1.Telmisartan improves insulin resistance in the rats with nonalcoholic steatohepatitis by SOCS-3/SREBP-1c pathway

Objective To evaluate the effects of telmisartan by SOCS-3/SREBP-1c pathway and its efficacy of improving insulin resistance (IR) in rats with high-fat diet-induced nonalcoholic steatohepatitis (NASH).Methods A total of 70 SD male rats were assigned randomly into 3 groups: A (normal control,20 rats,basic diet),B (model control,30 rats,high-fat diet) and C (treatment with telmisartan,20 rats,high-fat diet).After the IR-NASH model was made successfully,proved by 10 rats randomly from the group B with euglycemic hyperinsulinemic clamp technique (EHCT) and liver histology,the rats in the group C were intragastrically administrated telmisartan (5 mg/kg/d) for 4 weeks,and then all rats were tested with EHCT and sacrificed to test the blood chemistry,interleukin-6,homeostasis model assessment of insulin resistance,hepatic pathological analysis,and semiquantitative RT-PCR for determining SOCS-3 and SREBP-1c mRNA.Results Rats with high-fat diet developed steatohepatitis and insulin resistance at the 12th week and had more weight gain and higher liver index at the 16th week.IL-6,SOCS-3 and SREBP-1c mRNA expressions in the group B were up-regulated obviously,and each was positively correlated with the velocities of glucose infusion rates at 60~120 min.Blood chemistry and pathological observation in the group C were all improved;both SOCS-3 and SREBP-1c mRNA were down-regulated,and each negatively correlated with VGIR60-120,while serum IL-6 stayed at a high level.Conclusions Telmisartan can remarkably improve hepatic function and insulin resistance in rats with IR-NASH,the mechanisms of which would not be by path of reducing the secretion of IL-6,but by down-regulating the expressions of SOCS-3 and SREBP-1c mRNA.
2.Construction of vascular access core nursing team and its effect in blood purification patients

Objective:To explore and analyze the construction of vascular access core nursing team and its application in blood purification patients.Methods:From May 2018 to May 2020, convenience sampling was used to select 120 patients with blood purification in Department of Nephrology of Henan Provincial People's Hospital as the research object. The patients were divided into the control group and the observation group by random number table method, 60 cases in each group. The control group carried out routine nursing, and the observation group constructed a vascular access core nursing team to implement specialized nursing on the basis of the control group. The composition of vascular access, the success rate of one puncture, the incidence of complications, and nutritional indicators before and after intervention were compared between the two groups.Results:The observation group had a lower vascular access rate of central venous catheter, and a higher success rate of one puncture than those of the control group, with statistical differences ( P<0.05) . The incidence of complications in the observation group was lower than that in the control group, and the difference was statistically significant ( P<0.05) . After nursing, the levels of albumin, hemoglobin and prealbumin in the observation group were higher than those in the control group, and the differences were statistically significant ( P<0.05) . Conclusions:Establishing a vascular access core nursing team to implement specialized nursing and applying it in blood purification patients can effectively reduce the incidence of related complications, improve the nutritional status of patients, and have a good overall nursing effect, which is worthy of clinical promotion.
3.Study on the relationship between perinatal hemodynamics and retinopathy of prematurity

Objective:To explore the correlation between the index of hemodynamics in perinatal period and retinopathy of prematurity(ROP), so as to provide basis for the better prevention and treatment of ROP.Methods:From May 2017 to April 2019, the preterm infants were admitted to the Neonatal Intensive Care Unit of the First Affiliated Hospital of Zhengzhou University at birth and were hospitalized for more than 2 weeks, gestational age ≤ 35 weeks and birth weight ≤ 2 500 g. They were selected as the study objects.The perinatal data including heart rate, blood pressure, patent ductus arteriosus, ventricular septal defect, and NT-proBNP level on the 1 st, 7 th and 14 th day, respectively after birth were collected.They were divided into ROP group and non ROP group according to the results of the retinopathy screening report.The influencing factors of ROP were screened out by univariate analysis and multivariate regression analysis. Results:A total of 1 119 subjects were included, 105 infants with ROP were detected, and the prevalence of ROP was 9.4%.Among them, 12 cases of pre-threshold lesion type 1 and threshold lesions required treatment, accoun-ting for 1.07% of screened preterm infants .Univariate analysis and multivariate regression analysis revealed that gestational age, birth weight, total oxygen therapy time, and intrauterine growth restriction were all factors affecting ROP, and 2 hemodynamic related indicators, such as the level of NT-proBNP in plasma on the 14 th day after birth, and placenta previa or abruption were also related to ROP( OR=0.604, 0.647, 1.276, 2.361, 1.688 and 2.506, respectively, all P<0.05). Conclusion:The hemodynamic changes in perinatal period may be involved in the formation of ROP, and it is necessary to further clarify its mechanism.
