Objective To explore the role of plasma level of homocysteine ( Hey ) in progressive ischemic stroke (PIS)of carotid artery system and to investigate the relationship between Hcy and carotid artery atherosclerosis(AS).Methods Plasma Hcy concentrations were measured in 68 patients with progressive ischemic stroke of carotid artery system (PIS group)and 72 patients with non-progressive ischemic stroke of carotid artery system ( non-PIS group ).Plasma Hcy level and carotid artery intima-media thickness ( IMT ) were compared among PIS group and non-PIS group as well as normal control group.The linear correlation between plasma Hcy level and IMT in PIS group was analyzed.Results Plasma Hcy concentrations was respectively (22.52 ± 5.26) μmol/L,( 16.38 ± 4.23) μmol/L and( 10.13 ± 2.16) μmol/L in PIS group,non-PIS group and normal control group( F =56.87,P ＜ 0.01 ).Compared PIS group and non-PIS group with normal control group,the differences were statistically significant ( P ＜ 0.01 ).And there was also significant difference on plasma Hcy levels between PIS group and non-PIS group( P ＜ 0.01 ).The thickness of carotid artery IMT was respectively(1.12 ± 0.34)mm,(0.96 ± 0.27)mm and(0.43 ± 0.21 )mm in PIS group,non-PIS group and normal control group( F =28.47,P ＜ 0.01 ).Compared PIS group and non-PIS group with normal control group,the differences were statistically significant( P ＜ 0.01 ).And there was also significant difference on IMT between PIS group and non-PIS group ( P ＜ 0.01 ).In PIS group plasma Hcy level was positively correlated with the carotid artery IMT ( r =0.687,P ＜ 0.01 ).Conclusion The plasma Hcy level is closely related to the occurrence and development of carotid artery atherosclerosis and PIS of carotid artery system,detection of plasma Hcy level helps to predict the occurrence of PIS of carotid artery system and provide an evidence for the therapy decision.

Objective To investigate the correlation of smokers in wnt/β-catenin signaling pathway activation as well as their correlation with lung squamous cell carcinoma incidence.Methods In the Department of Respiratory Medicine of the People's Hospital of Wuhan University,30 cases lung squamous cell carcinoma patients with a history of smoking,30 cases lung squamous cell carcinoma patients with no history of smoking,30 cases bronchiectasis patients with a history of smoking,30 cases bronchiectasis patients with no history of smoking were chose respectively as A group,B group,C group and D group.After the patients were treated by surgery,the removal of diseased tissue used immunohistochemical staining for CD68 + testing,TNF-α,wnt-2,β-catenin and the expression of TCF-4 were measured by using Western blot hybridization method,then wnt-2,β-catenin,TCF-4 transcription of mRNA concentrations were detected by using RT-PCR method.Results Immunohistochemical studies showed that cells in TNF-α,CD68 + expression of A group,B group,C group and D group were respectively 0.895±0.036 and 0.931±0.237,0.125±0.018 and 0.631±0.064,0.083±0.042 and 0.311±0.062,0.053±0.004 and 0.196±0.020,the differences were statistically significant(P＜0.05).Westblot test Results showed that wnt-2,β-catenin,TCF-4 expression level showed a gradually decreasing trend,the difference was statistically significant(P＜0.05),testing of RT-PCR showed that wnt-2,β-catenin,TCF-4 translation level showed a gradually decreasing trend,the difference was statistically significant(P＜0.05).Conclusion Smoking for body wnt/β-catenin pathway has a cell activation,after cigarette activate wnt/β-catenin signaling pathway in the incidence of lung squamous cell carcinoma of the body will greatly enhance the smoking crowd wnt/β-catenin signaling transduction circumstances.The detection of wnt/beta-catenin signal transduction in smokers can be used to indicate the occurrence of lung squamous cell carcinoma.

Objective: To observe the effect of acupuncture around the diseased region combined with point injection on post-herpetic neuralgia. Methods: Point injection on 30 cases in the related segments of spinal cord after needling around the diseased region. Results: Twenty-seven cases got effect and the total effect rate was 90%. Conclusion: Acupuncture combined with point injection has remarkable curative effect on post-herpetic neuralgia.

