1.Glucocorticoid induced apoptosis in osteoblast via a mitochondria-mediated pathway

Objective To explore the molecular mechanism of osteoblast apoptosis mediated by glucocorticoid, provide experimental basis for clinical prevention and treatment of hormonal necrosis of femoral-head.Methods The MC3T3-E1 cells were divided into control group and three treatment group treated with different concentrations of dexamethasone (1, 10, 100μmol/L) for 24 hours.The apoptosis rate detected by TUNEL, the mitochondrial transmembrane electric potential after JC-1 staining by flow cytometry, the expression of Bax, Bcl-2 in mitochondrion and Cyt-C, apoptosis inducing factor ( AIF) in cytoplasm by Western blot.ResuIts With the increasing of Dex concentration, the rate of osteoblast apoptosis was significantly increased(F(3,28) =159.0,P=0.000), the positive cell rate of JC-1 stainning was significantly increased(F(3,28) =499.5,P=0.000), the expression of Bax(F(3,28) =17.4,P=0.000)in mitochondrion and Cyt-C(F(3,28) =22.4,P=0.000), AIF(F(3,28) =42.61,P=0.000)in cytoplasm were significantly increased.There were statistically significant difference of above indexes between control group and each experimental group(P<0.05).But there were no significant differences of Bcl-2 in mitochondrion among five groups(F(3,28) =0.62,P=0.607).ConcIusion Dexamethasone increases expression of Bax in mitochondrion, prompts mitochondrial permeability transition pore ( MPTP ) to be over-openning, induces apoptosis in osteoblast via a mitochondria-mediated pathway.
3.The regression model and correlation analysis between condensate infection level and influencing factors in ventilator pipe

Objective To explore the related factors influencing the condensate infection level in order to provide a reference for ventilator pipe care and reducing condensate infections.Methods A total of 80 patients using ventilator in ICU of the First Hospital of Liaoning Medical University was chosen as the research object,and the related factors influencing the condensate infection using correlation and regression analysis method were explored,and the regression model was estabhshed as well.Results Condensate infection were correlated with pipe length (r=0.837,P<0.01),temperature difference between ward and gas in the pipe (r=0.875,P<0.01),exhaled gas humidity (r=0.793,P<0.01) and ventilation (r=0.932,P<0.01),but the pipe length could not enter the regression equation (t=-0.09,P>0.05),and the regression equation was y=-168.08 +9.96x1 (temperature difference) +0.33x2 (exhaled gas humidity) +20.98x3 (ventilation).Conclusions Pipe length,temperature difference between ward and gas in the pipe,exhaled gas humidity and ventilation were the influencing factors,and medical personnel can control the infection of condensate considering the influencing factors.
5.Surgical Treatment of Intracranial Angiographically Occult Vascular Malformations Hemorrhage:40 Cases Report

ObjectiveTo review the surgical technique and outcome of intracranial angiographically occult vascular malformations (AOVM) hemorrhage. Methods40 patients with spontaneous intracranial hemorrhage episodes and accepted microsurgery were analyzed retrospectively. ResultsAll AOVMs were successfully resected. No operative death occurred.36 patients were followed up for mean 28 months. CT and/or MRI revealed no residual lesions. Most neurological complications improved postoperatively. 1 case demonstrated rebleeding in surgical site 48 months after operation. Another case, a follow-up MRI scan revealed a contralateral denovo lesion 44 months after operation.ConclusionA current neurosurgical procedure is a satisfactory method to prevents AOVMs rebleeding. Long-term follow-up evaluation is necessary despite the lesion has been totally resected.
6.Comparative study on bond strength of porcelain fused to CAD/CAM and to cast pure titanium

Objective:To compare the titanium-porcelain bonding strength between CAD/CAM and cast pure titanium.Methods:Pure titanium samples were prepaired by CAD/CAMand casting respectively and fused with porcelain.The bond strength between tita-nium and porcelain of the samples was measured with three-point test.The titanium-porcelain interface was investigated under scanning electronic microscope(SEM).Results:The bond strength of CAD/CAMpure titanium to porcelain was (35.95 ±3.74)MPa and cast pure titanium to porcelain was (28.37 ±1 .98)MPa(P <0.05).SEMobservation showed that there was thin transition layer between titanium substrate and ceramic in CAD/CAM bond interface,titanium and porcelain combined closely,no obvious pores.However, there was thicker transition layer and small pore in cast pure titanium to porcelain bonding interface.Conclusion:The bonding strength of porcelain to CAD/CAMpure titanium is greater than that of porcelain to casting pure titanium.
7.Adults Moyamoya Disease Patients Present with Cerebral Ischemia

