1.Length of stay for acute myocardial infarction patients in eastern urban area of China during 2001-2011

Objective China is experiencing increasing disease burden and economic burden of acute myocardial infarction(AMI),while the healthcare resources are limited and unevenly distributed.Hospitalization duration is an important measure of health resource utilization index.This study aims to describe the time trend of Length of stay(LOS) for AMI inpatient care in eastern urban areas of China,and to identify potential hospital-level factors associated with the LOS,which is an important indicator of resource utilization.Methods The study randomly selected a representative sample of patients admitted for AMI in 2001,2006 and 2011,from urban hospitals in eastern China.After central data abstraction from medical records,patients' characteristics,treatments and outcomes were compared between 3 hospital groups that were divided according to their median of LOS.Results Five thousand and forty-seven AMI cases were included in the analyses.The average LOS were (15.3±10.2),(13.5±8.7) and (12.2±7.8) d in 2001,2006 and 2011 respectively,with a decreasing trend(F=10.93,P<0.001).After adjusted for patients' characteristics,there were no significant different in LOS between years.Patients' characteristics were similar between high-,middle-and low-LOS hospitals,however,in high-LOS hospitals,use rates of aspirin,clopidogrel and statins were even lower(P≤0.001).LOS were longer 1.9 d in the hospitals with relatively small AMI inpatient treatment volumes(30-80 annually,t=2.97,P<0.01).Conclusions The average LOS for AMI in eastern urban areas of China is much longer than in western countries.Conclusion between hospitals have highlighted the gaps in care quality and efficiency.Standardized clinical pathways and hierarchical medical system are essential strategies in the new healthcare reform.
2.Effect of Yuzhenxifeng Decoction on the brain ferritin in PD mouse models

Objective To study the effect of Yuzhenxifeng Decoction on the brain ferritin in PD mouse models .Methods All ex‐perimental animals were devided into 4 groups:the control group ,the model group ,the positive drug group and TCM group .Prepare PD model mice with MPTP ,then use the immunohistochemical staining technique to observe the change of expression of Fn .Results the results showed that compared with the model group ,movement coordination disorder of rats in TCM group were relieved ;the Fn level of the model group on 6th ,13th ,20th were higher than control group(P<0 .05) ,and were 2 .21 times ,1 .15 times and 0 .36 times higher respectively .Compared with model group mice ,expression of Fn were enhanced in the treatment group .Conclusion Yuzhenxifeng Decoction can improve the expression of Fn in the brain ,which provide the basis for further study on mechanism of the treatment of Parkinson′s disease .
3.Surface characterization of morphology and nano-scale adhesionforce on ethanol saturated root dentin

Objective To observe the morphology of ethanol-wet dentin surfaces and detect their nano-scale adhesion force (Fad) by atomic force microscopy (AFM) in order to explore the potential mechanism of ethanol-wet bonding in improving clinical dentin bonding effectiveness.Methods Dentin slices from human premolar roots were prepared into flat ones, polished, and then randomly divided into five groups.All the specimens were acid-etched, rinsed, and left moist.They were then treated with 100% ethanol for 0s (control group), 20s, 60s, 3×60s, or stepwise ethanol application.Afterwards, each group was scanned for the morphology in air and the Fad was probed by AFM.One-way ANOVA followed by Tukey`s test was employed for multiple comparisons using SPSS16.0.Results Compared with control group, ethanol-wet dentine produced a less undulating and relatively smooth surface topography.Ethanol-wet protocol significantly decreased the value of Fad in the experimental groups (P<0.001), in which 3×60s and stepwise ethanol application groups showed the lowest Fad.No difference was found between these two groups (P>0.05).Conclusion When using AFM in air, ethanol-wet protocol with longer time can produce a less undulating and relatively smooth surface topography and decrease the Fad, whichindicates that the water saturated in dentin matrix was replaced more thoroughly by longer ethanol application time.This will benefit hydrophobicity of the dentin bonding interface.
4.Campus bullying in Shenzhen middle school students and its associations with quality of life

