1.Study on clinic nursing personnel's work load in our hospital

Objective To explore the collocation ratio of direct nursing time to indirect nursing time of clinic nursing personnel in our hospital and the difference between different departments, so as to offer academ-ic evidence to collocate the nursing personnel resources reasonably. Methods Observation method was adopted to choose six typical clinics such as respiration internal medicine and so on as observed objects, and then tested the working hours of 22 nursing proceedings which were defined by the topic group, so as to get the work load and average working hours of nurses. Results Time needed for direct and indirect nursing pro-ceedings in different departments was obtained, as well as the ratio of 5 proceedings accounted for the total 22, also deficiency of nursing personnel in 6 departments were tested. Conclusions Evaluation of clinic nursing personnel's work load as an important index to measure the labor intensity of chnic nursing personnel can offer important evidence to optimize allocation of clinic nursing human resources.
2.Changes in endoscopic detection rate of peptic ulcer and gastric cancer after 10 years' eradicative treatment of Helicobacter pylori

Objective To investigate the changes in the detection rate of gastric ulcer,duodenal ulcer and gastric cancer by gastroscopy after 10 years' eradicative treatment of Helicobacter pylori,in order to learn the characteristic alternations of these diseases and the significance of eradicative treatment of Helicobacter pylori.Methods The clinical and endoscopic data of gastric ulcer,duodenal ulcer and gastric cancer at our hospital in January 1994 to December 1995 and January 2004 to December 2005 were collected and analyzed.Results The detection rate of gastric ulcer,duodenal ulcer,and gastric cancer by gastroscopy during the period of January 2004 to December 2005(5.53%,10.07% and 4.03%,respectively) was significantly lower than that during the period of January 1994 to December 1995(8.17%,11.21% and 6.21%,respectively,P
3.Predictive value of C-reactive protein and procalcitonin in the early forecasting acute pancreatitis based on the new Atlanta classification criteria

Objective Based on the 2012 Atlanta classification criteria, to study the value of C-reactive protein (CRP) and procalcitonin (PCT) in the early forecasting acute pancreatitis (AP). Methods Eighty-three patients with AP were selected. The patients were divided into mild AP (MAP) group (39 cases), moderately severe AP (MSAP) group (31 cases) and severe AP (SAP) group (13 cases) according to the 2012 Atlanta classification criteria. Twenty-seven healthy people were selected as control group. The levels of serum CRP and PCT were measured. The predictive value of serum CRP and PCT levels for SAP, infectious pancreatic necrosis (IPN), organ failure and death risk was assessed using the area under the curve (AUC). Results The serum CRP and PCT levels in MAP group, MSAP group and SAP group were significantly higher than those in control group: (49.84 ± 12.26), (89.77 ± 22.10) and (123.69 ± 37.09) mg/L vs. (3.92 ± 1.37) mg/L, (1.15 ± 0.42), (2.44 ± 0.61) and (3.27 ± 0.96)μg/L vs. (0.41 ± 0.13)μg/L, and those in MSAP group and SAP group were significantly higher than those in MAP group, those in SAP group were significantly higher than those in MSAP group, and there were statistical differences (P<0.05). The Spearman correlation analysis result showed that AP severity was positively correlated with serum CRP and PCT levels (r = 0.652 and 0.714, P<0.05). The accuracy of serum CRP level for forecasting SAP and IPN was medium (AUC = 0.73 and 0.76), and for forecasting organ failure accuracy was low (AUC = 0.67). Serum CRP showed no significance in forecasting death risk (AUC = 0.46). The accuracy of serum PCT level for forecasting SAP, IPN and death risk accuracy was medium (AUC = 0.71, 0.86 and 0.80), and for forecasting organ failure accuracy was low (AUC =0.64). Conclusions Based on the 2012 Atlanta classification criteria, the accuracy of serum CRP level for forecasting SAP is higher than that of serum PCT level, the accuracy of serum PCT level for forecasting IPN and death risk is higher than that of serum CRP level, and accuracy of serum CRP and PCT levels for forecasting organ failure are low.
4.Chondrogenic differentiation of bone marrow mesenchymal stem cells induced by different growth factors

BACKGROUND:Articular cartilage is a highly diferentiated tissue, which is very limited in its ability to repair after injury. Stem cel therapy for cartilage repair has completely solved this problem. OBJECTIVE: To investigate the mechanism that diferent growth factors induce the diferentiation of bone marrow mesenchymal stem cels into chondrocytes. METHODS:Passage 5 rat bone marrow mesenchymal stem cels were induced by diferent growth factors and their combinations, including transforming growth factor beta 1 (TGF-β1) group, TGF-β1+insulin growth factor-1 (IGF-1) group, and bone morphogenetic protein 2 (BMP-2)+IGF-1 group, TGF-β1+BMP-2 group, TGF-β1+IGF-1+ BMP-2 group, and blank control group. At 21 days of induction, cels were stained with alcian blue and alizarin red; RT-PCR was employed to detect colagen I mRNA expression. RESULTS AND CONCLUSION:Alcian blue staining showed metachromasia in the cytoplasm and mesenchyma, and proteoglycan was expressed green; alizarin red staining showed no orange calcium nodules. The bone marrow mesenchymal stem cels were preliminarily deduced to diferentiate into chondrocytes, but could not express the cel phenotypes of bone cels. In the blank control group, the expression of colagen I mRNA was negative. Compared with the TGF-β1 group, the mRNA expression of colagen I was lower in the BMP-2+IGF-1 group, but higher in the TGF-β1+BMP-2 group and TGF-β1+IGF-1+BMP-2 group (P < 0.05). Moreover, the expression of colagen I mRNA was highest in the TGF-β1+IGF-1+BMP-2 group (P < 0.05). These findings indicate that TGF-β1 alone is able to induce the chondrogenic diferentiation of bone marrow mesenchymal stem cels, and the combination of TGF-β1, IGF-1 and BMP-2 can play the biggest role to induce the chondrogenic diferentiation of bone marrow mesenchymal stem cels.

