1.Application of single-incision laparoscopic surgery in digestive surgery

With advances in surgical techniques,laparoseopic surgical instruments and constant equipment innovation,minimally invasive surgery has become the main stream of this century.Wide range of applications of endoscopy and laparoscopy as the representative of the revolutionary changes in diagnosis and treatment of digestive diseases since the second half of last century is the main symbol.Patients obtained benefits from the minimally invasive technique to reduce the trauma and pain,shorten the course of treatment,and gain rapid recovery.Singleincision laparoscopic surgery (SILS) is proposed in recent years.It differs from conventional multiports laparoscopic surgery,with only a small 2-3 cm incision at the body's natural scar-umbilicus as the approach to assist laparoscope and surgical instruments into the abdomen cavity.In this paper,the surgical experiences of SILS based on 800 cases and understanding of the SILS at home and abroad were presented to talk about the application of SILS technique in digestive surgery.
2.Abnormality of sphincter of Oddi and biliopancreatic diseases

Abnormality of sphincter of Oddi is divided into the anatomical and functional ones. Anatomical abnormality includes stenosis and insufficiency, and the dyskinesia of sphincter of Oddi is composed of hypermotility and hypomotility. Many biliopancreatic diseases are associated with the abnormality of sphincter of Oddi, which include congenital pancreaticobiliary maljunction, gallstones, primary choledocholithiasis and hepatolithiasis, post-cholecystectomy syndrome, and pancreatitis. The proper acknowledgement of abnormality of sphincter of Oddi is of importance to improve the diagnosis and therapy of corresponding biliopancreatic diseases.
3.Improvement of minimally invasive therapies and techniques for intra-and extrahepatic bile duct stones

This article introduces the explorations in improving minimally invasive therapies and techniques for intra-and extrahepatic bile duct stones in this center,including laparoscopic choledocholithotomy without the placement of T tube,laparoscopic partial hepatectomy for the treatment of intrahepatic bile duct stones using an anastomosis stapler for liver partition,and modification of percutaneous transhepatic cholangioscopy.The improved therapies and techniques above have achieved good clinical effects,and a scientific judgment of their clinical effects still depends on a large number of cases and randomized controlled clinical trials.
4.Enteric motility alterations in experimental gallbladder cholesterol stones formation in guinea pigs

Objective To study the changes in small intestinal motility during the process of gallbladder cholesterol stone formation.Methods Forty guinea pigs were divided into two groups of 20 each,the experiment group fed on high cholesterol diet ( cholesterol 2% ),while the control group on normal diet.Animals were sacrificed at 8 weeks.Slow wave and tension of the isolated small intestinal muscle specimen were measured using a multi-channel physiological recorder,and its relation to gallstone formation was assessed.Results Compared with the control group,the frequency and amplitude of the slow wave significantly reduced in the experimental group (5.70 ± 1.05/min vs.17.45 ± 1.50/min and 0.23 ± 0.31 my vs.0.78 ±0.17 mv respectively,P <0.05).The t-value between the two groups were -27.083 and - 13.236 respectively; Frequency of contraction and muscle tension also reduced significantly (5.94 ± 1.25/min vs.15.85 ± 1.76/min and 0.78 ±0.002 g vs.1.20 ±0.11 g respectively,P <0.05).Thet-value between the two groups were - 19.448 and - 17.307 respectively.Conclusions Reduced small intestinal motility is present in guinea pigs fed on a lithogenous diet,and may have a role in the pathogenesis of cholesterol gallstone formation.
5.Intestinal Mucosal Immune Barrier in Guinea Pig Model of Pigment Gallstone