Objective To discuss the prenatal diagnosis of abnormalities of fetal limb bone.Methods We selected 18 cases which long bone of fetus less than 2SD of average volume of gestational weeks or long bone changed into angle or other fetus's abnormalities by first B-mode ultrasonic. All above cases was delivered at Capital Medical University of Obstetric and Gynecological Hospital during Jan. 2006 to Dec. 2009. We B-mode ultrasonic was used to measure fetus's biparietal diameter (BPD) ,femur length (FL) ,abdominal circumference(AC) ,head circumference(HC) ,humerus length (HL) ,amniotic fluid index (AFI) and structures of organ and calculated FL/AC, growth speed of long bone. The standard of achondroplasia is that FL and HL are less than 4SD of average of gestational weeks or FL/AC less than 0. 16. The standard of Osteogenesis Imperfecta is fetal long bone of fetus shows short and thick, curves into angle, fracture in uterus by X-ray, or skull shows thin or sink by X-ray. Results (1) By B-mode ultrasonic and X-ray exam of all 18 cases: 7 cases shows that HC ＞2SD, 10 cases shows too much amniotic fluid, 12 cases shows AFI ＞ 18.0, 9 cases shows abnormalities of narrow cavitas thoracis, disordered vertebral column, or unusual architecture of heart. For cases 1 to 14 are achondroplasia, among which,11 cases are FL＜4SD and HL＜4SD, 2 cases are FL ＜3SD and HL ＜4SD, 1 case is not only FL ＜2SD and HL ＜3SD but also hydroncus all over the body of fetus. The growth velocity of long bone of fetus in all the 14 cases is more slowly than the normal rate. For all the above 14 cases, 12 cases FL/AC ＜0. 16, 1 case FL/AC =0. 19, 1 case FL/AC =0. 20. The length of femur or humerus is shorter than the normal rate and have other abnormalities the above last two cases. For case 15 and 16, they don't show any abnormalities of bone growth though one year's follow up studying. For case 17 and 18, they are osteogenesis imperfecta. (2) The result of fetal perinatal period fate and autopsy: there are 8 female and 10 male in all the 18 cases. One case is labored after 39 weeks pregnancy, and it is low birth weight infant, weight ＜3%th. All the other cases are normally birth weight infant. All the 18 cases of abnormalities of fetal limb bone are examined by chromosomes check, among which, 9 cases are amniocentesis, 7 cases are cordocentesis,2 cases are checked chromosomes by fetus cord blood, all the caryotype are normal. In the 16 autopsy cases,14 cases are achondroplasia or hypochondroplasis. It can be seen amplifying extremities, hyperplasia chondrocytes of tubiform born, karyomegaly, anachromasis, hyperplasia capillaries though microscope and grow up into cartilage irregularly. Also can be seen hyperplasia chondrocytes of epiphyses, delaying osteosis.2 cases are osteogenesis imperfecta. It can be seen broadening of metaphyses, exility of bone trabeculae. For the other two cases which the fetus is alive, we do the follow up studying to their one year old one of them is low birth weight new born, their limb and height are all normal. Conclusions To diagnose fetal Achondroplasia, it is not only based on the significantly shorter of femur or humerus length but also based on the dynamics observing the long bone growth velocity and calculating FL/AC. For osteogenesis imperfecta fetus, it should be diagnosed by fractures in uterus though X-ray.

Objective To compare effect of the GlideScope and direct laryngoscopy in snoring surgery.Methods Fourty patients scheduled for orotracheal intubation of general anesthesia in snoring surgery wererandomly divided into GlideScope group ( A group) and direct laryngoscope group ( B group). Two groups were recorded values of heart rate (HR),systolic blood pressure (SBP), diastolic blood pressure (DBP)before induction (T1), after induction (T2), intubation (T3) and 1 (T4), 3 (T5), 5 (T6) min after intubation and plasma norepinephrine (NE) concentrations in T1, T5, T6. Results ( 1 ) Values of HR, SBP and DBP of two groups were compared in T1 and T2, the difference was not significant ( P ＞ 0.05 ); Values of SBP and DBP of two groups compared T1 to T2, and the difference was significant ( P ＜ 0. 05); (2) Values of HR, SBP and DBP of two groups compared in T3, T4 and T5, and the difference was statistically significant (P ＜0.05);(3 ) Values of the concentration of plasma NE of two groups compared in T1 ,T5 and T6, and the difference was statistically significant ( P ＜ 0. 05 ). Conclusion Cardiovascular response and stress response of orotracheal intubation using a videolaryngoscope is lower than using ordinary laryngoscope in snoring surgery.

Objective To compare the clinical effect of erythromycin and azithromycin in the treatment of mycoplasma pneumonia.Methods 120 children with mycoplasma pneumonia were randomly divided into study group and control group according to digital table method,60 cases in each group.The control group was treated with erythro-mycin,while the study group used azithromycin treatment.Clinical curative effect was compared between the two groups.Results The disappearing time of cough[(4.58 ±0.75 )d],fever[(4.75 ±1.74)d],pulmonary rales [(7.84 ±2.85)d]of the study group were significantly shorter than those in the control group[(4.58 ±0.75)d, (4.75 ±1.74)d,(7.84 ±2.85)d],and the differences were statistically significant (t =7.54,385,5.93,all P <0.05).The total effective rate in the study group (95.00%)was significantly higher than that in the control group (83.33%),and the difference was statistically significant (χ2 =8.08,P <0.05).There was no significant difference in the incidence of adverse reactions between the study group and the control group (χ2 =0.64,P >0.05 ). Conclusion Mycoplasma pneumonia in children for early diagnosis and treatment of dry,use of azithromycin curative effect was better than that of erythromycin.It could effectively relieve patients of fever,and alleviate the clinical symptoms of cough,pulmonary rales.

