1.Expression characteristics of 123I-vascular endothelial growth factor-binding sites on tumor cells

Objective To explore the expression characteristics of vascular endothelial growth factor (VEGF) receptor (VEGFR).Methods The 123I-VEGF165 and 123I-VEGF121 were marked to human umbilical vein endothelial cell (HUVEC),several human tumor cell lines (HMC-1,A431,KU812,U937,HEP-1,HEP-G2,HEP-3B and Raji),a variety of human tumors and adjacent non-neoplastic tissues as well as peripheral blood cells.Then,the specific binding site maximal binding capacity (Bmax),dissociation constant (Kd) and concentration of 50% required specific binding (IC5o) were analyzed.The affinity,quantity and specificity of different cells combined with 123I-VEGF165 and 123I-VEGF121 were judged.Results Two kinds of analogous 123I-VEGF165 binding sites on the surface of HUVEC were found.While,there was only one kind of 123I-VEGF121binding site.123I-VEGF121 was found on the special cell lines (HUVEC,HEP-1 and HMC-1) and special early tumors (early melanoma,ductal breast cancer,ovarian cancer and meningioma).Compared with peripheral blood cells and adjacent non-neoplastic tissues,the number of VEGFR of tumor cells was bigger.Among the 123I-VEGF165 marked VEGFR,the Bmax value of early melanoma,ductal breast cancer,hepatocellular carcinoma,papillary thyroid carcinoma,ovarian carcinoma,renal cell carcinoma were 45 ± 13,13 ± 3,25 ±8,5 ±2,42 ± 12,20 ±6,respectively.While among the 123I-VEGF121 marked VEGFR,the Bmax value of early melanoma,ductal breast cancer,ovarian carcinoma were 30 ± 8,8 ± 3,20 ± 6.123I-VEGF165 and 123I-VEGF121 had specific binding capacity with a variety of human tumor cells and tissues.Compared with 123I-VEGF121,there were more different kinds of tumor cells could be bound to 123I-VEGF165 with higher capacity.Conclusion 123I-VEGF165 may be a potential target of tumor imaging in vivo,and it is expected to be used to diagnose and treat tumors.
2.Studies on injury of endothelial cells by homocysteine

AIM: This study is to detail its possible mechanisms that homocysteine (Hcy) induces injury of cultured endothelial cells.METHODS: Hcy in sequential concentrations was added into the cultured human umbilial vein endothelial cells for 24 hours in serum-free medium. The lipid peroxidation, release of LDH, cell total protein content, cell apoptosis and necrosis were assessed. RESULTS: Hcy increased the apoptosis of endothelial cells.In high Hcy concentration the cells also showed obvious injurious and necrotic morphological changes. Lipid peroxidation increased, with LDH releasing up and cell total protein content down, and they showed a positive dose-effect relationship with the Hcy concentration. All the above effects of Hcy was strengthened by low density lipoprotein (LDL) which may suggest synergetic effects of Hcy and LDL.CONCLUSION: Our results indicate that Hcy has a strong oxidizing effect, which may be one of its major mechanism for injury of EC.
3.Prognostic value of cystatin C and NT-proBNP in patients with non-ST elevation acute coronary syn-drome

