ObjectiveTo investigate the protection of renal function and the prevention of acute renal failue(ARF) in perioperative period in obstructive jaundice(OJ) patients. Methods In experimantal group, short time parenteral nutrition(PN) was given preoperatirely.Small dosage of dopamine was intravenously dripped.and rapid infusion of 20% mannitol periodically was done.In control group routine treatment was done. Postoperative blood BUN and Cr dynamic changes were examined and occurrence rate of ARF were recorded. 2.Behavioral study of internet gaming addiction in adolescents

Objective To explore the facilitative mechanism of the behavioral improvement resulting from gaming experience in internet gaming addicts. Methods Seventeen gaming addicts and twenty normal subjects were involved in the study, and their behavioral performance were evaluated by using the revised paradigm of event-related design. Results 1. Mean reaction times (RTs) under valid and invalid cue conditions were ( (614.27 ± 85.25)ms,(645.4±80.24)ms)and ((710.24 ± 131.42)ms,(765.53 ± 142. 53)ms) respectively. There were statistically significant differences of mean RTs within each group (P < 0. 05 ) and between the two groups (P < 0.01 ). 2. There were statistically significant differences of mean RTs on three cue levels between addiction group (online game picture(643.65 ±90.82) ms,neutral face picture(616. 73 ±82. 19)ms,neutral picture(629. 21 ± 78.20)ms) and control group(game picture (744. 54 ± 140. 42) ms, neutral face picture ( 727. 98 ± 142.26)ms, neutral picture(741. 13 ±138. 21)ms, P<0.05).3. There were statistically significant differences on trends of mean RTs on three cue levels of addiction group between valid cue condition ( online game picture (619. 02 ± 91.00) ms,neutral face picture (604.90 ±90. 71 )ms,neutral picture (618.87 ±77.85)ms) and invalid cue con-dition( online game picture ( 668. 28 ± 86. 27 ) ms, neutral face picture ( 628. 56 ± 73. 53 ) ms, neutral picture (639.54 ±79. 54) ms, P<0. 05). Conclusion Both exogenous and endogenous attention orientation are improved in internet gaming addicts. Enhancement of endogenous attention function resulting from gaming experience might be greater than that of exogenous attention.
3.Down-regulation of ObR by lentivirus-mediated RNA interference inhibits growth of MCF-7 cells xenograft in a nude mouse model

ObjectiveTo investigate the inhibitory effect of lentivirusly-mediated ObR-siRNA on transplanted MCF-7 human breast cancer cells by intratumoral injection.MethodsA model of subcutaneous implanted tumor was generated through injecting MCF-7 human breast cancer cells into the nude mice.Thirty established mice with MCF-7 breast cancer cells xenograft were divided into 3 groups randomly,and mice in the experimental group were intratumorally injected with ObR-siRNA lentivirus,while the negative control group and blank control group mice were injected with the same dose of negative lentivirus and normal saline.All mice were subcutaneously injected with recombinant human leptin around the tumor site once a day.Tumor size was blindly measured every other day and the mRNA expression and protein expression levels of ObR in each group were determined.ResultsKnockdown of ObR-treated xenografted nude mice with a high leptin microenvironment was successfully established.Local injection of ObR-siRNA lentivirus significantly suppressed the established tumor growth in nude mice(P < 0.01,P <0.01 ).Real time-PCR and Western blotting showed that the mRNA and protein expression of ObR was decreased in the ObR-siRNA lentivirus group( P < 0.01,P < 0.01 ).ConclusionsIntratumoral injection of recomhinant ObR-siRNA lentivirus inhibits the growth of MCF-7 cells xenografts in the nude mice,suggesting that ObR might represent a therapeutic target in the genotherapies of human breast cancer.
4.Effect of insulin pump in treatment of type two diabetic patients with pulmonary infections

Objective To study the effect of continuous subcutaneous insulin injection(CSII) in treatment of type two diabetic patients with pulmonary infections. Methods From January 2001 to December 2005,83 type two diabetic patients with pulmonary infections were randomized to group CSII (42 cases) and group MSII(41 cases). The period of normalization of the blood glucose and cure of the pulmonary infections, cost and morbidity of low blood glucose were observed. Results Blood glucose of all patients in two groups reduced significantly. Significant difference was observed in the period of cure of the pulmonary infections, usage amount of insulin and morbidity of low blood glucose. In group CSII HbA1c, CRP and CHOL reduced significantly. Fasting CRP and high density lipoprotein-C (HDL-C) were remarkably increased. Conclusions Consecutive subcutaneous insulin pump injection is able to control the blood glucose better, to correct metabolic disorder, reduce the cost and hospital day and decrease the morbidity of low blood glucose. CSII is a better of method.
5.Clinical effect of recombinant adenovirus containing thymidine kinase suicide gene in preventing postoperative recurrence of hepatocellular carcinoma

