1.The Application of G5 percussive mobilizer in bronchial purification treatment of repiratory failure

ObjectiveTo investigate the clinical value of G5 Percussive Mobilizer in respiratory failure.Methods76 patients with respiratory failure were randomly divided into the treatment group (38 cases) and control group (38 cases). The treatment group were treated with G5 Percussive Mobilizer made in America,and the control group were treated with knock expectoration. The daily sputum volume, clinical symptoms, thoracic signs and arterial blood gas analysis changes were observed. ResultsThe daily sputum volume,clinical symptoms,thoracic signs and arterial blood gas analysis changes of the treatment group (31 cases effective,81.6% ) has significant difference compared with the control group (23 cases effective ,60.5% ) ( P < 0. 05 ). ConclusionG5 Percussive Mobilizer can eliminate the bronchial secretion rapidly and effectively,and it can help improve the ventilation and oxygenation function. It is safe, effective and easy to practice in bronchial purification treatment.
2.Social support and subject well-being of only-child-lost people:the mediating role of self-efficacy

Objective To investigate the relationship between social support and subjective well-be-ing in only-child-lost ( OCL ) people, and the mediating effect of self-efficacy in this relationship. Methods By stratified cluster sampling,206 only-child loss people were investigated with Social Support Rating Scale ( SSRS) ,General Self-Efficacy Scale ( GSES) and Index of Well-Being ( IWB) ,and data were analyzed with SPSS19.0 software and Amos17.0 software. Results ①Scores of SSRS,GSES and IWB in only-child-lost people were(31.78±8.18),(24.12±0.67) and (8.38±3.05). ②Correlation analysis showed that scores of GSES,SSRS and 3 dimensions of SSRS were positively correlated with scores of IWB ( r=0. 542,0.332,0.214,0.332,0.216;P<0.05) . ③Self-efficacy partially mediates the relationship between social support and subjective well-being through mediating effect test(χ2/ df=1.986,GFI=0.985,AGFI=0.942, RMSEA=0.069),mediating effect accounted for 16.08%.Conclusion Social support is an important factor which plays a direct or indirect role in subject well-being. Self-efficacy is the intermediate link between social support and subject well-being.
3.Development of a software for predicting the effects of nuclear and radiological terrorism events in city areas

Objective To develop a new software system that cau directly display the predicted results on an electronic map,in order to get a directly perceived understanding of the affected areas of nuclear and radiological terrorism events in city areas.Methods Three scenarios of events including spreading radioactive materials, dirty bomb attack,and explosion or arson attacks on the radiation facilities were assumed.Gaussian diffusion model was employed to predict the spread and deposition of radioactive pollutants,and both the internal and external doses were estimated for the representative person by using the corresponding dose conversion factors.Through integration of the computing system and Mapinfo geographic information system(GIS),the predicted results were visually displayed on the electronic maps of a city.Results The new software system could visually display the predicted results on the electronic map of a city, and the predicted results were consistent with those calculated by the similar software Hotspot(R)The deviation between this system and Hotspot was less than 0.2 km for predicted isoplethic curves of dose rate downwind.Conclusions The newly developed software system is of the practical value in predicting the effects of nuclear and radiological terrorism events in city areas.

5.Expression of EZH2 and Ki-67 in salivary adenoid cystic carcinoma and their clinical significance

Objective:This study aimed to investigate the expressions of EZH2 and Ki-67 in the salivary adenoid cystic carcino-ma (SACC) of humans and their correlation. Methods:A total of 42 cases of SACC tumor tissues and 5 cases of normal tissues were considered to determine the expressions of EZH2 and Ki-67 by immunohistochemistry. The relationship and correlation of such expres-sions with the clinicopathological characteristics were also analyzed. Results:The expression of EZH2 was notably higher in SACC than in normal tissues (P<0.05). EZH2 expression was detected in 66.67%(28/42) of the tumor tissues. This expression was correlated with pathological grade and clinical stage. By contrast, EZH2 expression did not correlate with gender, age, and localization. EZH2 was not expressed in normal tissues. The incidence of EZH2 expression in the Ki-67 positive group was 75.76%(25/33) and the incidence in the Ki-67 negative group was 33.33%(3/9). The difference between the two groups was statistically significant (P<0.05). Conclu-sion:The increased expression of EZH2 in SACC was related to tumor proliferation. EZH2 may participate in tumor cell proliferation via cell cycle management.
6.Risk factors for postoperative liver failure of patients with hepatocellular carcinoma and bile duct tumor thrombus

