1.Effect of different antiplatelet or anticoagulation therapy on postoperative pocket hematoma after electrophysiological devices placement

Objective To investigate the effect of different antiplatelet or anticoagulation therapy on postoperative pocket hematoma after electrophysiological devices placements (EPD). Methods The clinic data of 410 patients who took anti-platelet or anticoagulation therapy and needed to be implanted EPD from February 2012 to February 2015 were selected. According to the type of therapy, patients were divided into 4 groups:dual anti-platelet drug (DAP) treatment group (DAP group, 114 cases), low-molecular-weight heparin (LMWH) bridging treatment group (LMWH group, 98 cases), aspirin (ASA) alone group (ASA group, 94 cases) and warfarin group (104 cases). The incidence of pocket hematoma after electrophysiological devices placements was observed. The risk factors of pocket hematoma were analyzed. Results The incidence of pocket hematoma was higher in LMWH group than that in the other 3 groups: 18.4% (18/98) vs. 4.4% (5/114), 3.2% (3/94) and 2.9% (3/104), and there was statistical difference (P<0.05). But there were no significant differences in the incidence of pocket hematoma among DAP group, ASA group and warfarin group (P>0.05). The multiple Logistic regression analysis revealed that LMWH bridging therapy was an independent risk factor for the development of pocket hematoma (OR=7.105, 95%CI 1.872-17.283). Conclusions LMWH bridging therapy can increase the risk of development of pocket hematomas after electrophysiological devices placement.

Background and Purpose:Cervical cancer is one of the most common malignant neoplasms among women.The 5-year survival for patients with early stage disease is over 70%.While it is generally accepted that either radical surgery or radiotherapy can achieve cure for the majority of patients with early-stage cervical cancer,there is no standard approach to the patients with bulky disease,whose prognosis remains very poor in spite of the therapeutic advances achieved in recent years.Neoadjuvant chemotherapy has been used as part of the multidisciplinary treatment before either operation or radiation therapy.Although the approach still had many controversies,numerous studies have supported its effectiveness in bulky cervical cancer.The present study was conducted to explore whether neoadjuvant chemotherapy with the combination of vincristine,bleomycin and cisplatin could improve the operability and pathological response rate in Ⅰb_(2)~Ⅱb bulky cervical cancer patients.Methods:A total of fifty-six patients with bulky cervical cancer were histologically diagnosed and staged into Ⅰb_(2)~Ⅱb according to the standards of International Federation of Gynecology and Obstetrics(FIGO) and randomly selected,including 15 cases with Ⅰb_(2),15 with Ⅱa,26 with Ⅱb.They were treated by a regimen of PVB chemotherapy for 2 cycles and followed by operation.The immediate response and adverse effects were analyzed.Results:The overall response rate was 78.6%.The resectability rate for surgery after chemotherapy was 100%.No intolerable toxic and adverse effects were recorded during neoadjuvant chemotherapy.Preoperative neoadjuvant chemotherapy can apparently increase the possibility of resection by operation and enhance the remission rate.The primary tumor sizes were significantly decreased after chemotherapy before surgery(P
4.Chromatographic fingerprint of Tanreqing NaCl Injection

Objective To establish the fingerprint detecting standard for Tanreqing NaCl Injection. Methods Using baicalin as reference substance to calibrate the common peaks by HPLC-ELSD. According to the technical requirements on fingerprint of traditional Chinese medicine injection, their relative retention times and peak area ratios were calculated and the similarity of ten batchs of samples was determined. Results On the basis of systematic methodology, ten batches of injection samples were tested and analyzed so as to establish HPLC fingerprint for the injection. Conclusion The relative retention times and peak area ratios of 16 common peaks on HPLC fingerprint all conform to the requirement. So the established fingerprint can be used as the one of important foundations for quality control of Tanreqing NaCl Injection.
5.Effects of Jianpizhidong decoction on dopamine transporter mRNA in striatum of tourette's syndrome model mice

