1.Complicated Infection on Functional Outcome of Stroke

Objective To observe the influence of complicated infections on functional outcome of patients with stroke.Methods Two hundred and twenty-five stroke patiens confirmed by cranial CT scan or MR were prospectively divided into non-infection (145 cases) and infection (80 cases) groups. The body temperature of all the patients was recorded on admission and during hospitalization. the National Institutes of Health Stroke Scale(NIHSS), Barthel Index(BI) and modified Rankin Scale(mRS) were documented in all the cases. The difference in functioual outcome between the two groups was analyzed.Results The maximun body temperature of infection group was higher than that of the non-infection during hospitalization(P<0.01). The funtional outcome of the patients complicated with infections was significantly worse than that of the cases without infections assessed by BI(P<0.01), mRS(P<0.01) and NIHSS(P<0.01).Conclusion Infection of the stroke patients influenceed functional outcome and the prognosis.
2.The therapeutic value of preoperative neoadjuvent chemotherapy in patients with stage Ⅰ_(B2)~Ⅱ_b bulky cervical cancer

Background and Purpose:Cervical cancer is one of the most common malignant neoplasms among women.The 5-year survival for patients with early stage disease is over 70%.While it is generally accepted that either radical surgery or radiotherapy can achieve cure for the majority of patients with early-stage cervical cancer,there is no standard approach to the patients with bulky disease,whose prognosis remains very poor in spite of the therapeutic advances achieved in recent years.Neoadjuvant chemotherapy has been used as part of the multidisciplinary treatment before either operation or radiation therapy.Although the approach still had many controversies,numerous studies have supported its effectiveness in bulky cervical cancer.The present study was conducted to explore whether neoadjuvant chemotherapy with the combination of vincristine,bleomycin and cisplatin could improve the operability and pathological response rate in Ⅰb_(2)~Ⅱb bulky cervical cancer patients.Methods:A total of fifty-six patients with bulky cervical cancer were histologically diagnosed and staged into Ⅰb_(2)~Ⅱb according to the standards of International Federation of Gynecology and Obstetrics(FIGO) and randomly selected,including 15 cases with Ⅰb_(2),15 with Ⅱa,26 with Ⅱb.They were treated by a regimen of PVB chemotherapy for 2 cycles and followed by operation.The immediate response and adverse effects were analyzed.Results:The overall response rate was 78.6%.The resectability rate for surgery after chemotherapy was 100%.No intolerable toxic and adverse effects were recorded during neoadjuvant chemotherapy.Preoperative neoadjuvant chemotherapy can apparently increase the possibility of resection by operation and enhance the remission rate.The primary tumor sizes were significantly decreased after chemotherapy before surgery(P
3.Research progress of radiation-induced xerostomia in nasopharyngeal carcinoma

Radiation-induced xerostomia is the common radiotherapy-related complication of nasopharyngeal cancer and other head and neck cancers.The cause is that the radioactive rays damage salivary gland acini,ducts,blood vessels and nerves at different levels,resulting in dry mouth,mucositis,sialadenitis,and even dysphagia,trismus and other symptoms.In addition,radiation-induced xerostomia has a close relation to the radiation effect on the genes.Reasonable application of cytoprotective agents and sialagogues is the main treatment means.Chinese medicine also plays a crucial role in the prevention and treatment of radiation-induced xerostomia in recent years.
4.A preliminary study on hepatitis C virus infection of a human liver cell line HepG2 in vitro

Objective To establish practical cell model of HCV infection, and investigate the susceptibility of a human liver cell line HepG2 to hepatitis C virus in vitro. Methods A human liver cell line HepG2 was incubated with serum from a chronic hepatitis C patient for 6~8 hours. Both the plus and minus strands of HCV RNA in infected cells or supernatant were examined by reverse transcription polymerase chain reaction (RT PCR). The HCV NS 3,NS 5 antigens in infected cells were respectively detected with the monoclonal antibodies to antigens of their own by immunohistochemical assay. The minus strand of HCV RNA in infected cells were localized by in situ hybridization. Results The intracellular plus or minus stands of HCV RNA were first detected on day 3 post incubation and then could be intermittently detected until day 35 post incubation in cells or supernatant. The positive signals of NS 3,NS 5 antigens could be expressed within cytoplasm of infected cells. The minus strand of HCV RNA was located within cytoplasm by in situ hybridization. Conclustions These results show that a human liver cell line HepG2 is not only susceptible to HCV but also able to support its long time replication in vitro.
5.Schistosoma japonicum infection in obese mouse insulin resistance

