Objective To investigate the application value of combination of ambroxol and fluidmicronized ipratropium bromide in full dose and frequency in treating senile refractory pneumonia.Methods From January 2010 to November 2013,84 cases of elderly patients with refractory pneumonia were divided into 3 groups.On the basis of anti-inflammatory and symptomatic treatment,the control group (n=28) received routine sputum suction and normal saline irrigation;the routine dose group (n=28) received the intravenous ambroxol 15 mg twice daily and the inhalation of 2.5 ml of nebulized ipratropium bromide once daily plus treatment in control group while full dose group (n=28) received intravenous ambroxol hydrochloride 30 mg three times a day and the inhalation of 2.5 ml of nebulized ipratropium bromide four times a day.The severity of pulmonary infection was described by clinical pulmonary infection score (CPIS) and C reactive protein (CRP),and therapeutic efficiency was evaluated by clinical symptom remission time,average hospitalization days and effective rate.Results At the 5 day and 10 days after treatment,the CPIS in full dose group was (4.82 ± 1.21) scores and (4.39± 1.03),respectively,C-reactive protein (CRP) was (58.11 ± 11.12) mg/L and (57.57±12.69) mg/L respectively,and these CPIS and CRP values at the 5 day and 10 days were significantly lower than those in the control group (6.10 ± 1.10) scores and (5.36 ± 0.95) scores,(74.50± 12.79)mg/L and (68.46± 13.77) mg/L (all P＜0.05).The above Values were lower as compared with those [(5.43±0.84) and (5.21 ±0.83),(65.11 ±834) mg/L and (59.11± 13.77) mg/L (P＜ 0.05)] in routine dose group.The CPIS and CRP concentration in full dose group and routine dose group were lower at the 10 days than at the 5 days (P＜0.05).Besides,clinical symptom remission time (4.43 ± 1.75) days,average hospitalization duration(26.32 ± 4.92) days in full dose group,clinical symptom remission time (5.68 ± 2.29) day,average hospitalization (32.21 ± 5.63) days in conventional dose group were significantly lower than in the control group (7.79±2.74) and (36.71 ± 11.78) days (P ＜ 0.05).Clinical manifestation,time of remission,and average hospitalization days of the full dose group were also reduced as compared with the conventional dose group (P＜0.05).The efficiency rates in the three groups of control,conventional dose and full dose were 42.9％ (12/28),71.4％ (20/28),92.9％ (26/28),among which the efficiency of full dose group was significantly higher than those of the control group and the conventional dose group,(x2 =16.047,6.788,P＜0.01).The conventional dose group showed a higher efficiency than the control group (x2 =4.667,P ＜ 0.01).Conclusions Combination of ambroxol and fluid-micronized ipratropium bromidet in full dose and frequency for treating senile refractory pneumonia is a useful managemen in the elderly patients.

Objective To explore the value of real‐time three‐dimensional speckle tracking imaging (RT‐3D‐STI) in the early detection of globle and regional left ventricular myocardial systolic function in patients of coronary stenosis without regional wall motion abnormality. Methods 64 coronary stenosis patients without regional wall motion abnormality were divided into 2 groups according to angiography (CAG):without coronary stenosis group( n = 20) and coronary stenosis group ( n = 44), and all the myocardial segments of the patients were divided into 5 groups:without coronary stenosis group normal myocardial segments ( n = 340 ), coronary stenosis group normal myocardial segments ( n = 235 ), mild coronary stenosis myocardial segments( n = 126), moderate coronary stenosis myocardial segments( n =213) and severe coronary stenosis myocardial segments( n =174). Real‐time three‐dimensional full volume of left ventricle was obtained, left ventricular global area strain (GAS)and regional area strain(AS) of 17 myocardial segments, etc, were measured by RT‐3D‐STI respectively. The parameters between the two groups and among the five sub groups were compared, the correlation between GAS and coronary artery Gensini score was also analyzed. Results GAS in coronary stenosis group significantly reduced compared with without coronary stenosis group( P <0 0.5) S.egmental AS gradually reduced with the increasing of the degree of the coronary stenosis. There was no statistically difference of segmental AS among without coronary artery group normal myocardial segments, coronary stenosis group normal myocardial segments and mild coronary stenosis myocardial segments( P >0 0.5) S.egmental AS in moderate and severe coronary stenosis myocardial segments significantly reduced ( P < 0.05 ), and that in severe coronary stenosis myocardial segments was lower than that in moderate coronary stenosis myocardial segments, the differences were statistically significant ( P < 0.05 ). In addition, GAS was significantly correlated with coronary artery Gensini score( r =0.828, P <0.01). Conclusions RT‐3D‐STI can quantitatively assess the early changes of left ventricular globle and regional myocardial systolic function in patients with coronary stenosis.

A 54-kDa protein overexpressed by chloroquine-resistant Plasmodium berghei ANKA strain was first reported by us. In this paper, the localization of this protein by immunoelec-tron microscopy is presented. The results showed that the protein was mainly scattered inside the cytoplasm of the early date trophozoites and schizonts of erythrocytic stage of P. berghei ANKA strain , and some of it was also found in cytoplasm of erythrocytes infected with parasites. The protein content was much higher in chloroquine-resistant P. berghei ANKA strain than in chloroquine-sensitive P. berghei ANKA strain, suggesting the importance of this protein in understanding mechanism of chloroquine resistance in malaria parasites .

