1.Impact of HIV/AIDS on marriage and family structure

Objective To compare and analyze the main manifestations of HIV/ AIDS impact on marriage and family structure.Methods The relevant data of 931 HIV households and 995 non-HIV households were collected in five provinces with high HIV prevalence in China,using the method of questionnaire survey.The differences between people living with HIV/ AIDS(PLWHA)and non-HIV individuals in marriage and family structure were analyzed and compared.Results The percentage of PLWHA who were unmarried,divorced,widowed and remarried was higher than that of individuals in non-HIV households;the average size of the non-HIV households was 4.4 persons,while the size of HIV households was 4.0 persons;of the surveyed HIV households 41.1 % were stem families,while in the case of non-HIV households this percentage was lower,only 33.4 %.The rate of school enrollment among children in HIV households was lower than that in non-HIV households,while the rate of labor participation among the elderly in HIV households was higher than that in non-HIV households.Conclusion The results indicate that HIV/AIDS destroys the normal marriage among PLWHA,leading to an increased number of stem families and changes in family structure;the impact on the elderly and children in the HIV households is severe.All this means that HIV/AIDS weakens or even destroys the family function.
2.Research Progress of Multimodal Clinical Support System

Objective To explore the research progress of the multimodal clinical support system (CSS). Methods With recognized development and operation of the multi-model CSS, and compared to the traditional CSS, to explore the research progress of the multimodal CSS. Results Based on the realization of the concept, purpose and characteristics of the multimodal CSS, it has been known that the international research progress of the multimodal CSS. Conclusion The developing and evolving of the CSS model have offered a new assist to the multi-disciplinary treatment model, and have enhanced the improving system associated with the practice of evidence-based medicine. However, the application of clinical support system program (CSSP) in our country still needs more research.
3.Study on the mechanism of IL-35 in autoimmune diseases

As an anti-inflammatory factor, Interleukin 35 (IL-35) is composed of p35 subunit and EBI3 subunit. IL-35 plays an important role in many diseases, but many mechanisms are not clear. In recent years, it has been found that IL-35 plays an important immunomodulatory role in autoimmune diseases, such as rheumatoid arthritis, inflammatory bowel disease, psoriasis, idiopathic inflammatory myopathy, systemic lupus erythematosus, multiple sclerosis, type I diabetes, pemphigus and primary biliary cirrhosis, etc. This review summarizes the research progress of the mechanism of IL-35 in the above autoimmune diseases.
4.Content Determination of TancantongⅡA and Cryptotanshinone in Zhiwei Fuwei Wan by RP-HPLC

Objective To establish the method for determining the content of cryptotanshinone and tanshinone ⅡA in Zhiwei Fuwei Wan. Methods The determination of cryptotanshinone and tanshinone ⅡA was performed on Symmetry C18 column with mobile phase consisted of acetonitrile-0.2%phosphate (60∶40) at flow rate of 1.0 mL/min. The detection wavelength was set at 262 nm. Results The linear ranges of cryptotanshinone and tanshinone ⅡA were 0.081 4-0.407 2 μg (r=0.999 7) and 0.163 6-0.81 8 μg (r=0.999 5) respectively, the average recoveries were 99.70% (RSD=1.37%) and 99.22% (RSD=0.94%) respectively. Conclusion The method is simple and rapid for the quality control of the product.
5.Sex hormone-binding globulin of gestational diabetes mellitus pregnant women with well-controlled glucose and pregnancy outcomes

