1.Postoperative complications of endoscopic thyroidectomy via breast areola vs conventional open thyroidectomy

Objective To compare postoperative complications between endoscopic thyroidectomy via breast areola approach and open thyroidectomy.Methods From June 2011 to June 2014,475 patients underwent thyroidectomy.Patients were divided into traditional open group (251 cases) and endoscopic thyroidcctomy group (224 cases).Postoperative complications were recorded and compared.Results All operations were successfully completed.There was no case in endoscopic thyroidectomy group converted to traditional open surgery.There was no serious postoperative complication occurred,including injury to trachea,mediastinal emphysema,intraoperative massive hemorrhage and cardiopulmonary accidents.The difference of postoperative complication between two groups had no statistical significance (all P >0.05) as to permanent recurrent laryngeal nerve injury,Homer syndrome,permanent hypoparathyroidism,chyle leakage,postoperative hemorrhage and infection of incision.However,postoperative ecchymosis was more often in endo group (P <0.05).Conclusions Compared to traditional open thyroidectomy,endoscopic thyroidectomy via breast areola approach has significant cosmetic advantages without increasing the incidence of major postoperative complications.
2.Detection and clinical significance of Th17 cell proportion and expression of IL-17 in peripheral blood of patients with non-small cell lung cancer

Objective To investigate the relationship between the proportion of Th17 cells and IL-17 expression in the peripheral blood and their clinical significance in patients with non-small cell lung cancer (NSCLC).Methods The proportion of Th17 cells determined by flow cytometry (FCM) and IL-17 detected by enzyme-linked immunosorbent assay (ELISA).The data was analyzed from 60 untreated patients with NSCLC (NSCLC group) and 20 healthy volunteers (control group).Results The Th17 cells proportion in CD+3 T-cells in NSCLC group was significantly higher than that in control group [(1.23±0.41) % vs (1.05±0.28) %,t =1.679,P =0.097].The expression levels of Th17 cells in peripheral blood of patients with squamous cell carcinoma was significantly higher than that in adenocarcinoma patients [(1.31±0.39) % vs (1.09±0.41) %,t =2.093,P =0.041].The concentration of IL-17 of patients with NSCLC was significantly higher than that in control group [(21.11±7.87) pg/ml vs (17.64±5.07) pg/ml,t =2.280,P =0.027].The expression levels of serum IL-17 of squamous cell carcinoma patients was significantly higher than that of adenocarcinoma patients [(23.11±8.73) pg/ml vs (18.54±6.38) pg/ml,t =2.203,P =0.032].As clinical stage progressed,the proportion of Th17 cells and IL-17 expression were significantly downtrend [F =3.151,P =0.032,F =4.132,P =0.010].Conclusion The levels of Th17 cells and IL-17 in the peripheral blood of patients with NSCLC significantly correlated with the pathological types and TNM stage,might play an important role in the course of the disease.

Epithelial architecture is formed in tissues and organs when groups of epithelial cells are organized into polarized structures.The epithelial function as well as signaling across the epithelial layer is organized by polarity proteins.Epithelial polarity complexes form and regulate epithelial integrity.Polarity proteins establish and maintain cell polarity,regulate tight junctions and adherens junctions.Polarity proteins act as neoplasms suppressors in mammals and interact with oncogenes,disrupt cell polarity,induce tumorigenesis.Therefore,polarity mechanisms of epithelial cell could therapeutic targets for clinical therapy.
4.Effects of Acute Exhausted Exercise on the Myocardial HSP70 and Plasma ANP of Rats in a High Temperature Environment

