1.Practical analysis of hemostatic compression after high pressure injection of magnetic resonance contrast in patients with acute cerebral infarction

Objective:In the research, the traditional mercury sphygmomanometer was used to press the puncture point to evaluate the hemostatic effect.Methods:Eighty patients with acute cerebral infarction diagnosed clinically in Xuanwu Hospital of Capital Medical University from June 2018 to December 2019 were examined by magnetic resonance perfusion examination. The serial number of the patient was randomly divided into the group by random digital table, and the puncture point was pressed by mercury sphygmomanometer after needle extraction in the auxiliary pressing group, and the individual systolic blood pressure intensity was set as the pressing intensity, while the control group adopted independent vertical pressing technique. After examination, the puncture points of the two groups were qualitatively evaluated, such as secondary bleeding, hematoma, ecchymosis, normal and so on.Results:In the auxiliary compression group, there were 3 cases of secondary hemorrhage, 1 case of hematoma, 0 cases of ecchymosis and 36 cases of normal, while in the control group, there were 4 cases of bleeding, 2 case of hematoma, 1 case of ecchymosis and 33 cases of normal ( χ2value was 4.372-34.225, P<0.05). Conclusion:Traditional mercury sphygmomanometer-assisted pressing puncture point can make personalized nursing hemostasis plan for patients with acute cerebral infarction, reduce the phenomenon of puncture point bleeding, ecchymosis, and even subcutaneous hematoma caused by unstable pressing and unequal force, and practice high-quality nursing service.
2.Treating Parkinson's disease with high and low frequency repetitive transcranial magnetic stimulation: a clinical study

Objective To investigate the therapeutic effects of repeated transcranial magnetic stimulation (rTMS) at high and low frequency for the rigidity and tremors of Parkinson's disease (PD).Methods Sixty-nine patients with PD received rTMS at 5 Hz or 0.5 Hz.The efficacy was assessed using the unified Parkinson's disease rating scale (UPDRS),motor function tests and motor evoked potentials (MEPs).Results Thirty-two patients with rigidity from PD received 5 Hz rTMS therapy,and their UPDRS total scores decreased significantly.Their motor function test results improved significantly and their MEP relaxed motor thresholds were significantly elevated.Thirtyseven patients with tremors from PD received 0.5 Hz rTMS therapy,and their UPDRS total scores were significantly decreased.Their motor function test results improved significantly and their MEP relaxed motor thresholds were significantly elevated.Conclusion High frequency rTMS may improve the symptoms of patients with PD rigidity and low frequency rTMS may improve the symptoms of patients with PD tremors.
3.A study on CD90, IGF1R expression in hepatocellular carcinoma

Objective To investigate the expression of CD90,IGF1R in hepatocellular carcinoma.Methods CD90,IGF1R expression were detected by SP immunohistochemical staining in 36 cases of hepatocellular carcinoma,20 cases of normal liver tissue biopsied from patients of chronic cholecystitis undergoing cholecystectomy. Results The positive rate of CD90 (63.89%,23/36),IGF1R (52.78%,19/36) in hepatocellular carcinoma significantly increased (P < 0.05 ); The positive rate of CD90 was higher in UICC Ⅲ - Ⅳ stage group (79.17% ) than in UICC stage Ⅰ - Ⅱ group (33.33%,P <0.05) ;The positive rate of CD90 was higher in low differentiated group (76.92% ) than in well-differentiated group (56.52%,P <0.05).The positive rate of IGF1R was higher in UICC Ⅲ - Ⅳ stage group (70.83% ) than in UICC stage Ⅰ - Ⅱ group ( 16.67%,P < 0.05 ). The positive rate of IGF1R was higher in low differentiated group (84.62%) than in well-differentiated group (37.38%,P < 0.05 ).The expression of IGF1R was positively correlated with that of CD90 (P < 0.05 ).The median survival of CD90+ patients (85 days) was shorter than CD90- patients ( 505 days ) ( P < 0.05 ).The median survival of IGF1 R + patients (100 days) was shorter than IGF1 R- patients (408 days,P < 0.05). Conclusions CD90 or IGF1R expression correlates positively with the progression of HCC.
4.Expression of interleukin-33 and vascular endothelial growth factor-C in gastric cancer and its clinical significance

