1.Fixation with three kinds of implants to repair osteoporotic intertrochanteric fractures: univariate and multivariate analysis of failure

BACKGROUND:Intertrochanteric fracture is one of the common complications in patients with senile osteoporosis.Internal fixation is a common method in clinical therapy.Different internal fixators are selected in different studies.However,there are few studies on influential factors for outcomes of internal fixation at present.OBJECTIVE:To investigate related influential factors for outcomes of internal fixation in elderly patients with osteoporotic intertrochanteric fracture.METHODS:A total of 86 elderly patients with osteoporotic intertrochanteric fracture,who were treated in the Guanghua Hospital from May 2011 to May 2014,were enroled in this study.They were divided into three groups according to different manners of fixation.There were dynamic hip screw group (n=34),proximal intramedulary nail group (n=41) and proximal femoral compression plate group (n=11).Rate of fixation failure was compared among the three groups.The relevant factors affecting fixation effects were analyzed using univariate and multivariate analysis.RESULTS AND CONCLUSION:Internal fixation failure was detected in 5 patients (15%) in the dynamic hip screw group,11 patients (26%) in the proximal intramedulary nail group,and 4 patients (36%) in the proximal femoral compression plate group.Results of univariate analysis revealed that Evanse genotyping of patients,whether fractures were found on outer arm side,degree of osteoporosis,quality of reduction,steeple distance,whether combined with internal medicine diseases and selection of fixator were associated with internal fixation effects (P< 0.05).However,the internal fixation effects were not correlated with gender,age,hospital stay and ambulation time (P> 0.05).Multivariate analysis results demonstrated that unstable fractures,severe osteoporosis,fractures of the lateral arm,medical ilness,steeple distance > 25 mm,and different internal fixators are independent factors affecting internal fixation effects in patients.
2.Relationship between the levels of tumor necrosis factor alpha, interleukin-1beta and interleukin 6 of bone tissue and osteoporosis in ovariectomized rats

BACKGROUND:It has found that secretions of tumor necrosis factor α (TNF-α), interleukin 1β (IL-1β) and interleukin 6 (IL-6) are closely related to osteoporosis
OBJECTIVE:To further investigate the levels of TNF-α, IL-1β and IL-6 of bone tissue in ovariectomized rats and their relationship with osteoporosis.
METHODS: Sixty 3-month-old female Sprague-Dawley rats were randomly divided into control group and test group (n=30 per group). In the test group osteoporosis model was induced by ovariectomy folowed by intraperitoneal injection of aluminum nitride solution; while rats in the control group were given intraperitoneal injection of normal saline without ovariectomy. The experiment cycle was 12 weeks. One week after modeling, serum level of estradiol was tested, and the bone mineral density of rats measured with bone densitometry. In the meanwhile, the levels of TNF-α, IL-1β and IL-6 in bone tissue were detected to analyze the correlation between cytokines and bone mineral density.
RESULTS AND CONCLUSION: The level of serum estradiol in the test group was significantly lower than that in the control group (P=0.007); the bone mineral density of the lumbar spine and femur in the test group was significantly lower than that in the control group (P=0.006, 0.004). Compared with the control group, the percentages of trabecular bone and osteoblast area within the visual field in the test group were significantly lower, while the osteoclast area within the visual field was significantly higher (P=0.037, 0.029, 0.044). Compared with the control group, the levels of TNF-α, IL-1β and IL-6 in the test group significantly increased (P=0.032, 0.031, 0.025). And the bone mineral density of the lumbar spine and femur was negatively correlated with the levels of TNF-α, IL-1β and IL-6 in the test group (P < 0.05). In conclusion, the osteoclast activity is enhanced by elevating the levels of TNF-α, IL-1β and IL-6 of bone tissue of ovariectomized rats, and increasing the percentage of osteoclast area within the visual field. Moreover, the percentages of trabecular bone and osteoblast area within the visual field have a decline, which accelerates the process of bone resorption and leads to the formation of osteoporosis. 3.Analysis of Cognition and Satisfaction with Mental Health Service among Residents in Weifang

