Objective The immature platelet fraction (IPF) could be detected quantificationally in Sysmex XE-2100 with the software of XE-pro and IPF master. The study aimed to perform the methodological evaluation of IPF detection and investigate the clinical significance for the monitoring for bone marrow hyperplasia in cancer chemotherapy patients. Methods The high-level, middle-level and low-level whole blood samples were randomly chosen for detection repeatly 20 times to obtain interrun coefficient of variation (CV) for evaluation of the precision and reproducibility. Integrated quality controls were determined for continuous 20 days to obtain intrarun CV, and the stability and carryover was investigated.Furthermore, the correlation between results from Sysmex XE-2100 and results from flow cytometry was assessed.182 healthy subjects and 130 cancer patients undergoing chemotherapy were selected and the latter were divided into two groups according to platelet counts after therapy, one was normal PLT group, the other was decreased PLT group. The IPF of either group was measured and was compared with each other. Results The precision of IPF for high-level, middle-level and low-level were 4.71%, 4.33% and 4.95%, respectively, they were less than 5%.The interrun CV of IPF detection for middle-level and low-level were less than 5%.The interrun CV of IPF detection for high-level were less than 10%.The carryover ranged from 0.6% to 2.7%,and the average rate was 1.2%.A good correlation for IPF detection was shown between results from Sysmex XE-2100 and flow cytometry(r = 0.880 9,P < 0.01).Regarding clinical utility of IPF detection in treatment monitoring for chemotherapy effect, the median of IPF levels in decreased PLT group, normal PLT group, control group were 14.45% ,7.35% and 15.68%, respectively. There was significant difference among the three groups (H =49.032,P <0.01 ).The IPF level was higher in decreased PLT group than normal PLT group (t = -5.681, P < 0.O1 ), and was lower in normal PLT group after chemotherapy than the control group (t = -6.662 ,P <0.01 ).Conclusions The determination of IPF by the Sysmex XE-2100 owns high precision and good stability. IPF is an effective marker for evaluation of thrombopoietic condition in the cancer chemotherapy patients.

Objective To evaluate the nucleated red blood cell (NRBC)count of Sysmex XN-9000 automatic hematology anal-ysis lines comparing with manual method, and verify the accuracy of the analyzer results. Methods 60 blood samples with more than 1% of NRBCs detected by XN-9000 were counted NRBCs by traditional manual microscopy in blood smears, and verified the analyzer results. Results According to the reliability analysis, the results of total 60 samples were all within the range of the reliability;correlation analysis showed that correlation coefficient (r)of group NRBC (%)1~10 and>10 were 0.972 1 and 0.996 2,respectively.There were no significant differences between them (P>0.05).Conclusion Compared with manual method, the results of NRBC count of XN-9000 were within the range of reliability, and showed good correla-tion.The analyzer test results of NRBC were accurate and reliable and could be applied to the detection of clinical samples.
3.Clinical value of combined detection of Hcy ,HbA1c and nephrotic index in screening for early renal injury in type 2 diabetes mellitus
International Journal of Laboratory Medicine 2018;39(11):1329-1331,1335
Objective To study the clinical value of combined detection of homocysteine (Hcy) ,glycosy-lated hemoglobin (HbA1c) and nephrotic index in screening early renal injury in type 2 diabetes mellitus . Methods From January 2016 to January 2017 ,164 patients with diabetes were selected as the subjects .Ac-cording to the glomerular filtration rate ,they were divided into normal renal function group (n=49) ,renal insufficiency compensatory group (n=56) ,renal insufficiency decompensation group (n=59) ,and 60 healthy physical examination personnel were taken as the health control group .We also evaluated the diagnostic value of Hcy ,HbA1c and nephrotic index alone and combined detection in early diabetic nephropathy .Results The average level of Hcy ,HbA1c and nephropathy index in healthy control group ,normal renal function group , renal insufficiency compensation group ,and renal insufficiency decompensation group increased in turn .In the early diagnosis of renal injury in diabetic patients ,the sensitivity was HbA1c (97 .39% ) ,the specificity was Hcy (81 .16% ) ,the Jordan index (0 .76) ,and the rate of coincidence (92 .76% ) were all high in the index of kidney disease .The sensitivity (99 .13% ) and negative predictive value (98 .43% ) of the 3 indexes were im-proved .Conclusion Combined detection of Hcy ,HbA1c and nephrotic index is of high clinical value in early detection of early diabetic nephropathy .