Objective To investigate the clinical effect of surgical operation combined with gene therapy in preventing postoperative recurrence of hepatocellular carcinoma (HCC).Methods A total of 102 patients with single HCC (TNM stage 1-2,tumor diameter < 10 cm) who were admitted to Beijing YouAn Hospital,Capital Medical University,from July 2006 to February 2013 were enrolled,and among these patients,60 underwent the gene therapy with recombinant adenovirus containing thymidine kinase suicide gene (ADV-TK) before and after surgery (gene group) and 42 underwent surgical resection alone (surgery group).The patients were followed up regularly after surgery to observe postoperative recurrence.The t-test was used for comparison of continuous data between groups,and the chi-square test was used for comparison of categorical data between groups.The log-rank test was used for the analysis of cumulative recurrencc rate,and univariate and multivariate Cox regression analyses were used to identify influencing factors for recurrence rate.Results The 1-,3-,and 5-year recurrence rates of tumor were 13.8%,33.7%,and 47.7% in the gene group and 18.5%,53.2%,and 69.2% in the surgery group,and there was a significant difference between the two groups (x2 =2.643,P =0.041).The gene group had a significantly higher proportion of patients with pyrexia after surgery than the surgery group,and there were no significant differences in the incidence rates of other complications and length of hospital stay between the two groups.The multivariate analysis showed that gene therapy was an independent influencing factor for cumulative rumulative recurrence rate (odds ratio =2.752,95 confidence interval:1.1 64-4.251,P =0.038).Conclusion Gene therapy conbined with surgical resection can effectively reduce postoperative recurrence of tumor,and therefore,it holds promise for clinical application.
6.Clinical analysis of thyroid carcinoma in 110 children and adolescents

Objective To study the clinical and pathologic factors for prognosis of children and adolescents with thyroid carcinoma.Methods Clinicopathological data of 110 children and adolescents with thyroid carcinoma at the Affiliated Tumor Hospital of Zhengzhou University from January 1984 to March 2008 were collected and reviewed in this study.Univariate analysis was performed by using Log-rank test to determine predictors of survival,and multiva-riable analysis was performed by a Cox regression model.Results The overall 5- year and 10 -year survival rates were 95.5%,88.2%,respectively.Gender and histopathologic typing were not related to prognosis.The 5 -year and 10-year survival rates in group ≤12 years old were 91.7% and 81.3%,and those in group >12 years old were 98. 4% and 93.5%,respectively.The 5-year and 10-year survival rates in stage T1 and T2 were 100.0%,in stage T3 were 100.0% and 66.7%,respectively,and those in stage T4 were 58.3% and 0.The 5-year and 10-year sur-vival rates in N0 group and N1a group were 100.0%,and those in N1b group were 93.4% and 82.9%,respectively. For patients without the distant metastasis group,the 5-year and and 10-year survival rates were all 96.9%,which were significantly higher than those in the distant metastasis group(84.6%,23.1%).The 5-year and 10-year sur-vival rates in groupⅠwere all 97.8%,those in groupⅡwere 84.6% and 23.1%,respectively,and those in groupⅣwere all 75. 0%.The 5-year and 10-year survival rates in the solitary lesions group were 98.7%,in the multiple le-sions group were 88.2% and 64.7%,respectively.For differentiated thyroid carcinoma,the 5-year and 10-year sur-vival rates in low,and medium risk groups were all 100.0%and those in high risk group were 83.3% and 50.0%,re-spectively.There was a significant relationship between survival and age group,T stage,N stage,M stage,TNM stage, the number of lesions,and risk stratification(all P<0.05).The Cox regression analysis showed that T stage and M stage were the independent prognostic factors.Conclusions The primary tumor and distant metastatic are important factors affecting the prognosis of children and adolescents.Earlier diagnosis for primary tumor and aggressive manage-ments for metastatic lesions are critical to the survival rate of children and adolescents with thyroid carcinoma.
7.Characterization methods of flowability of traditional Chinese medicine powders.

