1.Relationship between plasminogen activator inhibitor 1 gene polymorphism and the level of endothelium-de- pendent vasodilatation in patients with type 2 diabetes

Objective To investigate the relationship between the polymorphisms of plasminogen activator inhibitor-1(PAI-1)gene and the level of endothelium-dependent vasodilatation (EDF) in patients with type 2 diabe- tes(T2DM).Methods The polymorphisms of PAI-1(4G/5G)gene were determined by polymerase chain reaction (PCR)and the EDF was assessed by non-invasive hish resolution B-mode ultrasonography in 66 T2DM patients and 33 controls.Plasma plasminogen activator inhibitor type-1(PAI-1)and fibrinogen(Fg)were measured.Results Frequencies of PAI-1 alleles and genotypes in T2DM patients and controls were all in accordance with the Hardy Weinberge quilibrium,without significant differences between T2DM patients and controls(P>0.05).PAI-1 and Fg were significantly higher in T2DM patients than in controls.There were significant differences in EDF and PAI- lamong different genotypes of PAI-1(P<0.05).Conclusion The PAI-1 genotype may affect EDF in T2DM pa- tients.
2.Effect of levosimendan on pulmonary artery pressure in patients with pulmonary hypertension undergoing mitral valve replacement

Objective To investigate the effect of levosimendan on pulmonary artery pressure in patients with pulmonary hypertension undergoing mitral valve replacement.Methods Twenty-four ASA Ⅱ or Ⅲ and NY-HA class Ⅱ or Ⅲ patients,aged 35-60 yr,with mean pulmonary artery pressure (MPAP) > 30 mm Hg,undergoing mitral valve replacement were randomly divided into 2 groups (n =12 each):control group (group C) and levosimendan group (group L).In group L,a loading dose of levosimendan 24 μg/kg was injected intravenously after aortic unclamping,followed by infusion of levosimendan at a rate of 0.2 μg· kg-1 · min-1 until 1 d after operation.Group C received the equal volume of normal saline.HR,MAP,MPAP,pulmonary capillary wedge pressure (PCWP),cardiac index (CI) were recorded at 5 min after induction (T0),at the end of CPB (T1) and at 1 h after operation (T2),and the pulmonary vascular resistance (PVR) and rate-pressure product (RPP) were calculated.The improvement in pulmonary hypertension was recorded.Results PCWP was significantly lower and CI higher at T1,2 in both groups,and HR was significantly higher at T1,2 and MPAP lower at T2 in group C,and MPAP and PVR were significantly lower at T1,2 in group L than at T0 (P < 0.05).HR,MPAP and PVR were significantly lower and CI was significantly higher at T1,2,RPP was significantly lower at T2 and the improvement in pulmonary hypertension was higher in group L than in group C (P < 0.05).Conclusion Levosimendan can improve pulmonary hypertension without increasing the myocardial oxygen consumption and with a significant increase in myocardial contractility in patients with pulmonary hypertension undergoing mitral valve replacement.
3.Study on quality standard for Xiaozhi Tablets

AIM: To establish the quality standard for Xaozhi Tablets(Lasiosphaera seu Calvatia, Rhizoma Dioscoreae Cirrhosae, Radix Sophorae Tonkinensis, Concha Ostreae, etc.). METHODS: Rhizoma Dioscoreae Cirrhosae and Radix Sophorae Tonkinensis were identified by TLC. Matrine was determined by HPLC. RESULTS: Rhizoma Dioscoreae Cirrhosae and Radix Sophorae Tonkinensis could be detected by TLC. Matrine showed a good linear relationship in the range of 0.2040~2.040?g and the average recovery was 97.64%, RSD was 1.49%. CONCLUSION: The methods are simple, sensitive and accurate with good reproducibility. The methods may be used for the quality control of Xiaozhi Tablets.
4.Association of body composition and β-cell function in obese females with normal glucose metabolism

