1.Concomitant cardiac valve replacement and coronary artery bypass grafting

BACKGROUND:Heart valve surgery combined with coronary artery bypass grafting has been gradual y used for treatment of coronary heart disease combined with cardiac valve disease.
OBJECTIVE:To retrospectively summarize the experience of combined coronary artery bypass grafting and valvular procedure.
METHODS:Total y 51 patients who underwent combined heart valve surgery and coronary artery bypass grafting were retrospectively analyzed, including 10 cases with aortic valve replacement, 14 cases with mitral valve replacement, eight cases with mitral valve replacement combined with tricuspid annuloplasty, four cases with aortic valve and mitral valve replacement, three cases with aortic valve and mitral valve replacement combined with tricuspid annuloplasty, seven cases with mitral valvuloplasty, and five cases with mitral valvuloplasty combined with tricuspid annuloplasty.
RESULTS AND CONCLUSION:Biovalve replacement was performed in nine patients, mechanical valves replacement in 31 cases, mitral valvuloplasty in 11 cases and tricuspid annuloplasty in 16 cases. There were total y 109 bypass graft vessels, and the average number of coronary artery bypass grafts was (1.92±0.73) branches. Four cases died within 30 days postoperatively, and 47 patients were successful y discharged from the hospital. Forty-five of 47 discharging patients were fol owed for 3-48 months. One case died of cerebral infarction within 6 months postoperatively, and another case died of cardiac dysfunction over 1 year after operation. The heart function of 45 survival patients was significantly improved. The comprehensive analysis showed that improving the heart function preoperatively, strengthening myocardial protection, shortening operation and myocardial ischemia time, and complete revascularization are the key factors for successful operation.
2.A Study on Method for Building Human Head Parameter Model

Objective To find a method for building a human head parameter model. Method Raw data of human head were acquired and filtered, a linear regression equation was formulated, and a parameter model was established. Result A human head parameter model was built. Conclusion A human head parameter model can be built by using the linear regression equation of human head to adjust the initial CT data, and a three-dimensional and human head parameter model can be built by OpenGL.
3.Analysis on Correlation Between TCM Syndrome Types and Cardiovascular Prognosis Risk Factors of Elderly Hypertensive Patients

Objective To observe the regularities of distribution of TCM syndrome types and correlation between TCM syndrome types and cardiovascular prognosis risk factors of elderly hypertensive patients in three different urban and rural communities.Methods Totally 299 elderly hypertensive patients who conformed to the hypertensive diagnostic criteria from 896 respondents which participated in the health monitoring questionnaire drafted by health management team of China Academy of Chinese Medical Sciences were screened. The syndrome types were differentiated according to Guiding Principle of Clinical Research on New Drugs of Traditional Chinese Medicine (for Trial Implementation). The levels of some risk indicators, such as blood glucose, blood lipid, CREA, ET, Hcy and mALB were measured by radioimmunoassay, enzymatic cycling assay and other methods. Age, gender, BMI and pulse pressure were recorded.Results The TCM syndrome types of 299 elderly hypertensive patients were deficiency of yin and yang type (74 cases), hyperactivity of yang due to yin deficiency type (65 cases), blood stasis syndrome type (62 cases), stagnation of phlegm-dampness type (61 cases), hyperactivity of liver-fire type (37 cases). Single factor analysis showed that there existed statistical significance among BMI, FBG, GHbA1c, CREA, TC, LDL-C, ET, Hcy and mALB in the 5 TCM syndromes (P<0.05,P<0.01), and the statistical significance was more apparent in the stagnation of phlegm-dampness type, blood stasis syndrome type and hyperactivity of yang due to yin deficiency type. Discriminant analysis showed BMI and Hcy have certain reference value for the diagnosis of TCM syndrome types in elderly hypertensive patients. Binary-Logistic regression analysis showed that the course of disease had a positive correlation with syndrome of deficiency of yin and yang.Conclusion Compared with other types of elderly hepertensive patients, stagnation of phlegm-dampness type and blood stasis syndrome type are more closely related to hypertension cardiovascular prognosis risk factors.
4.Literature Research of Compatibility Laws of Homologous Chinese Medicine of Medicine and Food in Prevention and Treatment of Alzheimer Disease