OBJECTIVETo characterize the flowability of traditional Chinese medicine (TCM) powders by using appropriate methods.
METHODWith highly flowable direct compression excipients and Chinese traditional medicine powder as raw materials, the flowability of material powders is determined by the Carr method and the Jenike method.
RESULTThe Carr method and the Jenike method for the determination of the flowability of Chinese herb extract powder and direct-pressing excipients have no obvious difference. But the flowability of Chinese herb extract powder is not as good as direct compression excipients.
CONCLUSIONFrom the characterization results of both methods, we can find that the Carr method better reflects the actual flowability of TCM extracts, while the Jenike method can be used for guiding the design of the hopper device.

8.Digital assisted chimeric deep circumflex iliac artery perforator flap in the reconstruction of mandibular composite defects

To evaluate the clinical effect of digital assisted chimeric deep circumflex iliac artery perforator flap (DCIAPF) in the reconstruction of mandibular composite defects. Methods From January, 2018 to January, 2019, 6 cases of mandibular tumor patients with postoperative defect within side were treated. Preoperative CTA was used to evaluate the deep branches of spin iliac artery.Digital simulation software and 3D printing technolo-gy was taken, vascularized iliac flap of the design guide of bone was made, and the rebuilding effect was simulated. DCIAPF was used to repair the defect of lower jawbone. The donor sites were sutured directly. The patients were fol-lowed-up in outpatient department for 3-6 months to evaluate the recovery of the patient′s shape, jaw height and oc-clusal function, as well as the complications in the donor area. Results Postoperation pathological examination re-sults: ameloblastoma in 2 cases, 4 cases of gingival cancer. The length of cut out ilium was 6.0-13.0 cm, carrying the flap area of 3.0 cm×1.0 cm-6.0 cm×5.0 cm.Six cases of DCIAPF and iliac bone flap survived.The shape, mandibular height and occlusal function were satisfactory.And no obvious complications were found in the donor area. Conclu-sion The blood supply of DCIAPF is rich with enough bone mass and height. The position of terminal skin perfora-tors is invariant. The complications of donor sites is less. With the help of digital technology, the accuracy of mandibular defect repair and the 3-dimensional wound repair can be realized, and provides an advantage condition for subsequent dental implant.It is one of the ideal method of reconstruction of mandibular defect.

Objective:To investigate the clinical application of 3-dimensional CT angiography(3D-CTA) in the repair of mandibular defects with deep circumflex iliac arterial flaps.Methods:From January 2019 to January 2020, 5 patients with postoperative defects of mandibular tumors were treated in the Department of Stomatology of the First Affiliated Hospital of Bengbu Medical College, including 2 males and 3 females. The ages of the patients ranged from 30 to 70 years, with an average of 52 years. CT angiography of both lower limbs was performed before operation. Data were processed using the CT workstation. According to the size and shape of the defect in the recipient site, the 3D model of the deep circumflex iliac arterial flap(DCIA) was constructed. the contour of the DCIA was observed and measured, and the data was recorded, including the diameter of the vassel at the starting point, the angle between the line from the starting point to the anterior superior iliac spine and the line between the bilateral anterior superior iliac spine, and the distance from the starting point to the anterior superior iliac spine. According to the 3D model, a composite tissue flap pedicled with DCIA was designed and prepared for mandibular defect repair. The actual situation during the procedure was compared with the preoperative 3D model simultaneously.Results:The diameter of the DCIA at the starting point was (2.30±0.24) mm. The angle between the line from the starting point to the anterior superior iliac spine and the line between the bilateral anterior superior iliac spine was 39.3°±6.1°, and the distance from the starting point to the anterior superior iliac spine was (5.83±0.24) cm. The shape and position of the DCIA were basically consistent with the three-dimensional model. All the flaps survived well after operation. All the patients were followed up for 3 to 6 months, the bilateral maxillofacial region were basically symmetrical. The postoperative imaging examination showed that the iliac osteocutaneous flap healed well, and the height and width of mandibular alveolar ridge were restored satisfactorily. There were no complications such as numbness in the donor site, abdominal hernia and gait abnormality.Conclusions:The 3D model of the DCIA produced by 3D-CTA can accurately locate the position and route of the vessel, clearly display the three-dimensional spatial relationship between the skin, muscles and bones around the target blood vessel, and significantly reduce the complications and operative time.