Objective To explore the relationship between sex hormone-binding globulin (SHBG) of gestational diabetes mellitus ( GDM ) pregnant women with well-controlled glucose and pregnancy outcomes. Methods Two hundred and fifty-one GDM pregnant women of 24 - 28 weeks in Shengjing Hospital of China Medical University were recruited from Mar. 2005 to Mar. 2010. Two hundred and sixteen cases of GDM with well-controlled glucose were defined as glycemic satisfied group, and they were treated by diet therapy ( 169 cases) or insulin therapy (47 cases) . Thirty-five cases with unsatisfied glucose were defined as glycemic unsatisfied group. One hundred and ninety-two healthy pregnant women of 24 - 28 weeks were defined as healthy control group. Serum SHBG and homeostasis model analysis of insulin resistance ( HOMA-IR) at 24 - 28 weeks and above 36 weeks were measured. GDM was diagnosed bytwo-step method according to the National Diabetes Data Group ( NDDG) criteria. The pregnancy outcomes and complications of the three groups were recorded. Results ( 1 ) Comparison of pregnancy outcomes and complications: glycemic satisfied group was less likely to develop hypertensive disorders in pregnancy ( 10. 6% ) , premature birth(8. 3% ) ,large for gestational age ( LGA) (8. 8% ) , neonatal asphyxia(3. 7% ) and neonatal hypoglycemia ( 2. 3% ) compared to glycemic unsatisfied group ( 42. 9% , 34. 3% , 31. 4% , 22. 9% and 11. 4% ,respectively). And the difference was statistically significant (P <0. 05 or P <0. 01). There was no significant difference for incidence of polyhydramnios, pueperal infection, postpartum hemorrhage, neonatal hyperbilirubinemia between the two groups ( P> 0. 05 ) . When compared to healthy control group(7. 3% ,2. 1% ,4. 2% ,2. 1% and 1. 6% ) ,no significant difference was found for incidence of premature birth( 8. 3% ) , pueperal infection ( 3. 2% ) , postpartum hemorrhage (5. 1% ) , neonatal asphyxia (3. 7% )and neonatal hypoglycemia(2. 3% ,P >0. 05). (2) Comparison of results of 24 - 28 weeks and above 36 weeks: serum SHBG of glycemic satisfied group [( 384 ± 88 ) , (457 ± 48 ) nmol/L]was significantly higher than that of glycemic unsatisfied group[(313 ±45) ,(401 ±73) nmol/L];HOMA-IR of glycemic satisfied group (5. 3 ±1.1,5.5 ±1.1) was significantly lower than that of glycemic unsatisfied group (7. 0 ± 1. 3 ,7. 6 ± 1. 7 ; P < 0. 01). Serum SHBG of glycemic satisfied group was significantly lower than that of healthy control group [( 492 ± 95 ) , (565 ± 40 ) nmol/L]; and HOMA-IR of glycemic satisfied group(5. 3 ± 1. 1,5. 5 ± 1. 1) was significantly higher than that of healthy control group (3. 6 ±0. 6,3. 9 ± 0. 5 ;P < 0. 01 ) . FPG of glycemic satisfied group [( 5. 84 ± 0. 28 ) , ( 5. 16 ± 0. 13 ) mmol/L]was significantly lower than that of glycemic unsatisfied group [(6. 13 ± 0. 16 ) , ( 5. 68 ± 1. 14) mmol/L; P < 0. 01]. FINS of glycemic satisfied group [( 20. 4 ± 2. 1 ) , ( 24. 1 ± 4. 2 ) mmol/L]was significantly lower than that of glycemic unsatisfied group [(24. 7 ± 4. 5 ) , ( 29. 9 ± 2. 7 ) mmol/L; P < 0. 01]. ( 3 ) Correlation analysis. Between 24 - 28 weeks, SHBG was negatively correlated with HOMA-IR in the three groups ( r = -0. 952, P <0. 01) ; and SHBG was negatively correlated with HOMA-IR in glycemic satisfied group ( r = -0. 903, P <0. 01). Conclusions Well-controlled glucose can not completely improve maternal and fetal outcomes of GDM pregnant women. High insulin resistance and low serum SHBG can influence pregnancy outcomes.
6.Repaglinide effected on proliferation, differentiation, and apoptosis of pre-osteoblasts MC3T3-E1 subclone 14 cell in hyperglycemia condition

Objective This study examined the effects of repaglinide on the proliferation, differentiation, and apoptosis of mouse pre-osteoblasts MC3T3-E1 subclone 14 cell under high glucose condition.Methods MC3T3-E1 cells cultured in vitro for 48h were divided into the following groups: control group and repaglinide with different concentrations (0.01, 0.1, 1, and 10 μmol/L).Cell proliferation was measured by CCK-8 assay.mRNA levels of type I collagen(COL-Ⅰ), osteopontin(OPN) and alkaline phosphatase(ALP) were assayed using quantitative real-time PCR.Expression of apoptosis related proteins (Bcl-2, Bax) was measured by western blot analysis.Results (1)Compared with control group, the proliferation rate of repaglinide with different concentrations increased, while the proliferation rate of the 1 μmol/L repaglinide group significantly higher than that of the control group(P<0.05).(2) There was no significant difference on the COL-Ⅰ mRNA expression among the repaglinide groups and the control group(P>0.05).Compared with control group, the OPN and ALP mRNA expression increased significantly in the 1 μmol/L repaglinide group(P<0.05), while the OPN expression was significantly declined in the 10 μmol/L repaglinide group(P<0.05).(3)The protein expression of Bcl-2 was positively related with repaglinide concentration(P<0.05), while the protein expression of Bax went down in the 1 and 10 μmol/L repaglinide groups(P<0.05).Conclusion When exposed to high glucose concentration, repaglinide in a certain concentration range may promote the proliferation and differentiation of MC3T3-E1 subclone 14, while restrain its apoptosis.
7.Effects of apatinib on quality of life and immune function in older adult patients with advanced non-small cell lung cancer

