2.Establishment and evaluation of the SD rat allergic rhinitis model.

OBJECTIVE:
To investigate method established and system evaluated in the model of SD rat with AR.
METHOD:
To establish AR model of SD rats by ovalbumin (OVA), 20 cases of SD rats were randomly divided into two groups, namely control group (10 cases) and AR group (10 cases). AR models were sensitized and challenged by OVA. Control group were used with normal saline instead of OVA. The score of pathology and praxiology were observed when the SD rats in AR group appeared typical symptom of allergic rhinitis, and levels of IL-4, IFN-γ, IgE in the serum were examined by ELISA. According to the behavioral score, nasal histology and content of IL-4, IFN-γ, IgE of serum, Rat allergic rhinitis model were judged successfully established or not.
RESULT:
Behavioral scores were significantly increased in OVA-challenged rats compared with the control group, P<0.05. Nasal epithelial goblet cells, eosinophils and lymphocytes in nasal mucosa in the AR rats exhibited obvious increase relative to the control group. IL-4, IgE levels in the AR rat exhibited obvious increase relative to control group while INF-γ levels exhibited obvious reduction (P<0.05).
CONCLUSION
The allergic rhinitis models in SD rat by OVA were successfully established. The levels of IgE, INF-γ and IL-4 in Serum can be used as objective evaluation of animal models of allergic rhinitis established successfully or not.

3.The clinical analysis and treatment of contact granuloma of larynx.

OBJECTIVE:
To explore the clinical features and treatments of contact granuloma of larynx.
METHOD:
To find the best treatments by analyzing the clinical features, related causes and the treatments of the 13 contact granuloma of larynx cases, which have complete data, of our department from Jan 2005 to Dec 2012, also by comparing the effect of conservative treatment and operation treatment.
RESULT:
Contact granuloma of larynx is more common in men (84.6%). The main causes are gastro esophageal reflux, chronic cough, habitual hawk and endotracheal intubation disease. However, a big part (38.4%) gets the disease without any other disease history. The main clinical feature of the disease includes: sensation of foreign matter in throat, easy to get exhausted when talking and voice hoarse, but also 30.7% without any feature in throat. Among the 13 cases, 6 were treated by operation and the first recurrence was 66.6%. The cure rate was 50% after one or two operations. The other 7 were treated in conservative way, including anti- Gastro esophageal reflux, avoiding excessive talking, reducing habitual hawk ,or without any medical treatment. The cure rate was 57.1%. The total cure rate was 53.8%. However, it is nonsense to comparing the two different treatments (Chi2 = 0.066, P > 0.05).
CONCLUSION
There is still no standard treatment for contact granuloma of larynx and the effect is not good enough. The treatment should be depending on individual case. Conservative treatment is the first choice. However, operation should be considered if the patient has obviously hoarse and his granuloma is too large to breathe well.

Objective To evaluate the auditory and verbal abilities of cochlear implantees using question_naires ,to explore basic rules of the development of auditory and verbal abilities ,and to analyze their relationships with some influencing factors .Methods A retrospective analysis of 107 cases of pre-lingually deaf children with cochlear implantation was conducted with Infant-Toddler Meaningful Auditory Integration Scale/Meaningful Audi_tory Integration Scale (IT -MAIS/MAIS) ,categories of auditory performance (CAP) and speech intelligibility rat_ing (SIR) for the evaluation of auditory and verbal abilities .An analysis was performed to assess influencing fac_tors ,such as postoperative time ,age at implantation ,physical age at follow -up ,time with hearing aids before im_plantation .SAS 9 .1 .3 was used in statistical analysis .ResuIts A total of 107 pre-lingually deaf children (48 males and 59 females) participated in this study .The postoperative time ranged from 0 to 49 months ,with an average of 9 .0 ± 10 .9 months .The results showed the postoperative time and age at implantation had significant influences on the scores of IT -MAIS/MAIS ,and postoperative time and physical age at follow -up had significant influences on the scores of CAP/SIR (P<0 .05) ,and hearing aids (HA) usage before implantation had no significant influences on these three questionnaires (P>0 .05) .ConcIusion The ages at implantation ,postoperative time and physical age had significant influences on the auditory and verbal abilities of pre-lingually deaf children with cochlear implanta_tion .Further studies are needed to explore their relationships with hearing aids usage before implantation .
5.The expression and significance of chemokines eotaxin and RANTES in the rat model of allergic rhinitis.

OBJECTIVE:
To explore the expression and significance of Eotaxin and RANTES in the rat model of allergic rhinitis (AR).
METHOD:
20 female SD rats in 6-7 weeks were randomly divided into control group and AR group (n = 10, respectively). AR rat model was made with ovalbumin stimulation. To detect pathological changes in mucosa and chemokine Eotaxin, RANTES in their nasal and lung tissues after execution.
RESULT:
Compared with the control group, Lung EOS cell counted higher in AR group and the difference was significant (P < 0.01); the AR rats nasal mucosa and lung tissue of Eotaxin, RANTES expression was significantly increased (P < 0.01).
CONCLUSION
There exist high expression of Eotaxin, RANTES, infiltration of eosinophils in nasal and lung tissue of model rats with allergic rhinitis, inferring that the upper and lower respiratory tract inflammatory response has obvious consistency.

