1.The effect of H-UPPP operation on obstructive sleep apnea hypopnea syndrome patients with hypertension

Objective To explore the effect of the H-uvulopalatopharyngology(H-UPPP)on the obstructive sleep apnea hypopnea syndrome(OSAHS)with hypertension sufferers.Methods The clinical data of 100 cases of OSAHS patients with hypertension were analyzed retrospectively,and the related parameters about changes of blood pressure and respiration were analyzed.Results The sargical operation treatment cure rate of OSAHS eas 37%,and the effective rate was 81%.In those patients.all the hypertension patients who in the first stage were completely cured (46/46),while for whom in the second stage,the cure rate was 37%(13/35).So the total effective rate of experimental group Was 88%(30/35).19 cases was ineffective.In those 19 patients,there were 16 patients in the second stage,while 3 cases in the third stage.Conclusion H-UPPP could completely cure OSAHS,meanwhile,it also obviously lowered hypertension which induced by OSAHS,and made the sufferers keep at relative normal blood pressure for a long time.So H-UPPP Was a best treatment method to the OSAHS patients with hypertension.

Objective: To study the method of identification and content determination of the important component in TongRenNiuHuangQingXin Tablet.Method:The major compounds in this drug were identifi ed by the thin layer chromatographic method and the contents of paeoniflorin were determined by HPLC.The HPLC procedure was used in the determination: the chromatographic column was Symmetry ShieldTM Rp18(3.9mm?150mm,5?m),the mobile phase was methanol-0.05mol/L potassium dihydrogen phosphate(25:75) with a flow velocity of 0.8ml/min,the detection wave length was 230nm,and the room temperature was 30℃.Results: Calculus Bovis Artifactus and Gensing were identified.The regression equation of paeoniflorin was in the range of 0.10-0.51?g.The average recovery of the procedure was 98.94%(RSD=2.05%).Conclusion: The identifi cation and determination methods established were specifi c and repeatable,which can be used to control the quality of TongRenNiuHuangQingXin Tablet.
3.Gene Expression of Bone Morphogenetic Protein in Degenerated Cervical Intervertebral Disc:Immunohistochemical and in Situ Hybridization Studies

Objective: To investigate the gene expression of bone morphogenetic protein(BMP)in human cervical disc.Methods:cervical disc specimen were obtained from patients undergoing anterior disc surgery,who had the signs of cervical disc herniation.Immunohistochemical and in situ hybridization studies were conducted to detect cells with BMP gene expression.Results:It showed positive staining of BMP-like immunohistoreactivity and its mRNA gene expression in degenerated discs,osteophytes,posterior longitudinal ligments,amd periosteum of vertebra.Conclusion:BMP may be relate to intervertebral disc degeneration as prolifertion-stimulating factor of chondrocytes that replace normal anular cells during disc degeneration but its mechanism needs further study.
4.Discussion on the diagnosis and surgical treatment of peripheral solitary pulmonary nodule

Objective To investigate the diagnosis and surgical treatment of peripheral solitary pulmonary nodule (SPN).Methods From January 2007 to October 2012,the clinical data of 69 cases of SPN treated were analyzed prospectively.And postoperative pathologic examination results as the gold standard to determine spiral CT enhanced check the accuracy and consistency test (Kappa test).Results Twenty-eight cases were diagnosed as malignant SPN by preoperative CT,25 cases were diagnosed as malignant SPN by postoperative pathology.The accuracy of CT judgment SPN benign and malignancy was 89.3% (25/28),the Kappa value was 0.503.All patients were followed up for 6-12 (9.6 ± 1.4) months.There was no recurrence or enlargement.Conclusions Enhanced spiral CT scan is more economical and practical way for identifying benign and malignant SPN.Malignant SPN diagnosed by CT should be considered having surgery and auxiliary thoracoscopy combined with intraoperative rapid freezing to determine the surgical approach.
5.Clinical Application and Effective Mechanism of Pacemaker with Atrial Fibrillation Preventive Pacing Algorithms

Objective To investigate mechanism and clinical effect of implantation of pacemaker with artial fibrillation(Af) preventive pacing algorithms (Vitatron 900E).Method 24 cases of patients with paroxysmal Af were observed in three phases (each phase two weeks):⑴Monitoring phase,in which onset mechanism of atrial arrtythmias was recorded, the device was programmed into DDD mode with 60 bpm;⑵Lower rate limit phase (LRL),DDDR mode with 85bpm LRL, and evaluating anti-Af effect by elevated rate;⑶Preventive pacing therapy phase,all preventive pacing therapies with programming to on or off, and this phase determined whether combined pacing therapies impacted on incidence of Af. Results 24 cases of patients were free of Af, in which 17 cases were done by using post-PAC-response or/and post-exercise-rate-control therapy only, and 7 cases of 17 casees were done by combining it with cordarone 0 1~0 2g/d also. The other 7 cases were done by DDDR 85 bpm only.Conclusions Af prevention pacing may be benefit for patients with paroxysmal Af,especially with complicated symptomatic bradycardia.
6.Effect of prostaglandin E1 on the expression of MCP-1 in cultured human umbilical vein endothelial cells