Objective To explore the correlation between intestinal mucosal immune barrier and pathogenesis of pigment gallstone and its possible mechanism.Methods Eighty guinea pigs were randomly divided into control group(group CON),pigment gallstone group(group PS),and intestinal mucosal protection group(group GLN).The guinea pigs were fed with normal diet in group CON,pigment gallstonein during diet in group PS,and glutamine-supplemented diet in group GLN for 8 weeks.The guinea pig model of pigment gallstone was established.The incidence of pigment gallstone was detected.The morphology of intestinal mucosa was observed,and the numbers of CD3~+T cell,CD40~+B cell,and IgA~+ plasma cell were counted.Results The incidence of pigment gallstone was significantly higher in group PS than in groups GLN and CON(P<0.05).Compared with group CON,the intestinal wall was significant thinner and represented obvious signs of inflammation in group PS,and the numbers of CD3~+ T cell,CD40~+ B cell,and IgA~+ plasma cell significantly decreased(CD3~+ T cell,21.8±2.5 vs 11.1±3.4,P<0.01;CD 40~+B cell,12.9±2.0 vs 10.7±3.6,P<0.01;IgA~+ plasma cell,12.4±3.4 vs 10.7±3.5,P<0.01).The signs of inflammation were less severe in group GLN than in group PS.There were significant differences in the numbers of CD3~+ T cell,CD40~+ B cell,and IgA~+plasma cell between groups GLN and PS.Conclusion Intestinal barrier dysfunction,including mechanical barrier and immune barrier,is involved in the formation of pigment gallstone.Glutamine has proved to improve the function of intestinal mucosal barrier and decrease the incidence of pigment gallstone.
6.A Retrospective Comparative Study Between Transumbilical Single-incision and Traditional Three-port Laparoscopic ;Appendectomy

Objective To discuss the safety and feasibility of single-incision laparoscopic appendectomy ( SILA ) . Methods A retrospective analysis was made on clinical data of patients diagnosed as having appendicitis by clinical manifestations , ultrasound or computer tomography at our department from June 2009 to December 2014.There were 189 cases of transumbilical SILA ( transumbilical group ) and 123 cases of traditional three-port laparoscopic appendectomy ( three-port group ) .Patients with incomplete records, combination with other surgery , pregnancy appendicitis , acute appendicitis over 72 hours were excluded .Intraoperative and postoperative data of two groups were compared . Results No conversion to open surgery was required in both groups .The operation time, postoperative exhaust time , and postoperative hospital stay had no significant differences between the transumbilical group and the three-port group [(45.5 ±12.7) min vs.(46.3 ±17.8) min, t=-0.461, P=0.650; (1.4 ±0.5) d vs.(1.5 ±0.6) d, t=-1.588, P=0.112;(3.8 ±1.6) d vs.(4.1 ±1.9) d, t=-1.554, P=0.121].The cosmetic scores at 3 months after surgery was higher in the transumbilical group than that in the three-port group [(4.5 ±0.6) points vs.(4.2 ±0.5) points, t=4.585, P=0.000].No significant differences were identified in the incidence of complications and pathological types between the two groups . Conclusion SILA is technically feasible and safe with better cosmetic effects .
7.Modified method for umbilical reconstruction after single-incision laparoscopic surgery

Objective:To introduce a modified method for umbilical reconstruction after single-incision laparoscopic surgery.Methods:Between June 2012 and June 2015,173 patients underwent single-incision laparoscopic cholecystectomy at our institution.All the patients adopted our modified method for umbilical reconstruction.Results:All the surgeries were successful.The umbilical incision healed well with good cosmetic result.Conclusion:The modified method for umbilical reconstruction can improve cosmesis and decrease the possibility of incision hernia after single-incision laparoscopic surgery.More studies are needed to evaluate definitive clinical advantages of this technique.

9.Research about Bevacizumab combined with FOLFOX protocol to treat metastasis colorectal cancer

The common therapy of colorectal cancer is FOLFOX scheme,which contains flurouracil,leucovorin and oxaliplatin.Numerous clinical trials have demonstrated that bevacizumab combined with FOLFOX scheme in cancer's therapy is safe and effective.But the adverse reactions including hypertension,neurovirulence,gastrointestinal bleeding and perforation are raised up.Scholars have carried out a series of studies for the overall survival times,tumor response rates and survival qualities for the patients with metastatic colorectal cancer which using the joint scheme,but they draw different conclusions the usefulness and safety of the joint scheme still need more RCT and meta-ananlysis to be proved.
10.Techniques of single access laparoscopic Dixon operation for the treatment of rectal cancer

The efficacy of laparoscopic Dixon operation for the treatment of rectal cancer has been proven,while at the same time there are opportunities to make the procedure less invasive.From January 2010 to March 2011,9 cases of single access laparoscopic Dixon operation have been performed at the Shengjing Hospital.An incision with the length of 3 cm was created in umbilicus and 3 trocars were inserted through the fascia on the abdominal wall.The dissection and anastomosis were performed through the incision.The resected tumors were removed through the umbilical incision or anus.The operation was successfully performed on 9 patients,and one more incision with the length of 12 mm was created on the lower abdominal wall in 1 patient in order to install the stapling device.The average operation time was 202 minutes.No postoperative bleeding,intestinal obstruction or anastomosis leakage was found.
