1.Application value of color Doppler ultrasound in the early diagnosis of diabetic nephropathy

Objective To investigate the clinical value of color Doppler in the early diagnosis of diabetic nephropathy (DN).Methods 69 cases of DN Phase I,II patients (group A) and 75 cases Phase III (B group) all underwent renal artery line Doppler flow imaging inspection .Renal aorta ( MRA) ,interlobar artery ( IRA) ,renal seg-mental artery blood flow (SRA) peak systolic velocity (Vsmax),resistance index (RI) and diastolic minimum speed ( Vdmin) and other hemodynamic parameters of two groups were analyzed .Results The Doppler spectrum renal blood flow characteristics of early renal damage were:between group A,B,the main renal artery (MRA),the kidney arterial segment Vdmin significantly reduce ,RI was significantly higher;the interlobar artery Vsmax (t=3.562 5,P=0.000 5),Vdmin (t=5.252,P=0.000 0) were significantly lower,RI was significantly higher (t=9.166 0,P=0.000 0);with the severity of renal dysfunction ,kidney color flow imaging reduce ,the kidney volume was significantly reduced in patients with stage Ⅲ.Conclusion Color Doppler ultrasound can clearly show the size of the patient′s kidney and renal artery blood flow parameters ,which has certain value in the diagnosis of early DN .
2.Clinical Observation on Treatment of 60 Cases of Primary Dysmenorrhea with Modified Shenhua Decoction plus Western Medicine

Objective To explore the efficacy of treating primary dysmenorrheal with modified Shenghua decoction plus western medicine. Methods 120 patients with primary dysmenorrhea were randomly recruited into a control group and a treatment group, with 60 patients in each group. The control group was treated with prostaglandin synthetic inhibitor, and the treatment group was treated with modified Shenghua decoction on the basis of control group. Results The total effective rate of the treatment group and the control group was 90% and 73.3%, respectively, showing significant difference (X2=4.011, P <0.05 ) . Conclusion Modified Shenghua decoction plus western medicine is effective in treating, primary dysmenorrheal.
3.The characteristics and treatment of rare embryonal tumors of central nervous system in children

Embryonal tumors of central nervous system (CNS) are highly invasive malignant tumors that mainly affect infants.Even if the current active treatment strategies are given, the prognosis is still very poor.The 2016 World Health Organization(WHO) classification integrates the genetic information of diseases with histopathology, so as to make the classification and diagnosis of these tumors more accurate.In the new classification, embryonal tumors of CNS include the most common medulloblastoma, and the rare embryonal tumor with multilayered rosettes, C19MC-altered, embryonal tumor with multilayered rosettes, not otherwise specified (NOS), medulloepithelioma, CNS neuroblastoma, CNS ganglioneuroblastoma, CNS embryonal tumor, NOS, atypical teratoid rhabdoid tumor, and CNS embryonal tumor with rhabdoid features.Further study and exploration on the biological nature of these rare brain tumors will provide scientific basis for new treatment methods.

5.TRANSGENIC RESCUE FROM FULMINANT HEPATITIS IN LEC RAT BY INTRODUCING HUMAN ATP7B cDNA

Objective To investigate the possibility of affecting transgenic rescue for Wilson disease using the human ATP7B transgenic LEC rat.Methods The 7.1kb transgene constructed with human ATP7B cDNA and chicken ?-actin promoter was introduced into the fertilized oocytes of LEC rats, an animal model of Wilson disease, by microinjection. The expressions of human ATP7B protein in the transgenic rats were detected by Western blot. The plasma AST and ALT activities, and the total bilirubin levels in transgenic rats were measured continuously from 6 to 16 weeks using non-transgenic rats and LEA rat as controls. The pathological and histochemistry changes in the liver of the transgenic rats at 13 weeks were analyzed. Results The intact and correct product derived from human ATP7B was confirmed in the liver of transgenic rats. At the age around 12 weeks, the plasma AST and ALT activities, and the total bilirubin levels in transgenic rats were significantly decreased, while the inflammatory reation in the liver of transgenic rats was much mild as compared with that of non-transgenic rats, and the granules of stained copper were less in the hepatocytes of transgenic rats. By the age of 16 weeks, the transgenic rats were phenotypically normal, and the survival rate was 100%. These data showed that the LEC rats were successfully rescued from fulminant hepatitis after introducing of human ATP7B gene. Conclusion The hepatitis in Wilson disease is directly related to the toxicity of excessive accumulated copper, which attributed to the functional deficiency of the ATP7B. Gene transfer probably is the effective method for the therapy of Wilson disease.
6.GENE THERAPY FOR HEPATIC CIRRHOSIS AND HEPATOMA IN LEC RAT BY INTRODUCING HUMAN ATP7B cDNA

