Objectives: Gastric endoscopic submucosal dissection (ESD) is currently astandard procedure. ESD enables en-bloc resection of large lesions, whileinducing larger artificial ulcers to a greater extent than conventional procedures.Several studies have reported that proton pump inhibitors (PPIs) prevent delayed bleedingand expedite the artificial ulcer healing process. Esomeprazole, anS-isomer of omeprazole, is reportedly one of strongest inhibitors ofgastric acid secretion. Previous studies have examined the effectiveness of esomeprazole.Our goal was to verify the effects of esomeprazole on artificial ulcers in a prospectivestudy.

Methods: A total of 185 patients underwent ESD for gastric neoplasms at ourhospital between January 2013 and June 2015. Among these 185 patients, 49 post-ESD scarlesions were included in this prospective trial. First, 20 mg esomeprazole was orallyadministered to all subjects before and after the procedure. We then evaluated the delayedbleeding rate and ulcer scarring rates at 4 weeks and 8 weeks after the procedure by usinga gastric ulcer stage system.

Results: There was one case of delayed bleeding (2.0%).Regardless of Helicobacter pylori infection status, ulcer scarring ratesat weeks 4 and 8 were respectively 28.6% (14/49) and 98% (48/49).

Conclusions: Our resultssuggest that oral administration of esomeprazole alone may be sufficient for prompthealing of artificial gastric ulcers induced by ESD (UMIN000009367).

Diet therapy is an effective method for treating obesity. However, its effectiveness varies from person to person. With that in mind and working towards the implementation of personalized nutrition education, we investigated the relationship between aspects of oriental medicine and genes related to obesity. We analyzed three genes associated with obesity (ADRB 3, UCP 1, and ADRB 2) and classified them as either wild type or mutant. The presence or absence of gene polymorphisms in the obesity-related genes was analyzed by using PCR-CTPP. We conducted a survey using questions that would be comprehensible to people unfamiliar with oriental medicine in order to assess their body constitution, by diagnosing two elements of body constitution according to oriental medical theory (stomach heat [wei re] and spleen vacuity [pi xu]). The study was conducted in 81 women and 28 men (mean age, 42.6 years ; mean BMI, 22.3 kg/m²). Among them, 7 participants were underweight, 83 were of normal weight, and 19 were overweight. The results of the analysis revealed that BMI was highest in those with stomach heat and lowest in subjects with spleen vacuity. Wild type ADRB 3 increased susceptibility to stomach heat, which led to higher body weight ; whereas, the presence of mutated ADRB 3 increased the likelihood of spleen vacuity, which led to lower body weight. These results suggest that by making comprehensive assessments of body characteristics, it is possible to identify individuals with a predisposition to obesity or low body weight.

[Objective]We performed a questionnaire for the purpose of investigating the present conditions of infection control education in Japanese acupuncture schools and to discuss the problems.
[Methods]We conducted a postal survey by sending out questionnaires to 153acupuncture schools in Japan from December 2008 to March 2009.
[Results]Replies were obtained from 69.9%of the schools (107 out of 153schools). In their responses, 65%of the schools taught hand-washing with soap and alcohol-based hand rub as methods of hand hygiene. Moreover 58%of the schools used only single use needles (SUNs), 40%used both reusable and single use needles, and 2%used only reusable needles. However, single use was observed only at 25%of the schools that introduced SUNs.
Furthermore, we inquired whether the students were taught to use sterilized gloves and finger cots. The results illustrated that a proportion of schools provided the instruction;the proportion of constant instruction was 17%, that of the instructor's option was 35%, that of no requisite was 48%.
[Conclusion]From the above-mentioned results, it became clear that the direction for use of SUNs was not instructed adequately. In addition, this aseptic technique for puncture is not yet standardized in the school's education. It is necessary to realize the measures based on evidence and to employ them in educational programs at acupuncture schools.

In Japan problems of the pediatric emergency-care system have recently increased the needs of consultation to deal with the growing anxiety of parents about how to raise their children and have decreased the number of pediatricians. We believe that one solution is to make pediatric primary care a required part of postgraduate clinical training and thereby increase the ability of nonpediatricians to provide pediatric primary care and to support the care of children. We hope that postgraduate clinical training programs will be reformed so that physicians of all departments can contribute to improving pediatric primary care.

　This report summarizes the Inohana Interprofessional Education （Inohana IPE） program and describes its current achievements and improvement process. Since 2007, a total of 5,679 students have participated in the program. Students who have graduated include 335 from the Faculty of Nursing, 197 from the Faculty of Medicine, 84 from the Faculty of Pharmaceutical Sciences.
　Through Inohana IPE we have created an environment that encourages students’ self-directed learning and the setting of behavioral objectives and have refined the program contents and evaluation method. As the first phase of the improvement, we developed competencies for interprofessional collaboration with 6 components and then created a matrix of learning-achievement goals and 4 learning steps based on the competencies. Furthermore, the number of faculty members participating in the program has increased.
　The two main tasks for the future are to develop an IPE program for nursing and to integrate IPE for basic education and continuing education.

The purpose of this study was to implement a community-based exercise program for postdischarge stroke survivors in the public health center. The subjects were fourteen male stroke survivors with chronic upper and lower limb paresis (63.0±5.9 years) . The study design was quasiexperimental, with each subject serving as his own control. All subjects have to meet following inclusion criteria: (1) first stroke resulting in hemiplegia, (2) at least 6 months post-stroke, (3) able to walk independently, and (4) free from grave aphasia, apraxia, agnosia, or disequilibrium. A 25-week (2 deek) training program consisted of a warm-up, lower and upper extremity muscle strengthening, recreational activities, walking training, and a cool down. The duration of this program is approximately 70 minutes a day. The outcome measure was the 12 functional fitness test items and questionnaire in activities parallel to daily living. Significant improvements (P<0.05) were found for handgrip strength, knee extension strength, tandem balance, standing and sitting, and timed up and go in the treatment period. However, the scores in activities parallel to daily living were not significant. These results suggest that they may attain significant functional improvements in response to implementation of a community-based exercise program. In conclusion, post-discharge stroke survivors can improve their functional fitness status by participating in a community-based exercise program.

During the perioperative period after total hip arthroplasty (THA), much attention has been recently paid to deep venous thrombosis, yet there are few reports concerning stroke after THA and there is no such data at all in Japan at present. The purpose of this retrospective study was to elucidate the occurrence rate and the characteristics of stroke cases during the THA perioperative period. A total of 1,551 primary THAs performed between January 1999 and December 2008 were investigated. Cerebral infarction occurred in three patients (0.19%) during three weeks after THA. Concerning the related factors, one male had foramen ovale, one female had untreated diabetes and atrial fibrillation, and one female had severe stenosis of the internal carotid artery. Cerebral infarction occurred at Day 1 in one male, at Day 2 in one female, and at Day 5 in the other female, and they underwent anticoagulant therapy just after their diagnosis. In all three patients, motor paralysis fully improved and they came back to the THA rehabilitation program within Day 9. One male was discharged at 4 weeks, and another two females were discharged at 8 weeks. Because many people eating a more European diet are now getting older in Japan, prophylaxis for not only DVT but also stroke after THA should be emphasized.