Objective To analyze the compatibility laws of homologous Chinese medicine of medicine and food in prevention and treatment of Alzheimer disease (AD). Methods We screened the clinical literature for the treatment of AD during 1988-2012 in TCM Clinical Diagnosis and Treatment Database, CNKI, VIP and Wanfang database. The nature, flavor, channel tropism, high frequency drug pair and compatibility laws of Chinese medicine and homologous Chinese medicine of medicine and food in prevention and treatment of AD were analyzed by frequency statistics and association rules. Results Two hundred and twenty-two clinical literatures were included. The top five frequently-used homologous Chinese medicines of medicine and food were Poria, Fructus Lycii, Fructus Alpiniae Oxyphyllae, Semen Persicae, and Rhizoma Dioscoreae. The top five frequently-used medicine were Rhizoma Acori Tatarinowii, Radix Polygalae, Rhizoma Ligustici Chuanxiong, Radix Salviae Miltiorrhizae, and Radix Angelicae Sinensis. The top five frequently-used drug pairs of homologous Chinese medicine of medicine and food were Rhizoma Dioscoreae→Poria, Poria→Pericarpium Citri Reticulatae, Rhizoma Dioscoreae→Fructus Lycii, Fructus Lycii→Fructus Alpiniae Oxyphyllae, and Fructus Lycii→Poria. The top five frequently-used drug pairs were Rhizoma Acori Tatarinowii→Radix Polygalae, Radix Salviae Miltiorrhizae→Rhizoma Acori Tatarinowii, Rhizoma Ligustici Chuanxiong→Rhizoma Acori Tatarinowii, Radix Salviae Miltiorrhizae→Rhizoma Ligustici Chuanxiong, and Rhizoma Acori Tatarinowii→Radix Astragali Seu Hedysari. Conclusion The commonly used homologous Chinese medicine of medicine and food in preventing and treating AD are tonic herbs with mild nature.

More and more DNA sequences have been obtained since the start-up of human genome project. Powerful system is badly needed for data mining on these DNA sequences. Based on a personal computer and Linux operating system, the Phred/Phrap/Consed software and Blast software were used to construct a platform for batch analysis of the sequences, including identifying raw DNA sequence from chromatogram file, vector sequence removing, contig analysis (sequence assembly), repeat sequence identifying and sequence similarity analysis. Result demonstrated that this robust platform could accelerate data analysis for large-scale DNA sequencing.
6.Clinicopathological features and risk factors for central compartment nodal metastasis in papillary thyroid microcarcinoma:a study of 1 401 patients

Objective:To determine the clinical features of papillary thyroid microcarcinoma (PTMC) and the risk factors for central compartment lymph node (CCLN) metastasis in PTMC. Methods:Data of 1 401 patients with PTMC who were treated in Tianjin Medi-cal University Cancer Institute and Hospital between January 2014 and December 2014 were retrospectively analyzed. Chi-square test and multivariate Logistic regression analysis were used to study the risk factors. Results:With regard to clinicopathological features, the sex ratio is 1:3.4 in patients with PTMC. CCLN metastasis affected 427 (30.5%) of the total 1 401 patients. Age (χ2=14.587, P<0.01), sex (χ2=21.636, P<0.01), tumor multifocality (χ2=35.505, P<0.01), tumor size (χ2=58.868, P<0.01), tumor site (χ2=8.385, P<0.05), and extracapsular/extraglandular invasions (χ2=26.481, P<0.01) were significantly correlated with CCLN metastasis. For patients with a soli-tary primary tumor, tumor location in the lower third of the thyroid lobe was associated with a higher risk of CCLN metastasis (48.1%). The male gender, age<45 years, tumor size>6 mm, extracapsular spread, and tumor bilaterality were independently correlated with CCLN metastasis. Conclusion:A prophylactic neck dissection of the central compartment must be considered particularly in male PT-MC patients with age<45 years, tumor size>6 mm, extracapsular spread, and tumor bilaterality.
7.X-ray and CT findings of soft tissue and bone infections secondary to acquired immunodeficiency syndrome