Objective To investigate the expression of interleukin-33(IL-33)and vascular endothelial growth factor C(VEGF-C)in gastric cancer tissues and serum,and to explore the relationship between these two indicators and gastric cancer lymph node metastasis.Methods The levels of IL-33 and VEGF-C in the tissues of gastric mucosa and serum were detected by immunohistochemical SP method and enzyme-linked immunosorbent assay(ELISA)in 98 patients with gastric cancer and 36 healthy subjects.Results The expression rates of IL-33 and VEGF-C in gastric cancer were 67.35%and 74.49%,which were significantly higher than the rates in normal gastric tissue(47.22%and 61.11%).The difference was statistically significant(P<0.01).The expression of IL-33 and VEGF-C was correlated with the degree of tumor differentiation,tissue infiltration,lymph node metastasis,distant metastasis and clinical stage(P<0.05).The positive rates of IL-33 and VEGF-C in gastric cancer lymph node metastasis group were higher than those in non-lymph node metastasis group(P<0.05).The serum concentrations of IL-33 and VEGF-C in patients with gastric cancer were(50.24±13.08)pg/mL and(210.73±58.35)pg/mL,respectively,which were higher than those in healthy control group(P<0.05);the expressions of serum concentration of IL-33 and VEGF-C in the cases with lymph node metastasis were higher than those without lymph node metastasis and the difference was statistically significant(P<0.05).Conclusion High levels of IL-33 in gastric carcinoma patients might induce the secretion of VEGF-C,promote lymph node metastasis,and be applied as an important index of the appraisal to the prognosis of gastric cancer.
5.High-frequency repetitive transcranial magnetic stimulation combined with neuromuscular electrical stimulation for the treatment of poststroke dysphagia: a randomized controlled trial

Objective To assess the therapeutic effect of high-frequency repetitive transcranial magnetic stimulation (rTMS) combined with neuromuscular electrical stimulation (NMES) on poststroke dysphagia.Methods A total of 90 patients with poststroke dysphagia were enrolled.They were randomly divided into either a NMES + rTMS group or a NMES + sham rTMS group (n =45 in each group).The Kuhota water drinking test and Standardized Swallowing Assessment (SSA) were used to assess the swallowing function.Results The scores of Kuhota water drinking test (F=82.001,P<0.001) and the SSA (F =33.743,P <0.001) before treatment,treatment of one course,treatment of two courses,and at 3 months after treatment in the NMES + rTMS group had significant differences.Compared with before treatment,they were improved significantly for treatment of one course (P <0.01 and P <0.05,respectively),two courses (all P<0.01),and at 3 months (all P<0.01) after treatment.The scores of Kuhota water drinking test (F =53.647,P<0.001) and the SSA (F=19.178,P<0.001) in the NMES + sham rTMS group also had significant difference.Compared with before treatment,they had significant improvement for treatment of one course (all P <0.05),two courses (P <0.05 and P <0.01,respectively) and at 3 months (all P<0.01)after treatment.The scores of Kuhota water drinking test for treatment of one course,two courses,and at 3 months after treatment (treatment of one course:t=2.217,P=0.02;treatment of two courses:t =2.406,P =0.019;at 3 months after treatment:t =2.128,P =0.037) and the SSA (treatment of one course:t =2.196,P =0.030,treatment of two courses:t =2.425,P =0.016;at 3 months after treatment:t =2.512,P=0.013) in the NMES + rTMS group were significantly better than those in the NMES + sham rTMS group.Conclusions High-frequency rTMS combined with NMES may significantly improve the swallowing function in patients with stroke.Its efficacy is superior to NMES.
6.Comparative analysis of immunological characteristics of patients with mild and severe influenza A (H1 N1)

