1.Advances in Diagnosis and Treatment of Differentiated Thyroid Cancer in Patients Showing Thyroglobulin Elevative and Iodine Scintigraphy Negative.

Thyroglobulin (Tg) and radioiodine whole body scan (WBS) have been commonly used in follow-up of patients with differentiated thyroid carcinoma (DTC). Tg is associated with radioiodine uptake in local or distant metastases. In minority of patients, the follow-up scan shows no functioning thyroid tissue, but the serum thyroglobulin is still elevated. Therefore, we review recent developments of diagnosis and treatment of those patients with differentiated thyroid cancer and with thyroglobulin elevation but negative iodine scintigraphy.

2.Correlation of 99 Tcm-MIBI gated myocardial perfusion SPECT imaging and two-dimensional speckle tracking imaging in swine model of myocardial infarction

Objective To evaluate the value of speckle tracking imaging( STI) by investigating the correlation and consistency between two?dimensional STI and 99 Tcm?MIBI G?MPI in the assessment of myo?cardial injury and myocardial infarction ( MI) of swine model. Methods Eight healthy and same?age adult female Tibetan swine were used to establish animal models of MI induced by open?chest ligation of the first diagonal branch of the left anterior descending coronary artery. The swine underwent ultrasonic cardiogram and G?MPI before and at 6, 24, 48, 72 and 96 h after the operation. The myocardial perfusion was evalua?ted. Ultrasonic sectional graphs were collected to analyze ultrasonic short?axis horizontal peak values of sys?tolic radial strain ( SR) and systolic circumferential strain ( SC) and local myocardial rotation angle ( Rot) of different segments of the left ventricle with off?line STI. Left ventricular ( LV) EDV, LVESV, LVEF by G?MPI and ultrasonography were compared. Pearson correlation analysis and repeated measures analysis of variance were used. Results Six models were successfully constructed. Six hours after the coronary artery ligation, the G?MPI showed no significant or very few perfusion defects;24, 48, 72 and 96 h after the liga?tion, G?MPI showed sparseness and defects at the anterior septum, anterior and lateral wall of apical seg?ment of apical segment. The peak strain values of SR, SC and Rot at the anterior septal apical segment, the anterior and lateral wall of apical segment decreased at different time points after the ligation. Six hours after the coronary artery ligation, the STI was positive. For healthy swine, the LVEF measured with G?MPI and ultrasonic STI showed good correlation ((68±3)% vs (70±4)%, r=0.84, P<0.05). The LVEF values of G?MPI and ultrasonic STI at 6, 24, 48, 72, 96 h after the ligation were (79±1)%, (80±4)%, (58±2)%, (54±4)%, (57±4)%, and(76±2)%,(76±3)%,(57±3)%,(55±3)%,(68±6)% (r:0.40-0?76, all P<0.05). LVEDV and LVESV measured by the two methods showed variation trends at different time points ( F=64.104, 30?069, 35.186, 54.903, all P<0.01) with good consistency after the ligation. Conclusions Ultrasonic STI could estimate the myocardial injury at an earlier stage and measure the severity of damaged myocardium quantitatively, and the resluts are in good agreement with myocardial perfusion and function by G?MPI.
3.Expression of potassium channel Kv1.6 in rat hippocampus after status epilepticus