Objective To investigate the effect of acute exhausted exercise in high temperature environment on the expression of myocardial HSP70 and plasma ANP level of in rats. Methods Forty eight male Sprague-Dawley rats was randomly and equally assigned to the following six groups:control group(C),exercise group(E),one-hour heat exposure group (H), heat exposure and exercise group (HE), exercise and 24 hours after exercise group(E'),heat exposure and 24 hours after exercise(HE'). Each group consisted of 8 rats. Rats in E,HE,E' and HE' ran on a treadmill until exhaustion, while rats in H and H' were exposed in a high temperature environment (33℃ ,50%RH) for one hour. Rats in C,E,HE and H were killed immediately after exercise or heat exposure. Rats in HE' and E' were killed 24 hours after exercise. Samples from myocardium and serum were collected for determining HSP70,ANP and CK-MB. Results(1) HSP70 levels in groups E and E' are significantly higher than that in group C(P<0.05 and P<0.01,respectively),and the level in group HE' is significantly higher than that in groups H and E'(P<0.01 );The HSP70 level in group H is significantly higher than that in group C. (2) The ANP and CK-MB levels in group E are Significantly higher than that in group C.and in group HE is higher than in group H (P<0.05);The HSP70 levels in group E' and HE' significantly decrease comparing to that in groups E and HE (P<0.01). Conclusions(1)Both heat exposure and exercise can induce the increased expression of myocardial HSP70 in rats and the peak expression is seen 24 hours after exercise. The increased expression of HSP70 may have protective effects on the heat-induced myocardial injury. (2) The increased ANP levels in plasma immediately after exhausted exercise improves the blood circulation in myocardium and hints the potential possibility of myocardial damage. However,the high temperature environment does not strengthen the increase of plasma ANP level that caused by exhaustive exercise.
5.Development of portable uterine contraction pressure monitoring system.

For the high cost and mobility issues, a home uterine contraction pressure monitoring system based on Windows CE platform was developed. In this paper, the design of hardware circuit, micro-controller system and LabVIEW program based on Windows CE are discussed. The clinical validation experiment in hospital for this system was made and the experimental results show that this system complies with the trend that current medical equipment is becoming portable, homely and networked. Through real-time monitoring uterine contraction pressure, occurrence of premature birth and abortion can be prevented effectively.

7.The curative effect of two methods of surgical treatment for thoracolumbar fractures

Objective To investigate the curative effects of the traditional surgical method of posterior pedicle Schanz screw fixa-tion with partially stripping injured paravertebral muscle in treating thoracolumbar fracture .Methods 52 cases with thoracolumbar fracture underwent posterior pedicle Schanz screw fixation to partially strip injured paravertebral muscle from January ,2010 to Jan-uary ,2012 .The patients were divided into partially stripping injured paravertebral muscle group (32 cases) and traditionally strip-ping injured paravertebral muscle group(20 cases) .The blood loss ,postoperative drainage ,kyphosis postoperative 3 d VAS scores , vertebral height correction rate ,Cobb angle correction ,Oswestry dysfunction indexes were copmared .Results Comparing with tra-ditionally stripping injured paravertebral muscle group ,the partially stripping injured paravertebral muscle group has better effect in transperative blood loss ,postoperative drainage and postoperative 3 d VAS scores ,the differences were statistically significant .But there was no statistical significance between the two groups in vertebral height correction rate ,kyphosis Cobb angle correction ,Os-westry dysfunction indexes .Conclusion Partially strip injured paravertebral muscle has less painful than traditional surgery does , being safe and effective with small invasion and easy to recover .
8.Preparation and characterization of poly (lactic-co-glycolic acid) microspheres for controlled release of osteogenic growth peptide