ObjectiveTo summarize X-ray and CT findings of soft tissue and bone infections secondary to acquired immunodeficiency syndrome (AIDS).MethodsThe data of X-ray and CT findings of soft tissue and bone infections in 18 patients with AIDS were retrospectively collected and analyzed.ResultsOf 18 patients with AIDS,the CT features of soft tissue demonstrated that subcutaneous patchy high density in 1 case which considered as cellulitis,round low density lesions with ring enhancement in 6 cases which considered as soft tissue abscesses,heterogeneous density lesions with peripheral enhancement in 1 case which considered as pyomyositis.Of 18 patients with AIDS,septic arthritis was found in 4 cases involving knee lesion in 3 cases and hip lesion.In the 4 case,the X-ray films showed bony destruction in 2 cases and the CT showed bone destruction in 3 cases and arthroedema in 4 cases.Of 18 patients with AIDS,osteomyelitis was found in 9 cases of which tuberculosis was considered in 8 cases and vertebral involvement in 6 cases.In the 9 cases,the X-ray films and CT displayed bony destruction,hyperostosis,small sequestra,and intervertebral space narrowing.Of 18 patients with AIDS,costal lesions were found in 3 cases in which the CT showed expandable bony destruction.Of 18 patients with AIDS,ilium and cacroihac joint lesions were found in 1 case in which the X-ray films and CT showed bony destruction,sequestra,and joint widening.Of 18 patients with AIDS,chronic pyogenic osteomyelitis of femur was found in 1 case in which the X-ray films showed bony destruction,hyperostosis osteosclerosis,and periosteal reaction.Conclusion The X-ray and CT features of soft tissue and bone infections secondary to AIDS are characterized.The X-ray and CT are useful tools to early diagnose soft tissue and bone infections secondary to AIDS.
8.CT Findings of Pulmonary Tuberculosis in the Patients with HIV Infection and AIDS

Objective To evaluate CT features of pulmonary tuberculosis in the patients with HIV infection and AIDS(HIV/AIDS).Methods CT findings of pulmonary tuberculosis were retrospectively analyzed in 48 patients with HIV/AIDS.Results The CT features of pulmonary tuberculosis in HIV/AIDS included: secondary pulmonary tuberculosis in 19 cases(39.6%), mostly showing patchings shadows,10 cases with large confluent consolidation; hematogenous pulmonary tuberculosis in 17 cases(35.4%), including 7 cases of acute miliary tuberculosis(showing miliary lesions evenly distributed in both lungs), and 10 cases of sub-acute disseminated tuberculosis(showing nodular patterns diffusely distributed in both lungs, asymmetrical in size and distribution); complex lesions in 10 cases(20.8%, showing patchy, nodular and miliary lesions). Among all the patients, 36 (75.0%) showed enlarged mediastinal lymphnodes, 13(27.1%) pleural effussion, 10(20.8%) pericardial effusion.Conclusion The characteristics of pulmonary tuberculosis in the patients with HIV/AIDS were mostly diffuse distribution.
9.Clinico-biological characteristics and prognosis of salivary duct carcinoma in 12 cases

Objective To explore the clinico-biological characteristics, treatment and prognosis of salivary duct carcinoma. Methods This study included 12 cases of salivary duct carcinoma treated in our hospital. Clinical data were retrospectively analyzed for patients admitted between April 1995 and October 2006. The clinical characteristics, histological features, imaging, therapy methods and prognosis were analyzed. Results Of 12 salivary duct carcinoma, there were 10 males, 2 females. The age of onset ranged from 53 to 73 year old and the average was 56 year old. Physical examination revealed a firm and unboundary mass accompanied by nerve infiltrating symptom. The histological appearance was characterized by solid cell nests with ductal structures and central comedonecrosis. Extensive resection and radical neck dissection was performed in 11 patients, postoperation radiation done in 10 patients and chemotherapy in 3 patients. One year survival rate was 83. 33% , that of 3 years was 41. 67% , and of 5 years was 25. 00% , the median survival time was 36 months. Conclusion Salivary duct carcinoma is a rare malignant salivary tumor and most patients are men. Regional extensive resection and postoperative radiation or chemotherapy are the mainstay of therapy. Lymph node metastases in level Ⅰ ,Ⅱ , Ⅲ are a common finding in patients with SDC and the prognosis is poor.
10.The abdominal CT findings of the penicilliosis marneffei in patients with acquired immunodeficiency syndrome

Objective To study the abdominal CT findings of the penicilliesis marneffei in patients with AIDS.Methods Abdominal CT imaging features of the the penicilliesis mameffei in 35 patients with acquired immunodeficiency syndrome (AIDS) were retrospectively analyzed.Results Abdominal lymph adenopathy was observed in 27 (77.1%) cases.Hepatosplenomegaly was observed in 18 (51.4%) cases.Lesions in the liver and spleen were found in 11 (31.4%) cases which included 3 cases of low density nodules in the liver (8.6%) and 5 cases of low density nodules in the spleen (14.3%).5 cases of diffuse low density in the liver (14.3%) and 5 cases of heterogeneous enhancement in the liver (14.3%) .Thickening of the intestinal wall was observed in 3 cases (8.6%). Ascites was observed in 4 cases (11.4%).Pancreatitis was found in 1 case(2.9%).Conclusions Penicilliosis marneffei in patients with AIDS usually involves multiple organs in the abdomen.Abdominal lymphadenopathy, hepatosplenomegaly and parenchymai lesions in the liver and spleen are the most common CT findings in the abdomen.