Objective:To explore the prognostic value of CysC and NT‐proBNP in patients with non‐ST elevation a‐cute coronary syndrome (NSTE‐ACS) .Methods :A total of 166 NSTE‐ACS patients hospitalized in our hospital from Jan 2012 to Dec 2012 were selected .They were followed up for 12 months ,then general data ,levels of CysC , NT‐proBNP ,hsCRP and cTnI etc .and incidence rate of MACE were recorded and measured .According to MACE occurrence during follow‐up or not ,156 cases were divided into non‐MACE group (n=137) and MACE group (n=19) ,risk factors for MACE in NSTE‐ACS patients were analyzed ,receiver operator characteristic curve (ROC) was performed ,and the optimal cutoffs of related indexes predicting MACE occurrence in these patients were analyzed . Results :Compared with non‐MACE group ,there were significant rise in age [ (60.26 ± 10.45) years vs .(64.16 ± 11.21) years] ,levels of CysC [ (1.02 ± 0.11) mg/L vs .(1.15 ± 0.12) mg/L] ,NT‐proBNP [ (251.97 ± 89.65) pg/ml vs .(347.93 ± 107.29) pg/ml] ,hsCRP [ (14.69 ± 3.53) mg/L vs .(17.13 ± 3.68) mg/L] and cTnI [ (0.36 ± 0.46) ng/ml vs .(0.90 ± 0.88) ng/ml] in MACE group ,P<0.05 or <0.01. Multi‐factor regression analysis indica‐ted that CysC ,NT‐proBNP and cTnI levels were independent predictors for MACE in NSTE‐ACS patients ( P<0.05 or <0.01) .ROC curves of CysC ,NT‐proBNP ,cTnI and hsCRP judging prognosis were drawn , only AUC of CysC and NT‐proBNP curves were >0.7 [CysC:0.784 ,95% CI:0.687~0.881 ;NT‐proBNP:0.753 ,95% CI :0.639~0.867] , and it′s analysis indicated that CysC=1.07 mg/L and NT‐proBNP=279.60 pg/ml were their optimal cutoff predicting MACE .Kaplan‐Meier survival curves with above two cutoffs as risk stratification cutoff indicated that survival time of high risk group was significantly shorter than that of low risk group (P<0.05) .Conclusion:Serum CysC and NT‐proBNP levels are independent predictors assessing prognosis in NSTE‐ACS patients .
4.Clinical study on the effectiveness and safety of terazosin in the treatment of benign prostatic hyperplasia patients with concomitant hypertension

Objective To evaluate the effectiveness and safety of terazosin in the treatment of benign prostatic hyperplasia (BPH)patients with concomitant hypertension.Methods A singlecenter prospective clinical observational study was conducted from March,2006 to March,2010 in our center.The main endpoints were the changes of IPSS total score,diastolic and systolic blood pressures at the end of 4 weeks and 3 monthes compared with the baseline,The second endpoints were Qmax value at the end of 4 weeks and 3 monthes compared with the baseline,Safety was assessed by adverse events.Results There were 212 patients in the study recruited,and 189 patients completed the study.All patients had BPH combined with hepertension.All patients were randomly devided into two statistical analysis group,blood pressure well controled and not well controled group,In the well controlled group,the IPSS socre reduced from 22.31 ± 5.18 at baseline to 15.64 ±3.91 at the end of the 4th weeks and 13.16 ± 3.53 at the end of 3rd monthes in the blood pressure well controled group populatin( P < 0.01 ).The Qmax were improved significantly from (7.87 ± 2.41 ) % at baseline to (14.19 ±2.64)% at the end of the 4th weeks and (15.69 ±2.77)% at the end of3rd monthes in the blood pressure well controled group populatin( P < 0.01 ).Terazosin had moderate effect in blood pressure decreasing (P < 0.05 ),and all patients were within normal blood pressure range.In the uncontrolled group,the IPSS socre reduced from 21.55 ± 4.82 at baseline to 15.44 ± 3.66 at the end of the 4th weeks and 12.96 ± 3.11 at the end of 3rd monthes in the blood pressure well controled group populatin (P < 0.01 ).The Qmax were improved significantly from ( 8.27 ± 2.27 ) % at baseline to ( 14.26 ± 2.87) % at the end of the 4th weeks and ( 15.51 ±2.92) % at the end of 3rd monthes in the blood pressure well controled group populatin( P < 0.01 ).Terazosin decreased BPH patient blood pressure with controlled patients and unctrolled patients additionaly to other blood pressure medicine (P < 0.05 ),and no severe side effect occured.At the end of the study,all patients were taking drug continuously and were followed.Conclnsion Terazosin can significantly improve the symptoms and quality of life in BPH patients with hypertension with good safety and compliance.
5.Preventive and Therapeutic Effects of Electroacupuncture in Treating Mild-moderate Female Stress Urinary Incontinence

