1.A Case of Abdominal Pain, Fever, Frequent Urination and Urinary Incontinence after Transvaginal Ultrasound-guided Oocyte Retrieval Successfully Treated with Kampo Medicine

Here we report a case with fever, abdominal pain, frequent urination and urinary incontinence after vesicolysis, for bladder hematoma after transvaginal ultrasound-guided oocyte retrieval, that was successfully treated with Kampo medicine based on daily changing sho for acute disease.
A 44 year-old woman underwent transvaginal ultrasound-guided oocyte retrieval under venous anesthesia in the morning, in order to perform in vitro fertilization, and got home around noon. She had right lower quadrant abdominal pain, frequent urination, hemoptysis, urinary incontinence, fever and chill in that evening. Emergency room ultrasonography revealed a 45 mm size bladder hematoma. She was admitted and the vesicolysis was performed. Antibiotics and hemostatics were administered, and were followed by self-sustaining flow back into the bladder. Although she became afebrile on day 3, her right lower quadrant abdominal pain remained and fever returned on day 5. Daiobotanpito was prescribed because of the right lower quadrant abdominal pain, frequent urination, urinary incontinence, constipation and a floating pulse. Additionally, goreisan was prescribed because of her tooth mark and leg edema. On day 6, her abdominal pain had largely disappeared, and her frequent urination and urinary incontinence were also improved. However, perspiration with fever and soft stools then appeared. Keishito was prescribed because of the perspiration, a weak pulse and weak abdominal strength. Additionally choreito was prescribed for the frequent urination and urinary incontinence. She started sweating continuously with no fever. The frequent urination and urinary incontinence then also disappeared, and she was discharged on day 7.
2.Knee joint troubles in tea pickers.

Investigation was made into knee joint problems among tea pickers through a questionnaire. Following this, physical examinations were given and radiography was taken.
Of the male respondents, 18.9% said that they have pains in the knee (s). The corresponding figure for the women was a high as 38.5%. The number of men and women who complained of knee pain increased with age. However, it was not clear whether the incidence of knee pain has any bearing on the number of years they had worked or working hours per day. Rather, it seems to depend on the configuration of the plantation.
Knee pain occurs more frequently in persons working on sloping and hilly lands than those working on flat land. The difference in Q-angle is wider in the hilly area than in the flat land.
X-ray examination of the knee joint of the persons who complained of sensations revealed degenerative changes in the form of the synovial cavity. Changes in the form of the patella were more evident in the persons working on the sloping land than on the flat land.
On the slope, tea pickers have to bend their knees, requring femoral quadriceps and crural triceps to maintain contraction for cours. Thus the level of stress on the kneecaps becomes high.
To prevent knee joint injuries, improvement of footings, mechanization of collecting work, decreases in the frequency of decending and ascending the slop, and having a rest at proper intervals are necessary.
Furthermore, it is necessary to strengthen femoral quadriceps by excercise and control weight.
3.A Meta-Analysis of Slow Pull versus Suction for Endoscopic Ultrasound-Guided Tissue Acquisition

Endoscopic ultrasound (EUS)-guided tissue acquisition is widely utilized as a diagnostic modality for intra-abdominal masses, but there remains debate regarding which suction technique, slow pull (SP) or conventional suction (CS), is better. A meta-analysis of reported studies was conducted to compare the diagnostic yields of SP and CS during EUS-guided tissue acquisition.
Methods:
We conducted a systematic electronic search using MEDLINE/PubMed, Web of Science, and the Cochrane Central Register of Controlled Trials to identify clinical studies comparing SP and CS. We meta-analyzed accuracy, sensitivity, blood contamination and cellularity using the random-effects model.
Results:
A total of 17 studies (seven randomized controlled trials, four prospective studies, and six retrospective studies) with 1,616 cases were included in the analysis. Compared to CS, there was a trend toward better accuracy (odds ratio [OR], 1.48; 95% confidence interval [CI], 0.97 to 2.27; p=0.07) and sensitivity (OR, 1.67; 95% CI, 0.95 to 2.93; p=0.08) with SP and a significantly lower rate of blood contamination (OR, 0.48; 95% CI, 0.33 to 0.69; p<0.01). However, there was no significant difference in cellularity between SP and CS, with an OR of 1.28 (95% CI, 0.68 to 2.40; p=0.45). When the use of a 25-gauge needle was analyzed, the accuracy and sensitivity of SP were significantly better than those of CS, with ORs of 4.81 (95% CI, 1.99 to 11.62; p<0.01) and 4.69 (95% CI, 1.93 to 11.40; p<0.01), respectively.
Conclusions
Compared to CS, SP appears to provide better accuracy and sensitivity in EUSguided tissue acquisition, especially when a 25-gauge needle is used.

