Objective To summarize local experience and skills of using Bard 3DMax mesh with-out fixation in lapamscopic total extraperitoneal inguinal hemioplasty(TEP) to treat patients with inguinal hernia.Methed From August 2006 to December 2007,38 patients suffered in inguinal hernia underwent TEP under general anesthesia,with using Bard 3DMax mesh without ixation.Results Thirty-eight cases had been performed 42 sides TEP.The operating time was 25 to 90 min.The mean operating duration was (35±13) min for unilateral hernia and (58±22) min for bilateral ones.The patients did not need analgesics after operation.The patients stayed time after operation was 1-4 days with the mean of (2.5±1.5) days.Hematnrrms occurred in 2 cases.There was no chronic inguinal pain,foreign body sensation cases and no recurrence after (13.0±5.5) months follow-up.Conclusions Using Bard 3DMax mesh without fixation in lapamscopic TEP can be performed safely and reliably.It also helps to shorten the operating time and reduces the complication rates of the operation.The procedure has advantages of faster postoperative recover,less pain,lower complications and recurrence rate.

Objective:To investigate the incidence rate and risk factors of acute kidney injury (AKI) after off-pump coronary artery bypass grafting (CABG),and to compare the effects of AKI on complications after operation and major adverse cardiovascular and cerebrovascular events (MACCE) after 3 years' follow-up.Methods:In the study,299 consecutive patients who underwent scheduled off-pump CABG from January 2010 to March 2012 were included.The patients were divided into AKI group with AKI and control group without AKI after operation.The data during perioperative stage were compared,and multivariable Logistic regression modeling was used to identify the risk factors of AKI.The complications were compared after surgery and the patients were followed up for 3 years to observe the difference of MACCE between the two groups.Results:AKI occurred in 37.1％ patients (111/299).The elevated serum creatinine levels (Wald =9.276,P =0.002,95 ％ CI 1.006-1.028),chronic obstructive pulmonary disease(COPD) (Wald =3.469,P =0.063,95％ CI 0.950-7.630),decreased left ventricular ejection fraction (LVEF) (Wald =4.414,P =0.036,95 ％ CI 0.965-0.999),and implantation of intraaortic balloon pump (IABP) before or after operation(Wald =6.745,P =0.009,95％ CI 1.336-7.925)were risk factors of AKI in multivariable logistic regression modeling.More complications occurred in AKI group,such as the duration of mechanic ventilation,the time of ICU and the length of stay post operation,reintubation,pulmonary infection,stroke,hemorrhage of digestive tract,the volume of blood transfusion (plasma and red blood cell) and renal replacement therapy(P ＜ 0.05).The difference of mortality rate had no statistical significance between the AKI group and the control group,but two patients died in the AKI group.The difference of MACCE between the two groups had no statistical significance after 3 years' follow-up either.Conclusion:The incidence of AKI was high (37.1％) after off-pump CABG.The elevated serum creatinine levels,COPD,decreased LVEF,and implantation IABP before or after operation were independent risk factors of AKI in multivariable Logistic regression modeling.More complications occurred in AKI group during perioperative period,but the difference of MACCE between the two groups after 3 years' follow-up had no statistical significance.

Objective-To-explore-the-feasibility-and-safety-of-endoscopic-total-thyroidectomy-via-chest-wall-approach-.-Methods-Endoscopic-total-thyroidectomy-via-chest-wall-approach-was-performed-in-20-patients-with-nodular-goiter-from-January-2008-to-December-2012.Subcutaneous-operative-space-was-created-through-an-unilateral-areola-incision-,-through-which-laparoscope-and-surgical-instruments-were-introduced-.The-neck-suture-suspension-technique-was-used-to-reveal-the-thyroid-gland-.By-using-an-ultrasonic-scalpel-,-the-thyroid-was-separated-and-removed-completely-.-Results-All-the-operations-were-successfully-completed-without-conversions-to-open-surgery-.The-operation-time-was-90-180-min-(mean,-120-min).The-blood-loss-was-15-40-ml-(mean,-20-ml).The-postoperative-hospital-stay-ranged-from-4-to-7-days-(mean,-5.8-days).There-was-1-case-of-temporary-hoarseness.No-severe-hemorrhage-,-wound-infection-,-or-subcutaneous-effusion-was-found-.-Conclusion-Endoscopic-total-thyroidectomy-via-chest-wall-approach-is-a-safe-and-feasible-procedure-.

