2.Down-regulation of coxsakie and adenovirus receptor during embryo implantation.

In this study, real-time PCR and immunohistochemistry were used to detect coxsakie and adenovirus receptor (CAR) expression. Both localization and quantity were evaluated in the uteri obtained at days post coitus (dpc) 2.5, 4.5, 6.5, 8.5. Outcome of PCR was assessed by 2(-ΔΔCt) method. Image Pro-Plus 6.0 software was used for quantifying mean density of CAR expression in immunohistochemical sections. We found relatively weak CAR expression in the mouse uteri during implantation window. PCR and immunohistochemistry revealed highest CAR expression was detected on dpc 2.5 followed by down-regulation of CAR at dpc 4.5 and 6.5 (with significant difference). At dpc 8.5, CAR expression was increased slightly again. It is concluded that during implantation, the expression of CAR mRNA and protein is declined, resulting in the impairment of tight junction between cavity epithelium cells. After implantation window closure, CAR appears again to maintain epithelium stability. CAR might play an important role during embryo implantation procedure.
3.Analysis on influence factors of medical staff blood-borne occupational exposure based on structural equation modeling

Objective To analyze the influence factors of medical staff blood-bome occupational exposure by structural equation modeling,in order to improve medical staff of blood-borne occupational exposure protection performance of the system to provide the theory basis for operation.Methods The influence factors of medical staff blood-borne occupational exposure and their correlation were analyzed by exploratory factor analysis and structural equation modeling fitting.Results Model fitting was ideal,hospital decision-makers and department management directly affected the behavior intention of medical staff,department management greatly influenced by the behavioral intention to the medical personnel,path coefficient was 0.27.Hospital decision-makers of behavioral intention to the medical personnel directly affect smaller,path coefficient was 0.03,but its indirect impact on behavioral intention by department management.Behavioral intention of occupational exposure protection action,the path coefficient was 0.80,behavioral intention determined the basic medical personnel blood-borne occupational exposure protection behavior.Conclusions Hospital should improve the system of standards and to establish effective communication channels,at the same time enhance the care ability.Enforcement departments should improve the standard system,equipped with adequate safety equipment,for the medical staff to provide the best working environment,so as to improve the medical staff of blood-borne occupational exposure protection behavior compliance.
4.The impact of blood-borne occupational exposure to the medical personnel on individual cognitive

Objective To discuss the influence of individual cognitive on medical personnel blood-borne occupational exposure protection action from the angle of behavior operation.Methods Medical staff of 14 hospitals in zunyi were investigated by questionnaire designed based on the theory of health belief model,and analyzed the data by structural equation model.Results Sample data and the assumption model was ideal,the blood-borne occupational exposure protective behavior of medical staff could be explained variance of 87% by susceptibility,severity,behavioral benefit and barrier cognition.The order of influencing factors from high to low were behavioral benefit,severity,behavioral barrier and susceptibility to cognition,and path coefficients were 0.39,0.27,-0.21,0.03.Conclusions Susceptibility,severity and behavioral benefit cognition have positive effection on protective behavior,the behavioral benefit cognition have more influence on blood-borne occupational exposure protective behavior of medical staff,and behavioral barrier cognition have negative effection,the results of health belief model can explain blood-borne occupational exposure protective behavior of medical staff better.
5.Application of Diana automated blood grouping analyzer in blood grouping and cross matching

Objective To evaluate the accuracy and reliability of Diana automated blood grouping analyzer in blood grouping and cross matching.Methods 2 300 patients′ABO and RhD blood groups were examined by conventional tube test and the fully auto-mated blood grouping analyzer and 900 patients′samples were tested using Diana automated blood grouping for blood cross matc-hing,and it was compared with polymatching method.Results The analyzer′s accuracy rate of blood grouping by two methods were 99.87% and 100.00%.The incompatibility occurred in 30 specimens in automatic blood type instrument,in 3 specimens in manual polymatching method.Conclusion The results of Diana automated blood grouping analyzer used for blood grouping and cross matc-hing blood testing are reliable.Its experimental operation is normalized and standardized with an advantage of low incidence of human er-ror.Moreover,the experimental results can be permanently preserved,which provides a convenience to search for medical proof.

7.Analysis on clinical characteristics of patients with ovarian clear cell carcinoma and ovarian endometriosis

Objective:To discuss the clinical characteristics of the patients with ovarian clear cell carcinoma(OCCC)and ovarian endometriosis(OMA),and to clarify the features of OCCC onset.Methods:A retrospective analysis was conducted on the clinical data of 80 patients with post-operative pathological diagnosis of OCCC and 80 OMA patients diagnosed by post-operative pathology,who received surgical treatment from March 2011 to May 2021.The analysis included general characteristics,clinical manifestations,laboratory indices,and imaging examination indexes.The age,body mass index(BMI),clinical symptoms such as abdominal pain,vaginal bleeding,and other symptoms(bloating,menstrual disorders,constipation,and abnormal vaginal discharge),preoperative serum carbohydrate antigen 125(CA125)levels,and ultrasound characteristics of ovarian cysts,such as cyst size,presence of pelvic effusion,and complexity,were compared between the patients in two groups.Multivariate Logistic regression analysis was used to identify the risk factors for OCCC,and the receiver operating characteristic(ROC)curve was drawn,and the area under the curve(AUC)was calculated.Results:Compared with OMA group,the age of the patients in OCCC group was significantly increased(P<0.05),the serum CA125 level was significantly increased(P<0.05),the diameter of ovarian cyst was significantly increased(P<0.05),and the percentage of the patients with pelvic effusion,other symptoms,and complex cysts was significantly increased(P<0.05).The multivariate Logistic regression analysis results showed that age≥40 years(OR=56.856,95%CI:5.611-576.082,P=0.001),complex ovarian cysts(OR=4.427,95%CI:1.025-19.114,P=0.046),concomitant pelvic effusion(OR=8.760,95%CI:1.574-48.760,P=0.013),and size of ovarian cysts(OR=1.782,95%CI:1.329-2.390,P<0.01)were the risk factors for OCCC.When the cut-off value of the ovarian cyst size was 7.35 cm,the sum of sensitivity(83.75%)and specificity(80.00%)was the highest,and the AUC was 0.883,indicating certain predictive value for identifying OCCC.Conclusion:The patients aged≥40 years with cysts≥7.35 cm in diameter,especially those with complex cysts accompanied by pelvic effusion,have a higher risk of malignancy to OCCC,and active intervention is recommended.
8.Correlation between self-management level and quality of life among 1 796 patients with type 2 diabetes mellitus in community in Sichuan province