Objective To investigate Schistosoma japonicum infection on mice high-fat diet of insulin resistance. Methods 36 male C57 BL/6 J mice were randomly assigned into three equal groups: normal control group ( NC group) , high-fat diet group ( HF group) and high-fat diet with Schistosoma japonicum infected group ( HSJ group) . Specimen was collected 6 and 12 weeks after high-fat diet, separately. The levels of fasting blood glucose ( FBG) , fasting plasma insulin resistance index ( FINS) and insulin ( HOMA-IR) were detected. Interferon-γ( IFN-γ) ,in-terleukin-4 ( IL-4 ) and singal transductor and activator of transcription-4 ( STAT4 ) singal transductor and activator of transcription-6 (STAT6)were detected by ELISA and immunohistochemical method. Results The mice from HF group showed higher levels of HOMA-IR than those from NC groups by the end of 6 and 12 weeks after infection( P<0. 01 );the levels of HOMA-IR in mice from HSJ group were lower than NC group and HF group by the end of 12 weeks(P<0. 05);the levels of IL-4 in mice from HSJ group were higher than NC group and HSJ group by the end of 6 and 12 weeks after infection ( P<0. 05 ); the levels of STAT6 in mice from HSJ group were higher than HF group by the end of 12 weeks after infection ( P<0. 05 );the levels of STAT4 in mice from HF group were higher than NC group by the end of 6 and 12 weeks after infection. Conclusion Schistosome japonicum chronic infection may improve insulin resistance in obese mice with induced STAT6 protein expressed in liver tissue and secrete IL-4,providing new ideas for the prevention and treatment of diabetes.

7.Effects of Jianpizhidong decoction on dopamine transporter mRNA in striatum of tourette's syndrome model mice

Objective To study the effects of Jianpizhidong decoction on dopamine transporter (DAT) mRNA in tourette syndrome(TS) model mice striatum,and provide neural biochemical proof for Jianpizhidong decoction in treating children with TS.Methods 32 male ICR mice were randomly divided into blank control group,model group,Tiapride group,Jianpizhidong decoction group.Except blank control group,the other 3 groups mice were made to TS models by intraperitoneal injection of 3,3'-iminodipropionitrile.The blank and model groups,other 2 groups were given corresponding drugs by intragastric administration respectively.Results The striatum DA TmRNA expression of TS model group,Tiapride group and Tianpizhidong decoction group( respectively(0.17 ± 0.017 ),(0.20 ± 0.021 ),( 0.24 ± 0.022 ) ) decreased than that ( 0.30 ± 0.036 ) of blank control group 13 %,10% and 16% respectively.Conclusion The function of improving symptoms of TS by Jianpizhidongdecoction may be relate to raised DAT gene expression to control the excessive hyperfunction of the DA neurons,and it maybe have better therapeutic than Tiapride.
8.Comparative study on determination of sperm DNA integrity by sperm chromatin dispersion test and by acridine orange staining test

