1.Early treatment of central herniation observation and prognosis of dual frontal concurrent severe cerebral contusion

Objective To investigate the bilateral frontal cerebral contusion complicated by severe central herniation early treatment .Methods 54 cases of severe bilateral frontal cerebral contusion complicated by central herniation patients were randomly divided into the observation group and control group using number table .The obser-vation group were taken the early surgical treatment and the control group were not underwent surgery .The clinical symptoms and prognosis of treatment were compared .Results After treatment ,the GCS score of the observation group was (12.98 ±3.85),which was higher than (11.35 ±4.12) of the control group(t=2.354,P<0.05),and the total time of the observation group ICU hospitalization time was significantly lower than the control group (t=10.007,P<0.01;t=19.278,P<0.01);The good recovery rate of the observation group (40.74%) was higher than the control group(14.81%)(χ2 =4.523,P<0.05),the residual rate of the observation group ,severe disability rate,plant sur-vival and mortality (33.33%,14.81%,7.41%,3.70%) were lower than that of the control group (44.44%, 22.22%,11.11%,7.41%),but the difference was not significant between the two groups (χ2 =2.587,2.135, 2.268,2.189,all P>0.05).Conclusion Dual frontal severe brain injury complicated by central herniation patients with early diagnosis and surgical treatment method using craniotomy decompression can significantly improve the cure rate and effectively improve the prognosis of patients with treatment .
2.Aanlysis and strategies of sources of stress of nurses in cardiovascular department

Objective To analyze the main causes of sources of stress for nurses in cardiovascular department. Methods We selected 110 nurses from three class-one hospitals in Chengdu and divided them into the nurses in cardiovascular department (the research group) and nurses in other departments (the control group). We compared the two groups in the following aspects: family, society, interpersonal relationship, work nature and work strength, continuing education and professional demand. In view of the high spirit stress existed in nurses in cardiovascular department, some corresponding stress-relief strategies were adopted. Results Two groups were statistically different in social, interpersonal relationship, work nature and work strength, continuing education and professional demand. While in the aspect of family, no difference was seen. Conclusions nurses in cardiovascular department had high spirit stress in social, interpersonal relationship, work nature and work strength, continuing education and professional demand. To analyze sources of stress and stress-relief strategy for them could improve physical and psychological health of them and thus enhance nursing quality.
3.Effect of managed health education on psychological and nutritional status of patients with HBV-related cirrhosis and ascites

Objective To evaluate the effect of managed health education on psychological and nutritional status of patients with HBV-related cirrhosis and ascites.Methods One hundred and twenty one patents with decompensated HBV-related cirrhosis were enrolled for health education and randomly divided into two groups:61 patients received managed health education (trial group) and 60 received conventional health education (control group).In control group patients were informed with their condition and diet note by medical orders.In control group the hepatologists evaluated the baseline condition,nutritional status and quality of life,set up personal files of patients and followed up with communication and counselling periodically from June 2010 to August 2012.The psychological conditions were assessed with self-rating anxiety scale (SAS) and self-rating depression scale (SDS) ; nutritional status was assessed with triceps skinfold thickness (TSF) and mid-arm muscle circumference (MAMC) and the quality of life was assessed with MOS 36-item shoort from healthy survey (SF-36).Results Before health education,there was no statistical difference in the scores of SAS,SDS,TSF,SF-36 and blood Hb,albumin (Alb) levels between two groups.After health education,scores of SAS and SDS were decreased in both groups (P < 0.0l),however,which in trial group were lower than those in control group (39.9 ±8.8 vs.49.0 ±9.4 and 40.6 ± 8.8 vs.51.9 ± 10.1 respectively,both P <0.01).The TSF scores and blood Hb and Alb levels in trial group were higher than those in control group (P < 0.05).The accunulated scores of 8 dimensions in SF-36 forms in trial group were significantly higher than those in control group (P < 0.01).Conclusion The managed health education model can better release psychological pressures,decrease mental disorders,elevate nutrition status,promote health and improve quality of life than conventional health education for patients with HBV-related cirrhosis and ascites.
4.Analysis of 43 cases of cesarean scar pregnancy treated with suction curettage under ultrasound ;guidance

