1.Studies on the mechanism of EGTA-induced fever and changes of cAMP and PGE levels in c. s. f. after EGTA intraventricular perfusion in rabbits

The levels of cyclic AMP and prostaglandin E (PGE) in the brain are raised in many kinds of fever induced by various pyrogens, we also know when the ratio of Na~+/Ca~(++) is changed in the brain, the body temperature changes. So it is suggested that cAMP, PGE and the ratio of Na~+/Ca~(++) are major central mediums in febril response. It is not clear what relationship is among these three factors and what are their roles respectively in the fever mechanism? It must be very helpful for the development of fever study if we can have them correctly answered. In this paper, we change the Na~+/Ca~(++) ratio by intraventricular perfusion with EGTA or CaCl_2, and observed the temperature response, levels of cAMP and PGE_2 in c.s.f. It is found that there was a positive correlation between cAMP and the body temperature(r=0.9913, P0.05). EGTA can raise body temperature. But CaCl_2 pretreatmentd could lower the EGTA fever.
2.Clinical observation on left heart function in essential hypertensive patients treated with Losartan

Objective To investigate the clinical effects of Losartan on hemorrheology and left heart function in essential hypertensive patients.Methods 43 essential hypertensive patients were treated with Losartan,50 ms/d~100 ms/d for twelve to seventeen months.20 essential hypertensive patients were treated with Fu Fang Jiang Ya Pian as control group.Then the changes of hemorrheology indexes and left heart function were tested in both groups.Results After the treatment with Losartan the blood pressure reduced effectively(P<0.001),the whole blood low shear rate hyperviacoSity(ηb),plasma viscosity,AI,whole blood low shear rate reduced viscosity,erythrocyte electrophoresis,Fib reduced obviously(P<0.01),but the control group had not statistical significance(P>0.05)before and after treatment.The whole blood high shear rate hyperviscesity,whole blood high shear rate reduced viscosity,Hct,RRI reduced too(P>0.05),but there was no statistical significance.LVM(P<0.01)and LVMI(P<0.05)were obviously reduced,E/A was increased(P<0.01),IRT(P<0.05)was shorten (P<0.05),while the cardiac diastole function improved effectively.The significant differences were not observed in the control group pre and pest treatment(P>0.05).Condusion Losartan can effectively reduce blood pressure.At the same time,it improves RBC deformation capability,reduce erythrecyte aggregation,Fib,Hct of essential hypertensive patients,and reduce blood viscosity.Losartan can also reduce myocardial hypertrophy,heart weight,and improve heart function.
3.Assay of Plasma (1→3)-?-D-glucan in Patients Suspected with Deep Fungal Infection

Objective To investigate the relationship between the concentration of plasma (1→3)-?-D-glucan (?-D-glucan) and deep fungal infection. Methods Thirteen patients were recruited in this study, who were suspected with deep fungal infection. G-test TE reagent for ?-D-glucan measurement was used to detect the plasma (1→3) ?-D-glucan in the patients by using UV-2450 spectrophotometer at 545 nm wavelength. The final concentrations were calculated according to concentration conversion formula. Results Nine of thirteen patients were confirmed as deep fungal infection by positive tissue culture, in whom high concentrations of ?-D-glucan were detected, the highest concentration was 352.94 pg/mL (mixed infection), with a mean value of 203.47 pg/mL. In the other four patients with negative culture, the ?-D-glucan concentration was over 54.40 pg/mL in three patients and 16.16 pg/mL in the another. Our results showed that the sensiti-vity, specificity, positive predictive value and negative predictive value of this test were 92.31%, 100%, 100% and 98.36%, respectively. Conclusion G-test TE method is a simple and rapid test and may be used for the diagnosis of patients with deep fungal infection.
4.Effect of candesartan on ERK1/2 protein expression in myocardium of epilepsy rats induced by kainic acid and its mechanism