Objective:
To investigate the prevalence of bullying and its association with quality of life in Shenzhen middle school students’ and to provide scientific basis for intervention measures.
Methods:
The study was a cross-sectional study with stratified cluster sampling. The short version of World Health Organization Quality of Life (WHOQOL-BREF), Center for Epidemiological Studies Depression Scale (CES-D) and Self-completed questionnaires, including demographic characteristics and bullying, were utilized to examine the study objectives. 2 103 school students from Shenzhen were selected to take part in the study during July-October of 2017. Multiple liner regression analysis was used to analyze the association between bullying and quality of life.
Results:
418 middle school students (19.9%) suffered bully in the past year. 326 students (15.5%) suffered traditional bullying and 182 students (8.7%) suffered cyberbullying in the past year. Male students, junior middle school students and those with bad grades as well as bad relationship with family and classmates were more likely to suffer bullying(χ2=8.89, 41.56, 14.83, 23.42, 32.26, 46.75, P<0.05). The prevalence of depression in students with bullying experience was significantly higher than those without bullying. Students with bullying experience reported significantly lower scores than those without bullying experience in physical domain, psychological domain, social relationship domain and environment domain, and the differences were of statistical significance(t=-7.54, -7.08, -6.88, -6.02, P<0.01).
Conclusion
The prevalence of bullying in middle school students was high. It was negatively associated with students’ quality of life. The findings underscore the importance of developing psychological interventions for students with bullying.
5.Core competence of neurology nurses from a clinical nursing perspective: a qualitative research

Objective:To explore the core competence that Neurology nurses should possess through qualitative research.Methods:In July 2023, a semi-structured interview was conducted by purposive sampling to select 15 clinical nurses and 7 head nurses in Department of Neurology of Xuanwu Hospital, Capital Medical University. Colaizzi 7-step analysis method in phenomenological research was used to analyze data.Results:The core competence model of neurology nurses were summarized into 7 themes, including clinical competence (symptom assessment ability of neurological diseases, critical rescue ability of neurological diseases, specialized skills related to neurological disease treatment), research ability, teaching ability, management ability, personal quality (communication ability, learning ability, teamwork ability), professional quality (patience, responsibility, love, empathy), and the supervision ability of neurology head nurses over nurses (development of continuing education on neurological diseases, promotion of career planning for neurology nurses) .Conclusions:The analysis of the core competence of Neurology nurses provides a basis for improving nursing quality, promoting disease prognosis for patients with neurological disorders, and enhancing their quality of life.
6.Study on plasma metabolomics analysis of Yanghe-patch in the treatment of impaired glucose tolerance with Yang-deficiency constitution

Objective To investigate the impact of Yanghe-patch acupoint in the treatment of impaired glucose tolerance(IGT)with Yang-deficiency constitution and its influence on serum metabolites.Methods The study enrolled a total of 63 patients with IGT were randomly selected and stratified into two groups:The control group(32 cases)and the treatment group(31 cases)at Ningbo Municipal Hospital of TCM Affliated to Zhejiang Chinese Medical University from October 2022 to January 2024,blood glucose,blood lipids,tumor necrosis factor-α(TNF-α),Yang-deficiency body mass score,and infrared thermal imaging temperature before and after treatment were compared between two groups.Furthermore,29 healthy individuals were selected as healthy negative controls,and their serum metabolites were analyzed by using liquid chromatograph-mass spectrometer(LC-MS)among three groups.Principal component analysis and orthogonal projections to latent structures-discriminate analysis were employed to identify characteristic differential metabolites.Additionally,key metabolic pathways were screened through network analysis by using MetaboAnalyst software.Results The treatment group after Yanghe-patch showed significant reductions in fasting blood glucose,glycosylated hemoglobin,triglycerides,low-density lipopro-tein and TNF-α levels(P<0.05).Additionally,Yang-deficiency constitution conversion points significantly decreased within the treatment group(P<0.05),while there were significant increases in the temperatures of Ren channel,Du channel,middle-Jiao and lower-Jiao(P<0.05).However,there was a notable decrease in upper-Jiao temperature(P<0.05).Metabolomics analysis identified varying levels of serum metabolites in IGT patients with Yang-deficiency constitution,such as 4-acetylaminobutyric acid,3-dehydrocholic acid,citric acid,and cystine etc.16 metabolites.Treatment group exhibited distinct expression patterns for 16 metabolites(including diaminoheptadecic acid,acetyl-L-carnitine and monobutyl phthalate etc.).The analysis by MetaboAnalyst shows that Yang-deficiency constitution with IGT was linked to dysregulation in the tricarboxylic acid cycle,aminoacyl bioanabolic pathway,and Gly-Ser-Thr metabolic axis.Additionally,Yanghe-patch intervention affects the glycine,serine and threonine(Gly-Ser-Thr)metabolic axis and sphingolipid metabolism.Conclusion The application of Yanghe-patch on acupoints shows potential in alleviating chronic systemic inflammation and improving symptoms associated with Yang-deficiency constitution in individuals with IGT.Metabolomics analysis has revealed a range of metabolites involved in their mechanism,among which the Gly-Ser-Thr metabolic pathway is considered crucial for regulating the constitution of Yang-deficiency constitution in individuals with IGT.
7.Study on the Synthesis of Shikimic Acid Derivatives and Their Reversal Effects on Paclitaxel-resistant Human Breast Cancer Cells