Objective To investigate the risk factors for postoperative liver failure of patients with hepatocellular carcinoma (HCC) and bile duct tumor thrombus through a risk evaluation model.Methods The clinical data of 107 patients with HCC and bile duct tumor thrombus who received hepatic resection at the Eastern Hepatobiliary Surgery Hospital from March 2002 to February 2011 were retrospectively analyzed.All patients were divided into the non-liver failure group (98 patients) and liver failure group (9 patients).Risk factors associated with liver failure were analyzed and a risk evaluation model was established.All data were analyzed using the bivariate regression model,and factors with significance were further analyzed using the multivariate regression model.Results Of the 107 patients,105 received hepatic resection + choledochotomy + thrombectomy and 2 received hepatic resection + extrahepatic bile duct resection + cholangiojejunostomy.The operation time was 2.0-5.5 hours,and the intraoperative blood loss was 200-3500 ml.In the non-liver failure group,5 patients had pleural and peritoneal effusion,3 had biliary bleeding,2 had incisional infection,1 had biliary infection,1 had bile leakage,1 had stress-induced ulcer of upper digestive tract and 1 had thoracic epidural hematoma.The bleeding of the patients with thoracic epidural hematoma was stopped after thoracic spinal decompression,but subsequent paraplegia occurred.In the liver failure group,2 patients died of postoperative acute liver failure,and 7 patients died of postoperative subacute liver failure (death caused by tumor recurrence or medicine was excluded).The results of univariate analysis showed that preoperative total bilirubin,albumin,pre-albumin,albumin/globulin ratio,distribution of tumor thrombus,operative blood loss and ratio of postoperative residual liver volume to the total liver volume were correlated with the postoperative liver failure in patients with HCC and bile duct tumor thrombus (OR =3.017,0.191,0.248,2.681,9.048,4.759,13.714,P < 0.05).The results of multivariate analysis showed that preoperative total bilirubin > 256.5 μmol/L,albumin/globulin ratio ≤ 1.3 and postoperative residual liver volume < 50% were the independent risk factors of postoperative liver failure (OR =5.537,11.107,172.450,P < 0.05).The risk evaluation model was Z =1.77 × preoperative total bilirubin + 2.408 × preoperative albumin/globulin ratio + 5.150 × ratio of postoperative residual liver volume to the total liver volume-17.288.The risk of postoperative liver failure increased as the increase of Z value.The risk of postoperative liver failure > 50% when the Z value > 0.Conclusions Preoperative total bilirubin > 256.5μmol/L,albumin/globulin ratio ≤ 1.3 and postoperative residual liver volume < 50% were the independent risk factors of postoperative liver failure.Risk evaluation model is helpful in screening the risk factors so as to decrease the incidence of postoperative liver failure.
7.Mediation role of self-efficacy between social support and depression of only-child-lost people

Objective:To investigate the relationship between social support and depression of only-child-lost (OCL) people,and the mediation role of self-efficacy in this relationship.Methods:By stratified cluster sampling,214 OCL people were enrolled,with 80 males and 134 females,ages from 49 to 83 years old.They were assessed by General Self-Efficacy Scale,Social Support Rating Scale,and Self-rating Depression Scale.Results:Univariate analysis showed that there were significant differences in age groups (t=2.85,P<0.05),with or without spouse (t=5.62,P<0.05),family location (t=3.95,P<0.05),per capita monthly income (F=3.48,P<0.05) among the social support scores.There was significant difference between the per capita monthly income and self-efficacy scores in QCL people (F=5.46,P<0.05).Correlation analysis showed self-efficacy and social support were positively correlated (r=0.26,P<0.01).Self-efficacy (r=-0.59,P<0.01) and social support (r=-0.59,P<0.01) negatively correlated with depression in OCL people.Self-efficacy partially mediated the relationship between social support and depression.Conclusion:The person who is <60 years old,with spouse and the high per capita monthly income,and lives the rural area,would have high social support levels among QCL people.The person who has high per capita monthly income would have high self-efficacy.Self-efficacy is one of the direct prediction for depression,and plays an indirect role between social support and depression.Intervention of depression among OCL people could be applied to change their cognition,and to enhance their self-efficacy.
8.Correlation study between lipid levels and the risk of multiple system atrophy