Objective To evaluate and verify the preventive and therapeutic effects of electroacupuncture (EA) plus pelvic floor muscle training (PFMT) in treating mild-moderate female stress urinary incontinence (SUI). Method By adopting a single-blind randomized controlled design, eighty-two SUI patients were randomized into an observation group of 40 cases and a control group of 42 cases. The observation group was intervened by EA plus PFMT, while the control group only received PFMT. Before the treatment and after 4-week treatment, the 1 h urine leakage amount, Incontinence Questionnaire Short Form (ICI-Q-SF) and improvement rate were evaluated in the two groups, to analyze the effects of the two methods in improving mild-moderate SUI. Result For mild SUI patients, the 1 h urine leakage amount and ICI-Q-SF score dropped significantly after the treatment in the observation group (P<0.05), and the observation group was significantly lower than the control group (P<0.05). For moderate SUI patients, the 1 h urine leakage amount and ICI-QSF score dropped significantly in both groups after the intervention (P<0.05), the observation group was significantly lower than the control group (P<0.05), and the improvement rate in the observation group was markedly higher than that in the control group (P<0.01). Conclusion EA plus PFMT caneffectively improve the urine leakage and urination in mild-moderate SUI patients. EA plus PFMT can effectively prevent the aggravation of moderate female SUI, and its effect is better than PFMT alone.
6.Investigation in the relationship between the number of complaints from outpatients and the time or the outpatient amounts in a certain grade A tertiary hospital

Objective To study the relationship between the number of outpatient complaints and the time or the outpatient amounts.Methods A retrospective analysis of 1289 cases of outpatients' medical complaints with basic records since 2009 in our hospital was carried out.The time regularity of the number of outpatient complaints,or the complaint rate was analyzed.And the correlation between the outpatient amounts and the number of outpatient complaints,or the complaint rate was also analyzed.Results In our hospital,the mean monthly number of outpatient complaints was (23.44±8.41)cases; the mean monthly outpatient account was (246 000±27 417)cases.The differences among monthly outpatient complaints,outpatient accounts and monthly complaints rate among every years were statistically significant.Within one year,the outpatient complaints numbers were lowest in February and January.And the outpatient complaints rate was low in November,January,February and December.There were statistically significant correlation between the monthly outpatient complaints and monthly outpatient accounts,and between the monthly outpatient complaints rate and monthly outpatient accounts.Conclusions There was time regularity in the outpatient complaints rates in one year.And there was certain correlation between the outpatient complaints rate and the outpatients' accounts.
7.Practice and outcomes of lean management at outpatient clinic process optimization

Targeting such setbacks as difficulty to registerand difficulty to see doctors at outpatient clinic services,the hospital adopted the lean management since August 2010.For example,diversified reengineering of the outpatient service process in place,which has improved outpatient traffic at the registration section by means of innovated appointment mode and outpatients total reservation;workload balance by means of through-counters for registration and fee payment as well as the timeinterval outpatient visits to doctors; reduced human costs and outpatient back-forth movement by means of integrated service and resources integration.Lean management plays a positive role in hospital management,as proven by much shortened outpatient waiting time,much higher outpatient satisfaction,for a win-win of both patients and the hospital.
8.Hepatic arterial transplantation of autologous bone marrow mesenchymal stem cells in treatment of decompensated liver cirrhosis

