1.Medication Status of Elder People in Community

OBJECTIVE: To probe into the medication status of the elder patients in community so as to provide reference for pharmacists to promote rational medication for the elderly people in community. METHODS: The medication status of 484 cases in the community was investigated through questionnaire and interview. RESULTS: The principal diseases affecting the health of the elder people included hypertension, diabetes mellitus, coronary heart disease, cerebrovascular disease. The average number of drugs they used totaled 2.92 kinds; the higher the educational background, the higher proportion of the elder people who read drug package instruction but the lower ratio of medication compliance. The elder people commonly had poor knowledge about the drugs and adverse drug reactions(91.74% and 60.74%);21.11% responded that they had ever received medication guidance from pharmacists, only 3.10% responded that they had once consulted pharmacists about medication problems. CONCLUSION: The medication of the elder people in community is characterized by multiple drug varieties, poor ADR knowledge and lower cognition on pharmacists. It is urgent to improve the pharmaceutical care ability of medical institution and enhance the social influence of pharmacists.
2.High-energy shockwave for the chronic pain of muscle and bone joint

BACKGROUND: At present, there are some reports regarding the treatment of high-energy shockwave to chronic pain of muscle and born joint, however,the therapeutic effects and mechanism are still uncertain and need further discussion.OBJECTIVE: To explore the curative effects, mechanism and clinical application of high energy shockwave to bone joint myofascitis.DESIGN: Non-randomized case controlled study based on diagnosis SETTING: Rehabilitation Medicine Department of First Affiliated Hospital of Sun Yat-sen University.PARTICIPANTS: Totally 90 patients with chronic pain of muscle and bone joint treated in Rehabilitation Medicine Department of First Affiliated Hospital of Sun Yat-sen University from July 2001 to April 2002 were divided into treatment group and control group each with 45 cases by the order of visiting time. There were 17 males and 28 females in the treatment group with mean age of 54 years and 15 males and 30 females in the control group with mean age of 63 years.METHODS: Shockwave therapy was applied to the treatment group while routine physical treatment was conducted to the control group. Simple McGill pain questionnaire(MPQ) was used to evaluate the general reactions of patients to pain and assess the shoulder joint territory and clinical effects.therapeutic effects.RESULTS: There was significant difference on sensory, affective and pain score, visual analogous scale(VAS) and pain presentation inventory(PPI) before and after treatment in the treatment group( t =5.69, 5.67, 7.06, 8.37,6. 21, P < 0.01 ). When compared with control group, there was significant difference on sensory, pain score, VAS between the treatment group and control group(t =4. 66, P < 0.01; t =2.52,3.40, P <0.05).CONCLUSION: Extracorporeal high-energy shockwave has reliable effects on treating chronic pain caused by bone joint myofascitis, which is characterized by its effective, quick and safe features.
3.IgG4-related kidney disease presenting as a cystic renal mass: a case report and literature review

Objective:To investigate the clinical characteristics, differential diagnosis and treatment of IgG4-related kidney disease (IgG4-RKD) presenting as a cystic renal mass.Methods:A 42-year-old male patient was found having a tumor in the left kidney in an annual physical examination. Subsequent CTU scan revealed a round shadow in the lower pole of the left kidney, with slightly lower, uneven density and obscure boundaries, which was slightly enhanced on contrast-enhanced CT imaging, suggesting a cystic mass (Bosniak category Ⅲ). Contrast-enhanced MRI showed a long T1, long T2 signal lesion in the lower pole of the left kidney, which was not obviously enhanced in the cortical phase but exhibited continuous and separated enhancement in parenchymal and delayed phases, suggesting a cystic renal cell carcinoma. A preoperative diagnosis of a left renal mass was made, for which the patient underwent a laparoscopic partial nephrectomy.Results:The resected cystic mass was grey white with obscured boundaries. Microscopically, the mass showed lymphoplasmacytic hyperplasia with lymphoid follicles, cystic formation, fibrosis hyperplasia and vascular obstruction. Immunohistochemistry revealed that the number of IgG4 (+ ) plasma cells was more than 50/HPF, and over 40% IgG (+ ) plasma cells were IgG4 (+ ). By virtue of the radiologic and pathologic findings, the patient was diagnosed with IgG4-RKD. At 2 weeks after operation, serum IgG level was 0.71g/L (reference: 0.03-2.01 g/L). Chest and abdominal CT scans performed 3 months after operation showed no evidence of other organ involvement and serum IgG4 level was within the normal range. Therefore no adjuvant therapy was prescribed.Conclusions:It is usually difficult to differentiate IgG4-RKD presenting as a cystic renal mass by radiography alone. The presence of other organ involvement, serum IgG4 test and renal biopsy can help the diagnosis and so as to avoid unnecessary surgery.

This study was to assess the chronic morbidity and metabolic disordelters in 262 patients with type 2 diabetes.Of all participants,64(24.4%)coexisted with peripheral neuropathy,34(13.0%)combined with peripheral vascular disease.41(15.6%)were diagnosed as diabetic retinopathy,and 46 (17.6%)had concurrent diabetic nephropathy.In comparison with diabetic patients without these complications,those with the chronic conditions generally had higher plasma glucose,blood pressure or body mass index.
5.Primary renal synovial sarcoma: a case report

Primary prostate synovial sarcoma (PPSS) is rare in clinic. One patient was admitted to our hospital in May 2021. The patient was admitted to the hospital because of the physical examination. Preoperative pelvic enhanced MR, PETCT and preoperative puncture pathology suggested that pelvic soft tissue sarcoma was likely. Robot-assisted radical resection of pelvic tumor was performed, and the unilateral PPSS was diagnosed by postoperative pathology, immunohistochemistry and gene detection. Patients were treated with ifosfamide + adriamycin adjuvant chemotherapy one month after operation, and Proton therapy radiotherapy five months after operation. Follow-up for more than 2 years showed that the patients were generally in good condition, and no recurrence or metastasis was found in imaging.
6.Treatment progress of cirrhotic portal hypertension

Cirrhotic portal hypertension (CPH) is a manifestation of decompensated liver cirrhosis, with ascites, portal collateral circulation formation, hypersplenism and splenomegaly as the typical clinical symptoms. In recent years, the incidence of CPH has been increasing year by year, and the treatment of CPH has gradually become a hot issue in medical research. In order to further explore the diagnosis and treatment scheme of CPH. We briefly describe the pathophysiological mechanism and diagnosis of CPH, and the current situation of CPH treatment and the new progress of internal and external treatment were reviewed.
