1.Correlation between the dosage of premixed insulin analogue treatment and continuous subcutaneous insulin infusion treatment in type 2 diabetes

To observe the patients with type 2 diabetes who were injected premixed insulin analogues after short time continuous subcutaneous insulin infusion( CSII ) treatment.It was found that the patients needed premixed insulin analogue injection 2 times daily when the dosage of CSII was less than 45 U/d,and 3 times daily when the dosage was greater than 45 U/d.The relationship of dosages between premixed insulin analogue treatment ( Y ) and CSII treatment(X) was calculated by following equations:2 times daily premixed insulin analogue treatment:Y =13.093 +0.395X,3 times daily premixed insulin analogue treatment:Y =23.114+0.405X.
3.Clinical Observation of Milrinone Injection in the Treatment of Hypertensive Heart Disease Complicated with Heart Failure

OBJECTIVE:To observe therapeutic efficacy and safety of Milrinone injection in the treatment of hypertensive heart disease complicated with heart failure. METHODS:A total of 120 patients with hypertensive heart disease complicated with chronic heart failure were randomized into control group and observation group according to random number table,with 60 cases in each group. Control group received routine therapy,including blood pressure controlling,anti-heart failure and other symptomatic treatment;observation group was additionally given Milrinone injection 0.3 mg/kg,continuous pump,qd. A treatment course lasted for 5 d. Both groups received one course of treatment each month for consecutive 6 months. Both groups were followed up for one year. 6 min walking distance(6MWD),BNP,LVEF,CI,LVEDD,heart rate,sitting systolic blood pressure(SiSBP)and sitting diastolic blood pressure(SiDBP),re-hospitalization rate and cardiovascular mortality were observed in 2 groups before treatment, after 3,6 months and 1 year of treatment;therapeutic efficacy of heart function and the incidence of ADR were compared between 2 groups. RESULTS:Before treatment,there was no statistical significance in 6MWD,BNP,LVEF,CI,heart rate,SiSBP and SiDBP between 2 groups;after 3 months,6 months and 1 year of treatment,6MWD,LVEF and CI of 2 groups were increased significantly,while BNP,LVEDD,resting heart rate,SiSBP and SiDBP were decreased significantly;the observation group was significantly better than the control group,with statistical significance(P<0.05). Total response rate of heart function was 90.00%in observation group,which was significantly higher than 63.33% in control group,with statistical significance(P<0.05). After 1 year of follow-up,re-hospitalization rate of observation group was significantly lower than that of control group,with statistical sig-nificance (P<0.05);there was no statistical significance in cardiovascular mortality (P>0.05). There were no statistical signifi-cance in the incidence of ADR between 2 groups (P>0.05). CONCLUSIONS:Milrinone injection shows good efficacy in the treatment of hypertensive heart disease complicated with heart failure,can significantly reduce BNP and re-hospitalization rate as well as improve prognosis with good safety.

5.Study on the time selection of pregnancy and delivery in women with systemic lupus erythematosus

Objective To analyze the appropriate time selection of pregnancy and delivery in women with systemic lupus erythematosus (SLE ). Methods Twenty-nine pregnancies in women with SLE in our hospital from 1998 to 2003 were retrospectively analyzed regarding the selection of appropriate time of pregnancy and delivery. Results All patients did not take any cytotoxic medicine for at least 6 months before pregnancy. Twenty-three conceptions occurred when SLE was inactive for at least 1 year. Two conceptions occurred when SLE was active without doctors' agreement SLE was diagnosed during pregnancy in the remaining 4 cases. The condition of all patients fluctuated and the gestational time at delivery ranged from 30 to 38 weeks after we modified the doses of glucocorticoid (prednisone). Among totally 29 living neonates, eight were premature neonates, three were FGR and one had serious congenital heart disease. Two neonates died of complications in early stage of neonatal period. None of the 29 neonates from all patients had neonatal lupus. Conclusion Pregnancy safety will be improved obviously if the condition of SLE is controlled and the patients are given reasonable doses of glucocorticoid and intensive monitoring. If pharmacotherapy does not work well and the condition threatens the safety of mother and fetus, or the fetus has matured, termination of pregnancy should be done on time, which reduces maternal complications and improves the perinatal mortality rate. The gestational time should be 34 to 38 weeks.
6.Progress of management and standardization strategy research in diabetics with insulin pump therapy outside hospital