Objective To evaluate the feasibility and safety of transabdominal ultrasound-guided suction curettage to treat cesarean scar pregnancy (CSP) and investigate factors affecting prognosis of CSP. Methods This was a retrospective case study of 43 cases who were diagnosed as CSP and treated by suction curettage under transabdominal ultrasound guidance as an initial treatment of CSP in Obstetrics and Gynecology Hospital of Fudan University over past 7 years (from 2007 to 2013);factors affecting prognosis of CSP were investigated. Results 39 of the 43 cases (91%) were successfully treated. There were no statistically significant differences in maternal age, gravidity, abortion frequency, and the time interval between current CSP and last cesarean delivery, the myometrium thickness between the gestational sac and the bladder wall between the success group and the failure group (all P>0.05). Statistically significant difference was found in crown-rump length (CRL) between the two group (median of the two group was 18.5, 2.0 mm) by rank sum test (P=0.047). Univariate logistic regression analysis demonstrated that CRL was strongly associated with the prognosis and the OR for no complications was 18.50, comparing CRL≤6 mm versus CRL>6 mm (P=0.020). Conclusion Transabdominal ultrasound-guided suction curettage is effective and safe in the treatment of CSP with CRL≤6 mm.
5.Survey on follow-up status in patients with chronic liver diseases

A questionnaire and telephone survey was conducted on 246 patients with chronic liver diseases in the Outpatient Clinic from August 2010 to May 2011.The distribution of diseases and the causes of losing follow-up were analyzed. Most subjects in this survey (182/246,74.0% ) were patients with chronic viral hepatitis( HBV or HCV)and cirrhosis; and their follow-up status was better than others (X2 =12.382,P < 0.05). There were no differences in gender( X2 =10.137,P > 0.05),but significant differences in age ( X2 =31.783,P < 0.05 ) and educational levels ( X2 =32.803,P < 0.05 ) between patients with chronic viral hepatitis and cirrhosis and patients with other liver diseases.The main causes of losing follow-up were neglect of disease by patients (38.3%) and lack of good communication between doctors and patients ( 18.3% ).It is important to reinforce the understanding and awareness of chronic liver diseases for patients.
6.Approaches and Methods of Promoting Doctor-patient Communication under "Internet +" Era

Under the"Internet +" era, some medical staff' s treatment philosophy cannot adapt to the current situation, and patients are prone to produce high expectations because of inadequate basic medical knowledge and the habit of acquiring medical information from Internet. However, when the difference between reality and expec-tations are large, they might be in disputes with medical staffs, and coupled with the lag of hospital management, the doctor-patient communication often is not smooth situation. Therefore, it is recommended that medical staffs improve humanistic quality meanwhile improving technology, patients should develop the habit of acquiring medical information from Internet correctly, hospital needs to optimize management and allocate medical resource reasona-bly, and society should also create positive atmosphere.
7.Mechanisms of delay endothelial cell replicative senescence by extracts from Panax ginseng, Panax notoginseng and Ligusticum chuanxiong.

OBJECTIVETo explore effect of extracts from Panax ginseng, P. notoginseng and Ligusticum chuanxiong on human umbilical endothelial cells (HUVECs) replicative senescence.
METHODHUVECs were induced to aging by generation cultivating to the eighth cells in order to establish a model of endothelial cells replicative senescence. The cultured HUVECs in vitro were divided into 4 groups, the eighth generation cell-senescence untreated group, Vitamin E group, herbal treated high dose and low dose groups. Changes of HUVECs aging were observed by method of SA-beta-gal stained HUVECs and cells cycle were analyzed. Contents of ROS in cells, the levels of anti-superoxide (O2-) and nitric oxide (NO) in cell mediums were examined. Western blot were used to analyse protein expression of NADPH oxidase p47phox, angiotensin type 1 and 2 receptor (AT1R, AT2R).
RESULTCompared with Vitamin E group, the positive cell numbers of beta-gal stained HUVECs were enhanced, cell proliferation was depressed, and the fluorescence intensity of ROS was increased, at the same time, less NO and more O2- in cells were produced in the eighth generation cell-senescence untreated group. Protein expression of p47phox, AT1R and AT2R in cells increased compared with Vit E group. Chinese herbs of high dose and low dose could improve condutions of HUVECs aging. Chinese herbs of high dose and low dose could reduce the positive cell numbers of beta-gal stained HUVEC, increase cell proliferation and decrease fluorescence intensity of ROS in cells, at the same time, cells secreting more NO and less O2-. Protein expression of p47phox, AT1R and AT2R in cells treated with Chinese herbs of high dose and low dose were decreased compared with Vit E group.
CONCLUSIONThe study indicated that extracts from P. ginseng, P. notoginseng and L. chuanxiong could delay endothelial cell replicative senescence. Herbal extracts downregulate the expression of NAD (P) H oxidase subunit-p47phox by means of ROS, hence decrease O2- production and ultimately delay HUVECs in vitro senescence.