BACKGROUND:Previous animal studies have revealed that osteogenic growth peptide (OGP) applied locally or systemically could promote fracture healing. But the disadvantages of short in vivo half-life and low oral bioavailability limit its clinical application.OBJECTIVE: To study the encapsulation and delivery of synthetic OGP (sOGP) from biodegradable polymeric microspheres in vitro so as to choose better carrier for the future study.DESIGN: Grouping observation and comparative trail.SETTING: Laboratory of School of Life Science and Technology, Xi'an Jiaotong University.MATERIALS: sOGP was synthesized by Xi'an Langene Bio-science Co., Ltd. with Fmoc system. The purity of sOGP after purification was over 98 % identified by reverse phase high performance liquid chromatography, and the molecular weight of sOGP was 1 523 650, which was consistent with the theoretical value (Mr 1 523 750); the result of whole sequence analysis of sOGP was consistent with the theoretical sequence of OGP. Poly (lactic-co-glycolic acid) (PLGA,50:50, Mr 30 000; 75:25 Mr 80 000) was obtained from Shandong Medical Instrumental Institute (Ji'nan, China)METHODS: PLGA with a 50:50 or 75:25 lactide to glycolide ratio was used for microsphere preparation using a modified double emulsion solvent extraction Water-in-oil-in-water (w/o/w) technique. The surface structure and appearance of microsphere was observed under scanning electron microscope; particle size distribution of microsphere was counted by laser diffraction particle sizer; efficiency of encapsulation, release time and the structural integrity of sOGP released from PLGA were assessed using high performance liquid chromatography (HPLC).RESULTS: ①Spherical microspheres of sOG-PLGA were formulated successfully. The average particle diameter of the PLGA 50:50 microsphere was (19.6±4.5) μm, efficiency of encapsulation (83.9±4.2)% with (83.9±4.2) % drug-loading efficiency, while the PLGA 75:25 microspheres showed an average size of (35.8±3.6) μm, efficiency of encapsulation (65.6±6.8)% with (65.6±6.8)% drug-loading efficiency. ②HPLC results indicated that sOGP were not chemically altered,physically aggregated but presented a intact structure as the original sOGP. An initial burst release was observed for both PLGA microspheres, especially from PLGA 75:25. sOGP was released from PLGA 50:50 microsphere for 56 days,and from PLGA 75:25 microspheres for over 70 days. The cumulative release of sOGP from PLGA 50:50 for 35 days was significantly lower than from PLGA 75:25 (P < 0.05).CONCLUSION: The controlled release of sOGP encapsulated within PLGA 50:50 is better than the delivery from PLGA 75:25. Moreover, the release time could meet the requirements for fracture or bone defect site.
9.Influence of curcumin in NO and S1 00βlevels in brain tissue of rats after focal cerebral ischemia reperfusion inj ury

to observe the infarction volume.Nitrate reductase assay was used to detect the level of NO in brain tissue of the rats.The level of S100βin brain was detected by ELISA method.Results Compared with model group,the brain infarction volumes of the rats 24 and 72 h after cerebral ischemia reperfusion in curcumin group were significantly decreased (P<0.05).Compared with sham operation group,the NO and S100βlevels in the brain tissue 24 and 72 h after cerebral ischemia reperfusion of the rats in model group were significantly increased(P<0.05);compared with model group,the levels of NO in the brain tissue 24 and 72 h after cerebral ischemia reperfusion in curcumin group were remarkably decreased (P<0.05);compared with modee group,the level of S100βin the brain tissue 72 h after cerebral iscemia reperfusion in curcumin group was remarkably decreased (P < 0.05 ). Conclusion Curcumin can significantly reduce the degree of ischemia reperfusion injury in the rats and reduce the levels of NO and S100βin brain tissue,which suggests that the decrease of NO and S100βlevels in brain tissue may be associated with the neuroprotective effect of curcumin.
10.Protective effect of mesenteric lymph duct ligation on organ functions in MODS rats

AIM: To study the protective effect of mese nt eric lymph duct ligation on the functions of liver, kidney and heart, and morpho logy in multiple organ dysfunction syndrome (MODS) rats subjected to two-hit. METHODS: Male Wistar rats were divided into three groups: the me senteric lymph duct ligation group, the non-ligation group and sham group. The M ODS model of two-hit was established by bleeding and LPS administration in both ligation group and non-ligation group. After 24 h, all rats were cannulated to f acilitate blood withdrawal for serum sample, then all rats were killed and organ s including kidney, liver, lung and heart were collected for making microscopic sectio ns. The biochemical indexes of hepatic and renal functions and myocardial enzyme in s erum were determined before and after experiment. RESULTS: After two-hit, the serum contents of AST, ALT, TBA, BUN , Cr and LDH-1 in both non-ligation group and ligation group, and UA content in non-ligation group were obviously increased than those in pre-experiment and sh am group (P