Objective To analyze and compare the accuracy and utility of determining sperm DNA integrity by sperm chromatin dispersion (SCD) test and acridine orange staining (AO) test.Methods The level of DNA fragmentation was determined by SCD test and AO test in 32 adult healthy fertile men and 27 idiopathic oligozoospermia (IO) patients.Sperm nuclei with large DNA dispersion halos or with mediumsized halos were normal and nuclei with small-sized halos or no halo were abnormal.The normal sperm DNA was double strands and stained by AO as green.The damaged sperm DNA was single strand,which were stained as green for native DNA and red for denatured DNA.Results The percentage of sperm nuclei with large halos,medium-sized halos,small-sized halos and no halo evaluated bv SCD test in IO patients were (49.9±13.8)%,(11.5±5.4)%,(11.9±6.1)%and(26.7±10.0)%,and they were(73.2±6.2)%,(14.7±6.3)%,(6.8±2.9)%and(5.3±2.2)%in healthy control group,respectively.There was significant difference between two groups(t=8.576,P<0.01;t=2.083,P<0.05;t=4.284,P<0.01;t=11.823,P<0.01).The percentage of sperm DNA fragmentation was (38.6±12.1)%in IO patients and was (12.1±5.2)% in fertile men,respectively.A statistically significant difference was found between IO patients and healthy control group under SCD test (t=11.995,P<0.01).AO test showed no significant differences between IO patients and healthy control group (t=1.626,P>0.05).The percentage of sperm DNA fragmentation evaluated by AO test in IO patients was (45.5 ±13.8)%,and it was (39.8±13.3)%in healthy control group.Conclusions Sperm DNA fragmentation may lead to male infertility.The SCD is effective in testing the sperm DNA fragmentation as a screening procedure to determine semen quality during basic infertility investigation for clinical use.
9.Effects of Jianpizhidong decoction on dopamine D2 receptor mRNA in striatum of tourette' s syndrome model mice

Objective To study the effects of traditional Chinese medicine of Jianpizhidong decoction on dopamine D2 receptor(DAD2R) mRNA in tourette syndrome (TS) model mice brain striatum,and provide the proof of neural biochemical for Jianpizhidong decoction in curing children TS.Methods 32 male ICR mice were divided randomly into control group,model group,Jianpizhidong decoction group and Tiapride group.Except control group,all mice in model group,Jianpizhidong decoction group and Tiapride group were made to TS models by intraperitoneal injection of 3,3'-iminodipropionitrile.The control gronp and model group were given normal saline,Jianpizhidong decoction group and Tiapride group were given corresponding drugs by intragastric administration respectively.Results The striatum DATmRNA expression of model group,Tiapride group,and traditional Chinese medicine group decreased(respectively(0.139 ± 0.013),(0.15 ± 0.021),(0.141 ± 0.019)) than that of control group (0.180 ± 0.028),and with 14.1%,3.0% and 3.9% down respectively.In model group DATmRNA expression was lower than control group (P < 0.01).In model group DATmRNA expression was lower than Tiapride group (P< 0.01).In model group DATmRNA expression was lower than traditional Chinese medicine group (P<0.01).In Tiapride group DATmRNA expression was lower than traditional Chinese medicine group (P < 0.01).Conclusion The function of improving symptoms of TS by Jianpizhidongtang may be relate to inhibite DAD2R hypersensitization to enhance the activity of substantia nigra-striatum.
10.Study on concomitant surgical correction of pelvic organ prolapse and TVT-O for treatment of stress urinary incontinence

Objective To investigate the necessity,safety and efficacy of transobturator tension-free vaginal tape (TVT-O) for treatment of stress urinary incontinence (SUI) during transvaginal corrective operation of pelvic organ prolapse (POP).Methods From Jan.2005 to Dec.2010,92 patients undergoing transvaginal pelvic reconstruction surgery for correction of POP concomitant TVT-O for treatment of SUI in Department of Obstetrics and Gynecology affiliated to Beijing Chaoyang Hospital as concomitant surgery group were enrolled in this retrospective study matched with 90 patients with mild SUI without SUI surgery as nonconcomitant surgery group and 120 patients without SUI as control group.Variable clinical index,clinical efficacy and complications were compared among those three groups.Results Compared with those in the other two groups,the mean age [(62 ± 11) years] was lower (P =0.007,0.038),the operation time only slightly increased (12.8 min and 12.9 min respectively) significantly in concomitant TVT-O group.The bleeding loss and the length of staying hospital after operation all exhibited no significant differences within three groups (P > 0.05).The effective rate for SUI was 96.7% (89/92) in concomitant TVT-O group,corrective operation of POP was ineffective for 74.4% (67/90) SUI,9.2% (11/120) patients presented new SUI in the patients without SUI preoperatively.Conclusions TVT-O is a simple,safe and effective method in the treatment of SUI,which is more suitable for performing simultaneously during the corrective operation of POP.Efficacy of SUI correction was limited in those patients undergoing only pelvic reconstructive surgery.However,a preventive anti-incontinence procedure is not recommended because of the lower incidence in POP patients without SUI preoperatively.
