1.Effects of E 4031 on isolated cardiac function and resting Ca2+ level of myocardial cells from rats with chronic heart failure.

OBJECTIVETo investigate the effects of E 4031, a sodium calcium exchanger (NCX) agonist, on the isolated cardiac function and resting Ca2+ level in myocardial cells from rats with chronic heart failure.
METHODSRats chronic heart failure model was established by abdominal aorta coarctation with; Isolated heart perfusion by Langendorff apparatus was used to detect heart function and the effects of E 4031 on haemodynamic indexes; Myocardial cells of rats in the model group were extracted quickly and co-incubated with calcium fluorescent indicator fluo3/AM and the impact of E 4031 on the fluorescence intensity in myocardial cells were evaluated by confocal microscopy.
RESULTSHeart function of rats in the model group detected by Langendorff perfusion was significantly reduced after 12 weeks, E 4031 at 10 micromol/L could improve their left ventricular developed pressure(LVDP) and systolic / diastolic maximum rate (+/- dp/dtmax). Compared with the control and sham operation groups, the resting Ca2+ fluorescence intensity of the myocardial cells of rats in model group was at a higher level and went through a process of transient rise and drop, then stably remaining at a low level after co-incubated with 10 micromol/L E 4031.
CONCLUSIONE 4031 can improve the isolated heart function of rats with chronic heart failure, which may be associated with its enhancing the activity of NCX in the myocardial cell membrane and stabilizing intracellular Ca2+ level.

2.Bilateral lumbar pedicle screw fixation for lumbar intervertebral disc herniation:recovery of the height of intervertebral discs

BACKGROUND:Lumbar fusion is a conventional effective measure to treat spondylolisthesis, spinal stenosis or with deformity. Bilateral pedicle screw fixation is recognized as the standard treatment for various spinal disorders, and has biomechanical and clinical advantages. OBJECTIVE:To evaluate the effects of bilateral pedicle screw fixation in the repair of lumbar disc herniation to restore disc height from the angle of imaging. METHODS: Clinical data of 42 patients with lumbar disc herniation were retrospectively analyzed. They al received bilateral pedicle screw fixation. Pain was evaluated before implantation, immediately and 1 month after implantation using Japanese Orthopaedic Association score of lower back pain and visual analog scale score. X-ray including anteriorposterior and lateral films of lumbar spine and MRI were used. CT was utilized to verify screw placement conditions and complications. RESULTS AND CONCLUSION:A total of 42 patients were folowed up for 3-6 months. Compared with pre-implantation, Japanese Orthopaedic Association score and visual analog scale score were significantly improved immediately after implantation (P < 0.01). There was no significant difference in Japanese Orthopaedic Association score between 1 month and immediately after implantation (P > 0.05). The height of intervertebral discs was significantly higher immediately and 1 month after implantation than pre-implantation (P < 0.01). The symptoms were lessened after fixation in al cases, and their qualities of life elevated. At 1 month, X-ray films and CT images revealed that no screw loosening, breakage or displacement occurred. The height of intervertebral discs was perfectly restored. No adverse events appeared in patients. These data indicate that bilateral pedicle screw fixation for lumbar intervertebral disc herniation can effectively restore the height of intervertebral discs, improve clinical symptoms and have biological and clinical superiority.

Objective:Performance appraisal of large-scale medical equipment has a significant impact on hospital management. Process equipment performance evaluation of digital is an important part of digital medical&hospital. How to fully utilize the large number of advanced medical equipment resources, how to monitor and manage equipment are focuses that hospital administrators pay much attention to. Methods: The effective performance appraisal of large-scale medical equipment can promote the resource allocation optimization, accurate and scientific management, reasonable and benign application, and then allows large-scale medical equipment fully and effectively applied to clinical practice. Results: Collecting basic data is the basis for performance appraisal of large-scale medical equipment; its instant validity, accuracy, and scientific can enhance the operability and the scientific accuracy of the performance appraisal for large-scale medical equipment. Conclusion:The acquisition of basic data has few specialized research in previous studies, the rules set up for basic data acquisition of large-scale medical equipment in this paper can improve the timeless, accuracy and normalization of the basic data using in the performance appraisal of large-scale medical equipment.

