Chinese Journal of Behavioral Medicine and Brain Science 2012;21(6):564-566
ObjectiveTo explore the theoretical construct of network values and design the network values questionnaire.MethodsThis research was to design the network values questionnaire based on an open-ended inventory survey, overviews of former studies and counseling of experts.Through stratified sampling,832 college students were investigated by the network questionnaire. The data were examined by item analysis, factor analysis, reliability and validity analysis.ResultsThe network values was a multi-dimensional construct which had three factors. The three factors cumulative total explained variance was 50.719%,52.545% and 50.510%.The questionnaire's internal consistency reliability was 0.931 and 0.809-0.874 for the three subscales. The questionnaire's split-half reliability was 0.902 and 0.772-0.864 for the three subscales.ConclusionThe theoretical construct of network values is rational. The questionnaire has good reliability and validity.
2.Effects of arsenic exposure during pregnancy on maternal and neonatal health: a review
Journal of Preventive Medicine 2023;35(7):591-595
Arsenic is a toxic metalloid, which may enter the human body through a variety of routes, including drinking water, food, and air. Previous studies have shown that arsenic exposure during pregnancy may cause pregnancy complications and adverse pregnancy outcomes, and maternal arsenic levels correlate with neonatal neurodevelopment and adult health. However, the negative impact of arsenic exposure during pregnancy on maternal and child health has not been widely accepted because of inconsistent study conclusions and unclear mechanisms. Based on international and national studies pertaining to the effect of arsenic exposure during pregnancy on maternal and child health during the period between 2007 and 2022, this review describes the influencing factors and biomarkers of arsenic exposure during pregnancy, associations of arsenic exposure during pregnancy with pregnancy complications and adverse pregnancy outcomes and impact of arsenic exposure during pregnancy on neonatal neurodevelopment, and discusses the mechanisms underlying negative health effects caused by arsenic exposure during pregnancy, so as to provide the evidence for assessing the hazards of arsenic exposure during pregnancy and formulating the control strategy.

A 53-year-old man presented with prunosus nodules and plaques on the trunk and extremities for half a month and with periorbital swelling for 4 days.Hematological examination in a local hospital showed thrombocytopenia, and pulsed corticosteroid therapy did not work.On physical examination, there were splenomegaly and multiple enlarged superficial lymphnodes.Serum calcium and 1-lactate dehydrogenase (LDH)were increased to 3.12 mmol/L and 853 U/L,respectively.Serum immunofixation electrophoresis evidenced the presence of a monoclonal immunoglobulin IgM (κ chain).Positron emission tomography (PET-CT) showed abnormal uptake in multiple lymph nodes, back wall of the pharynx, and spleen. The biopsy of a nodule in the neck revealed a diffuse infiltration of numerous atypical lymphoid cells in the subcutaneous fat tissue, which were medium-sized with round nuclei, obvious nucleoli and karyokinesis, Immunophenotyping of the abnormal lymphocytes indicated positive reactions for L26,CD79a,Bcl-2,cyclin D1,multiple myeloma oncogene 1 (partly),Ki-67 (>80%),but negative for CD5,CD21,CD23,CD38,CD3,CD10,Bcl-6,CD45RO,terminal deoxynucleotidyl transferase (TdT),myeloperoxidase (MPO),CD30,anaplastic lymphoma kinase (ALK),CD117,or CD34.Fluorescence in situ hybridization (FISH) revealed the presence of a fusion gene (t(11:14) CCND1/IGH) in the abnormal lymphocytes.Based on the above findings, the diagnosis was made as blastoid mantle cell lymphoma with skin and periorbital involvement complicated by hypercalcemia. After treatment with rituximab injection, cyclophosphamide, vincristine, doxorubicin, dexamethasone, and intermittent treatment with intravenous high dose of methotrexate and cytarabine (R-Hyper-CVAD),serum calcium returned to a normal level three days later, and the patient made a quick and excellent recovery on the 6th day, with the regression of skin lesions and poriorbital swelling.Unfortunately, the patient eventually died of severe pulmonary infection one month later.

The clinical data of patients with hypoparathyroidism accompanied by elevated and normal muscle enzymes were analyzed retrospectively. The results showed that there were no differences in age, gender, pathogenesis, etiology, and parathormone level between groups with elevated muscle enzyme and normal muscle enzyme.Serum calcium concentration in the group with normal muscle enzyme was within normal range.Serum calcium concentration in the group with elevated muscle enzyme was significantly lower than that in the group with normal muscle enzyme (P<0.01).As the serum calcium concentration rose, the elevated enzyme level gradually returned to normal range, suggesting that the decrease of serum calcium concentration resulted in the elevated muscle enzyme levels in patients with hypoparathyroidism.

Objective To summarize the experience in diagnosis and treatment of primary splenic neoplasm (PSN) to promote understanding of the disease. Methods The clinical data of all admitted patients with PSN treated in this hospital from February 1989 to February 2004 were retrospectively analyzed. Results A total of 63 patients were treated by surgery and pathologically confirmed as PSN. Twenty-one patients were treated by splenectomy alone. Twenty-seven patients underwent distal pancreatectomy and splenectomy. Fifteen patients received splenectomy combined with excision of tumor, the 5-year survival rate was 35.9%(15/42). Conclusion PSN should be diagnosed mainly according to clinical manifestations, image examination, and fine needle aspiration biopsy. CT and angiography play an important role in the identification of benign and malignant splenic mass. Early diagnosis, radical resection and comprehensive treatment are important for improving the prognosis of PSN.

Objective To explore the expression of transcriptional regulation factor FHL 1 in pancreatic fibrosis of chronic pancreatitis (CP) and clinical significance. Methods Serum samples from 20 CP patients treated in the Department of Gastroenterology , Nanjing General Hospital and 20 healthy controls in the corresponding period were collected .The level of FHL1 in the serum was detected by ELISA .Twenty male Wistar rats were randomly divided into control group and CP group according to the body weight .The CP rat model was established by injecting dibutyltin dichloride ( DBTC) through tail vein .All the rats were killed at 28 days after injection and the pancreas was collected , which was observed using hematoxylin and eosin ( HE) staining to investigate pathological changes . The expression of FHL1 in pancreas was measured by immunohistochemistry (IHC) and Western blotting (WB).Results The average of serum FHL1 level in two groups were (0.86 ±0.22)g/L and (1.18 ±0.22)g/L, respectively. The level of FHL1 in serum of CP patients was significantly lower than that of healthy cases , and the difference was statistically significant (t=10.54, P=0.001).Damaged pancreatic lobular structure , acinar atrophy and the obvious fibrous hyperplasia were observed in CP rats .The pathological score of abnormal structure , acinar atrophy, fibrosis, and inflammatory cell infiltration in the pancreas of the CP group was (2.2 ±0.4),(2.0 ±0.7),(2.4 ±0.3) and (2.8 ±0.6), respectively, but was all 0 in control group. The difference between two group was statistically significant ( t value =17.24, 9.01, 25.22 and 14.75, respectively, all P<0.001).The expression of FHL1 in the pancreatic tissue of CP group were less than that of normal group , and the difference was statistically significant (IHC:15 798 ±3 247 vs 37 570 ±9 254, t=2.438,P=0.016;WB:0.44 ±0.11 vs 0.89 ±0.03, t=4.289, P=0.002).Conclusions The protein of FHL1 was in low expression in the pancreas of CPs , indicating that FHL1 may participate in the formation of pancreatic fibrosis .
