1.Endovascular embolization for traumatic carotid cavernous fistula:a retrospective case series study of 20 cases

Objective To evaluate the clinical effect of endovascular embolization for traumatic carotid cavernous fistula (TCCF).Methods In 20 patients with TCCF,10 were treated with the detachable balloon embolization,4 were treated with the detachable balloon embolization and coils,2 were treated with coils alone,3 were treated with coils and Onyx,and 1 was treated with covered stenting.Two of them were embolized again because of leaking balloon.Results Eighteen of the 20 patients were embolizeded successfully once,2 were embolizeded successfully twice.The patency rate of internal carotid artery was 100%.No operation-related complications occurred.Conclusions Endovascular embolization is a simply,safe and reliable method of curative effect for the treatment of TCCF.
2.Sedative and Anti-stress Action of Shenqiwuweizi Capsule

OBJECTIVE: To study the sedative and antistress action of Shenqiwuweizi capsule. METHODS: The sedative and hypnotic experiments were made to observe the action of Shenqiwuweizi capsule on the mice's sleep induced by pentobarbital sodium at subthreshold dosage or hypnotic dosage. The swimming and hypoxia tolerance experiment were performed to observe the effects of Shenqiwuweizi capsule on the swimming time and hypoxia tolerance time in mice. RESULTS: Shenqiwuweizi capsule could obviously prolong the sleeping time of mice induced by pentobarbital sodium, increase the number of sleeping animals caused by pentobarbital sodium at subthreshold dosage, and significantly prolong the time of swimming and hypoxia tolerance in mice. CONCLUSION: Shenqiwuweizi capsule had sedative and anti-stress action.
3.Preparation and characterization of poly (lactic-co-glycolic acid) microspheres for controlled release of osteogenic growth peptide

BACKGROUND:Previous animal studies have revealed that osteogenic growth peptide (OGP) applied locally or systemically could promote fracture healing. But the disadvantages of short in vivo half-life and low oral bioavailability limit its clinical application.OBJECTIVE: To study the encapsulation and delivery of synthetic OGP (sOGP) from biodegradable polymeric microspheres in vitro so as to choose better carrier for the future study.DESIGN: Grouping observation and comparative trail.SETTING: Laboratory of School of Life Science and Technology, Xi'an Jiaotong University.MATERIALS: sOGP was synthesized by Xi'an Langene Bio-science Co., Ltd. with Fmoc system. The purity of sOGP after purification was over 98 % identified by reverse phase high performance liquid chromatography, and the molecular weight of sOGP was 1 523 650, which was consistent with the theoretical value (Mr 1 523 750); the result of whole sequence analysis of sOGP was consistent with the theoretical sequence of OGP. Poly (lactic-co-glycolic acid) (PLGA,50:50, Mr 30 000; 75:25 Mr 80 000) was obtained from Shandong Medical Instrumental Institute (Ji'nan, China)METHODS: PLGA with a 50:50 or 75:25 lactide to glycolide ratio was used for microsphere preparation using a modified double emulsion solvent extraction Water-in-oil-in-water (w/o/w) technique. The surface structure and appearance of microsphere was observed under scanning electron microscope; particle size distribution of microsphere was counted by laser diffraction particle sizer; efficiency of encapsulation, release time and the structural integrity of sOGP released from PLGA were assessed using high performance liquid chromatography (HPLC).RESULTS: ①Spherical microspheres of sOG-PLGA were formulated successfully. The average particle diameter of the PLGA 50:50 microsphere was (19.6±4.5) μm, efficiency of encapsulation (83.9±4.2)% with (83.9±4.2) % drug-loading efficiency, while the PLGA 75:25 microspheres showed an average size of (35.8±3.6) μm, efficiency of encapsulation (65.6±6.8)% with (65.6±6.8)% drug-loading efficiency. ②HPLC results indicated that sOGP were not chemically altered,physically aggregated but presented a intact structure as the original sOGP. An initial burst release was observed for both PLGA microspheres, especially from PLGA 75:25. sOGP was released from PLGA 50:50 microsphere for 56 days,and from PLGA 75:25 microspheres for over 70 days. The cumulative release of sOGP from PLGA 50:50 for 35 days was significantly lower than from PLGA 75:25 (P < 0.05).CONCLUSION: The controlled release of sOGP encapsulated within PLGA 50:50 is better than the delivery from PLGA 75:25. Moreover, the release time could meet the requirements for fracture or bone defect site.
4.The expression of miR-155 and miR-146a in peripheral blood mononuclear cells and plasma of rheumatoid arthritis patients