Objective To investigate the clinical feasibility of contrast?enhanced three dimensional T2WI turbo?spin?echo sequence with short?term inversion recovery and sampling perfection using different flip angle evolutions (3D STIR T2WI SPACE) sequence in the brachial plexus neurography. Methods Thirty two patients were prospectively chosen and performed with brachial plexus plain scanning on a 3.0 T MR scanner by using plain and contrast?enhanced 3D STIR T2WI SPACE sequence. Thirteen of them underwent plain scan, 9 of them underwent contrast?enhanced scan, and 10 of them underwent both plain scan and enhanced scan. The visibility of the brachial plexus were scored and contrast to noise ratio (CNR) were measured by two experienced radiologists. The results between plain and contrast?enhanced imaging were compared by t test. The 10 subjects received both enhance and plain imaging, were performed with paired t test. Results In 32 patients, the visibility score of brachial plexus nerve and CNR were 7.8 ± 1.3 and 24.97±3.41 in the plain scan group, and 13.1±1.7 and 38.49±4.95 in enhanced scan group, respectively. There were statistically significant differences in the two groups(t=-11.72,P<0.01;t=-10.47, P<0.01). In 10 cases with plain and enhanced brachial plexus imaging, the average score of the brachial plexus were 7.4 ± 1.7 and 13.3 ± 1.6, the average CNR were 26.23 ± 4.43 and 38.19 ± 5.03 respectively. There were statistically significant differences (t=- 8.22, P<0.01; t=- 5.64,P<0.01). The score results were analyzed for consistency. Plain images Kappa value was 0.684, which shows moderate consistency and enhanced images Kappa value= 0.822, which shows excelent consistency. Conclusions The contrast?enhanced 3D STIR T2WI SPACE sequences may suppress background tissue signals, which is helpful to display brachial plexus, therefore it is of important value for the early diagnosis of brachial plexus neuropathy.
6.Surgical diagnosis and correction of chordee among hypospadias

ObjectiveTo explore the importance and significance of classification diagnosis and treatment of chordee among hypospadias in children.MethodsA random sample review and controlled study was made in patients with hypospadias and chordee from 1989 to 2011 in First Affiliated Hospital of Sun Yat-Sen University,and the patients were divided into 2 groups.There were 232 cases in group A,who were all diagnosed as chordee only on the basis of clinical symptoms or an erection induced by artificial stimulation before operation.All the 232 cases were corrected empirically without objective assessment by degloving the penis,dissecting superficial and deep fascia,and cutting off the urethral plate,and so on during operation.There were 25 cases in group B,who were all strictly implemented on classification diagnosis and treatment of chordee,in sponge saline injection test during operation and correction effect evaluation after operation.Chordee was classified according to different etiology and pathology:skin type,fascia-type,urethra type,sponge type and the glans of penis type.There were different operating methods in different ways of folding tunica albuginea.ResultsIn group A,86 patients (37%) had postoperative recurrence of chordee,25 patients ( 11% ) had pain during erection after surgery,183 patients were satisfied with correction of chordee (79%).In group A,25 patients were in their adulthood when followed up,and 7 patients had IIEF-5 score <21 points.In group B,the single type of chordee among hypospadias were 7/25 (28%),merged type were 18/25 (72%).All patients were followed up without chordee recurrence or painless erection,and 100% patients or families were satisfied with correction of chordee.The efficacy difference were statistically significant between the two groups in chordee correction.In folding tunica albuginea group in group B,the penile erection length was (4.58 ± 1.59) cm before the correction,and (6.16 ±2.54) cm after correction.In non-folding tunica albuginea group,penile erection length was (4.O1 ± 1.18 ) cm before correction,and (5.82 ± 1.51 ) cm after correction.The difference was not significant between the 2 groups in penile erection length (P > 0.05 ).ConclusionsSurgeon should pay more attention to the correction,effect evaluated,classified etiology and pathological diagnosis of hypospadias and chordee.And the targeted selection of the correct treatment of chordee can further improve the therapeutic effect.Chordee of hypospadias can exist in single or merged according to the pathological classification.Chordee correction surgery can increase length of penis,and dorsal tunica albuginea plication had no significantly different effect on the length of the penis compared to other chordee corrective surgery.
7.Investigation of the Relationship between Cognitive Function and MRI Manifestation in Patients with SHPT