Objective To investigate the current situation of self-management level and quality of life among patients with type 2 diabetes mellitus (T2DM) of the urban community in Sichuan province and to explore the correlation between them. Methods Self-management level and quality of life of patients with T2DM from 20 community health services of 12 cities in Sichuan province was investigated with the self-designed diabetes self-management behavior scale and the short form 36 questionnaire (SF-36) by the method of a cross-sectional survey. The correlation between self-management level and quality of life was analyzed. Results A total of 1 796 patients with T2DM were investigated. The total average score of the self-management behavior and the scores of physical health and mental health of the SF-36 were (3.25±0.46) (full marks are 5), (277.3±77.5), (282.7±70.8). The score of the dimension of behavior on complications monitoring had a positive correlation with the score of mental health (r=0.194, P< 0.01). The total average score and scores of dimensions of diet therapy, exercise therapy, medication therapy, blood sugar monitoring and risk factors were positively correlated with the score of physical health (r=0.304, 0.265, 0.441, 0.467, 0.262, 0.361; P<0.05) and the score of mental health (r=0.380, 0.354, 0.364, 0.466, 0.352, 0.406; P< 0.05). Conclusions The self-management level and quality of life among patients with T2DM of the urban community in Sichuan province are not ideal. The better the self-management is, the higher the quality of life is. Communities should improve disease self-management skills and quality of life of patients.
9.A qualitative study on living quality of patients with rectal carcinoma and permanent colostomy

Objective To understand the patients′subjective evaluation of the quality of life, who are suffering from carcinoma and have been operated with permanent colostomy, and analyze relative influencing factors to provide an evidence for improving their quality of life. Methods According to phenomenological research method of qualitative research to choose purposive sampling, twelve patients had been underwent semi-structured interviews. We used Colaizzi′s seven-step analysis methods to analyze the data, inductive and refine the theme. Results There were 7 points which might affect the quality of patients′ life including negative emotion, effects of daily life, persecution of complications, sexual dysfunction, degradation of social roles, lack of relevant knowledge of disease and economic pressures. Conclusions We should pay much more attention on patients′ emotion reaction, provide psychological support and more stoma-related information, and take full advantage of enterostomal therapist and call on improving the medical security system by government in order to enhance patients′quality of life.

Objective To monitor the antifungal resistance of clinical isolates of Candida spp.in the East China region.Methods MALDI-TOF MS or molecular methods were used to re-identify the strains collected from January 2018 to December 2022.Antifungal susceptibility testing was performed using the broth microdilution method.The susceptibility test results were interpreted according to the breakpoints of 2022 Clinical and Laboratory Standards Institute(CLSI)documents M27 M44s-Ed3 and M57s-Ed4.Results A total of 3 026 strains of Candida were collected,65.33%of which were isolated from sterile body sites,mainly from blood(38.86%)and pleural effusion/ascites(10.21%).The predominant species of Candida were Candida albicans(44.51%),followed by Candida parapsilosis complex(19.46%),Candida tropicalis(13.98%),Candida glabrata(10.34%),and other Candida species(0.79%).Candida albicans showed overall high susceptibility rates to the 10 antifungal drugs tested(the lowest rate being 93.62%).Only 2.97%of the strains showed dose-dependent susceptibility(SDD)to fluconazole.Candida parapsilosis complex had a SDD rate of 2.61%and a resistance rate of 9.42%to fluconazole,and susceptibility rates above 90%to other drugs.Candida glabrata had a SDD rate of 92.01%and a resistance rate of 7.99%to fluconazole,resistance rates of 32.27%and 48.24%to posaconazole and voriconazole non-wild-type strains(NWT),respectively,and susceptibility rates above 90%to other drugs.Candida tropicalis had resistance rates of 29.55%and 26.24%to fluconazole and voriconazole,respectively,resistance rates of 76.60%and 21.99%to posaconazole and echinocandins non-wild-type strains(NWT),and a resistance rate of 2.36%to echinocandins.Conclusions The prevalence and species distribution of Candida spp.in the East China region are consistent with previous domestic and international reports.Candida glabrata exhibits certain degree of resistance to fluconazole,while Candida tropicalis demonstrates higher resistance to triazole drugs.Additionally,echinocandins resistance has emerged in Candida albicans,Candida glabrata,Candida tropicalis,and Candida parapsilosis.