Objective To explore the relationship between body composition and β-cell function in obese females with normal glucose metabolism. Methods Seventy-five obese women with normal blood glucose and without family history of diabetes were investigated. They were assigned to 4 groups based on body mass index (BMI). Body fat content was measured by dual-energy X-ray absorptiometry (DEXA), and intravenous glucose tolerance test (IVGTT) was performed. The acute insulin response (AIR), the area under the curve (AUC) of insulin (AUCins) and homeostasis model assessment (HOMA) for β-cell function (HOMA2-% B) were calculated. Insulin resistance index(HOMA2-IR) and the ratio of AUCins to AUC of glucose (AUCins/AUCglu) were calculated to assess insulin resistance. Results Women with higher BMI appeared to have more total body fat content and trunk fat content. The similar distribution was also found in other parameters, including the plasma glucose levels at 0 and 10 min, AUCins, AIR, AUCins/AUCglu and the difference of insulin level between 0 and 10 min [INS (10-0)] during IVGTF. AUCins, AIR, AUCins/AUCglu and [INS (10-0)] were positively correlated with the age, BMI,total body fat content and trunk fat content. After adjustment of age, the trunk fat content was independently associated with the AIR in a good linear manner. Conclusion The obese females show change in body composition with more trunk fat content. They show significant insulin resistance with compensated elevation of insulin secretion. Body composition assessment is a valid and more accurate method than BMI and waist circumference in predicting early damaged β-cell function in obese patients.
5.Digital anatomy of nucleus accumbens in the human brain

Objective To explore the locating, parameter measurement and 3D display of nucleus accumbens in human brain in terms of digital anatomy .Methods The raw data of the head specimen of a 45-year-old male adult with 0.5mm as the section spacing was collected by using digital milling machine .Three hundreds images of continual cross sections containing brain were chosen and the segmentation of the caudate nucleus , putamen and nucleus accumbens was accomplished with Photoshop CS .The nucleus accumbens on the images of continual coronal section reconstruction were distinguished according to Harvard Medical School ’ s segment method to calculate the volume of nucleus accumbens and collect the correlative location information .The caudate nucleus , putamen and nucleus accumbens were 3D visualize with the software of Amira 3.1.1.Results The nucleus accumbens , the adjoining structure and the lesion target of nucleus accumbens were all clearly visible .The left nucleus accumbens volume was 972.5mm3 , and the right was 830.6mm3 .The 3D coordinate value was the left ( -11.0, 24.4, 1.3) and the right (9.3, 23.9, 1.7).Conclusion The digital anatomy of nucleus accumbens can distinctly display the nucleus accumbens , form and confirm it ’ s volume, location and adjoining area , which is useful to clinician .
6.Effects of oxidized low density lipoprotein and vitamin E on the levels of IL-6,IL-8 and TNF-? in cultured human umbilical vein endothelial cells

AIM: To investigate effects of OX-LDL and VitE on the levels of IL-6,IL-8 and TNF-? in human umbilical vein endothelial cells(HUVEC). METHODS: Human umbilical vein endothelial cells were obtained by in vitro culture. HUVEC treated with or without Vit E was incubated with OX-LDL, and the levels of IL-6, IL-8 and TNF-? were determined by enzyme-linked immunosorbent assy technique. RESULTS:50 ?g/L,100 ?g/L, 200 ?g/L OX-LDL induced the release of IL-6,IL-8 and TNF-? by HUVEC in a dose-dependent manner. Compared with the control group , the levels of IL-6 and IL-8 were significantly increased at 6-12 h of stimulation with OX-LDL . Maximal levels of IL-6 and IL-8 occurred after 24-36 h, reaching a plateau maintained for at least 48 h. TNF-? rose after 2-6 h in HUVEC, and reached a maximum after 12 h. In contrast to IL-6 and IL-8, TNF-? declined after 48 h. However, when VitE (50 mg/L,100 mg/L,200 mg/L)was added, it can significant inhibited the release of IL-6, IL-8 and TNF-? in a dose-dependent manner, and after 48 h these cytokines have no diference between OX-LDL+VitE groups and OX-LDL groups. CONCLUSION: OX-LDL can obviously stimulate the production of IL-6,IL-8 and TNF-? in vascular endothelial cells, which can significantly be inhibited by VitE in a short time.
7.Antitumor Effects of Soluble TRAIL in Human Hepatocellular Carcinoma

