Objective To investigate the effect of conductive education applied in water exercise on cerebral palsy. Methods 58 children with cerebral palsy who could accept conductive education were divided into observation group (n=29) and control group (n=29) randomly. All the children accepted routine rehabilitation, and the observation group accepted water exercise in the model of conductive education. They were assessed with muscular strength, muscular tension (modified Ashworth Scale) and the Gross Motor Function Measure (GMFM) before and after training. Results The score of GMFM improved more in the observation group than in the control group (P<0.05), as well as the incidence of improvement of muscle tension (released 1 grade or more, 75.9% vs. 44.8%, P<0.05) and muscle strength (increased 1 grade or more, 82.8% vs. 58.6%, P<0.05). Conclusion Water exercise with the conductive education model can improve the gross motor function and muscle strength, release the spasticity in children with cerebral palsy.

Objeetive To probe the clinical diagnosis and surgical treatment of primary gastric lymphoma (PGL). Methods Clinical data of 23 PGL patients identified by postoperative pathology were analyzed retrospectively. Sixteen patients underwent subtotal gastrectomy,3 patients underwent total gastrectomy,2 patients underwent palliative resection and 2 patients underwent exploratory laparotomy only. Postoperatively 21 patients received adjuvant treatment(chemotherapy and/or radiotherapy). The variables analyzed were type of surgery, histological type in accordance with Kiel's classification, Involvement of lymph nodes. Ann Arbor stage classification. Results The overall 5-year survival rate of these patients Was 80%,that of low grade malignancy patients was 90%,and of high grade malignancy was 40%.Thirteen patients were classified as stage Ⅰ E and seven as stageⅡE and three as stageⅢor Ⅳ,the 5-year survival rate was 90%,67%and 0,respectively.Nineteen patients underwent radical resection with 5-year survival rate of 92.3%.No patients undergoing palliative resection have survived more than 5 years. The prognosis of PGL with low grade malignancy and that of early stages(IE and Ⅱ E)and those undergoing radical excision was better than those with high grade malignancy, that of advanced stage(Ⅲand Ⅳ),and that undergoing palliative resection. Conclusions Preoperative diagnosis relies mainly on gastroscopy with biopsy and CT scan. For patients with early stage disease, radical resection combined with adjuvant therapy is the key factor in improving the prognosis. Chemotherapy or/and radiotherapy is useful management for patients with advanced stage disease.

Objective To report the diagnosis and treatment of a case of renal inflammatory myofibroblastoma. Methods A 30-year-old man presented with space-occupying lesion of the kidney with no symptoms in physical examination. No abnormality was found in the laboratory tests.B-ultrasound showed a clear-edged,solid mass which was 4.5 cm?3.7 cm in size and had heterogeneous echo in the upper, middle part of the left kidney.CT showed a similar value of the mass to that of the normal tissue.Enhanced MRI showed heterogeneous intensification of the mass whose signal was slightly lower than that of the normal tissue.Preliminary diagnosis of renal cancer was made. Results Nephrectomy was performed. On pathological examination the tumor was mainly composed of spindle-shaped,fibrous cells, and positive staining for Vimentin,SMA and HHF35 were observed immunohistochemically.Inflammatory myofibroblastoma of the kidney was diagnosed.Follow-up of 54 months showed no recurrence and metastasis of the tumor. Conclusions Inflammatory myofibroblastoma is a kind of rare, benign or low-grade malignant tumor of the kidney, and the clinical diagnosis is often difficult.Definite diagnosis relies on pathology. For the sake of avoidance of resecting the kidney by mistake, pathological examination is the preferred choice during operation.

Objective To understand the spectrum of bile pathogen in patients with cholelithiasis and their drug sensitivity to antibiotics. Methods Microbes automatic detection system was applied to detect the spectrum of bile pathogen and their drug sensitivities in 366 cholelithiasis patients who underwent choledochotomy. ResultAmong 366 specimen ,common cultures were positive in 258 (70 5%) in which 268 strains of aerobes were detected, and most of them were E.coli(121 strains,45 2%), Klebsiella pneumoniae (24 2%) and Enterococcus(10 1%). 14 strains of anaerobes were cultured in 32 specimen with an incidence of 44%,among which the most common type was B.fragilis (57%).Most pathogens in all the bile specimen were gram negative,73% in aerobes and 86% in anaerobes. Aerobes were highly sensitive to imipenem, tazobactam/piperacillin (TZP) and amikacin, with sensitive rate of 94%,83%, and 63%, respectively. The sensitive rate of anerobes to imipenem as well as TZP was 93% and 86% repectively. Conclusions The main pathogen in patients with cholelithiasis were Escherichia coli,Klebsiella pneumoniae, intestinal cocci and B.frailis which were most sensitive to those antibiotics such as imipenem and TZP.

Journal of Third Military Medical University 2002;0(12):-
Objective To construct a bait vector containing human glucocorticoid receptor(GR) ligand binding domain (LBD) in yeast two hybrid system in order to screen its cDNA library Methods PCR was used to amplify GR LBD fragment from the fetal liver cDNA library with the primers designed in accordance with the sequence in GenBank GR LBD The product was inserted into pGEM(r) T Vector Systems After verified with restriction endonuclease digestion of SmaⅠ/SalⅡ, the vector was inserted into the "bait plasmid" pGBKT7 (named as pGBKT7 GR LBD) After confirmation with restricted endonuclease digestion and sequence analysis, the plasmid was transformed into the yeast cell AH109, and the transformants were selected on SD/ Trp plates The interaction between GR and SRC 1 was tested by ? gal activity with yeast two hybrid system Results The amplified product of 893 bp was inserted into pGEM(r) T Vector and proven to be successful with double restriction enzyme digestion Sequence analysis revealed that the sequence was correctly inserted into pGBKT7 with a right reading frame AH109 [pGBKT7 GR LBD] grew on SD/ Trp plates, but not on the other selective media GR could interact with SRC 1 Conclusion The bait plasmid pGBKT7 GR LBD constructed expresses GR LBD correctly, and can't activate the transcription of reporter gene alone in yeast two hybrid system

Objective To evaluate the role of myocardial contrast echocardiography (MCE) to select target vessel, to monitor the opacification zone and to avoid complications during transcoronary ablation of septal hypertrophy(TASH) for hypertrophic obstructive cardiomyopathy(HOCM). Methods In 37 HOCM patients cardiac apex four chamber view, parasternal left ventricular long axis view and left ventricular short axis view were recorded after injection of the myocardial agents and absolute ethanol 5 minutes later respectively. And the opacification zones of the myocardial agents and absolute ethanol were observed. Results Among 37 patients,2 patients gave up treatment because of no appropriate vessel to provide ablation by coronary Doppler flow imaging. Two of 37 patients gave up treatments and did not undergo myocardial contrast echocardiography because of diffused coronary arteries in coronary angiography. Among 33 patients who underwent MCE, 5 patients gave up TASH including one of the opacification of left ventricular mammary muscles, one of the opacification of right ventricular and right ventricular mammary muscles, one for the opacification zone out of the target area and two for the large opacification zone. Other 28 patients underwent TASH successfully. Just after operations and 7 days, left ventricular outflow tract pressure gradient decreased at no less than 50%. Distribution zone of absolute ethanol was smaller than that of myocardial opacification zone in some patients. Conclusions MCE is a credible reference for selection of target vessels in TASH for HOCM.