ObjectiveTo investigate the expression of miR-155 and miR-146a in peripheral blood mononuclear cells (PBMC) and plasma of rheumatoid arthritis (RA) patients.MethodsPBMC and plasma were separated from the peripheral blood of 34 RA patients and 15 healthy individuals.Total RNAs were isolated and miRNAs were purified.The levels of miR-155 and miR-146a were determined by quantitative reverse transcription PCR(qRT-PCR).U6 was used as housekeeping control.The amount of target miRNA was normalized relative to the amount of U6(ΔCt=ΔCtmiRNA-ΔCtU6).Relative expression levels were expressed as 2 △-ΔCt.Data were analyzed using SPSS 13.0 software.The test of homogeneity of variance and unpaired t-test was used to compare between groups.P values(2-tailed) less than 0.05 were considered as statistically significant.ResultsThe expressions of PBMC and plasma miR-155 were higher in RA patients than those in the healthy control individuals(0.08±0.08 vs 0.05±0.03,t=-2.225,P<0.05; 5.9±6.7 vs 1.3±2.0,t=-3.677,P<0.05).The expression of miR-146a in PBMC and plasma of RA patients and controls were(1.3±1.2 vs 0.8±0.6,t=-2.154,P<0.05)and(741±1001 vs 300±295,t=-1.669,P>0.05).According to their DAS28 value,RA patients were divided into high activity group (23 cases,DAS28≥5.0) and low disease activity group( 11cases,DAS28<5.0).The plasma miR-155 and miR-146a expressions were significantly higher in high activity group than those in low activity group.There were no significant differences in the expression of PBMC miR-155 and miR-146a between the two groups.ConclusionThe expression of PBMC and plasma miR-155 and miR-146a are higher in RA patients.The expression of plasma miR-155 and miR-146a are associated with RA patients' activity.Plasma miR-155 and miR-146a may be potential non-invasive biomarkers for RA diagnosis anddisease activity assessment.

6.Repairing closed rupture of Achilles tendon with rectangle frame and no tension short incision

Objective To improve the operation method of rupture of Achilles tendon for decreasing complication. Methods Total 39cases with closed rupture of Achilles tendon were selected in this study. Short incision was made at achilles tendon wall, reveal and anneal the broken ends, using Kirschner wire and steel-wire to make rectangle frame ,intradermic fixation of fracture away from the broken ends, then sutured ends with rarities and smoothing,functional exercise after 6weeks ankle rest position fixation. Results According to the Arner Lindholm evaluation system,the treatment outcome were excellent in 34 cases ,and good in 5cases. No complication including re-rupture skin ,tendon necrosis and infection were found after operation. Conclusion Repaired closed rupture of Achilles tendon with short incision and rectangle frame, assisted with ankle rest position fixation after operation wasa worthy way for treating closed rupture of a chilles tendon.
7.Reconstruct coronoid process of ulna with ilium-fascia lata complex tissue

Objective To analyze and explore the operative effect of coraccid process basilar part fractures combined injury of ligamental structure. Methods Use ilium-fascia lata complex tissue to reconstruct coronoid process of ulna and medial collateral ligament, recoverd anterior and wall horn anatomic structure of elbow joint. Total 9 cases of ulna coracoid process fractures concomitancy with elbow posterior dislocation . Among these one with olec-ranon fracture,five patients with head of radius fracture. Anterior trance joint approach was used,removal bone chips, restore defect with ilium-fascia lata complex tissue after cuting and trimming. Once reduction was achieved,fixed with screw. Repaird the collateral ligament and anterior joint capsule use the fascia lata, If there were combined fracture of the radius head or olecranon fracture,fixed through lateral approach,All injured extremities were treated with a posterior plaster splint with functional position for 3 weeks followed by elbow rehabilitation training. Results According to Moneys evaluation method,2 patients were classified as excellent ,5 as good,l as fair and 1 as poor. The excellent and good rate was 77. 8%. Two patients with gently ossifying myositis but no patients with wound infection,internal fixation loosening or break,elbow joint destabilizing orcorpus libemm. Conclusions Coracoid process basilar part fractures was simultaneous with the instability of osteal frame and soft tissue. To attain the favourable anatomical foundation for elbow joint functional recovery, it should rebuild the height and shape of coronoid process first and think highly of repair or rebuild medial collateral ligament and joint capsule. It reconstructed the coronoid process osteal frame and soft tissue instability at the same time to use ilium-fascia lata complex tissue. The operative procedure is simplify with reliability effect and fine functional rehabilitation.
8.Effects of guizhi tang on inflammatory cytokines in myocardial ischemia and hyperlipidemia rats.

