1.Study on Quality Control of Rupining Strapping

OBJECTIVE:To establish quality control method for rupining strapping.METHODS:The qualitation of Herba Epimedii,Rhizoma Bolbostemmae and fructus toosendan in rupining strapping was conducted by TLC.The principal drug-epimedium herb was determined by column chromatography-HPLC.RESULTS:Herba Epimedii,Rhizoma Bolbostemmae and fructus toosendan could all be identified in TLC.Good linear correlation of icariin was observed when the sample size was0.428?g~2.568?g(r=0.9998).The average recovery was94.57%(RSD=1.43%).CONCLUSION:The method is sensi-tive,accurate,and reproducible.It can be used for the quality control of rupining stripping.
2.Analysis of HLA-A, B local haplotype among Han population in Northwest China

Objective To analyze the HLA-A, B local haplotype frequencies of Han population in Northwest China. Methods The results of HLA-A, B local polymorphism were obtained by using the PCR-sequence specific oligonucleotide probes (SSOP) reverse dot blot. Haplotype was inferred by the heredity law of HLA. Results The high haplotype frequencies were A02-B46, A30-B13, A02-B40. Linkage disequilibrium parameters of 11 haplotypes had significant differences. Conclusion The HLA-A, B local haplotype frequencies of Han population in Northwest China differ from those of Han population in other areas. The haplotypes A30-B13, A01-B37 and A32-B44 present significant linkage disequilibrium.
3.REPORT OF HLA DISTRIBUTION IN 2315 VOLUNTEER DONORS OF CHINESE BONE MARROW BANK FROM NORTHWEST CHINA

Objective To report the HLA data of volunteer donors of Chinese bank from Northwest China and characterize the distribution of HLA genes in Northwest China. Methods HLA-A, B antigens of 2315 volunteer donors were examined by the method of microlymphocytetoxicity (MLT) test .The antigen frequencies(AF) were assessed by directly counting; and based on that gene frequencies(GF) were calculated. HLA data from other population were collected and distribution characteristics were compared. With the raw data, Hardy-Weinberg equilibrium, statistical parameters of forensic medicine interest for HLA were computed. Results A total of 18 specific antigens were detected in HLA-A and the most frequent antigen was A2 . AF and GF were 0.5136 and 0.3026, respectively. A total of 42 specific antigens were detected in HLA-B and the most frequent antigen was A13. Its AF and GF were 0.1978 and 0.1044, respectively. The heterozygosity(H), polymorphism information content(PIC), discrimination power(DP) and probability of paternity exclusion (PPE) of HLA-A were 0.8215, 0.8212, 0.9356 and 0.7798 accordingly; while H,PIC, DP and PPE of HLA-B were 0.9322, 0.9322, 0.9878 and 0.9528. Conclusion The polymorphism of HLA-A,B genes is characteristic in Chinese. In this research, the genetic trait of HLA in 2315 volunteers is consistent with Northern Han population.
4.Application of mechanical insufflation-exsufflation combined with external diaphragm pacemaker in elderly patients with mechanical ventilation

Objective:To explore the suitability of mechanical insufflation-exsufflation combined with external diaphragm pacemaker in elderly patients with mechanical ventilation and its effect on the complications and prognosis of mechanical ventilation.Methods:Using the convenient sampling method, a total of 40 elderly patients undergoing mechanical ventilation through nasal tracheal intubation who were admitted to the Geriatric Ward of the PLA General Hospital from September 2018 to December 2019 were selected as the research objects. The patients were divided into the observation group and the control group by random number table method, with 20 cases in each group. The control group continued the traditional airway nursing technique and limb rehabilitation training, while the observation group added mechanical insufflation-exsufflation combined with external diaphragm pacemaker on the basis of the control group. The incidence of pulmonary infection, changes in sputum production at 1 h and 24 h, diaphragm thickening rate, weaning time, success rate of weaning and mortality rate were observed in the two groups before and after intervention.Results:After the intervention, the incidence of pulmonary infection in the observation group was lower than that of the control group, the sputum production and diaphragm thickening rate at 1 h and 24 h were higher than those in the control group, the weaning time was shorter than that of the control group, and the success rate of weaning was higher than that of the control group. All differences were statistically significant ( P<0.05) . There was no statistically significant difference in mortality rate between the two groups ( P>0.05) . Conclusions:Compared with traditional airway nursing techniques, mechanical insufflation-exsufflation combined with external diaphragm pacemaker can help elderly patients with mechanical ventilation achieve airway clearance and respiratory muscle exercise more safely and effectively, improve strength and endurance of diaphragmatic muscle and enhance the ability of spontaneous cough in elderly patients. In addition, the intervention method is mild and easy for patients to accept, and it is worthy of being widely used in elderly patients with mechanical ventilation.
5.Megestrol acetate plus metformin for fertility-sparing treatment of atypical endometrial hyperplasia and early-stage endometrial adenocarcinoma:a prospective study

Objective To evaluate the efficacy of medroxyprogesterone acetate(MA)plus metformin as the primary fertility-sparing treatment for atypical endometrial hyperplasia(AEH)and early-stage grade 1 endometrial adenocarcinoma(G1 EAC)and the recurrence rate after treatment.Methods Sixty patients(aged 20-42 years)with AEH and/or grade 1 EAC limited to the endometrium were enrolled prospectively and randomized into two groups(n=30)to receive oral MA treatment at the daily dose of 160 mg(control)or MA plus oral metformin(850 mg,twice a day)for at least 6 months.The treatment could extend to 12 months until a complete response(CR)was achieved,and follow-up hysteroscopy and curettage were performed every 3 months.For all the patients who achieved CR,endometrial expressions of IGFBP-rP1,p-Akt and p-AMPK were detected immunohistochemically.Results A total of 58 patients completed the treatment.After 9 months of treatment,23(76.7%)patients in the combined treatment group and 20(71.4%)in the control group achieved CR;two patients in the control group achieved CR after converting to the combined treatment.The recurrence rate did not differ significantly between the control group and combined treatment group(30.0%vs 22.7%,P>0.05).Ten(35.7%)patients in the control group experienced significant weight gain of 5.7±6.1 kg,while none of the patients receiving the combined treatment exhibited significant body weight changes.Compared with the control group,the patients receiving the combined treatment showed enhanced endometrial expressions of IGFBP-rP1 and p-AMPK with lowered p-Akt expression.Conclusion Metformin combined with MA may provide an effective option for fertility-sparing treatment of AEH and grade 1 stage IA EAC,and the clinical benefits of metformin for controlling MA-induced weight gain and promoting endometrial expressions of IGFBP-rP1 and p-AMPK while inhibiting p-Akt expression warrants further study.