7.Clinical analysis of primary laryngeal amyloidosis.

OBJECTIVE:
To analyze the clinical features and treatment protocols of primary laryngeal amyloidosis.
METHOD:
Retrospective study of 5 patient hospitalized from 1996 - 2011. All of the patients by resection lesions, including four routine throat tumor resection, and burst throat by supporting laryngoscope in 1 case, all did not give lesions resection radiation and hormone therapy.
RESULT:
All the 5 patients recovered clinically. There were 3 patients followed up for 0.3-7.5 years with a mean time of 3.3 years without recurrence, 2 patients lost follow-up.
CONCLUSION
Middle ages seemed to be more vulnerable. The most common disease region is true vocal cord, followed by false vocal cord, epiglottis former clearance etc. Early surgical treatment of this disease is the most important treatment, larynx endoscopic and CT for the diagnosis of great value, and pathologic biopsy especially Congo red stain positive is the basis of the specific diagnosis of this disease.

8.Influence of nasal septal deviation on incidence of bilateral sinusitis.

OBJECTIVE:
To investigate the influence of nasal septal deviation on the incidence of bilateral sinusitis.
METHOD:
Two thousand and seventy-four cases of nasal septal deviation with chronic sinusitis available from January 2006 to March 2012 were reviewed retrospectively.
RESULT:
Of these 2074 cases, 1 687 (81.3%) patients had sinusitis in narrow side and 1582 (76.3%) in wide side, the difference was statistically significant (P < 0.01) and the relative risk value in narrow side (1.4) was higher than wide side (0.7) associated with the incidence of sinusitis. There was significant difference (P < 0.05) between the two group counting datas analyzed by rank sum test, and the mean rank order in narrow side was 1656.396, significantly greater than wide side (1 612.184). The incidence of sinusitis in men was 50.6% (1049/2074), in women was 49.4% (1025/2074), and there was no significant difference between them. Deviating leftward was 1148 cases (55.4%), deviating rightward was 926 cases (44.6%), and the difference was statistically significant (P < 0.05). Anatomic variations in narrow side and wide side were uncinate process hypertrophy (7.0%, 27.0%), middle turbinate hypertrophy (7.8%,38.0%), ethmoidal bulla hypertrophy (26.2%, 39.0%), paradoxical middle turbinate (5.4%, 15.5%), hypertrophic inferior turbinate (52.9%, 67.0%) and mucosa pachynsis (0.5%, 2.8%), respectively.
CONCLUSION
Sinusitis incidence in narrow side is higher than wide side. There is no significant difference between the incidence of sinusitis in men and women. The majority of nasal septum deviate leftward. Higher incidence of sinusitis in wide side is the result of compensatory mechanism. Only septoplasty may not restore the normal venting function in nasal cavity. Hypertrophic inferior turbinate are closely correlated with sinusitis.

9.Clinical therapeutic effect of obstructive sleep apnea hypopnea syndrome treated by uvulopalatopharyngoplasty.

OBJECTIVE:
To investigate the therapeutic effect of obstructive sleep apnea hypopnea syndrome (OSAHS) by modified uvulopalatopharyngoplasty (UPPP).
METHOD:
Forty-nine cases of OSAHS were treated by modified UPPP, and followed-up on 6 months, 1 year and 2 years after operation and carried out polysomnography (PSG) every time.
RESULT:
Therapeutic effect was evaluated by standard of Hangzhou (2002) . In 6 months, symptoms of snore, breath holding, headache and lethargy disappeared or reduced obviously, the total effective rate was 100%. In 12 and 24 months, the effective rate was 95.92% and 91.84% respectively.
CONCLUSION
This operation can enlarge venting area of palatopharyngeal cavity effectively and keep density of soft palate as well as the normal function of pharyngeal mucous membrane, and can also elevate operative effect and decrease postoperative complications.

10.Experiences of nasal reconstruction with forehead flap: reports of 13 cases.

OBJECTIVE:
To present the experience of nasal reconstruction with forehead flap.
METHOD:
nasal reconstruction with forehead flap were applied in eight nasal carcinoma cases after operation and 5 nasal trauma cases with defects.
RESULT:
These forehead flaps were alive in all patients, all incision healed in I stage, no post operative complications were found. The shapes of nose were satisfactory, there were no recurrence of tumor during 1 to 17 year follow up.
CONCLUSION
The method can be clinically applied for its simple procedure, reliable flap's blood supply, high survival rate and satisfied result.
