Objective To evaluate the diagnostic value of the Eating Assessment Tool (EAT-10) in screening dysphagia by using 2 and 3 as cut-off value with systematic review. Methods A retrieval on the diagnostic value of EAT-10 in dysphagia was done all through PubMed, Embase, the Cochrane Library, CBM, CNKI, WANFANG and VIP databases (from January 2008 to November 2016)."Quality As-sessment of Diagnostic Accuracy Studies" (QUADAS) was used to appraise included papers. Meta-Disc1.4 software was used to analyze heterogeneity and the combined effect of the data. Results Three papers were included and 394 subjects were included in the review. By using 2 and 3 as the cut-off value of EAT-10, the value of the weighted sensitivity was 0.84 [95%CI (0.79, 0.88)] and 0.77 [95%CI(0.71, 0.82)], the specificity was 0.64[95%CI (0.55, 0.72)] and 0.74 [95%CI(0.66, 0.81)],and the area under the receiver-operating-characteris-tic-curve (AUC-ROC) was 0.8621 and 0.8343, which illustrated the diagnostic accuracy of EAT-10 on dysphagia at the medium level. Conclusion The diagnostic ability of EAT-10 is good, but the sensitivity and specificity are different by using 2 and 3 as the cut-off value. The subjects should consider the specific clinical situation to determine the best cut-off value.

Objective To explore the therapeutic effect of Jinkoubao on oral ulcer in rabbits and its action mechanism.Methods Among 60 SPF NewZealand rabbits,6 cases were randomly selected for the ulcer model identification,and the rest was randomly and equally divided into 3 groups:the control group (NC group),normal saline drug film group (NS group) and Jinkoubao film group (JK group).The rabbit model of oral ulcer was constructed by applying 40 ％ glacial acetic acid to burn rabbit oral mucosa for duplicating the oral ulcer model.of the change situation of oral ulcer was observed on the same day for constructing model,on 3,5,7 d after medication.The EGF level in oral mucosal tissue was detected by using RT-PCR and the local histopatho1ogical changes in oral ulcer was observed by using HE staining method.Results Compared with the NS group,the ulcer area on 3,5,7 d after medication in the JK group was significantly deceased (P＜0.01).Compared with the NS group,the EGF level in oral buccal mucosal tissue in the NS group and JK group was markedly increased (P＜0.01).But the EGF level increase in the JK group was faster than that in the NS group,the difference was statistically significant(P＜0.01).The HE staining section of rabbit oral ulcer on 3,5,7 d after medication showed that the inflammatory cells decrease in the JK group was more obvious than that in the NS group,fibroblasts proliferation was obvious and epithelial hyperplasia was good.Conclusion Jinkoubao could greatly improve the symptoms of oral ulcer in rabbits and promotes its healing,which is possible to strengthen the repair capacity of oral ulcer by regulating the EGF level.

Objective To analyze the causal relationship between gastroesophageal reflux disease (GERD) and chronic obstructive pulmonary disease (COPD) based on the bidirectional two-sample Mendelian randomization (MR). Methods Genetic variation information of GERD and COPD was obtained from Genome-Wide Association Studies (GWAS) and used as instrumental variables. Inverse variance-weighted (IVW), weighted median and MR-Egger methods were used for MR analysis, and sensitivity analysis was performed to validate the robustness of the results. Results A significant positive correlation was observed between genetically predicted GERD and the incidence risk of COPD, but there was no statistical association between COPD and the incidence risk of GERD. Positive IVW result showed that the odds ratio (OR) was 1.305 7, the 95% confidence interval (95%CI) was 1.114 4 to 1.529 8, and the P value was 0.000 9; the reverse IVW result showed that the OR was 0.982 3, the 95%CI was 0.917 4 to 1.051 9, and the P value was 0.610 4. Sensitivity analysis did not find any potential bias. Conclusion MR analysis shows that GERD is a risk factor for COPD, and treating GERD may help prevent or delay the progression of COPD.