Objective To observe the effect of Ganoderma lucidum and Astragalus membranaceus on the liver nucleic acid content of the mice exposed to the noise. Methods Noise-exposed mice were given Ganoderma lucidum and Astragalus membranaceus by stomach perfusion. Measured the liver nucleic acid content of the mice liver with diphenylamine and orcinol method. 6.Pathologic diagnosis of transplantation liver puncture biopsy: clinicopathologic analysis of 906 cases/times

Objective To explore the histological morphological features of complications following liver transplantation. Methods In 639 patients with complications following liver transplantation, the percutaneous liver biopsy tissues were stained by HE method. Van Gieson, Masson, PAS, reticulin and immunohistochemical staining were performed. HBsAg, HBcAg, HCVAg, CMV-EA, CMV-LA, CMV pp65, EBVAg and the expression of CK19 were detected. The rejection was graded according to Banff standard and scored with RAI.Results 906 times of liver biopsies in 639 cases were performed. Acute cellular rejection (ACR) was most commonly seen in 386 cases ( 42.61 %), followed by complication of biliary tract (251 cases, 27.70 %), drug-induced liver damage (72 cases, 7.95 %), CMV infection (55 cases, 6.07 %), infection or recurrence of hepatitis virus (B or C) (45 cases, 4.97 %), ischemia-reperfusion injury (42 cases, 4.64 %), chronic rejection (32 cases, 3.53 %), obstruction of efferent tract (6 cases, 0.66 %), recurrence of the primary affection (5 cases, 0.55 %), and non-function of live grafts (4 cases, 0.44 %). The complications in 8 cases were difficult to diagnose ( 0.88 %). Compared with the previous report of complications, the incidence rate of ACR was decreased and that of complication of biliary tract and drug-induced liver damage increased in this study. Conclusion Percutaneous liver biopsy is valuable for the diagnosis of complications. It provides the evidence of settling plan of treatment and improves the survival rate.

8.Genetic advances in hydatidiform mole

Hydatidiform mole is the most common gestational trophoblastic disease and is characterized by gross trophoblastic hyperplasia resulting from abnormal fertilization of the oocyte. It is of great interest to researchers because of its propensity for local in vasion and spreading. Using cytogenetic and moleculargenetic techniques researchers have performed various studies on hydatidiform mole. There are advances in the understanding of karyotype, DNA ploidy, fertilization types, expression of imprinted genes, thedifferences between genetic classification and pathologic classification, and the correlation to its invasion and spreading.
9.Molecular mechanisms of Taishan Ganoderma Lucidum amylose in prevention of rat atherosclerosis

AIM: To study the change of blood lipid and difference of expressed gene in chest aorta cells of atherosclerosis model rats with or without the treatment of Taishan Ganoderma Lucidum amylose.METHODS: The concentrations of triglyceride (TG), total cholesterol (TC) and high-density lipoprotein cholesterol (HDL-C) in serum were measured by automatic serum biochemical assay. Differential-display reverse transcription-PCR (DDRT-PCR) was used to analyze the differential expressed gene in chest aorta cells of atherosclerosis model rat with or without the treatment of Taishan Ganoderma Lucidum amylose. The differential expressed cDNA was confirmed by reverse Northern blotting.RESULTS: Up-regulated and down-regulated cDNA were isolated in chest aorta cells of atherosclerosis model rats with or without Taishan Ganoderma Lucidum amylose treatment. The known up-regulated genes included cytB561, ERP72, nitric oxide synthase 2 and Gpx1. The known down-regulated genes included thymosin β4, FGFRI oncogene, ICAM-1, FK506 binding protein, c-myc and TNF-α. CONCLUSION: Taishan Ganoderma Lucidum amylose decreases the level of blood lipid.
10.Screening and identification of serum biomarker of anovulatory dysfunctional uterine bleeding and its expression in the menses

Objective To screen and identify the serum biomarker of anovulatory dysfunctional uterine bleeding (ADUB) , to determine the expression of biomarker protein in menses of ADUB pa-tients, and to investigate the relation between ADUB and the biomarker proteins. Methods Subjects included 128 ADUB patients and 93 age-matched controls( normal women ). Their serum and super-natant of mense were collected and stored for use at -80℃. The differential proteins in the serum of the 2 groups were detected by CM 10 and analyzed by Biomarker WizardTM3.2 software. Then, the differential proteins were identified by Trieine-SDS-PAGE gel separation, spectrometry identifica-tion, and immunoprecipitation. The expression of the protein identified above in the menses was test-ed by ELISA, RT-PCR, and Western blotting. SPSS 14.1 was applied for statistical analysis and chart drawing. Results Five differential protein peaks were screened and their peak values were 11.80, 13.59, 13.79, 13.85, and 14.20 km/z, respectively. The intensity of protein peak ( 11.80 km/z ) which was identified as serum amyploid protein A ( SAA ) of ADUB was significantly higher than that of the controls (P<0.05). While the intensity of protein peak (13.59 km/z) which was identified as vascular endothelial growth factor (VEGF) of ADUB was obviously lower than that of the controls (P<0.05). The intensity of protein peak 13.08, 13.85, and 14.20 was not different between the cases and controls. SAA expressed highly in the menses of ADUB but low in that of the controls. Conversely, VEGF expressed highly in the menses of the control but low in that of the ADUB. Conclusion Two biomarkers which might be related with ADUB have been correctly screened and identified as SAA and VEGF. It needs further study whether the increased expression of SAA and reduced expression of VEGF are the cause or result of ADUB.
