1.Cementless total hip arthroplasty:comparison of unilateral and bilateral effects

BACKGROUND:Bone cement has certain toxic effects on the human body. The probability of renovation is high after bone cement total hip arthroplasty. It is reported that the long-term effect of cementless total hip arthroplasty is apparently better than bone cement total hip arthroplasty, and can be renovated conveniently. OBJECTIVE:To investigate the clinical effect of cementless total hip arthroplasty on hip joint disease, and to compare the difference between unilateral replacement and bilateral replacement. METHODS: Clinical and folow-up data of 233 patients (280 hips), who were treated with cementless total hip arthroplasty in the Department of Orthopedics, Wuzhou Worker’s Hospital, Seventh Affiliated Hospital of Guangxi Medical University from July 2007 to December 2013, were retrospectively analyzed. According to the replacement program, they were divided into unilateral replacement group (n=186) and bilateral replacement group (n=47). Harris score of hip joint, visual analog scale score of thigh pain, the excelent and good rate of hip joint during final folow-up and complications were compared between the two groups before replacement, at 6, 12 and 24 months after replacement. RESULTS AND CONCLUSION:No significant difference in Harris scores was detected before replacement, at 6, 12 and 24 months after replacement (P > 0.05). Harris score was significantly higher at 6, 12 and 24 months after replacement compared with that before replacement in both groups (P < 0.05). No significant difference in the excelent and good rate was detected in the unilateral replacement group (87%) and the bilateral replacement group (86%) (P > 0.05). No significant difference in the visual analog scale score was seen before replacement, at 6, 12 and 24 months after replacement (P > 0.05). Visual analog scale scores were significantly lower at 6, 12 and 24 months after replacement than that before replacement in the two groups (P < 0.05). There was no significant difference in the incidence of complications after replacement in patients of both groups (P > 0.05). These findings confirm that the effects of cementless total hip arthroplasty for hip joint disease are evident, can effectively restore hip joint function. No significant difference was detected between unilateral replacement and bilateral replacement. Strict replacement operation and matching of prosthesis and medulary cavity can effectively reduce thigh pain after replacement.
2.EFFECT OF FISH OIL RICH IN EICOSAPENTAENOIC AND DECOSAHEXAENOIC ACIDS ON SERUM LIPIDS IN EXPERIMENTAL HYPERCHOLESTEROLEMIC RATS

The effects of fish oil rich in eicosapentaenoic acid (C20:5?-3; EPA) and docosahexaenoic acid (C22:6?-3; DHA) on serum total cholesterol (TC) and high density lipoprotein cholesterol (HDL-C) levels were investigated in experimental hypercholesterolemic rats. The fish oil contained about 15% EPA and 43% DHA. Each animal was given 1ml fish oil/day by intragastric feeding for 30 days. The results in twice experiments showed that TC concentration in the serum of rats fed fish oil was markedly decreased as compared to rats fed olive oil, but serum HDL-C elevated. Furthermore, the ratio of HDL-C to TC in the serum was also significantly elevated in rats fed fish oil.
3.Haploidentical blood and marrow transplantation for advanced chronic myeloid leukemia

ObjectiveTo investigate the efficacy of haploidentical blood and marrow transplantation (haplo-BMT) in the treatment of advanced chronic myeloid leukemia (CML).MethodsFrom November 2002 to October 2007,35 patients with advanced CML received haplo-BMT.Eleven patients achieved the second chronic phase (CP2) after treatment with imatinib or chemotherapy or both before pre-conditioning,but there were 13 cases in accelerated phase (AP) and 11 patients in blast phase (BP) at the time of transplantation.By the last follow-up date October 31,2011,the median follow-up time among living patients was 67 months (range,49 to 100 months).ResultsThe cases of HLA-antigen mismatched between donors and recipients as 1,2,and 3 antigens were 1,12,and 22 respectively.The number of mean mononuclear cells and CD34+ cells was (7.19+ 1.37) × 108/kg and (2.54± 1.50) × 106/kg,respectively.All but one patient achieved durable hematopoietic reconstitution. Hyperacute graft-versus-host disease (GVHD) occurred in 28.6% (10/35) patients.The cumulative incidence of grade Ⅱ to Ⅳ acute GVHD was 48%.Among 27 patients who survived longer than 100 days after transplant,16 (60 %) had chronic GVHD.Fiveyear overall survival (OS) rate was 46.2% and 45.5% in CML-AP and BP (P =0.97),respectively.Five-year probability of OS rate was 81.8%,30.8% and 27.3% in patients with CML-CP2,CML-AP and BP at transplant,respectively.The OS of CML-CP2 was significantly higher than CML-AP and BP at transplant (P<0.01 ).ConclusionHaplo-BMT is a feasible therapeutic mean for patients with advanced CML who have no matched donors available.It is better to perform haplo-BMT at CML-CP2 other than CML-AP or BP.
4. Effect of minimal residual disease monitoring by multiparameter flow cytometry pre-conditioning on prognosis of acute myeloid leukemia after allogeneic hematopoietic stem cell transplantation

