Objective To explore a new training strategy suitable for clinical education and talent cultivation in gy-necologic oncology.Methods Ninety undergraduates from each of the two grades of clinical medicine curriculum at Kunming Medical University were randomly divided into control group(n=40)and research group(n=50).The"4+3"teaching model of plan-do-check-action(PDCA)cycle integrated with TBL were adopted,whereas the control group was educated with classic methods.Two groups of students were evaluated with quantitative assessments and anonymous questionnaire surveys.Results Multidimensional questionnaire surveys indicated that the teaching model of the test group provided a better learning experience than classic teaching methods as proved by significant improvements in educational reform(58％),learning interest(64％),skill development(72％),capacity building of learning(66％)and satisfaction with the new model of training(70％)(P＜0.05,＜0.01).Further-more,the PDCA cycle can address the short-comings of the previous teaching iteration.In written exams,the class-room quiz scores,regular grades,and comprehensive scores of the research group were(18.38±5.81),(29.09±0.29),and(82.38±4.03)points respectively all higher than those of the control group(P＜0.01).Conclusions The PDCA cycle and the TBL-embedded teaching model promote the standardization of gynecologic oncology teach-ing procedures,thereby enhancing teaching quality and achieving precise alignment between student satisfaction and assessment performance.

Objective To establish a rapid detection method for Clostridium botulinum in food.Methods A rapid detection method based on RPA-CRISPR/Cas12a was developed by integrating recombinase polymerase amplification(RPA)with the CRISPR-Cas12a system.After the reaction conditions were optimized,the method's sensitivity,specificity,and usefulness were methodically confirmed.Results and Conclusion The optimized method achieved detection within 1 hour,with a limit of detection(LOD)of 1.91 copies/μL.No cross-reactivity was observed with non-target pathogens.The RPA-CRISPR/Cas12a-based detection method developed in this study exhibits high specificity,sensitivity,and operational simplicity and may provide a feasible solution for the rapid detection of foodborne pathogens.

Objective To develop a teaching strategy which is suitable for training talents and improving teaching quality in field of clinical gynecological oncology.Methods Seventy-eight clinical students of grade 2020 in a medi-cal university were divided into control group(n=38)and research group(n=40).The"3+2"teaching mode of team-based learning(TBL)and workshop(WS)were adopted,and the learning outcome was evaluated at preview preparation,knowledge application and the questionnaire survey of students,so as to promote the cultivation of clinical reasoning of medical students.Results Compared with traditional teaching method,TBL+WS teaching group had better academic performance.The difference of before class tests and final exams was more significant[17.53±6.15 points and(76.81±5.10)points,respectively,P＜0.001].However,among the eight dimensions of the classroom teaching questionnaire,the teaching quality dimension was the highest(97.5%).More students think that this model had a positive effect on cultivating clinical thinking and developing new knowledge.Conclu-sions This integrated teaching strategy improves the quality of obstetrics and gynecology teaching and suppots students'capacity building on clinical reasoning.

Objective To explore the effect of paclitaxel with carboplatin combined chemotherapy on hemocyte re-lated indexes,vascular endothelial growth factor(VEGF)and tumor markers in ovarian cancer(OC)patients.Methods OC patients combined chemotherapy(CC)group and ovarian benign tumor control group with 50 cases in each.The pathological changes in the ovarian cells were observed by HE staining and the expression of VEGF was observed by immunohistochemical staining(IHC).Blood cell was counted by flow cytometry(FC).Serum car-bohydrate antigen 125(CA125)and human epididymal protein 4(HE4)were measured by electrochemical lumi-nescence(ECL).Results Compared with control group,the ovarian tissue structure was disordered and the cell atypia was obvious in OC.VEGF expression was significantly enhanced.Platelet count(PLT),CA125 and HE4 were significantly increased(P＜0.01).However,necrotic cells were observed in ovarian tissue of CC group.VEGF expression was inhibited.PLT count and the level of CA125 and HE4 were significantly lower than those of control group after chemotherapy(P＜0.01).Conclusions Combined chemotherapy may regulate the level of VEGF,CA125 and HE4 in OC patients.