Objective: To detect the expression of basic fibroblast growth factor(bFGF) in oral mucosa with ulcer in rabbits. Methods: 72 New Zealand rabbits(with the weight of 3 000-3 500 g) were randomly divided into control group,model group,and treatment group(n = 24). 1,3,5 and 7 d after treatment buccal mucous membrane tissues of the rabbits were respectively taken from the 3 groups. The models of oral ulcer were examined by HE staining. The expression of bFGF mRNA was detected by RTPCR. The expression of bFGF protein was detected by immunohistochemistry. Results: The oral ulcer model of the rabbits was successfully established. Both RT-PCR and immunohistochemistry analyses showed that 1-7 d after treatment the expression levels of bFGF mRNA and protein were higher in treatment group than in model group(P ＜ 0. 05) and control group(P ＜ 0. 05),3-7 d after treatment were higher than in model group(P＞ 0. 05). Conclusion: bFGF may be a new therapeutic target for oral ulcer.

0bjective To synthesis evidence of self-efficacy-based interventions to improve self-management and health behaviour change in patients with diabetes. Methods PubMed, Cochrane, MEDLINE, CINAHL, EBSCO and Web of Science were searched from 1977 to August 2018. The primary outcomes were self-efficacy and self-care ability. Pooled effect sizes of standardised mean difference ( SMD ) were calculated if outcome measurements and the units were different, and if outcome measurements and the units were the same, such as measuring blood glucose by HbA1c, weighted mean difference ( WMD ) were used to calculate pooled effect sizes. Results Nine eligible randomised control trials (RCTs) including 1578 participants were identified. The results showed that interventions based on self-efficacy significantly improved patients′self-efficacy ( SMD=0.69, 95% CI :0.58 to 0.80, P＜0.05) and self-management ( SMD=0.93,95% CI : 0.74 to 1.12, P＜0.05), helped them control blood glucose ( WMD=-0.69, 95% CI :-0.85 to-0.52, P＜0.05), reduced hospitalisation and emergency times, and improved their quality of life ( SMD=0.52,95% CI : 0.32 to 0.72, P＜0.05). Conclusions Even though the results showed significant improvement in self-efficacy and self-management after interventions, the conclusion needs to be generalised with caution because of the quality of RCTs and high heterogeneity. Future trials could consider RCTs with high quality, appropriate sample size, specific component of diabetes management, and reliable and valid scales or objective measures as outcomes.

Objective To explore the status quo and influencing factors of caring efficacy of nursing undergraduate students so as to provide reference for improving their caring efficacy. Method A cross-sectional study was conducted. Totally 274 nursing undergraduate students from 10 general hospitals in Sichuan Province were surveyed by using general information questionnaire, professional model scale for nursing students, professional identity questionnaire for nursing students and caring efficacy scale. Results The average score of caring efficacy of undergraduate nursing students was (4.00 ±0.64), which was lower that of jurior nurse before training (4.55±0.34) and after training (4.98±0.27). The average score of career identity and professional model (3.63±0.68) were postively related with career effevtieness (all P＜0.001). The care from family, medical staffs, job-taking benefits and dimission risks, social comparison and self-reflection were the main factors influencing caring efficacy (P＜0.01), which explained 40.9％ of the varianceof caring efficacy among undergraduate nursing students. Conclusions Caring efficacy of the nursing students was above the middle level, but lower than that of clinical junior nurses. Their professional model and professional identity were positively correlated with caring efficacy. It is necessary to establish a good professional model, through example education, perfect career planning and improve the professional identity of nursing undergraduate students, so as to improve their caring efficacy of nursing students, and then improve the quality of nursing team reserves.

Objective:To investigate the status quo of readiness for hospital discharge of patients with coronary heart disease after percutaneous coronary intervention(PCI)and analyze the main factors affecting readiness for hospital discharge.Methods:With a cross-sectional survey method, a total of 343 patients with coronary heart disease after PCI in the Affiliated Hospital of Southwest Medical University were selected by convenience sampling method and investigated by General Information Questionnaire, Readiness for Hospital Discharge Scale, Care Transitions Measure and Self-Efficacy Questionnaire of coronary heart disease. Multiple linear regression analysis was used to analyze the influence factors.Results:The total score of Readiness for Hospital Discharge Scale of patients with coronary heart disease after PCI was 94.33±8.67. The score of quality of care transitions was 47.59±5.51 and that of self-efficacy was 39.19±8.08. Multiple linear regression analysis showed that five variables including self-efficacy, length of hospital stay, marital status, residence and family history of coronary heart disease entered into the regression equation ( P<0.01), which could explain 50.0% of the variations of readiness for hospital discharge. Conclusions:The readiness for hospital discharge of patients with coronary heart disease after PCI is at the middle level. The main influencing factors are self-efficacy, length of hospital stay, marital status,residence and family history of coronary heart disease. Nurses should attach importance to the self-efficacy of patients and pay attention to high-risk groups. By improving patients′ self-efficacy and giving targeted guidance ahead, in order to improve patients′ perception of readiness for hospital discharge.

