1.TRIPTERYGIUM WIIFORDII AND STEROID THERAPY FOR RELAPSING POLYCHONDRITIS

Aim: To report 3 cases of relapsing polychondritis admitted within a month, and to explore the therapeutic programme. Methods: Based on therapeutic effect of traditional immuno suppressant and their side effects, the programme was designed as follows: During acute episode, 60~90mg of tripterygium glucosides and 30~60mg of prednisone were medicated orally every day and 1ml(5mg) of diprospan was injected into the painful cortilaginous areas. After the symptoms subsided, the daily dosages of oral drug were gradually decreased to 30~60mg of tripterygium glucosides and 10~20mg of prednisone for four months' course. Results: All the 3 cases′ symptoms were obviously subsided within a half month. All of them were followed up for 22 months. hyperemia, swelling or pain were not fou and in all suffering areas. One case remained a mild cauliflower ear and another case a mild saddle nose. Conclusions: Otorhinolaryngologists need to improve the capabillity of detecting the relapsing polychondritis, in order to make the diagnosis and treatment as early and correctly as possible. Tripterygium wiifordii and steroid have a good therapeutic effect for relapsing polychondritis.
2.STUDY OF ABERRANT METHYLATION OF p16 GENE IN PATIENTS WITH COLORECTAL CANCER

To explore whether aberrant methylation in the promoter of p16 gene was associated with development and clinicopathological characteristics of colorectal cancer. Methylation-specific PCR(MSP) was used to detect hypermethylation of p16 gene in tumor tissues obtained from 32 patients with colorectal cancer. The results showed that the hypermethylation of p16 promoter was detected in 40.6% of tumor tissues. p16 hypermethylation in patients with Dukes stages of C and D tumors (63%) was higher than that in the stages of A and B tumors (25%). The highly and intermediately differentiated carcinomas had lower positive rate (30%) than the poorly differentiated carcinoma. 3.Human immunodeficiency virus vulnerabilities and related health risk behaviors of out-of-school children of the floating population in Beijing

Objective:To explore the vulnerability and related risk behaviors of the out-of-school adolescents floating with their parents from country to city.Methods:From September to October in 2005,260 out-of-school adolescents who were 14 to 20 years old were investigated in our study.Results:The out-of-school adolescents lacked of HIV/AIDS knowledge and life skills,and their psychosocial competence was at a lower level.The adolescents were open-minded to sex,but they were not conscious of using condoms.Attitude towards people with HIV was positively correlated with a well-being family,harmonious relation between their parents,a stable job,knowledge,life skills and psychosocial competence(such as in control of emotions,empathy).Their life skills were positively correlated with their mothers'schooling satisfaction with family income,a stable job,high scores of knowledge,social responsibility,and interpersonal relationship.Competence of HIV/AIDS prevention was positively correlated with schooling,a stable job,knowledge,life skills,and psychosocial competence.Conclusion:Out-of-school adolescents were vulnerable to HIV/AIDS.Individual level intervention combined with structural intervention could effectively reduce their vulnerability.Effective and feasible health education approach to enhance their life skills and psychosocial competence was a key point to health education on HIV/AIDS prevention.
4.Regulation of Th1 and Th2 Type Cytokines in the NCC Patients Treated with Praziquantel

Objective To explore immunological mark and the outcome of disease after pharmacological treatment in human NCC with praziquantel. Methods 35 patients were treated with praziquantel for 6 months. Levels of serum IL-2,IFN-γ, IL-12, IL-5, IL-10 and TNF-α were measured before and after treatment. Results Th1 type cytokines IL-2, IFN-γand IL-12 were up regulated after treatment( P < 0. 01 ) . Levels of TNF-α and Th2 type cytokines IL-5 and IL-10 were remarkable decreased after treatment( P < 0. 01 ). The levels of IL-2, IFN-γ and IL-12 from the patients with good response to the treatment is higher than those with no response to the treatment. Conclusion Th1 type cytokines were up regulated while Th2 type cytokines were down regulated in the NCC patients treated with praziquantel. The protective immunity may be related to the Thl cell activation.
5.The community health diagnosis of life-way disease in Peking University Health Science Center