Objective To investigate the protective effect of ascorbic acid on myocardium from ischemia and reperfusion injury and its electrophysiological mechanism. Methods Using modified Ferrier's arrhythmia induced by ischemia and reperfusion injury in isolated guinea pig right ventricular wall. Results Ascorbic acid at 5, 10, 50 and 100μmol/L slightly decrease incidence of ischemia arrhythmia (P ＞ 0. 05). In contrast, ascorbic acid at 10, 50 and 100μmol/L significantly reduced the incidence of arrhythmia during early reperfusion(2～12min) (P ＜ 0. 01 or P ＜ 0. 001). Ascorbic acid at 5～ 100μmol/L didn't reverse the abbrevating of APD of endocardium and epicardium and didn't affect the conduction time in endocardium (P ＞ 0. 05). However, ascorbic acid abbreviated epicardial conduc tion time significantly (P ＜ 0. 05 or P ＜ 0. 01 or P ＜ 0. 001 ). Conclusion The study suggests that ascorbic acid might exert antiarrhythmic effect via alleviating conduction abnormity to avoid transmural reentries during ischemia and early reperfusion.

Objective To observe the clinical efficacy of incipient diabetic nephropathy treated with Ligustrazine Hydrechloride Injection plus Candesartan Cilexetil.Methods 80 cases of incipient diabetic nephropathy were randomly divided into a treatment group and a control group,with 40 cases in each group.On the basis of treated with insulin and antidiabetic drugs,The control group was given Candesartan Cilexetii and the treatment group was given Candesartan Cilexetil together with Ligustrazine Hydrochloride Injection.Results Plasma level of ET and NO,UAER and Ua1-MG of the two groups were all reduced,while the improvement of the treatment group was better than the control group.Conclusion The combination treatment of Ligustrazine Hydrochloride Injection and Candesartan Cilexetil was an ideal one for patients with incipient diabetic nephropathy.

Objective To explore the effect of spironolactone on vascular remodeling of injuried rabbit artery. Methods 20 male New Zealand rabbits were randomly divided into two groups. The rabbit models of iliac artery angioplasty were set up by BC. After injury, control group was fed with standard forage, and experimental group was fed with standard forage and spironolactone(20mg?kg~ -1?d~ -1). The rabbits were killed four weeks after injury. The injuried segments of iliac artery were removed, and processed for histological and morphological analysis by imumohistochemistry and TD 2000 analysis system of pathology image. Results AUIEM and AUEEM in the experimental group significantly increased as compared with the control group(P0.05). Conclusion Spironolactone could improve vascular remodeling after angioploasty of iliac artery in rabbits.

Objective To reveal the relation between Nesiritide and vascular remodeling,the relation between Nesiritide and the levels of aldosterone.Methods 20 male New Zealand rabbits were randomly divided into two groups.All rabbit models were constructed with BC.After injury,the control group were fed with standard forage,the Nesiritide group received standard forage plus subcutaneous Nesiritide[0.1 mg/dissected free from arteria iliaca externa,processed for histological and morphological study by TD 2000 analysis system of pathology image.Data of vascular remodeling and the levels of plasma aldosterone were determined.Results There had significant difference between AUIEM and AUEEM(P ＜ 0.05),markedly significant difference was also found in residual cavity areas and ratios of stenosis(P ＜0.01).There had no significant difference between areas of media(P ＞ 0.05).Aldosterone reduced from the 28th day.Conclusions Nesiritide could benefit vascular remodeling after angioplasty,which might be through reducing aldosterone.

ObjectiveTo reveal the relation between rhBNP and vascular remodeling,the relation between rhBNP and levels of serum high sensitive C reactive protein (hs-CRP).Methods20 male New Zealand rabbits were randomly divided into two groups.All the rabbits were constructed to be models of angioplasty with BC.After injury,the control group were feed with standard forage,the rhBNP group received standard forage plus sibeutaneous rhBNP [0.1mmg/(kg · day),20 days].The rabbits were killed 4weeks after initial injury.The injured segments were dissected free from arteria iliaca externa,processed for histological and morphological study by TD 2000 analysis system of pathology image.The data of vascular remodeling was determined.The levels of hs-CRP were measured at the beginning and the end of the experiment.ResultsThere was significant difference in AUIEM and AUEEM[ (643.2±134.06) μm2 vs (493.7±139.32) μm2 ; (1495.1±204.98)μm2 vs (1265.9±232.56)μm2,P ＜ 0.05 ],extremely significant difference in residual cavity areas and ratios of stenosis[ (519.1±93.47) μm2 vs (308.6±86.50) μm2;(18.68±7.72) μm2 vs (38.04±8.32) μm2,P ＜ 0.01 ].There was no significant difference between the areas of media (P＞ 0.05).rhBNP reduced the levels of hs-CRP on the 28th day.ConclusionsrhBNP can benefit vascular remodeling after angioplasty which might be through its anti -inflammatory action.

Ectrodactyly,ectodermal dysplasia and clefting(EEC)syndrome is characterised by ectrodactyly,ectodermal dysplasia and clef-ting.It has been rarely reported till now.To introduce the causes,manifestation,diagnosis and treatment of this disease,we presented a case of EEC syndrome and reviewed the related literature.It may provide some informations for clinical doctors.