Objective To evaluate the effect of metformin treatment on the thyroid hormone profile in patients with polycystic ovary syndrome (PCOS).Methods A total of 148 patients with PCOS was specifically selected for either being diagnosed of hypothyroidism or subclinically hypothyroid or euthyroid without levothyroxine treatment before the starting of metformin.The serum levels of thyroid stimulating hormone (TSH) and free thyroxine (FT4) were measured before and after a 6-month period of metformin therapy.Results Thyroid function parameters did not change after starting metformin therapy in euthyroid patients with PCOS.In the 9 hypothyroid patients and 38 subclinical hypothyroid patients with PCOS,the basal median serum levels of TSH (P ＜ 0.05) decreased after a 6-month course of mefformin treatment [(2.77 ±1.47) μIU/ml vs (6.77 ± 2.46) μIU/ml;(2.90 ± 1.08) μIU/ml vs (4.05 ± 0.94) μIU/ml,P ＜ 0.05].No significant change in the serum levels of FT4 was observed in these patients (P ＞ 0.05).Conclusions Mefformin treatment has no effect on thyroid hormone,but has a TSH-lowering effect in hypothyroid and subclinically hypothyroid patients with PCOS.

Cotinine is the best biomarker for the assessment of the exposure to environmental tobacco smoke(ETS).With the"Framework Convention on Tobacco Control"implemented in China,the hazards from ETS are increasingly concerned,the determination of cotinine in human body which reflects the internal exposure to ETS received universal recognition day by day.It is imperative to create a standardized,simple and fast method to detect cotinine in human body.The recent determination techniques of cotinine,the application in different human samples and the direction of development were introduced in the present paper.

Objective To summarize the experience on bronchiallavage to cure pulmonary atelectasis and pulmonary infection after thoracotomy.Methods 48 patients were selected and treated by bronchiallavage under monitoring,and antibiotics was used according to sputum culture and drug sensitive test.The out come was analyzed and evaluated.Results All patients of the pulmonary atelectasis were cured and those with pulmonary infection were controled.There was no serious complication.Conclusion Bronchiallavage is applied to treat pulmonary atelectasis and pulmonary infection after thoracotomy,the curative rate is significant.The appliance of antibiotics is guided by drug sensitivity.It is such a safe and feasible therapeutic tool that the bronchofibroscope is applied to cure pulmonary atelectasis and pulmonary infection after thoracotomy.

Objective To deterine the expression of TNF-related apoptosis inducing ligand (TRAIL) and its receptor DR5 in murine myocardium with acute viral myocarditis(VM).Methods We builted the model of VM.Eight mice of the VM group and the normal control group were sacrificed on the 7th,10th,14th,21st,28th day after inoculation CVB3 virus.The myocardial histopathological scores were counted.The terminal reansferase-mediated dUPT-biotin nick end-labeling (TUNEL) assays was used to quantified apoptosis rate.The expression of TRAIL and DR5 protein and mRNA were determined by the method of immunohistochemistry and reverse-transcription polymerase chain reaction (RT-PCR).Results The expression of TRAIL and DR5 protein and mRNA were found in myocardium of both the normal control group and the VM group.The expression of TRAIL protein of the VM group(14th) were significantly higher than which in the normal group at the same time and the VM group(7th)(F=9.17,P<0.01).The DR5 protein of the VM group(10th,14th,21st)were significantly higher than which in the normal group at the same time and the VM group(7th)(F=13.32,P<0.01).The expression of TRAIL and DR5 protein were positive correlated with the myocardial histopathological scores and the apoptosis rate.The expression of TRAIL mRNA of the VM group(10th) were significantly higher than which in the normal group at the same time and the VM group(7th)(F=10.86,P<0.01).The DR5 mRNA of the VM group(10th,14th)were significantly higher than which in the normal group at the same time and the VM group(7th)(F=22.75,P<0.01).Conclusion High characteristic expressions of protein/mRNA TRAIL and DR5 were observed in the myocardium of mice with VM.The level was positive correlationed with the account of pathology and the rate of apoptosis.The apoptosis induced by TRAIL and DR5 may participate in the pathophysiological processes of VM.