Objective To comparatively analyze the immunological characteristics of patients with mild and severe influenza A (H1N1), and to provide the evidence for condition monitoring and treatment . Methods 52 cases with mild influenza A ( H1N1), 152 cases with severe influenza A ( H1N1) and 26 healthy subjects from July 1, 2009 to December 31, 2009 were enrolled in the study.Lymphocyte subsets in peripheral blood were analyzed by flow cytometry and the serum concentrations of interferon -γ( IFN-γ) and interleukin-4 (IL-4) were detected by enzyme-linked immune-sorbent assay (ELISA).Results The total lymphocyte counts were decreased obviously in patients with severe influenza A ( H1N1) than in mild pa-tients and in healthy subjects (P<0.01).The T lymphocyte, NK cells, CD4+T lymphocyte and CD8+T lym-phocyte were also decreased obviously in severe patients than in mild patients (P<0.01).The B lymphocyte and CD4+/CD8+were also decreased in severe patients than in mild patients but had no significant statistical difference (P=0.11, 0.175).The serum IFN-γlevels in patients with mild and severe influenza A (H1N1) were lower than those in control group, especially in patients with severe influenza A (H1N1) (compared with control group and mild group , P<0.01).And the changes of serum IL-4 levels were the same with the former, but there were no statistically significant differences in three groups (P>0.05).Con-clusion Immune dysfunction in patients with influenza A (H1N1) infection is associated with the severity of disease, especially cellular immunity .Therefore, monitoring of the immune system is valuable for the diag-nosis of influenza A(H1N1) infection.
7.Preparation and characteristics analysis of 5-carboxy fluorescein-dextran-coated superparamagnetic iron oxide nanoparticle

Objective To investigate the physical and magnetic properties and cytotoxity of 5-FAM-dextran-coated superparamagnetic iron oxide nanoparticles ( 5-FAM-dextran-Fe3 O4 ) , and to observe the cell-labeling character of these nanoparticles. Methods 5-FAM-dextran-Fe3 O4 were prepared by ultra-sonic and chemical coprecipitation method and the characteristics were evaluated. The size and distribution of 5-FAM-dextran-Fe3 O4 were measured by transmission electron microscope ( TEM) and Malvern Zetasizer. The organic structure of the coating was characterized by fourier translation infrared spectroscopy. The optical imaging ability was measured by ultraviolet visible spectrometer and the susceptibility was measured by vi-brating sample magnetometer. In vitro cytotoxities of 5-FAM-dextran-Fe3 O4 , dextran-Fe3 O4 and Fe3 O4 were detected by MTT assay. The area of labeled neuronal cells was observed by confocal microscopy after incuba-ted with nanoparticles under different magnetic intensities. One-way analysis of variance was used. Results The size of 5-FAM-dextran-Fe3 O4 was homogeneous under TEM, and the diameter ranged from 15 to 25 nm ( average=22 nm) by Malvern Zetasizer. The organic structure of the coating of Fe3 O4 was confirmed by fou-rier translation infrared spectroscopy. Ultraviolet visible spectrometer observation showed that the nanoparti-cles expressed unanimous green fluorescence under ultraviolet activation. The saturation magnetization, residu-al magnetization and coercivity of the nanoparticles by magnetometer detection were 86. 02 A · m2 · kg-1 , 15. 05 A·m2 ·kg-1 and 5414.01 A/m respectively, showing superparamagnetic character. MTT assay re-sults showed that 5-FAM-dextran-Fe3 O4 had no obvious cytotoxicity. The confocal microscopy observation in-dicated that the cell-labeled area reached the maximum under the magnetic field intensity of 500 mT ((0. 880±0.146) mm2, F=320.298, P<0.05). Conclusions 5-FAM-dextran-Fe3O4 prepared by ultra-sonic coprecipitation method have the advantages of small size, good homogeneity and magnetic property. Therefore, they might be used as the fluorescent magnetic bio-probe in laboratory and clinical studies.
8.The Application of Ventilator Lengthening Tube in Hyperbaric Oxygen Therapy for Patients with Tracheotomy

