2.Surgical treatment of benign prostatic hyperplasia in elderly patients

Objective To investigate the clinical efficacy of surgery treatment of benign prostatic hyperplasia in elderly patients.Methods 60 elderly patients with prostatic hyperplasia aged 80 years and over were admitted in our hospital from January 2005 to January 2009.56 cases were treated with transurethral plasmakinetic resection of benign prostatic hyperplasia,and 4 cases were treated with suprapubic prostatic hyperplasia enucleation.IPSS score,residual urine volume and maximum urinary flow rate (Qmax) in all patients were compared between pre-and post-surgery.Results 60 cases of patients were operated successfully without significant intraoperative capsular perforation,ureteral orifice and urethral sphincter injury.IPSS scores and residualurine volume were significantly less after surgery than before surgery [(20.3 ± 3.7) vs.(11.5 ± 2.1),(85.1 ± 13.0) ml vs.(37.7±7.9) ml,t=16.02,24.14,both P<0.05].The maximum flow rate Qmax was significantly higher after surgery than before surgery[(8.7 ± 2.1) ml/s vs.(14.9 ± 3.2) ml/s,t =12.55,P<0.05].All patients were followed up for 6-12 months.No incontinence and recurrence of benign prostatic hyperplasia were found.Conclusions Transurethral plasmakinetic resection for treatment of benign prostatic hyperplasia is safe and effective in elderly patients,with a low recurrence rate,and is worthy of promoting.
3.Study on the culture and the function of dendritic cells of patients with laryngeal squamous cell carcinoma in vitro

OBJECTIVE To culture dendritic cells from the peripheral blood ofpatients with laryngeal squamous cell carcinoma. METHODS Peripheral blood was collected from patients under sterile conditions. The mononuclear cells were separated by centrifugation through a density gradient (Ficoll-Hypaque). The adhesion cells, collected from the mononuclear cells, were cultured with interleukin-4 (IL-4, 100ng),granulocyte/macrophage colony stimulating factor (GM-CSF, 100ng) and tumor necrosis factor ? (TNF-?, 100ng) in RPMI 1640 medium for 7 days. The cell morphology was observed under electronic microscope, and the characteristic surface marker CD80 of the cells was analyzed by flow cytometry. MTT colorimetry was employed to assess the proliferation of the mixed lymphocytes. The mixed lymphocytes were stimulated by DC loaded with laryngeal carcinoma antigen. RESULTS A large quantity of cells characteristic of dendritic cell was obtained. Cytometry analysis showed that 33.76 % of mononuclear cells expressed the characteristic marker CD80. After being cultured for 7 days, the expression level of CD80 was significantly increased (69.15 %). Few DC with laryngeal carcinoma antigen could play a role in stimulating lymphocyte proliferation, and the proliferation effect was optimized when the ratio between DC and T cells was 1:5. CONCLUSION A large number of DCs with high purityand sufficient treatment dose can be cultured in vitro from the peripheral blood of patients with laryngeal carcinoma. This technique may play an important role in experimental studies for clinical applications.

5.Perioperative management of thoracolumbar fractures fixed with latero-anterior screw-plate system

[Objective]To discuss the perioperative management of thoracolumbar fractures fixed with latero-anterior screw-plate system.[Method]The entry point and direction of screw on 30 thoracic and lumbar vertebra specimens were marked based on the principle and demand of implantation of latero-anterior screw-plate system and were determined exactly by anatomic landmarks in order to make reference for operation.Sixty patients with thoracolumbar fractures were treated with anterior decompression,bone grafting and internal fixation from January 2000 to June 2007.Reference data such as the length of vertebral body screw and plate and the height of bracing gotten from the patients'X-ray and CT findings before operation were compared with practical data measured during operation.The entry point and the direction of screw were determined exactly by anatomic landmarks of thoraco lumbar vertebrae and were checked by X-ray during operation.[Result]The judgement of the entry point and the direction of screw was easy and accurate during operation and the reference data were in coincidence with practical data.The effects of operation were satisfactory.[Conclusion]The perioperative management is both scientific and practical.The surgical complications can be reduced in this way.
6.Experimental research of contracture of hip joint capsule induced by intragluteal injection

[Objective]To explore the mechanism of hip joint capsule contracture induced by intragluteal injection. [Method]Thirty young Chinese rabbits were divided into four groups of A(n=6),B(n=6),C(n=6) and D(n=12) at random.Rabbits in groups A,B and C were treated with deep intragluteal injection of penicillin dissolved in 2% benzyl alcohol(group A),2% benzyl alcohol(group B)and penicillin dissolved in physiologic saline(group C).Rabbits in group D were treated with immobilization of unilateral hip joint and deep intramuscular injection of physiologic saline on the homolateral buttock.Each rabbit was injected twice a day for 20 days.Specimens were obtained at 30,60 and 90 days after first injection and the evaluation of the functions of hind limbs was obtained at the same time.The thickness of hip joint capsules were measured with micrometer and sections of joint capsules were stained by Sirius red and were observed with microscope and polarization microscope.[Result]Capsules of hip joint in group A and B became thicker and contractural,and the change of joint capsules was reflected in the change of motion of hind limbs.Sections in groups A and B showed shrinked capsules with thick collagens in disordered arrangement;while there were no gluteus contracture and contracture of joint capsule in the other groups.[Conclusion]Deep intragluteal injection of penicillin dissolved in 2% benzyl alcohol or 2% benzyl alcohol into young rabbits can cause contracture of hip joints.The contracture of joint capsule is largely induced by tissue trauma and repair of joint capsules.The limited locomotion of hip joint induced by the contracture of gluteal muscle does not play an important role in the contracture of joint capsule.

