1.Osteosarcoma in a 14 month old female child.

A case of osteosarcoma in a 14 month old girl is reported. The child presented with a history of difficulty in standing for 2 months and x-rays revealed an osteolytic lesion in the upper third of the femur associated with widening of bone and periosteal reaction. Osteosarcoma was diagnosed by biopsy. Treatment was refused and the child died 13 weeks later. Only one other case of osteosarcoma in a child as young as 14 months has been reported. A review of literature showed 22 cases of osteosarcoma in children aged 5 years or younger.

4.Determinants of Body Weight Status of University Students: Exploratory Evidence from Universiti Sains Malaysia

Introduction: Overweight and obesity are major health concerns among young adults in Malaysia. This study investigates the association between socio¬demographic and health-lifestyle factors and body weight status among university students.
Methods: Data were obtained from random questionnaire-based face-to-face interviews of 414 full-time students from Universiti Sains Malaysia (USM). Ordered probit analysis was conducted and marginal effects of socio-demographic and health-lifestyle variables on body-mass-index (BMA calculated.
Results: Among socio-demographic variables, ethnicity (Chinese), gender (male), monthly household income bracket, and family history of obesity were significant factors in determining body weight status. As a health-lifestyle factor, self-reported main meal portion size was significantly associated with BMI.
Conclusion: It is concluded that health policy makers in institutions of higher learning should take appropriate intervention measures by focusing on socio-demographic and health-lifestyle related factors in addressing issues of overweight and obesity among university students. Policy recommendations include implementing specific nutrition awareness programmes among male students and promoting health education that elevated body weight issues could be associated with familial traits. Efforts to ensure affordable and accessible nutrient-rich foods in campus cafeterias may also be beneficial to the nutritional health of university students.
5.Dietary Compliance and its Association with Glycemic Control among Poorly Controlled Type 2 Diabetic Outpatients in Hospital Universiti Sains Malaysia

Introduction: Compliance with medical nutrition therapy is important to improve
patient outcomes. The purpose of this study was to determine dietary compliance
and its association with glycemic control among outpatients with poorly
controlled type 2 diabetes mellitus (T2DM) in Hospital Universiti Sains Malaysia
(HUSM). Methods: In this cross-sectional study, patients who had a glycosylated
hemoglobin (HbA1c) level of at least 6.5%, after attending a diet counseling
session at the Outpatient Dietetic Clinic, HUSM, were enrolled. Out of 150 diabetic
patients reviewed between 2006 and 2008, 61 adults (32 men and 29 women)
agreed to participate in this study. A questionnaire-based interview was used to
collect socio-demographic, clinical and diabetes self-care data. The patient’s
dietary compliance rate was determined by the Summary of Diabetes Self-Care
Activities (SDSCA) measure. Anthropometric and biological measurements were
also taken. Results: Only 16.4% of the respondents adhered to the dietary regimen
provided by dietitians. Among the 7 dietary self-care behaviours, item number
6 (eat lots of food high in dietary fibre such as vegetable or oats) had the highest
compliant rate (54.1%); whereas item number 3 (eat five or more servings of
fruits and vegetables per day) had the lowest compliant rate (23.0%). There was
a significant association between gender (p=0.037) and fasting blood sugar (FBS)
(p=0.007) with the compliance status. Conclusion: Dietary non-compliance is
still common among T2DM patients. Dietitians need to improve their skills and
use more effective intervention approaches in providing dietary counseling to
patients.
6.‘Focal’ Cardiac Concussion – An Under-Recognized Problem?

Cardiac concussion is a sudden, direct blunt trauma cause to the chest which led
to death. However, there are patients who did not completely fulfil this definition.
We report two cases which did not fit into the definition domain. Two male patients
presented to Emergency Department with moderate anterior chest pain after motor
vehicle crash showed transient anterior ST segment elevation at chest lead V2
and V3 with raised creatine kinase and normal troponin T. The electrocardiogram
changes fully resolved after 24 hours. Both patients were discharged uneventfully
after 24 hours monitoring in Emergency Department short-stay ward. Conventional
definition of cardiac concussion (commotion cordis) and cardiac contusion may be unsuitable to describe these cases. Therefore, we propose the diagnosis of focal
cardiac concussion. We also highlighted the ability of Emergency Department to
manage these patients in short-stay ward.

7.Wandering humeral head mimicking a breast mass.

A 74-year-old woman was incidentally found to have a left breast mass. The mass could not be adequately compressed to be visualized on mammography. Ultrasonography showed a heavily-calcified rounded mass in the left axillary tail of the left breast. Chest radiograph confirmed that the mass was a migrated humeral head. Remotely-displaced fracture-dislocations of the humeral head are very rare and to our knowledge, displacement into the breast, clinically mimicking a breast mass, has not been previously described.

8.Generalized asymptomatic red nodules in a 21-year old Filipino male.

A 21 year old Filipino male presented with swelling of the second right hand digit unresponsive to antibiotics. Amputation revealed chronic inflammation and negative cultures. He developed sterile conjunctivitis and a generalized eruption of asymptomatic red papules and nodules. First skin biopsy revealed a diffuse infiltrate of epithelioid and foamy histiocytes, diagnosed as "juvenile xanthogranuloma." The second biopsy revealed large histiocytes with a "ground-glass" eosinophilic cytoplasm, multinucleated giant cells, and mixed cell infiltrate. Immunohistochemistry showed histiocytes staining with (+)S100 and (+)CD68, and (-)CD1a. Final diagnosis was "multicentric reticulohistocytosis." Despite treatment with oral prednisone, methotrexate and alendronate, lesions were progressive.
CONCLUSION: This fascinating case manifests with overlapping features of both juvenile xanthogranuloma and multicentric reticulohistiocytosis, and lead the authors to suggest considering the spectrum of diseases called the non-Langerhans cells histiocytosis when presented with a generalized nodular eruption.

9.Scrotal leiomyoma presenting as angiokeratoma of fordyce.

Scrotal Leiomyoma is an extremely rare benign tumor originating from the tunica dartos muscle. It usually presents as a solitary, painless, unilateral, slow growing mass that is occasionally pedunculated. We report a case of a 31-year-old man, presenting clinically with multiple papules resembling Angiokeratoma of Fordyce but with a histopathologic diagnosis of Scrotal Leiomyoma. Scrotal Leiomyoma has not been reported to present clinically in this manner and our report is probably the first of its kind.

10.Vaginal agenesis: A case report.

Congenital anomalities of the vagina are rare congenital anomalies. Women born with this anomaly present with collection of blood in the uterine cavity or hematometra and pelvic pain. Presented is a case of a 12-year old girl with hypogastric pain and primary amenorrhea complicated by vaginal agenesis. She was managed conservatively by creating a neovagina with the use of bipudental flap or Modified Singapore flap. Management can be non-surgical or surgical but the management of congenital vaginal agenesis remains controversial. The decision to do a conservative surgical procedure or a hysterectomy depends on the clinical profile of the patient, the expertise of the surgeons, the extent of the anomaly, and it's association to other congenital anomalies.