Objective To study the effects of Jianpizhidong decoction on dopamine transporter (DAT) mRNA in tourette syndrome(TS) model mice striatum,and provide neural biochemical proof for Jianpizhidong decoction in treating children with TS.Methods 32 male ICR mice were randomly divided into blank control group,model group,Tiapride group,Jianpizhidong decoction group.Except blank control group,the other 3 groups mice were made to TS models by intraperitoneal injection of 3,3'-iminodipropionitrile.The blank and model groups,other 2 groups were given corresponding drugs by intragastric administration respectively.Results The striatum DA TmRNA expression of TS model group,Tiapride group and Tianpizhidong decoction group( respectively(0.17 ± 0.017 ),(0.20 ± 0.021 ),( 0.24 ± 0.022 ) ) decreased than that ( 0.30 ± 0.036 ) of blank control group 13 %,10% and 16% respectively.Conclusion The function of improving symptoms of TS by Jianpizhidongdecoction may be relate to raised DAT gene expression to control the excessive hyperfunction of the DA neurons,and it maybe have better therapeutic than Tiapride.
6.Study on UPLC Fingerprint of Gehua Formula Granules

OBJECTIVE:To establish UPLC fingerprint of Gehua formula granules. METHODS:UPLC method were adopted. The determination was performed on Zorbax Eclipse XDB-C18 column with mobile phase consisted of acetonitrile-water at the flow rate of 0.5 mL/min. The detection wavelength was set at 264 nm,column temperature was 25 ℃,and the sample size was 1 μL. Using tectorigenin as reference substance,UPLC chromatograms of 10 batches of Gehua formula granules were determined. The common peak identification and similarity evaluation were conducted by TCM Chromatogram Fingerprint Similarity Evaluation Sys-tem(2004 A edition). RESULTS:14 common peaks were identified in UPLC chromatograms of 10 batches of Gehua formula gran-ules and similarities were all higher than 0.90. UPLC chromatograms of 10 batches of samples were in good agreement with control fingerprint. CONCLUSIONS:Established UPLC fingerprint can provide reference for identification and quality evaluation of Gehua formula granules.
7.Effects of parathyroidectomy on bone metabolism and bone mineral density in hemodialysis patients with secondary hyperparathyroidism

Objective To observe the effects of parathyroidectomy(PTX) on bone metabolism and bone mineral density(BMD) in maintenance hemodialysis patients with secondary hyperparathyroidism (SHPT).Methods A total of 26 dialysis patients with SHPT were treated with PTX.Serum calcium,phosphorus,alkaline phosphatase(ALP) levels were determined by standard methods.The levels of serum intact parathyroid (iPTH),osteocalcin (OC),procollagen type Ⅰ aminoterminal propeptide (PINP),β-crosslaps (β-CTX) were measured by chemiluminescence.BMD was measured by dual energy X ray absorptiometry.iPTH,OC,PINP,β-CTX,serum calcium,phosphorus,ALP were measured before parathyroidectomy and 1,3,6,12,18,24 months after operation.Lumbar spine (LS) and femoral neck (FN) BMD were measured before and 24 months after PTX.Results Compared with that before operation,serum OC[(104.49±25.42) μg/L vs (695.46±355.62) μg/L,P < 0.01] and PINP levels [(248.36 ± 159.38) μg/L vs (809.28 ± 283.50) μg/L,P< 0.01] progressively decreased 3 months after PTX,and serum β-CTX levels [(1.60±0.64) μg/L vs (3.37±1.34) μg/L,P < 0.01] decreased 1 month after PTX.Compared with that before operation,BMD levels increased 24months after PTX in LS[(0.88±0.23) g/cm2 vs (0.78±0.23) g/cm2,P < 0.01] andFN[(0.96±0.19) g/cm2 vs (0.84±0.24) g/cm2,P < 0.01],and Z-scores were also increased in both LS[(-1.24±0.55) vs (-1.66± 0.24),P < 0.01] and FN[(-1.51 ±0.72) vs (-1.93 ± 0.40),P < 0.01].Correlation analysis showed that baseline iPTH was positively correlated with ⊿Z-score in FN (r=0.584,P=0.002) and LS (r=0.400,P=0.043),and so did the OC with ⊿Z-score in FN (r=0.651,P < 0.001) and LS (r=0.509,P=0.008).Conclusion The levels of OC,PINP and β-CTX are reduced and BMD is improved in hemodialysis patients with SHPT after PTX.
8.Study on concomitant surgical correction of pelvic organ prolapse and TVT-O for treatment of stress urinary incontinence