Objective To investigate the expression and distribution of Kv1.6 in rat hippocampus after status epilepticus (SE) induced by lithium and pilocarpine to explore the relationship between Kv1.6 and epileptogenesis.Methods Forty-eight healthy male Sprague-Dawley rats were randomly divided into experiment group and control group.The rats in each group were randomly assigned into 6-hour,1-day,2-day and 3-day subgroups (n =6).Electroencephalography (EEG) of rats in both groups were recorded to show the electrical activity of the rat brains.Nissel staining was performed to investigate the pathological changes,and immunohistochemistry and Western blotting were applied to analyze the expression and distribution of Kv1.6.Results (1) The EEG of rats in control group showed α wave with uniform amplitude,while the EEG in rats subjected to pilocarpine injection showed slow waves and spike waves with irregular amplitude and rhythm.Sustained spike waves were observed during SE.(2) Nissl staining showed that 6 hours after SE,the neural morphology of hippocampi in experiment group was as the same to that in control group.There was no change in neuronal morphology and numbers.However,1 day after SE,the structure of hippocampi was discrete and the number of neurons was decreased.Two and 3 days after SE,the number of neurons in hippocampi decreased further.Swelling and deformation of neurons were seen.Nissl bodies emerged,decreased or even disappeared.(3) There was no statistically significant difference in the expression of Kv1.6 in CA3 and CA1 areas between experiment group and control group 6 hours after SE (all P>0.05).One day,2 days and 3 days post SE,the expression of Kv1.6 in CA3 area (0.117 ±0.040,0.135 ±0.029,0.112 ±0.050) decreased obviously compared to control group (0.192±0.054,0.201 ± 0.062,0.184 ±0.042;t---3.861,3.745,5.579,all P <0.05);and the expression of Kv1.6 in CA1 area (0.114 ± 0.02,0.082 ± 0.031,0.106 ± 0.043) decreased significantly compared to control group (0.165±0.036,0.187 ±0.025,0.172 ±0.052;t=3.221,6.360,3.645,all P<0.05).The expression of Kv1.6 did not show statistically significant difference in DG area between experiment group and control group at all time points examined (P > 0.05).The Western blotting showed the same trend.Conclusion The decreased expression of Kv1.6 may play a role in the epileptogenesis.
4.Physiotherapy for Fracture in Earthquake: 11 Cases Report

5.Effects of ginsenoside Rg1 on free amino acids in brain of sports fatigue-mice

Objective: To study the effect and possible mechanism of Ginsenoside Rg1 in sports CNS fatigue and dose-response relationship.Methods: Kun Ming male mice were divided randomly into 6 groups: control group and five different doses groups(5,10,20,40,80 mg/kg).They were treated orally with Ginsenoside Rg1 for 2 weeks.The levels of amino acids of brain tissue were tested after swimming for 90 min and the exhaustive swimming time were recorded.Results: The levels of BCAA and the ratio of BCAA/AAA in groups 5,10 and 20 mg/kg were significantly higher than control group and the levels of AAA in groups 5,10,20 and 40 mg/kg were significantly decreased.The exhaustive swimming time in groups 5,10 and 20 mg/kg were much longer than in control group.Conclusion: Through adjusting the ratio of BCAA and AAA and decreasing the level of excitatory amino acids neurotransmitter,Ginsenoside Rg1 can elevate movement ability and resist against CNS fatigue.The roles of mid-low and low doses may be better.
6.A study on Changduqing enema combined with plasma exchange for treatment of endotoxemia in 34 cases with acute-on-chronic hepatic failure