Objective Moyamoya disease is a progressive cerebrovascular occlusive disease. The etiology of the disease is unknown. most think that Moyamoya usually presents with cerebral ischemia in children and intracranial hemorrhage in adults. But we have some adults present with cerebral ischemia .This article will help us know better about moyamoya disease in adults present with cerebral ischemia. And will let us know better about treatment and prognosis of this disease.Methods We reviewed our experience 88 adults angiographically diagnosed patients with moyamoya. There are 49 patients with moyamoya disease initially present with cerebral ischemia. We report their clinical presentation, radiological findings, management, and clinical outcomes. Results The incidence of moyamoya disease presents with cerebral ischemia is 55.7% in adults .The mean age of patients at symptom onset was 32 years (range, 18~64 years). Ischemic strokes or transient ischemic attacks were the predominant initial presentations.7 cases present intracranial haemorrhage in follow-up period. There was no difference in clinical representation, image and cerebral infarction in this 7 case.Conclusions Moyamoya usually presents with cerebral ischemia in adults higher than we think. diagnosis and treatment of moyamoya disease in adults is essential to minimize residual mental and physiologic deficits. If we find patients 30-40 years old ,with cerebral infarction that can’t explain in conventional mode, we should pursuit these patients, in order to exclude Moyamoya disease .Structured multicenter randomized clinical trials are needed to further assess the best treatment modalities for patients with moyamoya.
8.Progress in Stat3 and SirT1 gene in the pathogenesis of rheumatoid arthristis

Rheumatoid arthritis ( RA) is a chronic inflammatory disease involving progressive articular damage cause by inflam-matory cells and synoviocytes.Stat3 gene, which is confirmed to play a role in the pathogenesis of RA, regulates the metabolism and apoptosis of the fibroblast-like synoviocytes ( FLS) .SirT1 gene relating to the bone formation was indicated to play a role in the patho-genesis of RA in recent years.This review introduces the pathogenesis and targeted treatment of RA and makes a prediction about the relation between Stat3 and SirT1 gene.We hope it will provide a new direction for the research of RA.
9.Study on effect of jiangya tongmai recipe on vascular activating substances in patients of hypertension with left ventricular hypertrophy.

OBJECTIVETo study the effect of Jiangya Tongmai Recipe (JYTMR), a Chinese herbal medicine preparation for activating blood circulation to remove stasis, on vascular activating substances in treating patients of hypertension with left ventricular hypertrophy.
METHODSThe 37 patients with hypertension were randomly divided into two groups, the treated group (n = 21) and the control group (n = 16). They were treated with JYTMR and captopril respectively for 8 weeks. Left ventricular mass weight (LVMI), peak flow velocity of early diastole (Emax), peak flow velocity of atrial contraction (Amax), Emax/Amax, ejection fraction (EF), as well as levels of plasma endothelin (ET), calcitonin gene-related peptide (CGRP) and angiotensin II (Ang II) were measured before and after treatment and compared.
RESULTSJYTMR could enhance the left ventricular dilation and contractile functions, lower the levels of plasma ET and Ang II and increase the level of plasma CGRP.
CONCLUSIONJYTMR shows good effect in improving left ventricular function and regulating vascular activating substances, it could prevent and treat hypertension and its complications for prolonged treatment via multiple paths and links.

10.Application of antibiotic cement-coated locking plates in induced membrane technique for treating post-traumatic tibial osteomyelitis

Objective To investigate the clinical outcome of post-traumatic tibial osteomyelitis treated by induced membrane technique combined with the antibiotic cement-coated locking plate.Methods A restrospective case series analysis was made on 71 cases of post-traumatic tibial osteomyelitis treated by induced membrane technique from September 2014 to September 2014.There were 55 males and 16 females, aged 18-60 years(mean, 37.4 years).Mean length of bone defect following debridement was 4.5 cm(range, 4-11 cm).Antibiotic cement-coated locking plates were used to provide the stability of the bone defects after one-stage debridement.Bone grafting and exchanging the plates with ntramedullary nails were done during the second-stage surgery.Parameters were monitored dynamically after operation to evaluate infection recurrence and bone healing, including C-reactive protein (CRP), erythrocyte sedimentation rate (ESR), clinical features(sinus formation, redness and swelling, warmth and pain) and X-ray films of the involved limbs.Results All patients were followed up for 13-25 months (mean, 19.5 months).There was no sign of recurring infection and CRP and ESR were normal in all cases after the first stage surgery.However, four cases (6%) had recurrent infection with sinus exudates and their CRP and ESR were abnormal after the second stage surgery, and further treatments were carried out on these patients.Follow-up showed bony union in all patients within mean 5.5 months (range, 4-6 months).Conclusion Induced membrane technique with antibiotic cement-coated locking plates for treatment of post-traumatic tibial osteomyelitis can shorten bone healing time and decrease infection recurrent rate without obvious impact on osteogenic activity.
