1.Effect of the etanercept on the hip pain in children with ankylosing spondylitis

Objective To study the early effect of the etanercept on the hip pain in children with ankylosing spondylitis(AS).Methods Seventy-seven patients were randomly divided into etanercept group (group A,n =21 cases),diclofenac sodium group (group B,n =20 cases),sulfasalazine plus methotrexate group (group C,n =19 cases),placebo control group (group D,n =17 cases).Group A was treated with etanercept 0.8 mg/kg,once a week.Group B was given diclofenac sodium enteric-coated tablets 1 mg/kg,2 times a day.Group C was treated with sulfasalazine tablets 15-25 mg/kg,2 times a day and methotrexate tablets 10-15 mg/m2,once a week.Group D patients were given vitamin C 0.395 g,2 times a day,then the effect of different drugs on AS hip pain in children was observed.Results One day after administration,the visual analogue scale (VAS) score of the etanereept group and diclofenac sodium group was significantly different statistically compared with the placebo control group,and this effect continued until more than 14 days.Sulfasalazine with methotrexate group had no difference in the experimental time window compared with that of the placebo control group.The VAS score of the etanercept group compared with the diclofenac sodium was significantly different statistically,and this effect continued until more in 14 days.Conclusion Etanereept can relieve the hip pain in children with AS rapidly and continuously.
2.A dosimetric comparison of intensity-modulated radiotherapy versus conventional tangential irradiation for breast cancer after breast-conserving surgery

Objective To evaluate the dosimetric benefits of intensity-modulated radiotherapy (IMRT) for early stage breast cancer after breast-conserving surgery. Methods Ten patients with stage Tis~2N0M0 breast cancer receiving breast-conserving surgery were selected for the study. Six cases were on the left, four were on the right. A dosimetric comparison of IMRT with conventional wedged tangential technique was performed on each patient using three dimensional treatment planning. The total prescribed dose for both plans was 50 Gy/25 fractions. Dose volume histograms (DVH) were used to compare the planning target volume (PTV) and organs at risks(OARS). Results The PTV coverage in IMRT plan was similar to that of the conventional plan (97.83 % vs 97.61%). A better dose uniformity throughout the whole breast was achieved by IMRT plan. The percentage of PTV receiving less than 95 % prescribed dose and more than 103 % prescribed dose (inhomogeneity index IHI), decreased from 25.42 % to 2.71%: the percentage of PTV receiving more than 105 % prescribed dose (V105 %) decreased from 25.79 % to 1.08 %. A better amelioration of IHI and reduction of V105 % in IMRT plans were observed in the relatively large PTV subgroup. The Dmean and percentage of volume receiving more than 30 Gy (V30) of heart from 6.72 Gy to 4.95 Gy, and from 7.23 % to 1.04 %. the Dmean and percentage of volume receiving more than 20 Gy (V20) of ipsilateral from 9.19 Gy to 7.65 Gy, and from 22.34 % to 20.18 %; the Dmean of contralateral breast decreased from 5.12 Gy to 3.52 Gy;the Dmean of contralateral lung decreased from 2.76 Gy to 1.79 Gy; the Dmean of liver decreased from 7.23 Gy to 1.04 Gy for right side patients. Conclusion Forward planning IMRT based on a standard tangential beam arrangement significantly improves the dose homogeneity throughout the target volume of intact breast and reduces the dose to OARS, especially in patients with large breast volumes who might be proposed as candidates of IMRT for intact breast.