Objective To establish tree shrew model of type 2 diabetes mellitus (T2DM) combined with cerebral ischemia (IS),and to explore the regulatory mechanism of ischemic postconditioning (PC) on gene differential expression in cerebral cortex under metabolic abnormalities and cerebral ischemia condition.Methods Seventy tree shrews were divided into control,T2DM,IS,T2DM+IS and T2DM +IS +PC groups (n =14 each group).The experimental diabetes model was established by the combined use of high fat diet breeding with streptozocin injection in tree shrew.The local cerebral thrombosis was induced by photochemical reaction in tree shrews,and ischemic PC was established at 4h after cerebral ischemia followed by clipped ipsilateral common carotid artery three times (5 min/time).The metabolic status of tree shrews was measured by serum biochemical markers.TTC staining,HE staining,and electron microscopy were used to observe the changes of the body's metabolic status at 24h after IS.RNA-seq was used to analyze differentially expressed genes.Results The ultrastructure of brain cells was abnormal and the cerebral infarction area was the largest in T2DM+IS tree shrews (P＜0.01).Compared with control group,body weight of tree shrews in T2DM + IS group was significantly reduced (P＜ 0.01) while blood glucose,total cholesterol,low density lipoprotein-cholesterol,triglyceride,and C-reactive protein (CRP) levels were markedly increased(all P＜0.01).The RNA-seq analysis showed that there were 1 629 differentially expressed genes (1 109 up-regulated genes and 520 down-regulated genes) in T2DM + IS group vs control group.However,ischemic PC deceased the cortical infarction area (P＜0.01)and reduced blood glucose,lipid and CRP levels (P＜0.05),with 520 differential expression genes (203 up-regulated genes and 317 down-regulated genes).Conclusion Ischemic PC improves the metabolic disturbance-aggravated ischemic brain injury in T2DM tree shrews,which may be related to its regulation on gene expression in cerebral cortex.

Objective To observe what changes the brown adipose tissue (BAT) of T2DM rat models would have,including morphology,function and specially expressed uncoupling protein (UCP1) after the gastric bypass (Roux-en-Y,RYGB) and to explore the effects of RYGB on BAT of T2DM rat models and its related mechanism in order to provide a theoretical and experimental basis for treatment of T2DM patients with RYGB.Methods SD rats were given a high-fat and high-sugar diet for two weeks,by injecting streptozotocin (STZ) 30 mg/kg intraperitoneally to build models.Blood glucose was measured after 72 h and 1 week by the fast blood glucose meter.The models were built successfully if blood glucose at both times were ≥ 16.7 mmol/L.Feeding environment:individually caged,standard rat feed,natural circadian cycle,indoor temperature (18±2)℃,indoor humidity (50±2)％.50 rats were randomly selected and dividing into four groups according to intervention methods:diabetes operation group (group A,n=10),undergoing RYGB surgery with the whole stomach kept;diabetes sham operation group (group B,n=10),the same anesthesia and incision as the previous RYGB group.The operation mode was anterior gastric wall incision and suture,jejunum transection in corresponding position and in situ anastomosis with the same suture method as group A;diabetes control group (group C,n=10),normally feeding after building models;and the last one was the healthy control group (group D,n=10):no special treatment,adequate water feeding ensured.The rest of rats remained to be used.The body mass (BM),fasting blood glucose (FBG),fasting serum insulin(Fins)before and at the 1st,2nd,4th and 8th week after surgery were measured.The number of transversal ceils was calculated by IPP6.0 image software and the average radius of fat cells was calculated.UCP1 expression was tested with western blot.Results ① The fasting blood glucose,fasting serum insulin level and the body weight of dia betic rats were higher than those of the control group,but the insulin sensitivity index was significantly lower.② HE Staining showed:diabetes operation group (group A) rats,compared with diabetes control group and diabetes sham operation group(group B),had obviously higher brown fat cell counts transversally and average radius,and the difference was statistically significant (P＜0.01).Diabetes operation group (group A) rats had no significant difference from the healthy control group(group D) rats,and the diabetes control group (group C) rats had no significant difference from sham operation group (group B) rats as well.③ Western blot showed that after the gastric bypass surgery,compared with the diabetes sham operation group (group B) and the diabetes control group (group C),UCP1 expression of brown adipose tissue of the diabetes operation group (group A) increased significantly (P＜0.05).The diabetes sham operation group (group B) had no significant difference from the diabetes control group (group C),and the diabetes operation group(Group A) had no significant difference from the healthy control group (Group D) as well (P＞0.05).Conclusion RYGB can reduce the body mass and insulin resistance (IR) of diabetic rats and,at the same time,promote the expression of UCP1 of brown adipose tissue.RYGB might increase the activity of brown adipose tissue by regulating the UCP1 signaling pathway to improve body's insulin resistance.