The chronological and spatial rules of changes during focal cerebral ischemia and reperfusion in different brain regions with magnetic resonance diffusion-weighted imaging (DWI) in a model of occlusion of middle cerebral artery (MCAO) and the development of cytotoxic edema in acute phase were explored. Fifteen healthy S-D rats with MCA occluded by thread-emboli were randomly divided into three groups. 15 min after the operation, the serial imaging was scanned on DWI for the three groups. The relative mean signal intensity (RMSI) of the frontal lobe, parietal lobe, lateral cauda-putamen, medial cauda-putamen and the volume of regions of hyperintense signal on DWI were calculated. After the last DWI scanning, T2 WI was performed for the three groups. After 15 min ischemia, the rats was presented hyperintense signals on DWI. The regions of hyperintense signal were enlarged with prolonging ischemia time. The regions of hyperintense signal were back to normal after 60 min reperfusion with a small part remaining to show hyperintense signal. The RMSIs of parietal lobe and lateral cauda-putamen were higher than that of the frontal lobe and medial cauda-putamen both in ischemia phase and recanalization phase. The three groups were normal on T2 WI imaging. DWI had good sensitivity to acute cerebral ischemia, which was used to study the chronological and spatial rules of development of early cell edema in ischemia regions.

As the dressings currently used in clinic settings unflat shape in general, they can't be fitted completely on occiput, heel, elbow, knee and other body parts unflat. This paper developed one kind of foam dressing of special shape based on local anatomy. The foam dressing is waterproof and air permeable, it can cover the wound closely enough to prevent bacteria from invasion and infection. With a saturated absorption ratio of 1: 8 or higher, it can keep the wound clean and moisture by absorbing large amounts of wound inflammatory secretions and is almost completely permeable to oxygen and carbon dioxide. Assuring safety and effect meanwhile, it has better outcomes than common dressings in the same application settings.

Objective To examine the effect of positive and negative evaluative conditioning (EC) on neutral faces.Methods The experiment consisted of three phases:baseline phase,conditioning phase,and re-evaluative phase,in which 41 college students participated,watching sequences of neutral faces (CS) pairing to either a positive stimulus (USpos) or a negative stimulus (USneg).Their emotional experiences (valence and arousal) and physiological reactivity (eyeblink startle reflex and skin conductance) were recor ded.Results (1) In the re-evaluative phase,CSpos was rated significantly more positive than CSneg and CSneut,while CSneg was rated significantly more negative than CSneut (CSpos (5.05± 1.24),CSneg (3.73± 1.48),CSneut (4.46± 1.04),P＜0.05).(2)In the re-evaluative phase,the mean startle eyeblink response magnitude(T score) to CSpos was significantly smaller than the responses elicited by CSneg and CSneut (CSpos (45.04±5.56),CSneg (51.44±9.30),CSneut (54.52± 10.60),P＜0.01).Conclusion The findings suggest that neutral faces can acquire valences and approach motivation through EC.

Objective To analyze the epidemiological features of leprosy in China from 2011 to 2015,and to provide scientific evidences for prevention and treatment strategies.Methods An epidemiological analysis and a trend analysis were conducted based on the national leprosy surveillance data from 2011 to 2015.Results The leprosy detection rate in China decreased from 0.085 per 100 000 in 2011 to 0.049 per 100 000 in 2015,with an average annual decline rate being 12.9％.A total of 4 775 leprosy cases were newly detected during 2011-2015,including 106 (2.2％) children,1 499 (31.4％) females,518 (10.8％) floating people,4 041 (84.6％) multibacillary cases and 1 134 (23.7％) cases with grade 2 disabilities.From 2011 to 2015,328 relapsed cases were reported,including 153 (46.6％) cases recurring after combined chemotherapy.The prevalence rate of leprosy in China decreased from 0.407 per 100 000 in 2011 to 0.235 per 100 000 in 2015,with an average annual decline rate being 12.9％.By the end of 2015,there had been 3 230 registered leprosy cases and 124 counties with a prevalence rate above 1 per 100 000.Conclusions The detection rate and prevalence rate of leprosy in China were both decreasing continuously from 2011 to 2015.The high-epidemic provinces were Yunnan,Guizhou,Sichuan and Guangdong.However,leprosy control in middle-and low-epidemic provinces can not be ignored.
