Cancer Research and Clinic 2011;23(2):109-110,113
Objective To investigate and analyze the imaging performance of SPECT and X-ray in fibrous dysplasia of bone (FDB). MethodsSPECT and X-ray were performed in 35 cases of FDB proven pathologically during the period of 1999-2009. A retrospective study method was used to analyze the imaging results.ResultsTwenty-eight of 35 cases had lesions localized in only one bone (monostotic), 7 cases had them in many bones (polyostotic). Monostotic most occurred in bones of limbs, Polyostotic more involved in the unilateral limb. X-rays: Ground-glass-like changes in 22 cases, cystic swelling in 12 cases, loofah-like changes in one case. SPECT: Bone lesions showed localized increased radionuclide uptake. ConclusionManifestations of FDB are various. The conventional imaging is lack of specific performance. The imaging examination should be referred each other and selected reasonably for diagnosis and differential diagnosis.
3.A dosimetric comparison of intensity-modulated radiotherapy versus conventional tangential irradiation for breast cancer after breast-conserving surgery
Cancer Research and Clinic 2010;22(3):182-185
Objective To evaluate the dosimetric benefits of intensity-modulated radiotherapy (IMRT) for early stage breast cancer after breast-conserving surgery. Methods Ten patients with stage Tis~2N0M0 breast cancer receiving breast-conserving surgery were selected for the study. Six cases were on the left, four were on the right. A dosimetric comparison of IMRT with conventional wedged tangential technique was performed on each patient using three dimensional treatment planning. The total prescribed dose for both plans was 50 Gy/25 fractions. Dose volume histograms (DVH) were used to compare the planning target volume (PTV) and organs at risks(OARS). Results The PTV coverage in IMRT plan was similar to that of the conventional plan (97.83 % vs 97.61%). A better dose uniformity throughout the whole breast was achieved by IMRT plan. The percentage of PTV receiving less than 95 % prescribed dose and more than 103 % prescribed dose (inhomogeneity index IHI), decreased from 25.42 % to 2.71%: the percentage of PTV receiving more than 105 % prescribed dose (V105 %) decreased from 25.79 % to 1.08 %. A better amelioration of IHI and reduction of V105 % in IMRT plans were observed in the relatively large PTV subgroup. The Dmean and percentage of volume receiving more than 30 Gy (V30) of heart from 6.72 Gy to 4.95 Gy, and from 7.23 % to 1.04 %. the Dmean and percentage of volume receiving more than 20 Gy (V20) of ipsilateral from 9.19 Gy to 7.65 Gy, and from 22.34 % to 20.18 %; the Dmean of contralateral breast decreased from 5.12 Gy to 3.52 Gy;the Dmean of contralateral lung decreased from 2.76 Gy to 1.79 Gy; the Dmean of liver decreased from 7.23 Gy to 1.04 Gy for right side patients. Conclusion Forward planning IMRT based on a standard tangential beam arrangement significantly improves the dose homogeneity throughout the target volume of intact breast and reduces the dose to OARS, especially in patients with large breast volumes who might be proposed as candidates of IMRT for intact breast.

Objective To analyse the causes of non-union and delayed union in distal femur and assess the clinical outcomes of retrograde intramedullary interlocking nail(RIIN). Methods Between June 1995 and December 1998, 15 cases of non-union and delayed union of distal femur were treated with RIIN. There were 9 males and 6 females, the average age of the patients was 34.5 years(range 23 to 46 years). At surgery, the failed implants were removed in all 15 cases, bone graft was performed in 10 cases, closed reaming was done in 5 cases. 11 patients also received knee adhesion release operation. The X-ray examination and KSS knee functional score were used to evaluate the results. Results The average duration of follow-up was 14.5 months. All patients achieved solid bone union at an average of 6.4 months. There were no malunion, infection and refracture in this series. According to the KSS rating system, the average range of motion(ROM) was 93.5 degrees, increased by 28 degrees(42.7% ) compared with the preoperation ROM. The average knee score was 96 points. The excellent result was obtained in 13 cases, good in 2 cases. The average knee function score was 90.5 points. Conclusion The main reasons for non-union and delayed union were improper selection of the indications and use of implants. Retrograde intramedullary interlocking nail is one of the effective alteratives for treatment of distal femur non-union and delayed union. It can provide a stable and reliable fixation which is of benefit for early functional exercise. Bone grafting, closed reaming technique and knee joint adhesion release should be considered in order to enhance the bone healing and the improvement of knee ROM and function.
