1.Therapeutic effect of rehmannia polysaccharide on diabetic nephropathy rat model and its effects on PPARγsignal pathway

Objective To investigate therapeutic effect and molecular mechanism of RPS on Diabetic Nephropathy rats.Methods DN rats were induced by STZ injection and grouped into model group, low-dose RPS group, middle-dose RPS group, high-dose RPS group, Rosiglitazone group and normal group.24 h urine protein,kidney weight index,blood glucose level and TG, BUN and Scr level in normal and DN rats were detected.TGF-βlevel of serum of rats in all groups by ELISA were detected.mRNA and protein expression level of PPARγ,aP2 and GLUT4 by RT PCR and Western blot were also detected.Results DN rats were induced successfully because the 24 h urine protein, kidney weight index and the levels of blood sugar and TG, BUN,Scr l and TGF-βevels, and the mRNA level of PPARγ,aP2 and GLUT4 level in DN rats increased than normal group(P<0.05).The mRNA level of PPARγ,aP2 and GLUT4 level increased(P<0.05) and other indexes decreased(P<0.05) as the doses of RPS increasing.The therapeutic effects of Rosiglitazone group was better than high dose PRS group.The protein level of PPARγ, aP2 and GLUT4 in DN rat skeletal muscle were significantly lower than normal group(P<0.05), and RPS can increase their expression level obviously.Conclusion RPS has a dose-dependent therapeutic effect on DN rats by improving the expression level of related protein in PPAR gamma signaling pathways.
2.Diagnostic Significance of Myelo-Enhanced Computerized Tomography(MECT) in Spinal Stenosis

Lumbar spinal stenosis may be defined as any type of narrowing of the spinal canal, nerve root canals or intervertebral foramina. It may be local, segmental or generalized and may be caused by the encroachment of bone or soft tissue. The narrowing may involve the bony canal alone or the dural sac or both. Myelo-enhanced computerized tomography(MECT) has been accepted as especially useful diagnostic method for spinal steonsis after its application in the orthopedic fields. The goal of this study was to analyze the diagnostic accuracy and sensitivity of MECT and to determine the prevalance of stenosis in various regions of lumbar spine. During the period of 3 years, from January 1991 to December 1993, 40 cases of lumbar spinal stenosis were analyzed, all of whom took myelography, computerized tomography and surgical treatment. The results of the study were as follows: 1. The myelographic findings were, respectively, hourglass bilateral defect 15(37.5%), complete block 14(35%) and the unilateral focal defect 5(12.5%). 2. The most prevalent from of spinal stenosis was the combined central-lateral-foraminal stenosis(47.5%) and the degenerative change in the lumbar spine was the principal etiologic factor(55%) by Arnoldi classification. 3. The involved levels were as follows:1 level 18(45%), 2 level 16(40%) and 3 level 6(15%). And among single level, the most frequent level was L5-S1 intervertebral space(30%). The diagnostic accuracies according to operative finding and above results were 83% by myelography and 97.5% by MECT. The MECT showed absolutely superior to myelography or CT and it was especially useful in lateral stenosis and multi-level stenosis.

3.Clinical Observation on Penis Carcinoma.

A clinical observation was made on 16 patients of penis carcinoma admitted to the Department of Urology, Catholic Medical College Hospital during the period from August, 1972 to July, 1982 and the following results were obtained. 1. The incidence of penis carcinoma was 0.4% of total inpatients of urologic department and 3.9% of genitourinary cancer. 2. Age distribution was from 43 to 78, showing highest incidence at 50-59 years. 3. Clinical findings were; mass 56%, Ulcer 44% and inguinal lymphadenopathy 44%. 4. Treatment were partial amputation in 18.8%, partial amputation with lymph node dissection in 43.8% and total amputation with lymph node dissection in 25%. 5. Of 11 cases in whom lymph node dissection were performed, 4 cases were positive for metastasis. Of 7 cases with palpable lymph nodes, 3 cases were positive for metastasis. In 4 cases having lymph node dissection when lymph nodes were not palpable, 1 case was positive for metastasis.

