1.Investigation about the use of antibiotics for type Ⅰ incision thyroid surgery

Objective To investigate the use of antibiotics for type Ⅰ incision thyroid surgery of our hospital,so as to provide guidance for reasonable clinical use of antibiotics.Methods A total of 149 discharged patients of Xinhua Hospital,who underwent the type Ⅰ incision thyroid surgery from Jun 2015 to Jun 2016,were enrolled in the study.There were 117 cases underwent unilateral thyroid surgery,and there were 32 cases underwent bilateral thyroid surgery.According to the operation condition,the application of antibiotics in perioperative period,the number of days in hospital,the total cost of hospitalization,the infection situation and so on were analyzed.Results There were 69 cases(58.97%) who were perioperative prophylactic use of antibiotics,of which unilateral surgery 117 cases,and there were 20 cases(62.50%) who were perioperative prophylactic use of antibiotics,of which bilateral surgery 32 cases.The hospitalization days and hospitalization cost of the antibiotics used group were higher than those of the antibiotics unused group.All the cases were well stage wound healing,and one case occurred nosocomial infection.Conclusion It is not worthy for the prophylactic use of antibiotics for the type Ⅰ clean incision thyroid surgery to prevent the nosocomial infection,furthermore,the hospitalization days and hospitalization cost of patients increased.
2.Protective mechanism of 7-nitro indazole in neonatal rats with hypoxic-ischemic brain damage

Objective To investigate the protective mechanism of 7-nitro indazole (7-NI) in neonatal rats with hypoxic-ischemic brain damage. Methods Seventy-two Wistar rats of 7-day-old were randomly divided into the sham-operation、HIBD and 7-NI treated group. The HIBD models were established in 7-NI treated and HIBD groups.7-NI (100 mg/kg) or peanut oil (10 ml/kg) was injected intraperitoneally 0.5 h before hypoxia in 7-NI treated group or HIBD group. NOS,NO,SOD and MDA were examined at different time (1 h、2 h、6 h、8 h). Terminal deoxynucleotidyl transferase mediated dUTP-biotin nick end labeling(TUNEL) method was used to detect neuronal apoptosis. 3.Expression of vascular endothelial growth factor after hypoxic-ischemia brain damage

Objective To study the expression of vascular endothelial growth factor(VEGF) mRNA in neonatal rats with HIBD and to explore the function of VEGF in HIBD. Methods Fourty-eight Wistar rats of 7-day-old were assigned randomly to the control or HIBD groups. The left common carotid artery of rats in HIBD group were isolated and ligated, then exposed to hypoxic environment (8%O 2) for two hours. In control group the rat left common carotid artery were isolated, but not ligated and not exposed to hypoxic environment. Animals were killed in 3 h, 12 h, 24 h and 3 d after HIBD. The expression of VEGF mRNA in hippocampus were examined by RT-PCR.Results VEGF164 was the most prominent transcript of VEGF mRNA splice variants. The expression of VEGF mRNA was present in control group, but there were no significant differences between time points ( F =0.176, P =0.911). 4.Application of Phadiatop UniCAP in detection in children with allergic diseases

Objective To evaluate a blood test,UniCAP system(screening IgE to food or inhalant allergens,eosinophil cationic protein,ECP)in differentiation the capability of lgE-mediated allergic diseases in children. Methods 26 atopic dermatitis,33 asthma/bronchiolitis,20 allergic purpura, 15 Kawasaki disease patients and 20 normal children were tested total lgE,lgE to food or inhalant allergens,ECP by UniCAP system and compared.Resuits Fx5E positive rate are 73.1%,24.2%,25.0%,13.3%in atopic dermatitis,asthma,allergic purpura,Kawasaki disease respectively,and the Phadiatop positive rate are 23.1%、69.7%、10.0%、6.6%.The serum levels of ECP、TIgE of patients were higher than the mormal controls,with the highest level in asthma, and lowest in Kawasaki disease,the differences were significant(P<0.05).Conclusions Food orinhalant allergens plays an important role in the inflammatory process of asthma,atopic dermatitis,and some role in allergic purpura,Kawasaki disease.Food allergens are more important to atopic dermatitis.and inhalant allergens are more important to asthma.
5.An empirical research on infection control culture assessment under the " macro infection control" concept

Objective:To design an infection control culture assessment scale to comprehensively assess a hospital in general.Methods:Based on the American mature safety culture survey scale(HSOPSC), we developed an infection control culture assessment scale. With this scale, a cross-sectional study was conducted to survey 684 medical staff between October 2018 to January 2019. 607 valid questionnaires were recovered. Reliability and validity of the assessment were then examined by Delphi method, Exploratory factor analysis and confirmatory factor analysis, while the positive response ratio of the dimension was calculated.Results:The scale of infection control culture assessment included 30 entries and 7 factors with eigenvalue>1, and a cumulative variance contribution rate of 57.97%. The 7 factors were basic infection control ability, hospital management atmosphere, mutual support between coworkers, reporting of negative events, interdepartmental collaboration, organizational learning and continuous improvement and working load. Cronbach′s coefficient of the entire assessment was 0.909, and split-half reliability was 0.941. Confirmatory factor analysis showed GFI, CFI, RMSEA, IFI, AGFI and other indicators were better fitted. Among all the factors, the organizational learning and continuous improvement and working load were lower than 50%, which should to be improved for the infection control culture. Conclusions:The assessment scale proved good reliability and validity, which may be widely used in hospitals. Regular measurement and improvement could effectively improve the infection control awareness of medical staff and ensure medical quality.
6.Gliosarcoma of cerebral hemispheres: a clinicopathologic study of 10 cases.

OBJECTIVETo study the clinical and pathologic features of gliosarcoma of cerebral hemispheres.
METHODSThe clinicopathologic features of 10 cases of gliosarcoma involving cerebral hemispheres were reviewed. Immunohistochemical study was carried out using EnVision method.
RESULTSThe mean age of the patients was 54 years and the male-to-female ratio was 6 to 4. Clinical symptoms included headache (6/10), nausea/vomiting (5/10), and sensory or motor impairment (4/10). Nine of the cases were primary gliosarcoma, with maximum diameter ranging from 2.4 to 5.5 cm (mean = 4.2 cm). The remaining case represented secondary gliosarcoma involving skull base and extracranial tissues. Histologic examination showed a biphasic pattern in all cases. Regarding the glial component, there were 9 cases of pleomorphic glioblastoma and 1 case of giant cell glioblastoma. Reticulin stain was positive in all cases. Immunohistochemical study showed that the tumor cells variably expressed GFAP (10/10), p16 (4/10), EGFR (1/10), CD68 (1/10) and p53 (6/10). The Ki-67 index ranged from 15% to 70% (mean = 34%). Six patients had follow-up data available. One patient was disease-free for 45 months and 5 patients died of the disease at 3 to 17 months after the operation (mean duration of survival = 9 months).
CONCLUSIONSGliosarcoma is a highly aggressive tumor, often locates in the deeper part cerebral hemispheres and has a relatively short duration of symptoms. It carries a poor prognosis. GFAP immunostain and reticulin stain are helpful in confirming the diagnosis. p53 and p16 are also expressed in some cases.