BACKGROUND:Autologous bone marrow mesenchymal stem cel s can treat decompensated liver cirrhosis, however, little evidence has addressed the control ed clinical research in hepatitis B patients with decompensated live cirrhosis.
OBJECTIVE:To evaluate the clinical efficacy and safety of autologous bone marrow mesenchymal stem cel s in the treatment of hepatitis B with decompensated live cirrhosis.
METHODS:A total of 67 hepatitis B patients with decompensated live cirrhosis were divided into two groups according to their wishes to receive stem cel transplantation. The control group (34 patients) only received oral administration of nucleoside analog antivirus and supportive treatment. The treatment group (33 patients) received autologous bone marrow mesenchymal stem cel s transplantation via hepatic artery plus antivirus and supportive treatment. The liver functional index, clinical signs and symptoms, adverse reactions were observed and compared at 4, 12, 24 weeks after treatment.
RESULTS AND CONCLUSION:After treatment, al patients’ symptoms were improved to varying degrees. After 4 weeks of treatment, the liver functional indexes were al significantly improved compared with before treatment, the levels of alanme aminotransferase, cholinesterase and prothrombin activity in treatment group were significantly ameliorated compared with control group (P<0.05). At 12 and 24 weeks of treatment, the alanme aminotransferase, albumin, total bilirubin, cholinesterase and prothrombin activity in control group and treatment group showed statistical y significant differences compared with before treatment (P<0.05). At the same time point, al the indicators in the treatment group were significantly ameliorated compared with control group (P<0.05). The Child-pugh score and model for end-stage liver disease score declined at 4, 12, 24 weeks after treatment, showing significant differences compared with before treatment. The difference was also significant at the same time point between two groups. The treatment of nucleoside analogue antivirus combined with autologous bone marrow mesenchymal stem cel s transplantation on hepatitis B patients with decompensated liver cirrhosis is an effective method to improve liver function and blood coagulation function, with symptom improvement, safety and low risk.
9.Effect of tirofiban combined with reteplase in patients with ST segment elevation acute myocardial infarction

Objective To study the thrombolysis effect and safety of tirofiban combined with half dose reteplase in patients with ST segment elevation acute myocardial infarction .Methods 60 patients with ST segment elevation acute myocardial infarction in our hospital from January 2011 to March 2013 were selected as research object ,and they were divided into reteplase group (control group ,n=32) and tirofiban combined with reteplase group (observation group ,n=28) ,then the recanalization rate of infarct-relat-ed artery at different time and incidence of complications were compared .Results The recanalization rate of infarct-related artery of observation group at different time were all higher than those of control group (P<0 .05) ,while incidence of complications of two groups had no significant difference(P>0 .05) .Conclusion The thrombolysis effect of tirofiban combined with half dose reteplase in patients with ST segment elevation acute myocardial infarction is better ,and it does not increase the incidence of complications .
10.Antimicrobial activity and GC-MS analysis of essential oil from lavender extracted by supercritical CO2 extraction and hydrodistillation.

OBJECTIVETo investigate the antimicrobial activity in vitro and chemical composition of essential oil from lavender extracted by supercritical CO2 extraction (SFE-CO2) process and hydrodistillation.
METHODThe antimicrobial activities against 4 bacteria and 4 fungi strains of these two oils were evaluated by using the agar disc diffusion and agar dilution method to determine the inhibition zone, minimal inhibitory concentration (MIC) and minimal bactericidal/fungicidal concentration (MBC/MFC). A GC-MS method was established to determine the chemical components of essential oils.
RESULTThese two oils presented remarkable antimicrobiat activities against all tested strains in vitro. Compared with the hydrodistillation product, SFE-CO, oil showed better antimicrobial activity against either bacteria or fungi of which MIC values were 0.63-3.33 g x L(-1) and the MBC/MFC values were 1.04-5.00 g x L(-1). By GC-MS analysis, 34 and 29 compounds identified cover 95.51% and 98. 39% of total peak area of substances appeared. The main differences between SFE-CO2 oil and hydrodistillation oil were the amounts of linalyl acetate and 5-methyl-2-(1-methylethenyl)-4-hexen-1-ol acetate.
CONCLUSIONResults presented here may suggest that the essential oil of lavender extracted by SFE-CO2 possesses has better antimicrobial properties, and therefore it is a potential source of antimicrobial ingredients for pharmaceutical industry.