Objective To investigate the possibility of affecting transgenic therapy for the hepatic cirrhosis and hepatoma in Wilson disease by human ATP7B cDNA. Methods The 7.1*!kb transgene consisting of human ATP7B cDNA and chiken ?-actin promoter was introduced into the LEC rats which is an animal model of Wilson disease by microinjection.The plasma AST and ALT activities in transgenic rats were measured continuously from weeks 17 to 30 using non-transgenic and LEA rats as controls.The histological and histochemistry changes of liver in the transgenic rats at 30 and 60 weeks of age were examined. Results The plasma AST and ALT activities in transgenic rats were kept at the relatvie lower levels from 17 to 60 weeks of age, as compared to the age-matched non-transgenic rats.By the age of 60 weeks,none of the transgenic males developed cholangiofibrosis or hepatoma,whereas all of the non-transgenic rasts had severe cholangiofibrosis at the age of 30 weeks and one male rat had hepatoma at 60 weeks.The transgenic rats were phenotypically normal,and the survival rate was 95.7%.In addition,the distribution and the numbers of the granules of stained copper in the hepatocytes of the transgenic rats did not show any significant difference between 30 and 60 weeks.Conclusion The human ATP7B successfully delayed the onset of hepatic cirrhosis,and suppressed the development of hepatoma in the LEC rats by gene transfer.The hepatic cirrhosis and hepatoma in Wilson disease may be not directly related to the copper accumulation.
7.THE EXPERIMENTAL STUDY ON THE EFFECT OF DIFFERENT DIETARY FATTY ACID COMPOSITION ON SERUM LIPID LEVEL

Objective: To study the effect of different ratios of n-6 to n-3 polyunsaturated fatty acid on serum lipid in mice, on the premise that the ratio of SFA to MUFA to PUFA is 1∶1.7∶1.2 or 1∶1∶1. Methods: Ten experimental diets were made from six oils different in fatty acid composition to raise the mice for 10 w. Then the blood lipids were determined in mice fed with different experimental diets. Results and conclusion: Based on the ratio of S/M/P 1∶1.7∶1.2, which is the current dietary fatty acid intake, n-6/n-3 at 1~5∶1 will have lower serum lipid level. Based on the ratio of S/M/P 1∶1∶1 , n-6/n-3 at 1∶1 will have lower serum lipid level. Based on the ratio of n-6 /n-3 10∶1 which is the current dietary fatty acid composition abroad , S/M/P 1∶1.7∶1.2 will have lower serum lipid level.
8.The Characteristics of the Species Distribution and Detection of Vancomycin Resistance Genotypes in Enterococci

Objective To study the characteritics of the species distribution, vancomycin resistance phenotypes, and genotypes in Enterococci in Shenzhen Second People,s Hospital. Methods Screening vancomycin-resistant Enterococci (VRE) with agar screen, and detecting the phenotypes and genotypes of vancomycin resistance or intermediate enterococci with E test and multiple PCR-restriction fragment length polymorphism. Results Among 310 Enterococcus spp strains isolated f rom clinical samples, 235 strains (75.8 %) were Enterococcus faecalis, 71 strains (22.9 %)were E. f aecium, 3 strains (1.0%) were Enterococcus gallinarum, and 1 strains (0.3%) was Enterococcus casseliflavus. 7 strains were vancomycin-resistant or intermediate to Enterococci, vanA for 2 strains were detected, vanC1 for 3 strains were detected, vanC2/3 for 1 strains was detected, 1 strains was unknown. Their phenotypes confirmed to the genotypes. Conclusion 2 strains vanA were found. It is important for clinical laboratory to detect VRE with right methods to instruct rational application of antibiotics clinically and prevent VRE prevalence.