Objective:To analyze the effects of apatinib on quality of life and immune function in older adult patients with advanced non-small cell lung cancer.Methods:A total of 187 older adult patients with advanced non-small cell lung cancer admitted to Taizhou Cancer Hospital from January 2017 to January 2021 were included in this study. They were divided into the control group ( n = 93) and the observation group ( n = 94). The control group was treated with carboplatin combined with pemetrexed and the observation group was treated with apatinib based on carboplatin and pemetrexed. Sign and symptoms remission was compared between the observation and control groups. The levels of tumor markers, immune function, and quality of life score were compared between the two groups before and after treatment. Results:Total remission rate in the observation group was significantly higher than that in the control group (88.30% vs. 69.89%, χ2 = 9.59, P < 0.05). After treatment, carbohydrate antigen 125, carbohydrate antigen 50, and carcinoembryonic antigen in the observation group were (16.25 ± 5.47) μg/L, (15.23 ± 3.27) μg/L and (5.91 ± 2.66) mg/L, respectively, which were significantly lower than (21.49 ± 6.61) μg/L, (19.11 ± 3.48) μg/L and (10.14 ± 2.73) mg/L in the control group ( t = 5.91, 7.86, 10.73, all P < 0.05). The percentage of CD3 + and CD4 + cells, and the ratio of CD4 +/CD8 + cells in the observation group were (69.34 ± 8.85)%, (38.15 ± 6.52)%, (1.40 ± 0.33), respectively, which were significantly higher than (64.51 ± 8.74)%, (33.55 ± 6.33)%, (1.23 ± 0.25) in the control group ( t = -3.75, -5.36, -3.97, all P < 0.05). Quality of life score was increased in each group ( P < 0.001). The amplitude of increase in quality of life score was greater in the observation group compared with the control group ( P < 0.001). Conclusion:Apatinib can effectively reduce the level of tumor markers and improve immune function in older adult patients with advanced non-small cell lung cancer and improve quality of life.

9.Optimization of Paper-roasted Technology of Aucklandiae Radix by a Multi-index Comprehensive Evalua-tion Method

Objective:To optimize the paper-roasted technology of Aucklandiae Radix. Methods:With methanol-water (65: 35) as the mobile phase, the contents of costunolide and dehydrocostuslactone were determined by HPLC. Senna was used to make diarrhea model in mice, and the sample decoction was administered for the mice by i. g at the dose of 10 g·kg-1 and the diarrhea index was de-termined. The paper-roasted technology of Aucklandiae Radix was optimized by L9 (34 ) orthogonal design taking the contents of cost-unolide and dehydrocostuslactone, and the diarrhea index in mice as the indices with the paper-roasted time, the paper-roasted temper-ature and the layer number as the influencing factors. Results:The variance analysis results show that the layer number and the paper-roasted temperature has the significant impact on the contents of costunolide and dehydrocostuslactone and the diarrhea index in mice. The best paper-roasted technology of Aucklandiae Radix was as follows:a layer of paper was covered by 4mm Aucklandiae Radix and repeated the same operation 5 times, and put them in drying baker at 120℃ for about 2 hours. After the essential oil of Aucklandi-ae Radix permeated on the paper with burnt flavor and dark brown in color, Aucklandiae Radix could be taken out. Conclusion:The optimum paper-roasted technology of Aucklandiae Radix is reasonable and feasible, which can provide reference for the establishment of the quality standard for paper-roasted Aucklandiae Radix pieces and the expanding of the clinical application.
10.Effects of Nie Ji tuina combined with acupoint massage on cellular immune function in children with recurrent respiratory tract infection

Objective:To explore the effects of Nie Ji tuina combined with acupoint massage on cellular immune function in children with recurrent respiratory tract infection (RRTI).Methods:Randomized controlled trial. A total of 96 children in the hospital were enrolled as the observation objects between June 2020 and October 2021. According to random number table method, they were divided into control group (acupoint massage) and observation group (Nie Ji tuina combined with acupoint massage), 48 in each group. All were treated for 8 weeks and followed up for 8 months. TCM syndromes were scored before and after treatment. The levels of CD4 +, CD8 + and CD4 +/CD8 + ratio in peripheral blood were detected by flow cytometry. The frequency of RRTI attacks during follow-up was observed, and the clinical curative effect was evaluated. Results:There was no significant difference in total response rate between observation group and control group [93.75% (45/48) vs. 81.25% (39/48); χ2=3.43, P=0.064]. After treatment, scores of TCM syndromes (cough, fatigue, shortness of breath and laziness to speak, spontaneous sweating) were significantly lower in observation group than control group ( t=3.95, 9.64, 7.68, 8.34, P<0.01). After treatment, levels of CD4 + and CD4 +/CD8 + were increased ( P<0.01), while CD8 + level was decreased in both groups ( P<0.01), but there was no significant difference between the two groups ( P>0.05). During follow-up, observation group times of recurrence attacks were fewer than those in the control group ( t=10.60, P<0.01). Conclusion:Nie Ji tuina combined with acupoint massage can improve disease resistance ability and reduce RRTI frequency in children.
