1.Endoscopic Frontal Sinus Surgery through the Agger Nasi Cell

Objective To discuss the effectiveness of endoscopic frontal sinus surgery through the agger nasi cell. Methods Twenty-seven patients with chronic frontal sinusitis were treated with endoscopic frontal sinus surgery. The frontal sinus was opened by resecting the agger nasi cell. Results The frontal sinus were opened successfully in all the patients. No complications occurred except periorbital bruising in 3 eyes. Nasendoscopy showed that the frontal sinus drainage pathway was normal after the operation. During a 6-month follow-up, the symptoms of the frontal sinus disappeared in 18 of the patients, improved in 7, and were not changed in 2. Conclusions Endoscopic frontal sinus surgery through the agger nasi cell is effective for chronic frontal sinusitis.The method is worth being widely used.
2.Sanitary Quality of the Blood Dialysate in Hospitals and Its Infuential Factors in Hu'nan

Objective To understand the sanitary quality of the blood dialysate in hospitals in Hu'nan and its influential factors. Methods Pyrogen,total count of bacteria,22 physical and chemical indexes including calcium,chloride,fluoride,zinc,sodium and other indexes in the blood dialysate samples collected from blood dialysis rooms in 99 hospitals distributed among 14 cities and states in Hu'nan were measured according to the Sanitary Standards of Drinking Water and the Sanitary Standards for Disinfection Technology. The data of analysis were evaluated based on the standards for the blood dialysate ruled by ASAIO and AAMI. The questionnaire on the status operation and maintenance of the water purification system for blood dialysate was performed simultaneously. Results The sanitary quality of the blood dialysate samples from 31 hospitals were completely accorded with the related standards with a qualified rate of 32%. The qualified rates were 44.4%,71.7%,90.9%,97.0%,99.0%,99.0%,99.0% for pyrogen,total count of bacteria,calcium,chloride,fluoride,zinc,sodium in the blood dialysate samples respectively. The blood dialysate from the water treatment system established and put into production during 2000-2003 showed higher qualified rate(45.1%)compared with that during 1995-1999(16.7%),?2=9.293,P
3.Clinical effects of keen osteoarthritis and meniscus injury by comprehensive treatment

Objective To investigate the projects and effects of the comprehensive and conservative treatment of patients with keen osteoarthritis and meniscus injury.Methods 120 patients with keen osteoarthritis and meniscus injury were randomly divided into the control group A(n =40),B(n =40)and comprehensive treatment group C(n =40).Patients in group A were treated with ozone and glass sodium injection;Patients in group B were treated with ozone,glass sodium and Triamcinolone Acetonide injection;Patients in group C were treated with ozone, glass sodium and Triamcinolone Acetonide injection,superlaser local irradiation,glucosamine tablet oral,Lornoxicam intravenous injection.The effects of the treatment according to keen joints function scores were compared after two weeks and six months.Patients in group C were randomly divided into group C1(n =20)and group C2(n =20)after one year,patients in group C1 with the same treatment as patients in group A and oral glucosamine tablet for one course of treatment,patients in group C2 without any further treatment.The follow up was completed for two years. Results The excellent and good rate in group C was better than that of group B and group C after two weeks (70.0%,22.5%,42.5%,χ2 =18.152,6.146,P <0.01 or P <0.05).After six month,the excellent and good rate in group C was better than that of group A and group B (87.5%,42.5%,60.0%,χ2 =17.802,7.813,all P <0.01).There was no significant difference between group C1 and group C2 after one years(t1 =2.014,P <0.01). There was significant difference between group C1 and group C2 after two years(t2 =16.152,P <0.01).Conclusion The effects of comprehensive conservative treatment are better than simple keen joint injection in patients with keen osteoarthritis and meniscus injury.The function of keen joint with maintenance care treatment is better than without maintenance care.Comprehensive conservative and maintenance care treatment is a good clinical treatment project.

5.Mastoidectomy in the treatment of secretory otitis media.

OBJECTIVE:
To investigate mastoidectomy efficacy in treating secretory otitis media.
METHOD:
Retrospective analysis of 22 cases (24 ears) with chronic secretory otitis media,20 ears were treated with intact canal wall mastoidectomy combined with facial recess opening,4 ears were treated with opened mastoid surgery,3 ears simultaneously accepted tube insertion. Ventilation tube was pulled out in 6 months. Hearing test was inspected before and after surgery.
RESULT:
None of the patients had hearing loss, 19 ears had varying degrees of hearing improvement. Seventeen ears were type A tympanometry curve, 7 ears were C-shaped curve. No recurrence of otitis media was observed after 6 - 36 months followed-up.
CONCLUSION
Mastoidectomy may improve eustachian tube function and decrease the risk of recurrence of secretory otitis media.