Objective To optimize the extraction conditions of volatile oil from Angelica Sinensis and Cassia Bark in Wenyang purgation granules.Methods The volatile oil was extracted from Angelica Sinensis and Cassia Bark by the method of steam distillation light oil device with diethyl ether extraction. The yield of the volatile oil was chosen as the evaluation index. The time of dip in water, the ratio of water to herbal medicine material and the time of distillation were used as the main factors. The optimum extraction conditions were investigated by the L9(34) orthogonal design. Results The optimal conditions for extraction process of volatile oil from Angelica Sinensis and Cassia Bark in Wenyang purgation granules were as follows:the time of dip in water was 3 h;the ratio of water to herbal medicine material was 10∶1;the time of distillation was 6 h. Conclusion The optimized conditions of extraction process are stable and feasible.

ObjectiveTo observe the effects of chemotherapy on mitochondria ultrarnicrostructure and stereological characteristic of mucinous cystadenocarcinoma (MC) and serous cystadenocarcinoma (SC) and to investigate clinical significance and possible mechanism of chemotherapy on two cystadenocarcinoma.MethodsThe ultramicrostructure changes of mitochondrion were observed with electronic microscope and the stereological changes and its interrelation of volume density (Vv), surface density (Sv) and ratio of surface density(Rsv,μm-1)of mitochondria of the cells in ovarian carcinoma were studied and compared with the normal control. ResultsElectron microscope reveals that the mitochondria swelling and cristae disorganizing were seen in ovarian carcinoma.The Vv(0.072±0.059)and Sv(0.057±0.040)of MC mitochondria were decreased markedly(P =0.004 and P =0.029, respectively), while its Rsv (0.891 ±0.400) μm-1 increased obviously (P =0.015); but the Vv and Sv of mitochondria of the SC cells were increased markedly (0.267±0.12,P =0.001 and 0.122±0.057,P =0.003,respectively).After chemotherapy,cancer cells mitochondrion Sv elevated significantly (0.060±0.037, P = 0.053), showing a similar MC mitochondria of variability. Conclusion Therearedifferentcharacteristicsintheultramicrostructureandstereologicalcharacteristicinthe mitochondria of two types ovarian carcinoma. Effects of chemotherapy may be associated with the ameliorate of construction and function of ovary cells.

ObjectiveThe trial was to examining the effectiveness and cost effects of family intervention for rehabilitating chronic outpatients with schizophrenia in the country.Methods90 subjects were randomly assigned to the family intervention group and the control group. Both groups received the same treatments, but the family intervention courses mainly containing mental health education were given to the family intervention group for one year. During the time, all subjects were evaluated with standard rating scales and self made criteria. ResultsThe family intervention group demonstrated clinical results significantly superior to those of the control group on overall improvement according to the scores on the SDSS, the SAPS, the SANS and the MRSS. Substantially, the direct and indirect average cost in the family intervention group was significantly lower than those of the control group. Conclusions Family intervention is effective not only in making the schizophrenics recover from illness but also in both increasing their social functions and reducing their medical cost.