Objective To investigate the necessity,safety and efficacy of transobturator tension-free vaginal tape (TVT-O) for treatment of stress urinary incontinence (SUI) during transvaginal corrective operation of pelvic organ prolapse (POP).Methods From Jan.2005 to Dec.2010,92 patients undergoing transvaginal pelvic reconstruction surgery for correction of POP concomitant TVT-O for treatment of SUI in Department of Obstetrics and Gynecology affiliated to Beijing Chaoyang Hospital as concomitant surgery group were enrolled in this retrospective study matched with 90 patients with mild SUI without SUI surgery as nonconcomitant surgery group and 120 patients without SUI as control group.Variable clinical index,clinical efficacy and complications were compared among those three groups.Results Compared with those in the other two groups,the mean age [(62 ± 11) years] was lower (P =0.007,0.038),the operation time only slightly increased (12.8 min and 12.9 min respectively) significantly in concomitant TVT-O group.The bleeding loss and the length of staying hospital after operation all exhibited no significant differences within three groups (P > 0.05).The effective rate for SUI was 96.7% (89/92) in concomitant TVT-O group,corrective operation of POP was ineffective for 74.4% (67/90) SUI,9.2% (11/120) patients presented new SUI in the patients without SUI preoperatively.Conclusions TVT-O is a simple,safe and effective method in the treatment of SUI,which is more suitable for performing simultaneously during the corrective operation of POP.Efficacy of SUI correction was limited in those patients undergoing only pelvic reconstructive surgery.However,a preventive anti-incontinence procedure is not recommended because of the lower incidence in POP patients without SUI preoperatively.
9.HPLC fingerprint of Platycodon grandiflorum in different picking times

AIM:To establish HPLC fingerprints of Platycodon grandiflorum in different picking times in Henan province.METHODS:HPLC chromatography condition:Hypersil C18 column(250 mm?4.6 mm,5 ?m);the mobile phase was acetonotrile with 0.05% phosphoric acid and the gradient elution mode was applied in chromatographic separation;The flow rate was 0.5 mL/min;The temperature of column was 30 ℃;the detective wavelength was set at 210 nm;RESULTS:The overall similarity in samples collected in autumn was higher than that in spring,and autumn was the appropriate for harvest.CONCLUSION:This method is simple and accurate with a good reproducibility.It provids a reliable scientific basis for the quality control of Platycodon grandiflorum.
10.HPLC fingerprints of Platycodon grandiflorum from different habitats and the determination of platycodin-D

AIM: To establish HPLC-fingerprints and quantitatively determine platycodin-D from Platycodon grandiflorum.METHODS: HPLC analysis was carried out on Hypersil C18 column(250 mm ? 4.6 mm,5 ?m),with a mobile phase of acetonitrile-0.05 mol/L phosphoric acid system,gradient elution,with a flow at 0.5 mL/ min,an ultraviolet detection wavelength was at 210 nm for fingerprint and at 206 nm for platycodin-D,column tem-perature at 30 ℃.RESULTS: Twelve common peaks were identified in chromatograms with reference to platycod-in-D peak from the 18 batches of the samples.CONCLUSION: The method of the HPLC-fingerprint and quantita-tive analysis is rapid,simple and accurate with a good reproducibility and can be used for the quality control of Platycodon grandiflorum.