8.Rapamycin decreases irradiation-induced hematopoietic system damage.

OBJECTIVETo explore the protection function of rapamycin in hematopoietic system damage induced by irradiation.
METHODSSix to eight week old C57BL/6J male mice were used for experiment. Mice received 4 mg/kg rapamycin by i.p.injection every other day for 5 times. The day after the last injection, mice were exposed to a dose (5 Gy) of total body irradiation (TBI). Peripheral blood was measured by a complete blood count at 0.5, 1, 2, 3, 5, 7, 40, 70 days after TBI. The hematoxylin-eosin staining was used to observe the pathologic changes in sternum obtained from mice at day 5 after TBI. CFU-S of spleen was measured by immerging in Tellyesniczky solution for 24 h at day 5 after TBI.
RESULTSBefore TBI, WBC and LYM decreased in rapamycin-treated mice compared with control (P<0.01); RBC and HGB increased (P<0.05); there was no difference in PLT; HE staining of bone marrow from rapamcin-treated and control mice before irradiation showed no difference in marrow cellularity. After TBI, WBC and LYM decreased significantly, with no difference at 0.5 d to 7 d between rapamycin-treated and control. The counts of WBC and LYM in rapamycin-treated mice restored to normal at 40 d and 70 d. RBC and HGB decreased at irradiation group at 3 d to 7 d, but rapamycin stimulated them to a higher level, both of them tended to normal at 40 d and 70 d. HE staining of bone marrow after 5 day of 5 Gy irradiation, nucleated cells in control decreased significantly, but restored in rapamycin-treated mice. CFU-S results showed the colony number in rapamycin-treated mice was much higher than control mice after 5 Gy irradiation, with 40.00±12.86 and 13.20±2.31 (P=0.035), respectively.
CONCLUSIONAdministration of rapamycin to mice before irradiation protected the mice from hematopoietic damage induced by irradiation by maintaining the bone marrow nucleated cells, slowing down decrease and promoting the restoration of peripheral blood cells and protecting hematopoitic stem/progenitor cells in spleen.

9.Lipopolysaccharide regulates neutrophil inflammation through activating the LRG1/ROCK1 signaling

【Objective】 To investigate the role of lipopolysaccharide (LPS) in regulating the inflammatory response of neutrophil through the leucine-rich α-2 glycoprotein 1 (LRG1)/ Rho-associated protein kinase (ROCK1) signaling. 【Methods】 HL-60 cells were treated with 1 μmol/L all-trans retinoic acid (ATRA) and 12.5 μL/mL dimethyl sulfoxide (DMSO) for 72 h and 96 h, and the morphological changes were observed by Wright-Giemsa staining. The expression of CD11b was detected by flow cytometry. LPS induced the activation of dHL-60 and human peripheral blood neutrophils. The transcription and secretion levels of LRG1, ROCK1 and inflammatory cytokines were detected by qPCR and ELISA, respectively. The expression levels of LRG1 and ROCK1 after the activation of dHL-60 were detected by Western blotting. Furthermore, dHL-60 was treated with the recombinant protein LRG1 and ROCK1 inhibitor Y-27632; the transcription levels of inflammatory cytokines were detected by qPCR. 【Results】 Neutrophils were activated by LPS. The expression levels of LRG1 and ROCK1 were significantly increased, and the transcription levels of inflammatory cytokines were significantly increased. The recombinant protein LRG1 activated dHL-60 in vitro, and the transcription levels of ROCK1 and inflammatory cytokines were significantly increased. Using the ROCK1 inhibitor Y-27632, the production levels of inflammatory cytokines were significantly reduced. 【Conclusion】 LPS can regulate the production levels of neutrophil inflammatory cytokines through activating the LRG1/ROCK1 signaling, thus exacerbating the inflammatory response.