Objective: To study the basic information of life way disease and the corresponding risk factors of behavior in the community of Peking University Health Science Center, understand the background issue and social support system, and analyze the requirement for community health service so that the critical issue for health can be dealt with and the comprehensive program of prevention and treatment of the disease can be accordingly supplied. Methods: The random face to face questionnaire about life way disease and risk factors in 1 051 residents over 6 years of age in the community of Peking University Health Science Center was conducted according to the typical sampling principle in 1 primary school, 1 middle school, 1 university and its community, and the investigations of health data from the hospital and the police office in the community were also conducted through discussing about life ways risk factors. Accordingly , the data obtained by way of EXCEL were analyzed and processed using SPSS 11.5. Results: Diagnostic data of demography, epidemiology, behavior environment, education and organization were obtained concerning the public health of the Peking University Health Science Center community. Conclusion: The key health issue in the Peking University Health Science Center community is significantly influenced by the bad way of life. And some behavior risk factors, such as fatness, lack of outdoor exercise in the community members. Thus, the major necessary health service of Peking University Health Science Center residents is to carry out a comprehensive management program of life way disease for the whole people.
6.Removal of large acoustic neuromas by enlarged translabyrinthine approach

Objective: To investigate the surgical methods and outcomes of the enlarged translabyrinthine approach in the removal of large acoustic neuromas. Methods: A large mastoidectomy involved complete exposure of the sigmoid sinus, the dura behind the sinus for at least 1 cm, the superior petrosal sinus and the middle fossa dura. The jugular bulb was exposed and pressed downwards if necessary. The internal auditory meatus was skeletonized and uncovered for at least 270°.The debulking of the tumor began inside the anterior and inferior poles in order to find the brainstem and the facial nerve root as early as possible, and then the dissection of the nerve was done medially to laterally. Intraoperative facial nerve monitoring and postoperative CT and MRI were done in all cases. Results: Total removal was achieved in all 18 patients with tumors larger than 3 cm (mean size: 4.2 cm). There were no deaths or other complications such as intracranial infection and persistent cerebrospinal fluid leakage. There were no obvious cerebral sequelae. The facial nerve was preserved both anatomically and functionally in 14 cases, with Grade Ⅰ or Ⅱ in 8 cases, Grade Ⅲ or Ⅳ in 6 cases. Nerve interruption occurred in 4 patients who all had severe facial palsy or nerve interruption before operation. Sixteen patients resumed work within 1-3 months. Conclusion: Total removal of large acoustic neuroma could be acomplished via the translabyrinthine approach, with good preservation of facial nerve function and minimum incidence of morbidity.
7.Clinical value of carbon nanoparticle in gastrectomy of stomach neoplasms

Objective To evaluate the clinical application value of local injections of carbon nanoparticle in gastrectomy of stomach neoplasms.Methods Forty cases of stomach neoplasms with local injections of carbon nanoparticle in gastrectomy of stomach neoplasms were as experimental group,and 40 cases of stomach neoplasms with conventional gastrectomy of stomach neoplasms were as control group.The number of removed tiny lymph node and transferred tiny lymph node was compared.Results There was significant difference in the average number of removed tiny lymph node N1,N2,N3 between experimental group and control group (5.120 ± 0.455 vs.2.900 ± 0.245,3.890 ± 0.367 vs.1.750 ± 0.256,1.790 ± 0.224 vs.0.590 ± 0.054)(P < 0.01).There was significant difference in the average number of transferred tiny lymph node N1,N2,N3 between experimental group and control group (1.090 ±0.087 vs.0.430 ± 0.044,0.550 ± 0.052 vs.0.340 ± 0.027,0.410 ± 0.044 vs.0.130 ± 0.013)(P < 0.05).There was no significant difference in postoperative complications,mortality and length of hospital stay(P> 0.05).Conclusion Carbon nanoparticle in gastrectomy of stomach neoplasms is a simple,safe,easy method,and it has certain supplementary role in conventional gastrectomy of stomach neoplasms.
8.Research progress in appropriate technologies of traditional Chinese medicine in the prevention and treatment of bedridden patients with urinary retention in traumatic orthopedics