Objective:To systematically evaluate the application effect of somatosensory interactive games in patients with chronic obstructive pulmonary disease (COPD) in order to provide a basis for clinical application.Methods:Randomized controlled trials and quasi-experimental studies on the application of somatosensory interactive games in patients with COPD were retrieved from PubMed, Embase, Cochrane Library, CINAHL, Chinese Biomedical Literature Database, Wanfang, China Knowledge Network and VIP. The retrieval time limit is from the establishment of the database to October 2022. RevMan5.2 software was used for Meta analysis.Results:Ten articles involving 611 patients were included. The results of Meta-analysis showed that the somatosensory interactive game group was superior to the conventional training group in improving FEV 1/FVC% ( WMD=6.83, 95% CI 3.71-9.95, Z=4.29, P<0.01), 6-minute walking distance (6MWD) ( WMD=13.36, 95% CI 0.50-26.23, Z=2.04, P<0.05), Hospital Anxiety and Depression Scale ( WMD=-1.64, 95% CI -2.31--0.97, Z=4.78, P<0.01), COPD Assessment Test score ( WMD=-2.95, 95%CI -4.08--1.82, Z=5.13, P<0.01) in COPD patients. However, there was no significant difference in FEV1 % ( WMD=2.91, 95% CI -1.61-7.43, P=0.210) and dyspnea ( SMD=0.63,95% CI -0.24-1.49, P=0.150) between the two groups. Conclusions:Compared with conventional training, somatosensory interactive game training can effectively improve lung function and quality of life in patients with COPD, relieve anxiety and depression. Due to the limited quantity and quality of the included literature, the reliability of the conclusions still needs to be verified by more high-quality studies.

To meet the requirements of social development, this article explored the significance of establishing the course of nursing financial management and described the current development situation of establishing the course of nursing financial management. This article also put forward some thinking and suggestions for establishing the course of nursing financial management involving promoting nursing financial research, perfecting the compilation of teaching material, strengthening the construction of teaching staff to improve the teaching level, carrying out diversified teaching form to meet different demands and evaluating teaching comprehensively to improve the quality of teaching.

Objective:To investigate the application value of double contrast-enhanced ultrasonography (DCEUS) combined with serum pepsinogen (PG) in the diagnosis of early gastric cancer(EGC).Methods:Eighty-two patients suspected of EGC from July 2020 to July 2022 in the People′s Hospital of Guangxi Zhuang Autonomous Region, and preoperative DCEUS examination and PG test were performed, and the patients were divided into benign lesion group(13 cases), early gastric cancer group(57 cases) and progressive gastric cancer group(12 cases) using postoperative pathology as the gold standard. Parameters for comparison included time to peak (TTP), peak intensity (PI), enhanced intensity (EI), serum pepsinogen Ⅰ (PGⅠ), serum pepsinogenⅡ (PGⅡ) and their ratio (PGⅠ/PGⅡ). The sensitivity, specificity, and accuracy of DCEUS and PG alone and in combination for the diagnosis of EGC were analyzed by plotting the ROC curve, and its diagnostic value was compared.Results:In the comparison of DCEUS parameters, PI and EI values were higher in the malignant group than in the benign lesion group and TTP was the opposite, with statistically significant differences (all P<0.05). In the comparison of PG detection, PGⅠ and PGⅠ/PGⅡ were lower in the malignant group than in the benign lesions, and lower in the progressive gastric cancer than in the EGC, while PGⅡ was the opposite, with statistically significant differences (all P<0.05). As shown by the ROC curve results, the sensitivity of DCEUS and PG alone and in combination for the diagnosis of EGC was 80.7%, 73.7% and 87.7%, respectively; the specificity was 76.0%, 72.0% and 80.0%, respectively; and the accuracy was 79.3%, 73.2% and 85.4%, respectively. The area under curve (AUC) of the two modalities alone and combined were 0.784 (95% CI=0.669-0.898), 0.728 (95% CI=0.606-0.850) and 0.839 (95% CI=0.734-0.943), respectively, and the combined diagnosis had a higher diagnostic value than the single diagnostic modality. Conclusions:The combined diagnostic modality of DCEUS and PG can further improve the diagnostic efficacy of EGC and reduce its underdiagnosis rate, which has good application value.