Objective:To understand cognition and satisfaction degree of mental health service among residents in Weifang,and provide reference for further development of mental health work.Methods:6174 residents were selected with random sampling method and a cognition survey was conducted using a self-designed questionnaire.Among them,553 residents were selected to investigate their satisfaction with mental health service.Results:The residents have unscientific cognition on mental health service,and they did not understand the delivery of community mental health services.Residents who have received mental health service were more satisfied with informed consent and patient privacy protection,and had low satisfaction with the clinic environment and the charge of inspection items.Conclusions:To improve residents' cognition and satisfaction of mental health service,we should further perfect the professional mental health institutions and personnel recruiting,and forcefully carry out mental health education.
4.Experimental research on the effects of metformin on steroid -induced osteonecrosis of mice femoral head

Objective To explore the effect of prevention and treatment on steroid-induced osteonecrosis of mice femoral head(ONFH) treated with metformin. Methods Thirty-six Kunming mice were randomly divided into three groups (n = 12):A (Control Group), B (Model Group)and C (Prevention Group). For producing ONFH mice models, did the intraperitoneal injection of horse serum (10 mL/kg) to B and C firstly. After two weeks, continuing the intraperitoneal injection of horse serum (5 mL/kg) again with the prednisolone intramuscularly [45 mL/(kg· day), totally for 5 days]. Meanwhile, feeding normal saline 10 mL/(kg·day) to B and feeding metformin hydrochloride [0.2 g/(kg·day)] to C. For A, mice were only given normal saline intramuscularly and intragastrically in equal quantity at the same time. The contents of serum cholesterol (TC), triglycerid (TG), plasma von willebrand factor (VWF) and plasminogen activator inhibitor 1 (PAI-1) were determined at the 2nd, 4th and 6th week after treatment. The micewere sacrificed at 2nd, 4th, and 6th weekafter treatment, and femoral heads were harvested to do histopathology analysis. Results The appearance and shape of the femoral head and the surface of cartilages were normal. The percentage of empty osteocyte lacunae in B was significantly higher than that in C (P < 0.05), there was no significant difference between A and C (P>0.05). TC and TG contents in C were significantly lower than that in B in 2th、4th、6th weeek(P<0.05), and higher than that in A(P<0.05). VWF and PAI-1 level in C were significantly lower than that in B at 2nd and 4th week (P<0.05), but there were no statistical significance at 6th week. there were no statistical significance for the comparison between A and C. Conclusion Metformin can prevent steroid-induced ONFH by improving hyperlipemia, hypercoagulability and hypofibrinolysis, then effectively prevent osteonecrosis.
5.Serological and molecular study of a novel B(A) allele with multiple missense mutations.

OBJECTIVE:
To explore the molecular basis for an individual suspected as AwB subtype through DNA sequencing.
METHODS:
ABO serology was carried out with the standard tube method. To identify the ABO gene haplotype, the amplicons of exon 7 were cloned and sequenced.
RESULTS:
Serological results showed that the forward typing was AwB and the reverse typing was B. Sequencing analysis revealed that the sample has contained an O01 allele in addition with c.297A>G, c.657C>T, c.796C>A, c.803G>C, c.930G>A variants as compared with the A101 allele.
CONCLUSION
Through sequencing analysis, the sample with an AwB subtype by serological testing was identified as a novel B(A) phenotype, which was unreported previously.

Since the enrollment in 2002, the cultivation model of "4+4" program of clinical medicine in Shanghai Jiao Tong University School of Medicine has been continuously explored and practiced. With the goal of cultivating high-level compound outstanding medical innovative talents with multi-disciplinary cross-capacity, through strengthening the heuristic teaching, establishing the medical-engineering cross-course, emphasizing the training of scientific research ability, and taking teaching reform in the basic clinical single-circulation organ system integration course, we have established a talent training system with the characteristics of Shanghai Jiao Tong University School of Medicine, which is characterized by "thick foundation, strong practice, re-transformation, shaping norms, and international integration", and intend to make further exploration in the field of post-graduation education convergence.