Urinary retention is a common complication of bedridden patients in traumatic orthopedics. The severe condition can even cause permanent bladder injury and renal failure, which brings great pain and psychological pressure to patients and seriously hinders their rehabilitation. The traditional Chinese medicine appropriate technologies have the characteristics of safety, effectiveness, low cost, simplicity and easy learning and achieve good clinical effects in the prevention and treatment of bedridden patients with urinary retention in traumatic orthopaedics, including Chinese herbal medicine, acupuncture and moxibustion, massage, etc. The authors summarize the research progress in appropriate technologies of traditional Chinese medicine in the prevention and treatment of bedridden patients with urinary retention in traumatic orthopedics from aspects of action principles, operation methods and effects of acupuncture points, moxibustion therapy, manipulation therapy, external treatment of traditional Chinese medicine and acupoint injection, so as to provide a reference for further nursing research and clinical application.
9.Establishment of HPLC chromatogram and content determination of 8 nucleoside components of Aspongopus

Objective:To establish HPLC chromatogram for Aspongopus; To determine 8 nucleoside components of uracil, adenine, uridine, uric acid, hypoxanthine, adenosine, xanthine and canine quinolinic acid; To provide reference for quality control and evaluation.Methods:The Agilent ZORBAX SB-Aq chromatographic column (4.6 mm×250 mm, 5 μm) was used for gradient elution with mobile phases consisting of a methanol (A) and 0.05% phosphoric acid (B). The column temperature was 25 ℃, the flow rate was 0.8 ml/min, and the detection wavelength was 254 nm. HPLC chromatograms for Aspongopus were established and the contents of 8 components were determined.Results:The characteristic chromatogram of 15 batches aspongopus herbs was established. A total of 10 common characteristic peaks were identified and 8 were identified. The similarity between the characteristic chromatogram of samples and the control chromatogram was 0.969-0.997. The content determination showed that the linear range of uracil, adenine, urin, uric acid, hypoxanthine, adenosine, xanthine and xanuric acid was among 0.002 0-0.644 0, 0.001 4-0.448 0, 0.001 0-1.257 0, 0.005 4-6.221 0, 0.001 0-0.724 0, 0.001 0-0.644 0, 0.002 0-1.113 0, 0.003 8-2.059 0 μg, respectively, with a good linear relationship ( r≥0.999); the repeatability and stability of RSD were <2.0%, and the average sampling recovery rate was between 99.36% and 103.40%. Conclusion:The characteristic chromatogram and content determination method established in this study are simple, reliable, reproducible and accurate, and can be used for the qualitative and quantitative analysis of Aspongopus and can provide a reference for the quality evaluation method of the Aspongopus.
10.Correlation between serum N-terminal pro-B-type natriuretic peptide level and echocardiographic parameters in patients with chronic Keshan disease

Objective:To study the correlation between serum N-terminal pro-B-type natriuretic peptide (NT-proBNP) level and echocardiographic parameters in patients with chronic Keshan disease, providing reference for diagnosis and prognosis evaluation of chronic Keshan disease.Methods:Ninety-nine patients with chronic Keshan disease who received standardized treatment at Jingchuan County People's Hospital in Pingliang City, Gansu Province from January to December 2020 were selected. Among them, 16 patients were classified as cardiac function grade Ⅱ according to New York Heart Association (NYHA), 69 as grade Ⅲ and 14 as grade Ⅳ. The patients underwent echocardiography and their serum NT-proBNP level was measured using fluorescence immunochromatography. The differences in serum NT-proBNP levels among patients with different cardiac function grades were compared, and the correlation between cardiac function grades, serum NT-proBNP level and echocardiographic parameters was analyzed.Results:The serum NT-proBNP levels in patients with cardiac function grades Ⅱ, Ⅲ, and Ⅳ were (1 107.26 ± 268.03), (2 125.98 ± 293.02), and (8 268.59 ± 2 659.50) pg/ml, respectively. The differences among the three groups were statistically significant ( F = 13.94, P < 0.001). The serum NT-proBNP level was positively correlated with cardiac function grades ( r = 0.44, P < 0.001), left ventricular end-diastolic diameter, left ventricular end-systolic diameter, and left atrial diameter ( r = 0.45, 0.52, 0.38, P < 0.001), and negatively correlated with fractional shortening and left ventricular ejection fraction ( r = - 0.39, - 0.46, P < 0.001). Conclusions:The serum NT-proBNP level in patients with chronic Keshan disease with different cardiac function grades is different, and is positively correlated with echocardiographic parameters reflecting the degree of cardiac structural and functional impairment. The NT-proBNP level may become an early diagnostic, grading, and prognostic indicator for chronic Keshan disease.