OBJECTIVE:To struc turally modify shikimic acid ,and to investigate the reversal effects of its derivatives on paclitaxel-resistant human breast cancer cells MCF- 7/PTX. METHODS :Using shikimic acid as the lead structure ,1-position carboxyl group was structurally modified to synthesize a series of shikimic acid derivatives through esterification ,amidation, hydrogenation and reduction ,etc. Using non-drug resistant cells MCF- 7 as reference ,MTT assay was used to screen derivatives with inhibitory activity as well as half-inhibitory concentration (IC50)and reversal index (RI)of derivatives to MCF- 7/PTX. With the drug resistance-related transgelin 2 as the target ,the molecular docking of the active derivatives with the drug resistance-related protein was carried out by using Glide 1.0 computer-aided design software. RESULTS :Totally 15 derivatives were obtained (T1-T15), of which T 4-T15 were obtained for the first time. MTT assay showed that (3R, 4S, 5R) -N-benzyl-3, 4, 5-trihydroxy-1-cyclohexene-1-formamide(T7),(3R,4S,5R)-N-(3,4,5-trihydroxy-1-cyclohexenylmethyl)-benzylamine(T14), (3R,4S,5R)-3,4-O-isopropyl-5-O-acetyl-1-cyclohexene-1-methyl formate (T15)inhibited MCF- 7 and MCF- 7/PTX cells to a certain extent ;IC50 values of T 7,T14 and T 15 combined with pacliaxel to MCF- 7/PTX cells were significantly lower than that in negative control (Paclitaxel alone )group(P<0.05). RIs of T 14 and T 15 were higher ,and RIs of the highest dose were 8.8 and 9.3, which were equivalent to positive control verapamil (10.8). Th e results of molecular docking showed that the hydroxyl groups at positions 3,4 of T 7 could form multiple hydrogen bonds with ; Arg625 and Asp 627 in the catalytic region of transgelin 2. In addition to the hydrogen bond mentioned above at T 7,the mail:batistuta28@126.com secondary amine side chain at position 1 of T 14 could also form hydrogen bond with Glu 657 of transgelin 2. When the hydroxyl group on the T 15 mother nucleus was derived from the donor group ,the binding of the hydroxyl group to transgelin 2 was closer and the inhibition was enhanced. CONCLUSIONS : The derivatives T 7,T14 and T 15 have certain reverse activity to paclitaxel-resistant human breast cancer cells. The polyhydroxy structure of the mother nucleus is the main structural region of the hydrogen bond between shikimic acid and its derivatives and transgelin 2. The derivation of its power supply group or the introduction of secondary amines and hydrophobic groups into the 1-carboxyl group of shikimic acid is benifit for enhancing the drug resistance reversal effect of derivative .