5.Management of Difficult Bile Duct Stones by Large Balloon, Cholangioscopy, Enteroscopy and Endosonography

Endoscopic management of bile duct stones is now the standard of care, but challenges remain with difficult bile duct stones. There are some known factors associated with technically difficult bile duct stones, such as large size and surgically altered anatomy. Endoscopic mechanical lithotripsy is now the standard technique used to remove large bile duct stones, but the efficacy of endoscopic papillary large balloon dilatation (EPLBD) and cholangioscopy with intraductal lithotripsy has been increasingly reported. In patients with surgically altered anatomy, biliary access before stone removal can be technically difficult. Endotherapy using two new endoscopes is now utilized in clinical practice: enteroscopy-assisted endoscopic retrograde cholangiopancreatography and endoscopic ultrasound-guided antegrade treatment. These new approaches can be combined with EPLBD and/or cholangioscopy to remove large bile duct stones from patients with surgically altered anatomy. Since various endoscopic procedures are now available, endoscopists should learn the indications, advantages and disadvantages of each technique for better management of bile duct stones.
6.Endoscopic Ultrasound-Guided Tissue Acquisition by 22-Gauge Franseen and Standard Needles for Solid Pancreatic Lesions

Background/Aims:
Recently, a three-plane symmetric nee-dle with Franseen geometry was developed for endoscopic ultrasound-guided fine needle biopsy (EUS-FNB). In this ret-rospective study, tissue acquisition per pass was compared between 22-gauge Franseen FNB and standard fine needle aspiration (FNA) needles in patients with solid pancreatic le-sions.
Methods:
Consecutive patients who underwent EUSFNA or EUS-FNB for solid pancreatic lesions between Octo-ber 2014 and March 2018 were retrospectively studied. The tissue acquisition rate and the diagnostic performance per session, per pass, and at first pass were compared.
Results:
A total of 663 passes (300 by the FNB needle and 363 by the standard FNA needle) were performed in 154 patients (71 FNB and 83 FNA). The tissue acquisition rate per session and at first pass in the FNB and FNA groups was 100% and 95% (p=0.13) and 87% and 69% (p=0.007), respectively. The multivariate analysis revealed that among the patients, EUS-FNB (odds ratio, 3.07; p=0.01) was associated with a higher first-pass tissue acquisition rate. While the tissue ac-quisition rate reached a plateau after the 4th pass with FNA, it reached a plateau after the 2nd pass with FNB. Among the 129 malignant cases, the histological tissue acquisition rate per session was similar (100% and 94%), but the sensitivity by histology alone per session was higher for FNB than for FNA (93% and 73%, p<0.01).
Conclusions
The results of our retrospective analysis indicated that compared with a standard FNA needle, a 22-gauge Franseen FNB needle was associated with a higher first-pass tissue acquisition rate.
7.Ligation of Lumbar Arteries and Stent Graft-Conserving Aneurysmorrhaphy for Type II Endoleak

Background : Persistent endoleak is a major cause of aneurysmal enlargement or rupture after endovascular aneurysm repair (EVAR). Although several reports have described ligation of lumbar arteries and stent graft-conserving aneurysmorrhaphy as useful strategies, treatment for type II endoleak after EVAR is controversial. Objectives : We investigated the early results in 5 patients who underwent ligation of lumbar arteries and stent graft-conserving aneurysmorrhaphy for type II endoleak. Methods : A>10 mm increase in aneurysm diameter after primary EVAR or a maximum diameter>65 mm serve as indications for intervention for type II endoleak. Under general anesthesia, following transperitoneal exposure of the abdominal aorta, the infrarenal aorta was banded using a tape at the proximal landing zone. After the aorta was opened without clamping, the lumbar arteries were ligated, and a stent graft-conserving aneurysmorrhaphy was performed. Results : The mean interval from the primary EVAR was 47±17 months. The mean operation time was 215±76 min. Blood transfusion was necessary in 4 patients (estimated blood loss 1,260±710 ml). No in-hospital deaths were observed, and the mean postoperative hospital stay was 26±20 days. One patient developed aspiration pneumonia and 1 developed surgical site infection post-surgery. The diameter of the aneurysm changed from 68±8 to 47±5 mm during hospitalization and decreased further to 36±7 mm at the last follow-up. Conclusions : The early results of ligation of lumbar arteries and stent graft-conserving aneurysmorrhaphy were favorable. Although this strategy could be useful for aneurysmal dilatation secondary to persistent type II endoleak after EVAR, the indications for this approach should be determined following careful evaluation of the patient's status considering the invasiveness of the procedure.