Objective:
To investigate the effect of minimal residual disease (MRD) monitoring by multiparameter flow cytometry (MFC) pre-conditioning on prognosis of acute myeloid leukemia in first complete remission (CR1-AML) after allogeneic hematopoietic stem cell transplantation (allo-HSCT) , and to explore the value of MRD monitoring by MFC in the prognosis evaluation on allo-HSCT in CR1-AML.
Methods:
Between April 2012 and March 2015, consecutive 186 patients with CR1-AML who underwent allo-HSCT were analyzed retrospectively. MRD in BM before conditioning was detected by eight-color MFC. Any level of residual disease was considered to be MRD positive.

Objective:
To analyze the effect of NCCN (2015) risk stratification on prognosis of patients with acute myeloid leukemia (AML) after allogeneic hematopoietic stem cell transplantation (allo-HSCT) .
Methods:
Retrospective analysis of 258 patients with AML in CR (186 cases in CR1, 72 cases in CR2) who underwent allogeneic HSCT in our hospital between April 2012 and March 2015 according to NCCN (2015) risk stratification. Accurate classification of the acetabular injuries and appropriate treatment plan are great challenges for orthopedic surgeons because of the irregular anatomical structure of the acetabulum and aggregation of important vessels and nerves around it. Letournel-Judet classification system has been widely applied to classify acetabular fractures. However, there are several limitations, including incomplete inclusion of fracture types, difficulty in understanding and insufficient guidance for surgical treatment, etc. Serious complications such as traumatic arthritis are common due to wrong classification and diagnosis and improper selection of surgical strategy, which brings a heavy burden to the society and families. Three-column classification, based on anatomic characteristics, has advantages of containing more fracture types and being easy to understand, etc. To solve the problems existing in the diagnosis and treatment process based on Letournel-Judet classification, achieve accurate diagnosis and treatment of patients with acetabular fractures, and obtain satisfactory prognosis, the Orthopedic Trauma Emergency Center of Third Hospital of Hebei Medical University and the Trauma Orthopedic Branch of the Chinese Orthopedic Association organized experts from relevant fields to formulate the Expert consensus on the accurate diagnosis and treatment of acetabular fractures based on three-column classification ( version 2023) in terms of principles of evidence-based medicine. Based on the three-column classification, 15 recommendations were proposed, covering the diagnosis, treatment, complication prevention and management, etc, so as to provide reference for accurate diagnosis and treatment of acetabular fractures.
7.A consensus on the management of allergy in kindergartens and primary schools

Abstract
Allergic diseases can occur in all systems of the body, covering the whole life cycle, from children to adults and to old age, can be lifelong onset and even fatal in severe cases. Children account for the largest proportion of the victims of allergic disease, Children s allergies start from scratch, ranging from mild to severe, from less to more, from single to multiple systems and systemic performance, so the prevention and treatment of allergic diseases in children is of great importance, which can not only prevent high risk allergic conditions from developing into allergic diseases, but also further block the process of allergy. At present, there is no consensus on the management system of allergic children in kindergartens and primary schools. The "Consensus on Allergy Management and Prevention in Kindergartens and Primary Schools", which includes the organizational structure, system construction and management of allergic children, provides evidence informed recommendations for the long term comprehensive management of allergic children in kindergartens and primary schools, and provides a basis for the establishment of the prevention system for allergic children.