6.Clinical efficacy of mouse nerve growth factor in the treatment of sudden deafness.

OBJECTIVE:
To study the clinical efficacy of mouse nerve growth factor (NGF) in the treatment of sudden deafness.
METHOD:
A retrospective analysis was performed on 115 cases of hospitalized patients who were suffered from sudden deafness. Patients were divided into two groups according to treatment medicine. Control group: patients were treated with intravenous vasodilators, energy mixture, steroid pulse therapy, and methylcobalamin neurotrophic therapy. NGF group: intramuscular NGF treatment was added on the basis of conventional therapy mentioned above. Both treatments lasted 14 days, the total efficiency were compared. Patients were further divided into sub-groups according to age, duration and the level of pre-treatment PTA, and the treatment efficiency was further compared. By SPSS 11.0 statistical analysis, a P < 0.05 was considered as statistical significant difference.
RESULT:
(1) The total efficiency of NGF group was significantly higher than control group. (2) Regard of age, the efficiency of NGF treatment group was significantly higher than control group. (3) For the patients whose duration were less than 7 d, or the PTA < or = 60 dBHL, the efficiency of NGF group were significantly higher. For the patients whose duration were more than 7 d, or the PTA>60 dBHL, the efficiency of NGF therapy was not superior to the traditional treatment.
CONCLUSION
NGF can significantly improve the symptom of patients with short duration or low PTA. For this kind of patients, NGF adjuvant therapy should be recommended. For the patients with longer duration and higher level of PTA, NGF therapy is not advocated. NGF treatment should not be in consideration of the age.

0.05). In the xenogeneic fresh nerve graft group, the xenogeneic nerve segment was rejected and absorbed by the recipient. CONCLUSION Xenogeneic acellular nerve could sustain facial nerve regeneration, and may be a substitute to autograft for repairing facial nerve defects.
9.Chlamydia trachomatis serovar D genital tract infected mice model: establishment and evaluation

ObjectiveTo construct a mouse model for studying pathophysiology and mechanism of human Chlamydia trachomatis genital infection.MethodsInnate immunity-deficient C3H/HeJ female mice were infected intravaginally with human C.trachomatis serovar D urogenital isolates for screening the highest violent clinical strain.The clinical strain UT0603 as well as standard strain D/UW-3/CX were then used to reinfect na(i)ve mice,the lower genital tract shedding were monitored by swabbing every 3-7 day over the entire infection period by culture.Some mice were sacrificed at early infection stage to detection of in site Chlamydia growth by immunofluorescence assay,then all the mice were sacrificed at later infection stage to evaluate upper genital tract gross pathology and histopathological characterization.ResuIts In the lower genital tract,Chlamydia shedding time course were significantly prolonged in clinical strain infected mice.Chlamydia not only growth in the lower genital tract,the live organism also ascending and growth in the upper genital tissue.The gross appearance under naked eyes and dilation and inflammation scores under microscope all showed that the genital tract pathology from the clinical strain infected mice were much more severe than standard strain infected control mice.Conclusion Together,all these results demonstrated that a mouse model for Chlamydia genital infection was constructed.
10.Repair of facial nerve defects by using acellular nerve allografts implanted with Schwann cells in rats

Objective To observe the effects of in vitro isolated Schwann cells co-cultured with chemically acellular nerve allografts on improving repair of large facial nerve defects. Methods A total of 30 Wistar rats were equally randomized into three groups, ie, experimental group, allograft group and autograft group. Nerve defect of 12 mm in length was made in the left inferior buccal branch of facial nerve and repaired with acellular nerve allograft implanted with Schwann cells, acellular nerve allograft and fresh tibial nerve autograft respectively. At the 5th month postoperatively, the function and morpholo-gy of the regenerated nerves were observed by electrophysiological method, methylene blue staining and transmission electron microscope. Results In experimental group, the recovery rate (operation side/normal side) of amplitude of nerve-muscle action potential was (35.8±2.5)%, the lantency recovery rate (normal side/operation side) (65.8±2.9)%, the number of the regenerated axon 1 570±188 and the myelin thickness (0.383±0.031) μm. The results in the experimental group were significantly supe-rior to those in the acellular nerve allograft group (P < 0.05), with similar results to fresh nerve autograft group (P > 0.05). Conclusion Transplantation of Schwarm cells in acellular nerve allograft can im-prove repair of large facial nerve defects.