Objective To investigate the effect of prostaglandin E1(PGE1) on the expression of monocyte chemoattractant protein-1(MCP-1)in human umbilical vein endothelial cells (HUVECs) and its possible mechanism. Methods Endothelial cells were incubated with oxidized low-density lipoprotein (ox-LDL group) in the presence or absence of prostaglandin E1. The level of MCP-1 in the supernatants was determined by enzyme linked immunosorbent assay (ELISA), the expression of MCP-1 mRNA in cultured endothelial cells was detected by in-situ hybridization and the protein expression of NF-κB was analyzed by Western blot. Results Compared with ox-LDL(100 μg/ml),PGE1 markedly lowered the levels of MCP-1[(0. 327±0. 051),(0. 214±0. 213),(0. 247±0. 228)pg/ml vs. (0. 655±0. 013)pg/ml], inhibiting the expression of MCP-1 mRNA [(0. 061±0. 008), (0. 033±0. 006),(0. 026±0. 004)A/μm2 vs. (0. 220±0. 032)A/μm2] in the cultured HUVECs in a dosedependent manner (0. 001, 0. 01, 0.1 mol/L). Western blot analysis demonstrated that the amount of NF-κB p65 was attenuated after treatment with prostaglandin E1 for 24 hours. Conclusions Prostaglandin E1 can downregulate the expressions of MCP-1 and NF-κB induced by ox-LDL in HUVECs, which may thereby defend the blood vessel endothelial cell function.

0.05).The acute toxicity was more severe in the no-response group and response-LCAF group than in the response-CF group.Conclusion:LCAF did not improve the local control rate and survival rate on grades Ⅰ&Ⅱ esophageal carcinoma when 40Gy had been given by CF.Grades Ⅲ&Ⅳ esophageal carcinoma will get benefit from LCAF.

Objective To observe the characteristics and treatment of malignant peripheral nerve sheath tumor originated from neurofibromatosis and the outcome of the patients. Methods A case was reported and the related literatures were reviewed. Results and Conclusion Malignant peripheral nerve sheath tumor is a rare soft tissue malignancy, which is with highly invasion. There are some difficulties with the diagnosis and treatment, and the outcome is poor. Early diagnosis is very important for treatment.
9.Expression and role of CD4+CD25+CD127low/-T cells of the peripheral blood in rheumatoid arthritis patients

Objective To investigate the expression of regulatory T cells in the peripheral blood of rheumatoid arthritis patients and to explore the possibility of using CD127 instead of FOXP3 in the study of regulatory T cells. Methods Thirty-two patients with active rheumatoid arthritis (RA) [ including 20 patients who hadn't been treated with disease modifying antirheumatic drugs (DMARDs) and 12 patients whose disease were poorly improved after DMARDs therapy], 25 patients with primary Sjogren's syndrome (pSS) and 24 healthy controls were selected. The percentages of CD4+CD25high, CD4+CD25+CD127low/'- and CD4+CD25+ FOXP3+T cells were analyzed by 3-color flow cytometry. Meanwhile, the levels of CRP, ESR, immunoglobulin and complement were measured. Results Percentages of CD4+CD25+CD127low/-T cells and CD4+CD25+FOXP3+ T cells in RA patients were significantly lower than those of healthy controls (P<0.01 for each category of T cells),but the percentage of CD4+CD25high T cells didn't show significant changes (P>0.05). The expression of CD4+CD25+CD127ow/- and CD4+CD25+FOXP3+T cells showed no difference between RA and pSS patients (P> 0.05 for each). Differences of the three group cells between untreated RA patients and poorly-improved RA patients were not significant (P>0.05 for each case). CD4+CD25+CD127low/-T cells were positively associated with CD4+CD25+FOXP3+T cells (r=0.698, P=0.001), but the two groups had no correlation with the levels of CRP, ESR and anti-CCP antibody and RF (P>0.05 for each case). Conclusion The percentage of CD4+CD25+CD127low/-T cells is decreased in the peripheral blood of active RA patients and positively correlates with CD4+CD25+FOXP3+T cells.It suggests that CD127 may be an ahemative marker of FOXP3 in the study of Treg cells.
10.Factors leading to delay in decision to seek treatment in patients with acute myocardial infarction in Beijing

Objective To investigate the factors associated with delay in decision to seek treatment in patients with acute myocardial infarction(AMI) in Beijing. Methods This prospective,cross-sectional,multicenter survey was conducted from November 1,2005 and December 31 ,2006. The participants consisted of 799 patients with STEMI admitted within 24 h of symptom onset to 19 hospitals in Beijing. Data were collected by semi-structured interviews and medical records review. The patients were categorized into an early decision group and the a late decision group based on the 30 min cut-off. Results The median(25%,75%) decision delay in STEMI patients was 60(20, 180)min. Factors associated with late decision in an univariate analysis were age ≥65 years, retirement or unemployment, history of myocardial infarction,symptom onset at home and intermittent symptoms, whereas presence of bystanders such as friends,coworkers or even strangers,unbearable symptoms,dyspnea,sweating,syncope and attribution of symptoms to cardiac origin were related to early decision. Multivariate logistic analysis showed that history of myocardial infarction,absence of syncope, intermittent symptoms,bearable symptoms and attribution of symptoms to noncardiac origin were independent predictors of decision delay>30 min. Patients in the early decision group had more chances to receive acute reperfusion therapies(P=0.001) and shorter time intervals from symptom onset to reperfusion therapies(P<0.001). Conclusions To a great extent patients with AMI in Beijing delayed in decision to seek treatment. History of myocardial infarction, symptom characteristics and symptom attribution were associated with decision delay.
