1.CT Evaluation in the Recurrence of Larynx Carcinoma After Operation

Objective To assess the dagnostic value of computed tomography(CT) for recurrent carcinoma of larynx after operation. Methods 45 CT cases of recurrent carcinoma of larynx were analysed retrospective and were compared with pathology results.Results CT was capable of demonstrating the existence,border and range of the recurred tumor and showing the information about inetastasis in lymph node of neck,the involved throat space and the invaded blood vcsscls in cervical.Conclusion CT is an cxact,rcliable and convenient method to evaluate the recurrence of larynx carcinoma,and is helpful to make therapy planning.
2.The effect of urinary kallidinogenase on serum concentrations of hydrogen sulfide, neuron-specific enolase, and S100β in patients with cerebral infarction

Objective To investigate the effect of urinary kallidinogenase on serum concentrations of hydrogen sulfide (H2S),neuron-specific enolase (NSE),and S100β in patients with cerebral infarction (CI).Methods From June 2011 to June 2014,80 patients with CI were chosen as study objectives.All patients were divided into two groups:40 patients in study group (urinary kallidinogenase group),and 40 patients in control group.The death rate,the rate of complication and National Institute of Health Stroke Scale (NIHSS) were compared between two groups.The concentrations of H2S,NSE,and S100βwas compared between two groups.Results In study group,the death rate was 5.00% (2/40),the rate of complication was 22.50% (9/40);in control group,the death rate was 12.50% (5/40),the rate of complication was 15.00% (6/40);and no significant significance was found between two groups (P > 0.05).The NIHSS was (11.2 ± 3.2) in the study group,and (15.7 ± 2.7) in the control group,with statistically significant difference between two groups (P < 0.05).After treatment,the concentrations of H2 S,NSE,and S100β of two groups were decreased significantly (P <0.05).At 1w,2w,and 3w,the concentrations of H2S,NSE and S100βhad statistically significant difference between two groups (P < 0.05).Conclusions Urinary kallidinogenase has a cerebral protective effect,which can decrease the concentration of H2S,and increase the concentrations of NSE and S100βin CI patients.
3.The effect of AN69 ST membrane on filter lifetime in continuous renal replacement therapy without anticoagulation in patients with high risk of bleeding

ObjectiveTo evaluate whether AN69 ST membrane would prolong filter lifetime in continuous renal replacement therapy (CRRT) without anticoagulation in patients with high risk of bleeding.Methods A single-center, prospective, randomized, double-blind control trial with crossover design was conducted. From March 1st to December 31st in 2013, patients who were admitted to Department of Critical Care Medicine of the Fourth Hospital of Hebei Medical University meeting CRRT treatment indications, but could not receive systemic anticoagulation because of high risk of bleeding were studied. The selected patients were randomly divided into two groups according to a random number table, and four filters consisting of two AN69 ST100 membrane filters (A) and two traditional AN69 M100 membrane filters (B) were used for them. GroupⅠ with the filter order of A-B-A-B, and groupⅡ with the order of B-A-B-A. The clinical data of patients was recorded in detail, and conventional AN69 ST and AN69 membrane filter lifetime, their influence on coagulability, and the incidence of bleeding complications were compared.Results Seventeen patients were enrolled, with 10 in groupⅠ, and 7 in groupⅡ. The basic medical characteristics including gender, age, acute physiology and chronic health evaluationⅡ (APAECHⅡ) score, sequential organ failure score (SOFA), Acute Renal Injury Network (AKIN) stage, activated partial thromboplastin time (APTT), prothrombin time (PT), international normalized ratio (INR), platelet count (PLT), and use of mechanical ventilation were not significantly different between two groups. But the use of vasoactive drug was more frequent in groupⅡcompared with that of groupⅠ[100.0% (7/7) vs. 30.0% (3/10),χ2 = 8.330,P = 0.010]. AN69 ST filter lifetime (n =34) was (15.92±2.10) hours, there was no statistically significant difference compared with that of AN69 membrane (t = 0.088,P = 0.942), filter lifetime of which (n = 34) was (16.12±1.38) hours. It was also found by Kaplan-Meier survival analysis that there was no significant difference between the two membrane filter lifetime (χ2=1.589,P =0.208). Logistic regression analysis showed that the life of the first filter was not correlated with coagulation indicators, including APTT, PT, INR, and PLT [APTT: odds ratio (OR) = 0.977, 95% confidence interval (95%CI) = 0.892-1.071, P = 0.623; PT:OR = 1.001, 95%CI = 0.901-1.109,P = 0.988; INR:OR = 1.078, 95%CI = 0.348-3.340,P = 0.896;PLT:OR = 0.996, 95%CI = 0.974-1.019,P = 0.735]. The application rate of vasoactive drugs, which was different between two groups for basic medical indications showed no effect on filter life time (OR = 2.541, 95%CI = 0.239-26.955,P = 0.439). Reasons of clotting in filters were also analyzed, and it was found that blood coagulation in the filter ranked the top (88.2%), and the other reasons were catheter-related problems, death, and unscheduled transport. No difference in blood coagulation function was found in both groups after treatment for 12 hours, and there was no bleeding complication.ConclusionDuring the CRRT without systemic anticoagulant, both surface-treatment with polyethyleneimine AN69 and AN69 ST membrane cannot prolong filter lifetime.
4.A case-control study on the influential factors of precocious puberty girls