6.Construction of insulin B chain DNA vaccine that prevents mice from autoimmune diabetes

Objective To construct insulin B chain DNA vaccine and to perform a vaccinating trial for preventing mice from autoimmune diabetes induced by multiple low dose streptozotocin. Methods RT PCR was used to clone insulin B chain, and insulin B chain DNA vaccine was constructed. DNA vaccine was injected into tibilias anterior muscle of C57BL/6 mice in treatment group (T), prophylactic group (P). The morphology of mice islets was investigated by pathology, blood samples were taken to determine glucose and insulin. Group T and group P were compared with group of normal control mice (C) and group of untreated diabetic mice (D). Results (1) Insulin B chain gene was cloned by RT PCR, which ligated with pcDNA3 plasmid. Insulin B chain DNA vaccine was constructed. (2) In group P, after injection of DNA vaccines, 3 (3/10) and 4 (4/10) mice became diabetic in the second week and the third, fourth week respectively, the incidence was significantly less than those of groups D and T. The insulin level showed no difference between group C and the non diabetic mice of group P. The change of pancreas of diabetic mice in group P was similar to group D. Conclusion Insulin B chain DNA vaccine can prevent pre autoimmune diabetes.

Background and purpose:Vascular endothelial growth factor-C(VEGF-C) is ranked first as a lymphoangiogenic factor, which induces lymphatic proliferation and spread of solid tumors. Lymph node status remains the strongest prognostic factor in a variety of human malignant tumors including uterine cervical carcinomas and lymphnode metastasis is stimulated by tumor cell aggressiveness, such as abilities of growth and metastases.We examined the correlation between VEGF-C expression and tumor aggressiveness in cervical carcinomas with respect to clinicopathologic features and patient outcome and to see whether the expression of VEGF-C and its receptor in cervical carcinoma tissue and their paratumour tissue had a role in tumor metastasis and other clinical significance. Methods:48 fresh cervical cancer tissues and their paratumour tissue were examined by RT-PCR to detect the VEGF-C/KDR/Flt-4mRNA expression.Results:There was a significant difference of VEGF-C/KDR/Flt-4mRNA expression between the tumor , paratumour tissue and the normal controls. And there was no significant relationship between VEGF-C mRNA, KDR mRNA or flt-4mRNA expression with the pathological types and clinical stage of invasive carcinoma of cervix. 8.New First-line Treatment Drug for Advanced Non-small Cell Lung Cancer-Afatinib

Afatinib is an irreversible inhibitor for the ErbB family of tyrosine kinases with oral administration. In two randomized, open-label and multinational phase III trials, the progression-free survival was significantly prolonged by afatinib compared with peme-trexed plus cisplatin (LUX-Lung 3) or gemcitabine plus cisplatin (LUX-Lung 6) in the treatment-naive patients with advanced non-small cell lung cancer ( NSCLC) with activating EGFR mutations. The patient-reported symptoms such as cough and dyspnoea, and certain health-related quality of life after the treatment by afatinib were also better than those treated by control dugs. Afatinib was ap-proved by FDA in July 2013 as the first-line treatment drug for the patients with metastatic NSCLC with EGFR exon 19 deletions or exon 21 (L858R) mutations. The action mechanisms, pharmacokinetics, clinical trials and adverse events were reviewed in this paper.
9.Argumentation study in clinical axillary temperature measurement

Objective To survey the difference of measurement results between axillary temperature measurement used in clinic and standardized axillary temperature measurement in textbooks. Methods For the same patients and in the nearest time interval, the two methods mentioned above were used to mea-sure the axillary temperature. Totally 782 patients were measured. Results There was no statistical sig-nificance between the measuring results of the two methods. Conclusions Axillary temperature measure-ment used in clinic should be standardized so that more scientific data will be got,effective operation and humanized service can be realized.
10.Study on Quality Standard for Shule Granules

Objective:To establish the quality standard for Shule granules. Methods:By TLC, Angelic asinensis, Panax pseudog-inseng, Fructus liquidam-baris and Leonurus in Shule granules were qualitatively identified;and contents of notoginsenoside R1 and gin-senosides Rg1 in Shule granules were quantitatively determined by HPLC. Results: In the qualitative identification, the clear spots showed good resolution and specificity. The content of notoginsenoside R1 and ginsenosides Rg1 , two main ingredients in Shule gran-ules, was 0. 166 6 and 0. 681 8 mg·g-1 , respectively. Conclusion:The TLC and HPLC-UV determination method is simple, accu-rate and reproducible, and can be used as the quality control method for Shule granules.