OBJECTIVETo explore the effects of Guizhi Tang on the inflammatory cytokines in myocardial ischemia and hyperlipidemia rats.
METHODThe early changes of hyperlipid and atherosclerosis are caused by utilizing multiple factors including feeding hyperlipid and propylthiouracil and high doses of vitamin D3 for 12 weeks. Sixty male SD rats were randomly divided in to 5 groups: control group, model group, simvastatin group, low-dosage Guizhi Tang group, high-dosage Guizhi Tang group. At the end of six weeks treatment, pituitrin(pit) is abdominal cavity injected every 24 hours for a total of three times. Detecting the serum levels of SES, CRP, NO, SOD, MDA and the content of cardiac muscle tissue SOD, MDA, The expression of TNF-alpha in cardiac muscle tissue was detected by immunohistochemistry.
RESULTGuizhi Tang significantly decreased levels of SES, CRP and MAD, increased levels of NO and SOD, Guizhi Tang markedly decreased the level of protein expression of TNF-alpha in cardiac muscle tissue.
CONCLUSIONGuizhi Tang may inhibit the proinflammatory factors and oxidation in myocardial ischemia and hyperlipidemia rats.

9.Operative treatment of complex posterior tibial plateau fractures via posterior approach

Objective To evaluate the clinical results of operative treatments for the complex posterior tibial plateau fractures via posterior approach. Methods Eleven cases with complex posterior tibial plateau fracture from June 2008 through June 2010 were reviewed retrospectively. There were 7 males and 4females, with age from 33 years to 60 years (average, 47.8 years). According to AO classification, there were 41-B2.2.4 type in 2 cases, 41-B3.1.2 type in 3, 41-B3.3.2 type in 3, 41-B3.1.2 type combined 41-B3.3.2 type in 2, 41-C3.3 type in 1. Carlson posterior lateral approach were used in 5 cases, posterior medial approach were used in 3 cases, and posterior medial and/or lateral approach combined with anterior approach were used in 3 cases. All fractures were fixed with plates. Autogenous ilium grafts were used if necessary.Results All cases were followed up. The average follow-up time was 1.6 years (range, 3-24 months). At the final follow-up visit, bone union was obtained in all cases. The mean Rasmussen score was 16.7 (range, 15-18), and the mean HSS was 86.2 (range, 75-96). The postoperative knee range of motion were 0°-135°, 0°-130° and -10°-125° in 5 cases with posterior lateral plateau fractures, 3 cases with posterior medial plateau fractures and 3 cases with anterior and posterior plateau and intercondylar fractures respectively. There was no vascular and nerve injuries. Loosing or breaking of hardware's was not found. Conclusion The Carlson posterior lateral and/or medial approach is preferred for the complex posterior plateau fractures, with the advantages of direct reduction and stabilization.
10.Clinical application of ultrasound guided percutaneous transhepatic gallbladder puncture and drainage in the treatment of pregnancy combined with acute biliary pancreatitis

Objective:To investigate the clinical value and safety of ultrasound guided percutaneous transhepatic gallbladder puncture and drainage (PTGBD) in the treatment of pregnancy combined with acute biliary pancreatitis (ABP).Methods:The clinical data of 47 pregnant combined with ABP from 2010 to 2019 in Shengjing Hospital of China Medical University were retrospectively analyzed. Among them, 19 patients received regular medication (conservative group), and the other 28 patients received PTGBD (PTGBD group). Clinical indexes of two groups including acute physiology and chronic health evaluation acute physiology and chronic health evaluationⅡ(APACHEⅡ), serum amylase, white blood cell and total bilirubin were respectively recorded before and 3 days after treatment.Results:There were no statistical difference in the indexes before treatment between 2 groups ( P>0.05). The APACHE Ⅱ, serum amylase, white blood cell and total bilirubin after treatment were significantly lower compared with those before treatment, conservative group: (5.9 ± 1.9) scores vs. (7.2 ± 1.3) scores, (736.8 ± 64.2) U/L vs. (969.2 ± 124.3) U/L, (10.3 ± 1.4) × 10 9/L vs. (14.7 ± 2.1) × 10 9/L and (55.3 ± 9.5) mmol/L vs. (67.1 ± 10.2) mmol/L; PTGBD group: (4.1 ± 1.7) scores vs. (7.0 ± 1.2) scores, (465.5 ± 77.9) U/L vs. (1 001.8 ± 112.5) U/L, (8.4 ± 2.2) × 10 9/L vs. (13.5 ± 2.6) × 10 9/L and (38.4 ± 10.6) mmol/L vs. (73.7 ± 12.5) mmol/L, and the indexes in PTGBD group were statistically lower than those in conservative group, and there were statistical differences ( P<0.05). In conservative group, 5 cases died (both mother and infant) after deterioration of the condition and 1 infant died after birth. In PTGBD group, 3 cases died (both mother and infant) after deterioration of the condition and 1 infant died after birth. The maternal cure rate and fetal survival rate in PTGBD group were statistically higher than those in conservative group PTGBD: 89.3% (25/28) vs. 14/19 and 85.7% (24/28) vs. 13/19, and there were statistical differences ( P<0.05). Conclusions:Among patients with pregnancy combined with ABP, early PTGBD is more effective than conservative medication, and quickly relieves symptoms and reduces complications.