4.EXPERIMENTAL OBSERVATION OF 65Zn METABOLISM ON RATS FED ON CROPS FROM ENDEMIC AND NON- ENDEMIC DISTRICTS OF KESHAN DISEASE

Albino rats fed on crops from endemic and non-endemic districts of Ke-shan desease, were used in this experiment, which was studied by 85Zn tracer metabolic method after 8 weeks feeding. Uptake of 85Zn (ZnCl2) by blood, heart, liver, kidneys and brown adipose tissue were determined at 24 and 72 hours after injection of this tracer respectively, and its distribution was thoroughly examined. The results revealed that the uptake of 8aZn was higher in the groups fed on crops from endemic districts, particularly significant in liver, kidneys and brown adipose tissue. This may reflect the lacking or deficiency of zinc in the bodies.The regular differences of 85Zn between two groups were certainly related to the crops produced from endemic or aon-endemic districts. In other words, there should be some differences between crops from endemic and non-endemic areas. Thus more attention should be paid to the factors in endemic crops, which induced lacking or deficiency of zinc in the experimental animal and to its relationship with etiology of Keshan disease.
5.Study on oxaliplatin plus S-1 versus XELOX program in adjuvant chemotherapy of postoperative gastric cancer

Objective To investigate the efficacy of oxaliplatin plus S-1 versus the XELOX(oxaliplatin + capecitabine) program in the adjuvant chemotherapy in the patients with postoperative gastric cancer .Methods A total of 81 cases with postoperative gas-tric cancer were randomly divided into the treatment group and the control group .The treatment group (41 cases) received the chemotherapy of oxaliplatin +S-1 ,while control group was given the chemotherapy of XEOLX program .Results The 1-year and 2-year recurrence-free survival (RFS) of the treatment group and the control group were 51 .3% vs .61 .5% ,25 .6% vs .20 .5% ,there were no statistically significant differences between the two groups (P=0 .361 ,P=0 .591) .The 1-year and 2-year overall survival period of the treatment group and the control group were 64 .1% vs .69 .2% ,30 .8% vs .25 .6% ,respectively ,there were also no sta-tistically significant differences between two groups(P=0 .631 ,P=0 .615) .The mainly adverse reactions of the two groups were myelosuppression ,gastrointestinal reactions ,hand-foot syndrome ,mucositis ,peripheral nerve toxicity ,hepatic and renal function damage .The occurrence rate of hand-foot syndrome in the control group was significantly higher than that in the treatment group (P=0 .001) ,the adverse reactions were improved after symptomatic treatment .Conclusion Oxaliplatin plus S-1 and oxaliplatin plus capecitabine are therapeutic equivalence and well tolerated in the patients with postoperative gastric cancer .

7.Establishment of specialist nursing clinic for breast cancer

In order to promote rehabilitation exercises for breast cancer patients and to improve their quality of life,as well as to promote the development of nursing profession, our hospital has estab-lished a specialistnursing clinic for breast cancer since January 2016. We standardized nurses' qualification and training requirements, and performed management in patients'symptom assessment, nursingtechniqueguidance for breast cancer, sleep guidance, health education, long-term follow up, etc. After one year of implementation, symptom assessment was conducted 1283 times, rehabilitationexerciseguidance was 329 times, lymphatic drainage guidance was 297 times, sleep guidance was 159 times, health education was 591 times, sexual life with spouse guidance was 107 times, in-hospital nursing consultation was 48 times, family on-site demonstration was 29 times. The satis-faction rate was 98.4% which was higher than average level of the hospital according to outpatient investigation.
8.Clinical Study on Ankylosing Spondylitis

Ankylosing spondylitis is a chronic inflammatory disease which most frequently affects the sacro-iliac joints of young men, and also the synovial joints of the whole spine. Bony fusion of these joints and ossification along the longitudinal ligaments lead to total immobility of the vertebrae. Sixty-two cases of ankylosing spondylitis treated at the Severance Hospital from June 1980 to June 1986 were analysed clinically and the followings were obtained. 1. The average age was 38 years and the sex ratio between the male and the female was 58: 4(Male=94%). 2. The most frequent location of pain was low back (32%), and the pain was usually bilateral (81%). 3. The main stiff regions were the hip joint (35%) and the lumbar spine (27%). 4. The most frequent joint affected was the hip (44%), and the extraarticular manifestations were pleuropulmonary disease (29%), gastrointestinal disorder (15%), iritis (10%), etc. 5. The characteristic X-ray findings were sacro-iliac changes (72%), facet joint blurring (53%), and syndesmophyte (37%). 6. Laborstory findings showed increased ESR (85%), positive HLA B-27 (97%), negative rheumatoid factor (94%), and the serum immunoglobulins snd complements were not decreased.

9.The Changes in Axial and Radial Diffusivity in a Patient with Clinically Mild Encephalitis/Encephalopathy with a Reversible Splenial Lesion

Multiple studies have established that mild encephalitis/encephalopathy with a reversible splenial lesion (MERS) exhibits reversible diffusion restriction in the white matter, including the splenium. There have been a few previous reports of the change in fractional anisotropy (FA) of MERS cases. Herein, we report the longitudinal changes in axial and radial diffusivity (AD and RD), and FA in a 15-year-old boy patient with MERS. Our case demonstrated that a MERS lesion had a significant drop of AD in the early period and gradual recovery. On the contrary, RD did not show any significant change.