The role of insulin pump in the treatment of diabetes is becoming more and more significant. Based on the systematic literature review, collation and analysis, this article introduces the management of insulin pump outside the hospital from the development trend of insulin pump, application and common problems. In order to enhance the therapeutic satisfaction of patients and improve their quality of life, our aims are to put forward the strategy of standardized management and provide a reference for establishing and improving the standardized management system of insulin pump therapy outside hospital.

Petrochemical-derived plastics have become a source of pollution for decades, and finding alternative plastics that are
environmentally friendly has become a matter of urgency. Polyhydroxyalkanoate (PHA), a biopolyester synthesized by
microbial cells, has properties that make it suitable as a biodegradable plastic material. The diversity of PHA makes it
applicable to a wide range of products, from packaging to biomedical devices. The main challenge in commercialization
of PHA is the cost of production. Although many studies have been focused on obtaining high yields of PHA, up until
now, there is no absolute definition of efficient production of PHA, as there are many factors that could contribute to the
efficiency of a process. Efficiency in PHA recovery also contributes to the commercial viability of PHA production. This
review focuses on the efficiency of PHA biosynthesis from several aspects relating to the criteria for efficient production.
The development of new strategies for improved production, including utilization of low cost carbon sources, genetic
modification of PHA-producing microbes, and fermentation strategies are discussed here. Advances in recovery of PHA,
as well as the potential of biological recovery techniques, are also highlighted in this review.

Evidence-based medicine was applied in the clinic teaching and ward inspection of superficial bladder cancer as far as no consensus was reached on its treatment.Some preferred to total cystectomy while others favored transurethral resection of bladder tumor and bladder instillation.In order to solve this problem,evidence-based medicine was applied in our clinic teaching procedure.Students were asked to read relative papers and draw the final conclusions with evidence-based medicine.Clinical guidance was provided for the students to promote clinical application.

10. Evaluation of 99mTc-labeled PSMA-SPECT/CT imaging in prostate cancer patients who have undergone biochemical relapse

Using conventional imaging modalities, it is difficult to detect recurrent lesions in prostate cancer patients who have undergone biochemical relapse, especially in patients with low prostate-specific antigen (PSA) levels. We retrospectively reviewed the files of fifty patients with histopathologically confirmed prostate cancer who underwent 99mTc-labeled prostate-specific membrane antigen (PSMA) single-photon emission computed tomography (SPECT)/computed tomography (CT), magnetic resonance imaging (MRI), and bone scan within a 30-day period. PSMA-SPECT/CT indicated metastatic lesions in 39 patients and had a higher detection rate (78.0%) than bone scan (34.0%) or MRI (40.0%). The diagnostic efficiency of PSMA-SPECT/CT imaging for bone and lymph node metastases (50.0% and 42.0%) was better than bone scan (34.0% and 0.0%) or MRI (24.0% and 20.0%). PSMA-SPECT/CT provided a higher detection rate at serum PSA levels of ≤1 ng ml-1, 1-4 ng ml-1, 4-10 ng ml-1, and >10 ng ml-1. No correlation was found between Gleason score, PSA level, and the tracer tumor/background ratio of metastatic lesions. With the aid of PSMA-SPECT/CT imaging, the therapeutic strategy was changed for 31 patients, and this may have enhanced their clinical outcome. In conclusion, PSMA-SPECT/CT imaging could detect more metastatic lesions and achieve a higher detection rate than conventional imaging modalities at different serum PSA levels in prostate cancer patients who had undergone biochemical relapse.