Objective To Observe and evaluate the effect of hyperbaric oxygen therapy for patients with tracheotomy using ventilator lengthening tube. Methods 75 patients with tracheotomy in our hospital from January 2017 to January 2018 were divided into the study group (n = 38 cases) and the control group (n = 37 cases) according to the random number method. The control group used conventional oxygen inhalation while the study group used the ventilator tube after prolonged special oxygen hyperbaric oxygen pipe three (built-in ventilation pipe check valve) and bellows connecting an oxygen supply device, compared two groups of patients with oxygen inhalation methods suction phlegm oxygen concentration times and cabin, every time when treating. Results The rate of oxygen inhalation in the study group was 97. 37% (37/38) higher than that of the control group (81. 08% (30/37)), and the difference was statistically significant (P<0. 05). The number of sputum sucking in the study group was (1. 02 ± 0. 36) times less than that of the control group (2. 32 ± 0. 53), and the difference was statistically significant (P<0. 05). The total time of hospitalization in the study group was (16. 4 ± 2. 4) d, and the total time of hospitalization in the control group was (21. 7 ± 3. 2) d, the difference was statistically significant(WTBX〗P<0. 05). The score of GCS in the study group was higher than that of the control group, and the difference was statistically significant (P<0. 01). Conclusion The ventilator extension tube for hyperbaric oxygen therapy in patients with tracheotomy improves the oxygen concentration, ensures the curative effect, reduces the oxygen concentration in the cabin and reduces security risks; the operation method is safe, simple and practical and convenient for clinical application.

Objective:To better promote the standardization of public health management in China, and provide evidence for the development and improvement of the standardization strategy and management system in public health field in China.Methods:This paper summarizes and analyzes the information about the standardized management mechanism collected from international organizations related with standardization in public health.Results:The standards in public health varied in different management systems of the international organizations, and there were great differences in organization nature, standard types, application, release, organization structure, standard development principles, advantages, transformation, promotion and implementation, and evaluation.Conclusion:China can benefit from the studying of the working mechanism of the international organization related with standardization in public health to facilitate its own standardization in public health.
10. Clinical application of STR genotyping diagnosis for hydatidiform mole and nonmolar gestation

Objective:
To investigate the value of short tandem repeat (STR) genotyping in the diagnostic workup of molar and non-molar gestations with correlation of histological characteristics.
Methods:
Six hundred and fifty-six cases were selected based on clinically suspected hydropic abortion and/or molar pregnancy from July 2015 to September 2017 at Beijing Obstetrics and Gynecology Hospital. DNA was extracted from dissected chorionic villi and paired maternal endometrial FFPE tissue samples by Simplex OUP™ FFPE DNA Tissue Kit. STR genotyping was performed by PowerPlex 16 HS system.
Results:
DNA genotyping was informative in 649 of 656 cases, leading to identification of 215 hydatidiform mole gestations and 434 non-molar gestations. Most of non-molar gestations (375 cases, 86.4%) were diploid hydropic abortion. Various trisomy syndromes were found (53 cases, 12.2%), including trisomy 2, 3, 4, 7, 8, 13, 16 and 21. Only 2(0.5%) digynic triploid gestations were detected. Moreover, 4 cases (0.9%) of uniparental disomies (homologous or heterologous) were found. There were 196 cases with histologic diagnostic suspicious of hydatidiform moles were accurate sub-classified. Among them, 59 cases hydatidiform moles were under-diagnosed as diploid hydropic abortions, and 28 cases diploid hydropic abortions were over-diagnosed as hydatidiform moles.Compared with partial moles(PHM), there were no specific histomorphological features between the various types of non-molar gestations and partial moles for definitive diagnostic separation. There was no significant difference in the expression of p57kip2 among PHM, trisomy and diploid hydropic abortions group (