The therapeutic potential of soluble TRAIL (sTRAIL) in hepatocellular carcinoma(HCC) was studied. The expression of TRAIL receptors was detected in 60 HCC tissues, 20 normal liver samples and 2 HCC cell lines (HepG2 and SMMC-7721) by in situ hybridization. Before and after HepG2 and SMMC-7721 were treated with sTRAIL protein with various concentrations,the apoptosis rate was observed by using flow cytometry and in situ terminal deoxynucleotidyl tranferase (TdT) labeling. The results showed death receptor 4 (DR4) and DR5 were expressed in 60 HCC tissues and 20 normal liver samples, while the expression intensity of DR in HCC tissues was stronger than in normal liver samples. DcR1and DcR2 were not detectable in 54 (90 %) and 25 (41.7 %)HCC tissues, while in 20 normalliver samples, DcR was detectable. The high expressionof DR and low expression of DcR in HCC tissues were significantly differed from the low expression and high expression in normal liver samples. The expression of DR5, DR4 and DcR2 in both HCC cell lines was detectable, but the expression of DcR1 was not detectable. The expression of DR in HCC tissues was related to the differentiation and grades of HCC. In the poor differentiated HCC, the expression of DR was decreased (P<0.01). The expression of DR in Ⅲ/Ⅳ grades was significantly lower than that in Ⅰ / Ⅱ grades (P<0.05). The expression of DR was not related to gender, age, HBsAg, AFP, tumor sizeand metastasis. The expression of DR in the HCC drugresistant lines was decreased. After treatment with TRAIL (100 ng/ml) for 24 h, the apoptosis rate of HCC cells, Jurkat cells and human cholangiocarcinoma cell line QBC939 was 10 %, 70 %,50 % respectively. It was suggested that the TRAILR expression is prevalent in HCC with different expression patterns of different receptor types. HCC is resistant to TRAIL-mediated apoptosis.The treatment of TRAIL alone has a limited effect on inducing apoptosis of HepG2 and SMMC-7721.
8.Retrospective study on the use of epinephrine in patients with drug-induced anaphylactic shock in Chongqing from 2015 to 2022

OBJECTIVE To evaluate the rationality of epinephrine in the treatment of drug-induced anaphylactic shock, and to provide a reference for further standardizing the treatment measures of anaphylactic shock. METHODS According to the relevant data of the reports of severe adverse drug reaction (ADR) of drug-induced anaphylactic shock provided by Chongqing ADR Monitoring Center from 2015 to 2022, the selection of treatment drugs, and the application of epinephrine in anaphylactic shock were analyzed retrospectively; the clinical outcomes of anaphylactic shock with different epinephrine administration methods were investigated. RESULTS A total of 1 415 cases of severe ADR related to drug-induced anaphylactic shock were reported, with a male-to-female ratio of 1.04∶1; the drugs that caused allergic shock mainly included anti-infective drugs (47.92%), TCM injections (9.12%); the patients who suffered from drug-induced anaphylactic shock within 10 min after medication accounted for 43.96%; 97.24% of patients were cured or improved, and 2.76% of patients died or did not been improved. Among 1 415 patients, 63.39% of patients were treated with epinephrine, and the patients who preferred epinephrine treatment accounted for 53.14%; the intramuscular injection, subcutaneous injection, intravenous injection and intravenous drip accounted for 33.78%, 30.32%, 25.75% and 1.23%, respectively. The initial dose range of epinephrine was 0.01-10 mg, and the most frequent single dose was 1 mg (44.70%). Excessive single doses of intramuscular injection, subcutaneous injection and intravenous injection accounted for 51.03% (148 cases), 53.13% (136 cases) and 91.47% (193 cases) respectively, and the risk of overdose in intravenous injection was higher (P<0.05). The patients receiving initial treatment with epinephrine had a higher improvement rate/cure rate than those who did not use epinephrine (98.14% vs. 96.23%, P=0.029); the patients who preferred epinephrine had a higher improvement rate/cure rate than those who did not preferred epinephrine (98.14% vs. 95.17%, P=0.031); the improvement rate/cure rate of patients receiving intramuscular injection of epinephrine was higher than those without intramuscular injection (99.01% vs. 96.69%, P=0.038). CONCLUSIONS There are some unreasonable phenomena in the treatment of drug-induced anaphylactic shock, such as inappropriate selection of drugs, insufficient use of epinephrine, delay of administration, inappropriate route of administration and excessive single dose.