8.LOCALIZATION OF PRIMARY HYPERPARATHYROIDISM WITH ~(99m)Tc-MIBI

Parathyroid scanning was performed with 99mTc-MIBI in 10 patients who had clinical and biochemical findings suggesting primary hyperparathyroidism but had no enlarged parathyroid or palpable thyroid. Seven cases were scanned with both 99mTc-MIBI and 99mTc and three cases with 99mTc-MIBI only but perchlorate was administered during the scan. A series of images were acquired for all cases at different time intervals after 99mTc-MIBI was injected intravenously to allow the dynamic studies. The P/T ratios (counts in parathyroid counts in thyroid)were also designed to see whether the agent had different uptakes and metabolic characteristics in parathyroid and thyroid tissues from 99mTc-MIBI. Surgical finding and histological examination were taken as gold standard against which US. CT and scintigraphy were judged. Finally, 10 adenomas found exactly at the sites predicted by scintigraphy and 2 hyperplastic glands were removed from the 10 patients. Ten of 10 99mTc-MIBI scans, two of 9 US scans and three of 8 CT scans were positive for a parathyroid adenoma, but 2 hyperplastic glands were missed by all of the three methods. It was shown that the suitable time for imaging was at 1h-2h or 30min-1h(if perchlorate was used) after 99mTc-MIBI was given, so as to render both adenoma and background clear. Despite the limited number of cases, the preliminary experience thus suggests that 99mTc-MIBI scintigraphy maybecome the technique of choice for the localization of hyperparathyroidism in stead of 21Tl.
9.Clinical application of minimally invasive direct cardiac surgery: 108 cases report

Objective The aim of this study is to summarize primarily the application range and the clinical effect of the minimally invasive direct cardiac surgery (MIDCS).Methods From April 2010 to August 2011,108 operations had been performed in our centre using MIDCS,among of which 44 males and 64 females,mean age (44.3 ± 16.4 ) years old,mean weight (60.0 ± 12.0 ) kg.These operations included 29 MVR operations (replacement with mechanical valve 19,with tissue valve 10),17 AVR operations( replacement with mechanical valve 11,with tissue valve 6 ),5 MVP operations,6 MVR + AVR operations (replacement with mechanical valve 5,with tissue valve 1 ),12 VSD repair operations and 23 ASD repair operations,9 MIDCAB operations,1 MVR + CABG oparation,2 PECD correction,1 repair of ruptruec chordae tendineae of tricuspid septum and 1 Ebstein deformity correction operation.Concomitant procedures included 19 TVP operations and 11 radiofrequency ablation operations.Except MIDCAB operations,other operations are performed with closed type extracorporeal circulation.Conversion to median sternotomy was necessary in only two patients ( 1.9% ).Results Mean cardiopulnonary bypass time and aortic eross-clamp time were ( 104.4 ± 59.3 ) minutes and ( 66.7 ± 52.8 ) minutes respectively,74 patients ( 92.5 % ) recover to beat automatically after heart ceased operation.Median mechanical ventilation time was ( 14.5 ± 11.9 ) hours,Median intensive care unit stay was ( 18.8 ± 15.3 ) hours,median hospital stay post operative was (6.9 ± 2.9) days.Median incision length was (5.3 ± 1.2 )cm.Median draining volume was( 337.6 ± 240.9 )ml in the first day after operation,No transfusion occurred in 72 patients(66.7% ).Hospital mortality was 0.No re-exploration for bleeding and sternal wound infection.When leaving hospital,94 being of heart functional class Ⅰ,8 of class Ⅱ,6 of class Ⅲ.Conclusion MIDCS was associated with good operative effect in the near future,superior safety and broad application range.Furthermore MIDCS has minimally invasive,less postoperative bleeding,fewer blood transfusions,good cosmesis,as well as the absence of sternal wound infection.It may be safely performed on selected patients with low postoperative mortality and morbidity.
10.Research on the treatment of liver failure rats with transplantation of alginate microencapsulated hepatocytes in vivo based on poly-ornithine.

This study aims to explore the effects of alginate-poly ornithine-alginate (A-PLO-A) and barium alginate-poly ornithine-alginate (B-PLO-A) microcapsules as cells carriers during implantation. Mice hepatocytes coated in A-PLO-A and B-PLO-A microcapsules were implanted into rats with acute liver failure caused by intraperitoneal injection of D-galactosamine. The rat survival rate, liver cell growth, proliferation and metabolism within the microcapsules were investigated, as well as its effect on the improvement of rat acute liver failure. The influence of A-PLO-A-free microcapsules, B-PLO-A-free microcapsules, isolated liver cells, A-PLO-A microcapsule-coated and B-PLO-A microcapsule-coated liver cells was studied. It was found that the chemical-free microcapsules showed no positive effect on the rats with liver failures, with a death rate of 100% in both groups 3 days after the implantation. The ALT, AST and ALB levels were all improved in the isolated liver cell group, the A-PLO-A microcapsule-coated and the B-PLO-A microcapsule-coated groups. The survival rate of both microcapsule-coated liver cell groups was significantly higher than that of the chemical-free microcapsule group and the isolated liver cells group. The microcapsules were retrieved after 4 weeks' implantation, which were observed to be smooth with no cells attaching to the surface. No apparent fibrosis was observed. This research demonstrated the physical stability and the biocompatibility of the PLO-based alginate microcapsules and therefore they could be used as liver cell carriers during implantation.