Objective To investigate the relenvant factors of precocious puberty and provide the basis for precaution of high risk group of precocious puberty .Methods A 1:1 case-control study was performed on 50 healthy girls and 50 precocious puberty girls .Results Univariate analysis showed an significant differences in the exposure rates of following five factors between cases and controls:intake of nutritious and health products ,favoring meat and poultry,long time watching television,addiction to TV ads,family economic condition (The Chi-square values are 7.045,9.073,17.478,14.063,12.246 respectively,all P<0.05).The Logisitc regression showed that favoring meat and poultry,long time watching television and addiction to TV ads had significant differences among precocious puber -ty children(The Wald′s Value were 4.846,6.850,5.662 respectively,all P<0.05).Conclusion favoring meat and poultry,long time watching television and addiction to TV play an important roles in precocious puberty .
5.Construction of cRGD-iron oxide nanoparticles and its application in the tumor diagnosis by magnetic resonance imaging

Objective:To construct superparamagnetic iron oxide nanoparticles targeting tumor angiogenesis and evaluate their potential value as contrast agent in magnetic resonance imaging (MRI) .Methods:Ultrasmall superparamagnetic iron oxide (USPIO) nanoparticles targeting tumor angiogenesis were prepared by using co-precipitation chemical method. Cyclic RGD(cRGD) containing the sequence of Arg-Gly-Asp were conjugated USPIO nanoparticles by using chemical conjugative method to prepare superparamagnetic imaging agent targeting tumor angiogenic vessles. The physical and chemical properties of cRGD-USPIO nanoparticles were detected. The specific binding capabilities of cRGD-USPIO and USPIO to human lung adenocarcinoma cells (A549) and human umbilical vein endothelial cells (HUVEC) were tested by Prussian blue staining. A549 xenografts were established in nude mice, then USPIO and cRGD-USPIO were injected though tail vein, and the MRI signal enhancement effect of cRGD-USPIO was evaluated.Results:We successfully prepared the cRGD-USPIO nanoparticles. Its core diameter was 5-10 nm and the average diameter was (43.97±10.10) nm and the quality saturation magnetic intensity was 59.94 A·m~2·kg~(-1). Cell-binding test suggested that cRGD-USPIO group showed strengthened positive staining. In vivo MRI experiments showed that signals of tumor were significantly reduced in cRGD-USPIO group than that in USPIO group (P<0.01). Conclusion:The constructed cRGD-USPIO nanoparticles can be developed as a potential tumor-specific MRI contrast agent for the early diagnosis of cancer.

7.Diabetic Patients Complicated by Lower Respiratory Tract Nosocomial Infection:Clinical Analysis

OBJECTIVE To assess the pathogen features and risk factors among diabetic patients complicated by lower respiratory tract nosocomial infection.METHODS A retrospective study was carried out to survey the clinical data of diabetic patients complicated by lower respiratory tract infection during from 2002 to 2005.RESULTS The main responsible pathogens were Gram-negative bacteria,their rate was 54.5%.While the rate of Gram-positive bacteria was 35.7 %.The rate of fungi was 9.8%.The risk factors of nosocomial infection included:old age,high level of blood sugar,consciousless status,longtime usage of antibiotics,oxygen inhalation and other aggressive manipulations.CONCLUSIONS In diabetic patients with lower respiratory tract infection,the incidence and mortality were all onrelatively high level.They must be given to pay attention.The chief pathogens were Gram-negative bacteria or fungi.So we should strengthen supervision and avoid risk factors,which are the keys in saving critical diabetic patients.

Objective:To observe influence of qingzhifugan pellet on mixed-type animal model of fatty liver with chronic hyperlipemia and alcoholized hepatic damage. Methods:The mixed-type animal model of fatty liver was established by administering with high lipids and alcohol feeds. Therapeutic experiments were conducted with a self-made new drug the qingzhifugan pellet. The efficacy of the tested drug was evaluated comprehensively by such measures as sero-level of lipids and ferments,as well as the histological pathology of liver. Results:It was shown that the tested drug can decrease the sero-level of lipids,promote the metabolism of lipids in liver,recover the alcoholized hepatic damage,and relieve the liver fat denaturation. Conclusion:It suggested that the tested drug possessed certain effect for treating or improving chronic hyperlipemia and alcoholized hepatic damage in the animal model.
10.Establishment of the method to evaluate cardiac toxicity by real-time cell analysis system on human embryonic stem cells

Aim To establish an in vitro early drug cardiac tox-icity evaluation method by human embryonic stem cells derived cardiomyocytes ( hESC-CM) and real-time cell analysis Cardio (RTCA Cardio) system. Method The hESC-CM were cultured at RTCA Cardio E-Plate 96. Impedance signals from hESC-CM were analyzed for beating rate, contraction amplitude and beating rhythm irregularity to determine the optimum inoculation density and detection duration. Based on this, we used 0. 1 % DMSO to be the solvent and quinidine (0. 2, 0. 78, 3. 13, 12. 5, 50 and 100 μmol·L - 1 ) known as affecting cardiac activity to validate this method. Result The results revealed no significant changes in the cell index (CI), transient pulse patterns, beating rate and amplitude of hESC-CM. Quinidine will affect the CI and transi-ent pulse patterns of hESC-CM and decrease the beating rate and amplitude of hESC-CM when its concentration ≥3. 13 μmol · L - 1 . And this effect is concentration-dependent, the higher the concentration,the more time they need to recover beating and the more significant the beating rate and amplitude inhibition of quinidine on hESC-CM. Conclusion The method established by hESC-CM and RTCA Cardio system can detect the effect of quinidine on the contraction of hESC-CM, and this indicates that this method has the potential to be an attractive high-throughput tool for screening potential drugs in early evaluation of drug car-diotoxicity.