Objective To look for more serum biomarkers supporting the diagnosis of multiple system atrophy ( MSA) and providing more evidence for early treatment.Methods All patients and healthy controls were enrolled from January 2011 to March 2015 in the First Affiliated Hospital of Zhengzhou University.Demographic features and biochemical examination results were collected.The t test was used to compare the lipid levels between MSA patients and controls.LSD-t test was used to compare the lipid levels among subtypes of MSA patients.Multivariate Logistic regression analysis was conducted to analyze the influencing factors.The relevance between lipid levels and onset age, disease duration and Hoehn & Yahr stage was calculated by Spearman correlation coefficients.Results Participants included 195 MSA patients and 195 age-and gender-matched controls with no neurological diseases.The levels of total cholesterol ((4.33 ±0.90) mmol/L), triglyceride ((1.27 ±0.71) mmol/L), low-density lipoprotein (LDL;(2.70 ±0.76) mmol/L) were significantly lower in patients than in controls ((4.52 ±0.85), (1.47 ± 0.86), (2.85 ±0.71) mmol/L ,t=2.056,2.528 and 2.149 respectively, all P<0.05).The levels of total cholesterol ((4.28 ±0.96) mmol/L) and triglyceride ((1.20 ±0.64) mmol/L) were significantly lower in MSA-P patients than in control group ((4.52 ±0.85), (1.47 ±0.86) mmol/L;LSD-t=1.983, 2.566, both P<0.05).After adjusting for age, gender and histories, the odds ratio ( OR) was 0.31 (95%CI 0.15-0.65, P =0.002 ) for MSA patients in the highest quartile of triglyceride and 0.38 (95%CI 0.17 -0.83,P=0.016) for those in the highest quartile of high-density lipoprotein (HDL), compared with the lowest quartiles.And HDL level was in a significantly positive correlation with onset age (r=0.15, P=0.039).Conclusion Our data suggest that triglyceride and HDL may be associated with the prevalence of MSA, and the lower levels of HDL, the earlier onset of MSA.
9.Preliminary investigation of paravaginal support structure in women by Hyaline imaging of three-dimensional pelvic floor sonography

Objective We aim to evaluate and discuss the feasibility of Hyaline imaging of three-dimension sonography in observation of paravaginal support structure in normal nonporous women.Methods Total of 45 normal infertile women were chosen.Three-dimensional volume datasets were collected at rest by transperineal ultrasound.The three-dimensional Hyaline images were acquired off-line.We observed the morphologic features of paravaginal support structure on the axial plane in middle vagina and measured the anteroposterior and lateral horizontal distances between bilateral paravaginal support structure and ureter.The consistency between two sonographers were analyzed by intraclass correlation coefficient (ICC).Results In three-dimensional Hyaline imaging sonogram,paravaginal support structure was appeared as typically tenting-like shaped.The anteroposterior distances between paravaginal support structure and center of ureter were listed as followed:left (-0.31 ± 1.29) mm,right (0.47 ± O.99) mm.While the lateral horizontal distances were left (12.67 ± 6.70) mm,right (13.01 ± 5.75) mm.There were no statistical differences between bilateral both anteroposterior and lateral horizontal distances.The consistency between two sonographers for distinguishing paravaginal support structure on the middle-vaginal plane and measuring distances between paravaginal support structure and center of ureter was remarkably high (ICC is 0.87 and 0.82).Conclusions Hyaline imaging technology of three-dimensional pelvic floor sonography could be used to observe paravaginal support structure in women.The normal paravaginal support structure is typically showed as bilateral symmetrical tenting-like structure,which is on the same level as the center of ureter.It provides normal anatomic radiological evidence and helps study defect of paravaginal support structure caused by pregnancy or delivery.

Pharmacological perturbation studies based on protein-level signatures are fundamental for drug dis-covery.In the present study,we used a mass spectrometry(MS)-based proteomic platform to profile the whole proteome of the breast cancer MCF7 cell line under stress induced by 78 bioactive compounds.The integrated analysis of perturbed signal abundance revealed the connectivity between phenotypic behaviors and molecular features in cancer cells.Our data showed functional relevance in exploring the novel pharmacological activity of phenolic xanthohumol,as well as the noncanonical targets of clinically approved tamoxifen,lovastatin,and their derivatives.Furthermore,the rational design of synergistic inhibition using a combination of histone methyltransferase and topoisomerase was identified based on their complementary drug fingerprints.This study provides rich resources for the proteomic landscape of drug responses for precision therapeutic medicine.