Liver is the main target organ of the metastasis of colorectal cancer.More and more researches demonstrate that including micro-environment of cancer,signal transduction pathway,abnormality of chromosomes and the gene which control cell proliferation and apoptosis are the most important mechanisms in liver metastasis of colorectal cancer.The metastasis of colorectal cancer is the result of all these factors.To study the mechanism can shed some light on prevention and therapy in liver metastasis of colorectal cancer.
5.Study on the medication scope of 581 optimized ancient prescriptions

Objective The medication scope of 581 prescriptions contained in the book of Two hundred one party governance inspection record is studied through data mining.Methods After regulating drug names individually according to the authoritative literature,the statistical range of medication is given using the software Excel 2003.Results A total of 360 kinds of medicines are obtained.Conclusion The spectrum of 581 optimized ancient prescriptions is formed,which is the focus of our further research.
7.Role of intracellular protein tyrosine kinase activation in vascular smooth muscle cells migration mediated by angiotensin Ⅱ

Objective To investigate the role of intracellular protein tyrosine kinase (PTK) activation in vascular smooth muscle cells (VSMCs) migration mediated by angiotensin Ⅱ (AngⅡ). Methods Intracellular PTK activities were investigated by means of ?- 32 P incorperation in cultured rat VSMCs stimulated by AngⅡ. The migrated VSMCs were enumerated in a modified Boyden Chamber. Results Without any specific stimulus, only a few migrated cells were detected. However, the number of migrated cells were increased, under the condition of AngⅡ at the concentration of 1?10 -10 , 1?10 -9 , 1?10 -8 , 1?10 -7 and 1?10 -6 mol/L, by 3.7, 4.6, 5.9, 6.6 and 6.3 times respectively, compared with the control group. Intracellular PTK activities were also increased by 1.0,1.7,2.4,3.2 and 2.4 times compared with the control group, under the condition of AngⅡ at the concentration of 1?10 -10 , 1?10 -9 , 1?10 -8 , 1?10 -7 and 1?10 -6 mol/L, respectively. Statistical analysis showed that there was a significant positive correlation between AngⅡ-induced VSMCs migration and intracellular PTK activation. 9.Analysis of the status quo and needs of community care for disabled elderly in Beijing

Objective:To study the current status and needs of Community Care for disabled elderly in Beijing and put forward the suggestions for improving the elderly Community Care system. Methods:Descriptive statistical a-nalysis, Chi-square test and Multivariate logistic analysis were adopted to analyze the situation of Community Care for Disabled elderly using data from the community survey of elderly population in Beijing. Results:The status of health is not optimistic for Disabled elderly, there are high demands for the community health services and welfare facilities for elderly and the needs for services provided by part-time home care, day care and volunteers are high, but those services and facilities provided by communities were found to be inadequate. Conclusion:China has a large popula-tion base of Disabled elderly and its number grows fast, but community care supply is in shortage and cannot meet the needs of those people. It is therefore urgent to improve community health services and community care system.
10.Orthotopic heart transplantations for end-stage heart diseases

Objective To summarize the outcomes and clinical experience of orthotopic heart transplantations in Shanghai Zhongshan Hospital.Methods From May 2000 through October 2005,141 patients,101 males and 40 females,diagnosed as having dilated cardiomyopathy in 118,hypertrophic cardiomyopathy in 2,restrictive cardiomyopathy in 2,end-stage ischemic heart disease in 9, primary malignant cardiac tumor in 4,valvular heart disease in 3 and others in 3,underwent orthotopic heart(transplantations) at our center.The operative procedures included 120 bicaval anastomotic cardiac transplantations,19 conventional Stanford orthotopic cardiac transplantations and 2 total heart transplantations.The immunosuppressive therapy strategies included Cyclosporine A or tacrolimus,corticosteroids and mycophenolate mofetil.For the latest 28 patients,induction therapy with Daclizumab was applied.Results There were 3 operative deaths with an operative survival of 97.9 %.During the follow-up from 1 month to 65 months,the actuarial survival rate was 90.8 % after 1 year,84.6 % after 3 year and 81.4 % after 5 year.Acute rejection,infections,graft failure and metastasis of primary cardiac tumor were the main causes of death in the 1st postoperative year,while acute rejection and graft coronary vasculopathy were the leading causes of death thereafter.Postoperative complications included acute rejection,infections,renal dysfunction and graft failure.Conclusions Orthotopic heart transplantation was proved to be a reliable choice for endstage heart disease with excellent outcomes.More attention should be paid to surveillance and management of acute rejection, infections and graft coronary vasculopathy in the long-term follow-up.