Objective To investigate the relationship between cognitive function and magnetic resonance imaging (MRI) manifestations in patients with secondary hyperparathyroidism (SHPT) by analyzing brain MRI diffusion-weighted imaging,diffusion tensor imaging,serological markers,and cognitive function.Methods We evaluated 49 random patients with SHPT.We adopted two functional scales to evaluate cognitive function and performed MRI fractional anisotropy (FA) and apparent diffusion coefficient (ADC) to evaluate white matter function of brain functional areas.We recorded serological markers,and conducted statistical analysis to analyze correlation among cognitive function scores,brain MRI in white matter functional areas,and biochemical parameters.Results We found that at the frontal area,the FA and ADC values were correlated with the MMSE language and memory function scores;at the parietal area,the FA and ADC values were correlated to the MoCA visuospatial executive function scores;the other areas had no significant correlation with cognitive function scale score.Cognitive function was negatively correlated with immunoreactive parathyroid hormone (iPTH) levels but positively correlated with education level,while it was not correlated with sex,age,blood pressure,and serum calcium levels in patients with SHPT.Conclusion In patients with SHPT,cognitive function is correlated with iPTH level and education level.The FA and ADC values of brain MRI in the frontal and parietal lobes might play a diagnostic role in evaluating cognitive function and locating injury for patients with SHPT,which requires clinical attention.
8.The value of MR T2* in evaluation of liver iron overload related to long term blood transfusion

Objective To investigate the value of MR T2* in evaluation of liver iron overload caused by long term blood transfusion.Methods Thirty one patients with long term blood transfusion were collected.Both serum ferritin(SF)and CRP levels were measured and the adjusted serum ferritin(ASF)concentration was calculated.MRI of the maximum cross-sectional liver was performed using a 1.5T scanner (Siemens Avanto).The T2* value of the liver was measured,and the correlation analysis was carried out.Twenty one normal volunteers served as controls.The transfusion group was divided into iron overload group and non overload group,and differences between groups were analyzed.Results The correlation coefficient in 31 patients between liver T2* value and SF,ASF and blood transfusion volume was-0.695,-0.728 and -0.657 respectively,and the difference was statistically significant(P<0.05).The correlation coefficient between SF and blood transfusion volume was 0.518,and the difference was statistically significant(P<0.05).There was difference in T 2* value between three groups.Conclusion MR T2* has some practical value in the evaluation of liver iron overload in patients with long term blood transfusion.
9.Mobile phone dependence and gray matter volume in college freshmen

Objective:
This study aimed to investigate the correlation between brain gray matter volume(GMV) and MPD based on voxel-based morphometry(VBM) method.
Methods:
By using principle of random sampling method, 566 students were recruited from 5 different majors in a certain university. An electronic questionnaire was used to collect the general situation and MPD. Moreover, 266 students were obtained high-resolution 3-dimensional T1 structure images by 3.0 T MRI based on voluntary principle. A multiple regression model with VBM8 was performed to detect GMV associated with MPD scores.
Results:
Among the 266 participants, 81 students(30.5%) reported MPD. There were no statistical differences of MPD scores between different characteristics, such as gender, residential area, sibling and perceived family income, as well as academic performance(t/F=-1.04,0.40,-1.30,0.23,1.48,P>0.05). An inverse correlation was found between GMV of the anterior cingulate gyrus(ACC) and right fusiform gyrus(FFG) with MPD scores(FDR corrected, P<0.05), and no positive associations were found.
Conclusion
College students with higher severity of MPD have lower GMV of ACC and right FFG, and these results suggested that MPD was correlated with brain reward, executive control and emotional related GMV.