ObjectiveTo observe the effect of enema with Chinese medicine Changduqing combined with plasma exchange for treatment of endotoxemia in patients with acute-on-chronic liver failure.Methods A prospective randomized controlled trial was conducted, and 68 patients with acute-on-chronic hepatic failure who were hospitalized from January 2009 to June 2012 in the First Affiliated Hospital of Henan University of Traditional Chinese Medicine were randomly divided into treatment and control groups, 34 cases in each group. The patients in control group were treated with plasma exchange combined with western medicine comprehensive treatment such as protecting liver, reducing enzyme, removing jaundice, and in cases with hepatitis B, antiviral therapy was added. The treatment in patients of treatment group was the same as that in the control group, but additionally Changduqing enema of TCM decoction was given(ingredients: radix et rhizoma ginseng 10 g, flos magnolia officinalis 6 g, radix et rhizoma rhei 6 g, fructus aurantii immaturus 6 g, radix paeoniae rubra 10 g),twice a day combined with 3-4 times of plasma exchange treatment, once every 72 hours. The course of treatment of both groups was 4 weeks. The changes of total bilirubin (TBil), alanine aminotransferase(ALT), albumin(ALB), prothrombin activity(PTA), endotoxin, the improvement of clinical symptoms and signs such as lacking in strength, poor appetite, abdominal distension, oliguria, bleeding tendency, hepatic encephalopathy, infection and ascites were observed before and after treatment, and the survival rate in 24 weeks of follow-up was also investigated.Results Before treatment, the comparisons of TBil,ALT, ALB and PTA between the two groups were of no statistically significant differences(allP>0.05); compared with those before treatment, the levels of TBil and ALT were obviously decreased, and PTA was markedly increased after treatment in both groups, the degree of change being more prominent in treatment group〔TBil(μmol/L): 89.6±52.3 vs. 124.6±64.4, ALT(U/L):52.4±32.1 vs. 98.3±42.5, PTA:(53.8±11.5)% vs.(41.2±10.2)%, allP<0.05〕. The ALB levelsbefore and after treatment of both groups showed no significant difference(bothP>0.05). After treatment, the endotoxin(kU/L) in both groups were decreased in inchoate, developing and fully-developed stages and the descent being more significant in treatment group(inchoate stage: 0.094±0.015 vs. 0.109±0.032, developingstage: 0.102±0.019 vs. 0.146±0.062, fully-developed stage: 0.124±0.022 vs. 0.196±0.074, allP<0.05). The 24-week survival rates of developing and fully-developed stages in patients of both groups were lower than the rate in patients of inchoate stage, and the rates of treatment group in inchoate and developing stages were remarkably higher than the rate in control group〔inchoate stage: 94.1%(16/17) vs. 83.3%(15/18), developed stage: 85.7%(12/14) vs. 42.9%(6/14),bothP<0.01〕.ConclusionEnema with TCM Changduqing combined with plasma exchange for treatment of patients with acute-on-chronic liver failure can remove bilirubin, ameliorate endotoxemia, elevate PTA level and increase the survival rate of patients in 24 weeks of follow-up.

8.THE HEPATOPROTECTIVE EFFECT OF OAT ?ETA-GLUCANS ON LIVER INJURY IN HYPERLIPEMIA RATS

Objective To investigate the therapeutic effect of oat ?-glucans on liver injury in hyperlipemia rats.Method Rats were fed on high fat forage for 3 w to establish hepatic injury model,then the model group was orally given oat ?-glucans at doses of 133,266 and 533 mg/(kg bw.d) respectively.At the same time,there were 3 control groups including 200 mg/(kg bw.d) chitosan oligosaccharide,normal and liver injury(model group fed with distilled water).Serum levels of ALT and AST,the content of MDA in serum and liver homogenate was assayed.Results Oat ?-glucans at high dose obviously decreased the levels of ALT,AST and MDA in the liver injury models,SP had a better protective effect on the liver injury than oligosaccharide did.Besides,in the treated group with ?-glucans,degenerative and necrotic changes of the liver cells were apparently milder than those of the untreated group.Conclusion Oat ?-glucans can obviously relieve the liver injury in hyperlipemia rats.
9.The value of CMV viral load test in diagnosis of cytomegalovirus infection in infants

Objective To assess the value of CMV viral load test in the diagnosis and prognostic judgment of infantile cytomegalovirus infection with whole blood and urine specimens. Methods 50 infants with active CMV infection were selected, which pp65 antigen was positive in serological detection and either CMV-IgM positive or the titer of CMV-IgG ≥40. The viral load in whole blood and urine specimens was detected before and after a course of preemptive ganciclovir treatment and the pp65 antigen was determined after treatment. Results 98% patients were manifested as cytomegalovirus viral load quantitative measurement in whole blood positive before the treatment, while 14% after. The positive ratio of pp65 assay after therapy was 6%. And there was no significant difference between the results of the two kinds of detection methods measurement in urine before and after treatment was 98% and 90%, respectively. The results of urinary viral load quantitative detection did not coincide with clinical characteristics of CMV infection (P<0.05,Sp=0.11,PVP=0.55, (Youden's index)=0.09). Conclusions Good coincidence could be found between CMV-DNA quantitative measurement in whole blood and pp65 antigenemia assay. And the former could be used as a diagnostic index of CMV positive infection. While single urinary viral load quantitative detection had no significance for the distinction between active and latent CMV infection and dynamic monitoring of CMV treatment.
