing, which was a kind of splicing mutation and confirmed by the first-generation sequencing. Conclusions Patients with dystonia might share similar clinical manifestations with PKS. Particularly, they should be differentiated with young-onset Parkinson's disease combined with focal dystonia. Clinical observation and genetic testing are important approaches to differentiate them.
10.Effect of rectal diclofenac on incidence of hyperamylasemia and hyperlipasemia after double-balloon enteroscopy

To evaluate the efficacy of the prophylactic use of diclofenac in prevention of hyperamylasemia and hyperlipasemia in patients undergoing double-balloon enteroscopy (DBE).One hundred and sixteen patients undergoing peroral DBE were enrolled in the study.Total 121 procedures were performed,in 56 procedures diclofenac sodium 50 mg was administered by rectum 1 h before the peroral DBE (intervention group) and in 65 procedures no medication was given (control group).Serum amylase and lipase levels were measured before and 4 h,24 h after the procedure.At 4 h after DBE the incidence rates of hyperamylasemia and hyperlipasemia in intervention and control groups were 33.9%,38.5% (P > 0.05) and 21.4%,36.9% (P >0.05),respectively.At 24 h after DBE,the incidence rates of hyperamylasemia and hypedipasemia in intervention and control groups were 19.6%,24.6% (P > 0.05) and 10.7%,15.4% (P > 0.05),respectively.Mild acute pancreatitis developed in one patient and gastrointestinal hemorrhage occurred in another patient.Hyperamylasemia and hyperlipasemia is common and transient after double-balloon enteroscopy,and rectal administration of diclofenac sodium cannot effectively reduce the incidence.
